Paraneoplastic neuropathies.

PURPOSE OF REVIEW: To review recent advances in paraneoplastic neuropathies with emphasis on their definition, different forms and therapeutic development. RECENT FINDINGS: A strict definition of definite paraneoplastic neuropathies is necessary to avoid confusion. With carcinoma, seronegative sensory neuronopathies and neuronopathies and anti-Hu and anti-CV2/Contactin Response Mediator Protein 5 antibodies are the most frequent. With lymphomas, most neuropathies occur with monoclonal gammopathy including AL amyloidosis, Polyneuropathy-Organomegaly-Endocrinopathy-M component-Skin changes (POE

Anti-neurofascin-155 antibody mediated a distinct phenotype of chronic inflammatory demyelinating polyradiculoneuropathy.

BACKGROUND: To investigate Ranvier's autoantibodies prevalence and isotypes in various peripheral neuropathy variants, compare clinical features between seronegative and seropositive patients, and elucidate immune mechanisms underlying antibody generation. METHODS: Antibodies against anti-neurofascin-155 (NF155), NF186, contactin-1 (CNTN1), CNTN2, contactin-associated protein 1 (CASPR1), and CASPR2 were identified through cell-based assays. Plasma cytokines were analyzed in anti-NF155 antibody-positive chronic inflammatory demyelinating polyneuropathy (NF155+ CIDP) and Ranvier's antibodies-ne

The mouse neuronal cell surface protein F3: a phosphatidylinositol-anchored member of the immunoglobulin superfamily related to chicken contactin.

Several members of the Ig superfamily are expressed on neural cells where they participate in surface interactions between cell bodies and processes. Their Ig domains are more closely related to each other than to Ig variable and constant domains and have been grouped into the C2 set. Here, we report the cloning and characterization of another member of this group, the mouse neuronal cell surface antigen F3. The F3 cDNA sequence contains an open reading frame that could encode a 1,020-amino acid protein consisting of a signal sequence, six Ig-like domains of the C2 type, a long premembrane reg

Development and Inter-Laboratory Validation of Multiplex Flow Cytometry Assay for Autoimmune Nodopathy.

BACKGROUND AND PURPOSE: Autoimmune nodopathies (AN) are a group of rare disorders caused by autoantibodies targeting cell-adhesion molecules located at the node of Ranvier. Reliable and efficient antibody testing for AN has become essential for optimal care of autoimmune neuropathies. In this study, we developed a multiplex flow cytometry (FCM) assay capable of simultaneously detecting four major AN antibodies and validated its diagnostic performance through inter-laboratory collaboration. METHODS: Multiplex FCM assay was established using live cell lines stably expressing each of the four AN

The Discovery of Autoimmune Nodopathies and the Impact of IgG4 Antibodies in Autoimmune Neurology.

In the past decade, significant progress has been made on the understanding of IgG4-mediated autoimmune diseases, of both the central and the peripheral CNS. In addition to the description of diverse antigenic targets, the description of IgG subclasses associated with specific pathogenic autoantibodies has provided useful insights into the pathophysiology and, more importantly, into the therapeutic implications of the autoantibody subclasses. This understanding has affected how myasthenia gravis, autoimmune encephalitis, and autoimmune neuropathies are treated. In the case of autoimmune neurop

Prevalence and clinical implications of diabetes mellitus in autoimmune nodopathies: A systematic review.

BACKGROUND AND AIMS: Autoimmune nodopathies comprise a newly-established subtype of immune-mediated peripheral neuropathies, characterized by circulating autoantibodies that target nodal-paranodal proteins, including contactin-1 (CNTN1), contactin-associated protein-1 (Caspr1), neurofascin-155 (NF155) and neurofascin-isoforms (NF140 and NF186). Emerging evidence suggests that diabetes mellitus (DM) may confer increased risk for autoimmune nodopathies. METHODS: A systematic search was performed including studies reporting on patients harboring nodal/paranodal antibodies (CNTN1, Caspr1, NF155,

Genome-wide association study in bipolar patients stratified by co-morbidity.

BACKGROUND: Bipolar disorder is a severe psychiatric disorder with high heritability. Co-morbid conditions are common and might define latent subgroups of patients that are more homogeneous with respect to genetic risk factors. METHODOLOGY: In the Caucasian GAIN bipolar disorder sample of 1000 cases and 1034 controls, we tested the association of single nucleotide polymorphisms with patient subgroups defined by co-morbidity. RESULTS: Bipolar disorder with psychosis and/or substance abuse in the absence of alcohol dependence was associated with the rare variant rs1039002 in the vicinity of th

The clinical features of anti-CNTN1 antibodies associated autoimmune nodopathies with nephropathy: a case series.

OBJECTIVE: The concurrence of anti-contactin-1 (CNTN1) antibody-associated autoimmune nodopathies (AN) and nephropathy has been sporadically reported, though the underlying relationship remains unclear. This case series study aimed to investigate the clinical characteristics and renal pathology of anti-CNTN1 antibody-associated AN with nephropathy. METHODS: A retrospective case series was conducted including 69 patients diagnosed with AN, among whom 11 patients tested positive for anti-CNTN1 antibodies. Clinical manifestations, electrophysiological data, laboratory results, and renal biopsy f

Distinct Epitopes Are Associated With Clinical Phenotypes in Autoimmune Nodopathies With Anti-Contactin1 Autoantibodies.

BACKGROUND AND OBJECTIVES: Autoimmune nodopathy with anti-contactin1 (CNTN1) autoantibodies is a rare sensory-motor neuropathy characterized by subacute-onset sensory ataxia and variable disease courses. Comorbidities such as glomerulonephritis and diabetes mellitus are observed in some patients. The diversity in clinical presentation may reflect differences in the targeted CNTN1 epitopes. METHODS: To investigate the relationship between clinical features and the underlying epitopes, we analyzed serum samples from 16 anti-CNTN1-positive patients. Binding epitopes were assessed using cell-base

Comparative Analytical Performance of Autoimmune Nodopathy Diagnostic Assays.

BACKGROUND: Autoimmune nodopathy (AN) is a disabling peripheral nerve disorder mediated by four pathogenic autoantibodies targeting the node of Ranvier: anti-neurofascin-155 (Nfasc155); anti-Nfasc155 and Nfasc186 (PanNfasc); anti-contactin-1; and anti-contactin-associated protein 1 autoantibodies. Several autoantibody detection assays exist; however, which assay yields the best performance remains unclear. We evaluated the performance of five diagnostic assays individually and in combinations to identify a gold standard assay for diagnosing AN and improve diagnostic accuracy. METHODS: Sera fr

Diagnosis of a floppy neonate with misleading clues: unraveled as congenital hypomyelinating neuropathy.

Contactin-associated protein 1 (CNTNAP1)-related congenital hypomyelinating neuropathy (CHN) is a rare type of peripheral neuropathy and has a clinically heterogeneous presentation. We report a neonate with an atypical presentation in the form of global hypotonia, facial diparesis and partial response to neostigmine challenge test. There was no clinical improvement on initiation of anticholinesterase drug for suspected congenital myasthenia and hence stopped. Detection of a pathogenic variant in CNTNAP1 gene by clinical exome sequencing and subsequent reverse phenotyping confirmed CHN as the a

Proprioceptive endings at muscle spindles as a possible target of autoantibodies.

Autoimmune nodopathies are characterized by autoantibodies targeting proteins of the nodes of Ranvier of peripheral nerves. Anti-Contactin-1 (CNTN1) autoantibodies are associated with sensorimotor neuropathy and severe sensory ataxia. While damage to the nodal architecture due to axoglial disjunction is considered the primary pathogenic mechanism of autoimmune nodopathies, the reason for the severe sensory ataxia, accompanied by proprioceptive impairment, remains unclear. We therefore aimed to investigate the expression of CNTN1 in muscle spindles and the binding of IgG from patients with anti

Antibodies in Autoimmune Neuropathies: What to Test, How to Test, Why to Test.

Autoimmune neuropathies are a heterogeneous group of immune-mediated disorders of the peripheral nerves. Guillain-Barré syndrome (GBS) and chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) are the archetypal acute and chronic forms. Over the past few decades, pathogenic antibodies targeting antigens of the peripheral nervous system and driving peripheral nerve damage in selected patients have been described. Moreover, the detection of these antibodies has diagnostic and therapeutic implications that have prompted a modification of the GBS and CIDP diagnostic algorithms. GBS diag

Anti-neurofascin autoantibody and demyelination.

Demyelination diseases involving the central and peripheral nervous systems are etiologically heterogeneous with both cell-mediated and humoral immunities playing pathogenic roles. Recently, autoantibodies against nodal and paranodal proteins, such as neurofascin186 (NF186), neurofascin155 (NF155), contactin-1 (CNTN1), contactin-associated protein 1 (CASPR1) and gliomedin, have been discovered in not only chronic demyelinating conditions, such as multiple sclerosis (MS) and chronic inflammatory demyelinating polyradiculoneuropathy, but also in acute demyelinating conditions, such as Guillain-B

Neuro-renal syndrome (NRS) in autoimmune nodopathies and paranodopathies: A warning to consider the diagnosis when anti-node and paranode antibody tests are difficult to perform.

INTRODUCTION: Neuro - renal syndrome (NRS), a neuroimmunomediated entity that presents with acute nephrotic syndrome (primarily membranous glomerulonephritis), recurrent proteinuria, and even acute renal failure requiring dialysis, has been reported with varying prevalence in patients with immunomediated neuropathies and paraneoplastic syndromes. Pathophysiologically, IgG 3 and IgG 4 antibodies target the peripheral nervous system (PNS) and central nervous system (CNS), leading to predominantly distal weakness, sensory ataxia, and electrophysiological abnormalities. The patient tends to be ref
