The effects of continuity of care on hospital utilization in patients with knee osteoarthritis: analysis of Nationwide insurance data.

BACKGROUND: Korea's rapidly aging population has led to a rise in the prevalence of knee osteoarthritis (which reached upwards of 21.3% in 2017) in elderly people aged 65 years and over. Most patients with knee osteoarthritis require ongoing management in the community or through primary care. Continuity of care is a desirable attribute of primary care. However, previous studies on the association between continuity of care and health outcomes have focused on specific disease populations, particularly diabetes mellitus and hypertension. The objectives of this study were to determine whether th

Interprofessional management of a complex continuing care patient admitted with 18 pressure ulcers: a case report.

Interprofessional practice (IP)--ie, collaborative practice--involves interaction and knowledge-sharing between professionals from different disciplines in order to meet the needs of the patient. This approach to care is well suited to patients with pressure ulcers, whose complex and varying presentations require the monitoring and consultation of an IP team. A 44-year-old man with anoxic brain injury was admitted to a complex continuing care facility with 18 wounds, 17 of which were pressure ulcers. The patient was at high risk for further skin breakdown as a result of immobility, incontin

OBJECTIVE: To evaluate the effects of a three-dimensional interconnected continuity of care intervention (hospital-community-family linkage) on self-care capacity, negative emotions, stigma, quality of life, and complication rates in patients with ostomies. METHODS: A retrospective comparative study was conducted on 94 patients with intestinal or urinary ostomies treated from October 2021 to June 2024. Patients receiving routine nursing care were assigned to the control group (n = 47), while those receiving additional three-dimensional linked continuity care intervention were assigned to the

Continued tobacco use among tobacco-related head and neck cancer patients seeking care in a tertiary care facility, South India.

INTRODUCTION: Continued tobacco use in cancer patients leads to decreased treatment efficacy and safety, decreased survival, decreased quality of life, and an increased risk of cancer recurrence and primary tumours at other sites. OBJECTIVE: To determine the prevalence of continued tobacco usage during the first 6 months of diagnosis among tobacco-related head and neck cancer patients seeking care from a tertiary care centre and the factors associated with it. METHODS: A facility-based cohort study was conducted at a tertiary care centre in Puducherry, India. Newly diagnosed head and neck ca

Development of a Continuity of Care Program for Postoperative Enterostomy Patients With Colorectal Cancer Guided by Heider's Balance Theory.

AIM: Patients with colorectal cancer undergoing enterostomy often encounter complex physical, psychological, and social challenges after discharge. Effective continuity of care is essential to support postoperative recovery and long-term adaptation. However, existing care programs frequently lack a comprehensive and theoretically grounded framework. Guided by Heider's balance theory, this study developed a continuity of care program for postoperative enterostomy patients with colorectal cancer, thereby providing a reference for clinical nursing practice. METHODS: A comprehensive search of dom

Challenges to continuity of care among patients with long COVID-related taste and smell disorders: a qualitative study.

Taste and smell disorders are predominant symptoms of acute SARS-CoV-2 infection and can persist in individuals with post-acute sequelae of SARS-CoV-2 infection (PASC), commonly referred to as Long COVID. These sensory impairments continue to significantly affect daily life and wellbeing, yet clinical understanding and care strategies remain insufficient. There is an ongoing need to address the specific healthcare requirements of this population. The aim of this study was to describe the experiences and perceptions of individuals with Long COVID related taste and smell disorders regarding symp

Patient perspectives on factors that influence continued treatment engagement in primary care behavioral health: A qualitative exploration.

INTRODUCTION: Primary care behavioral health (PCBH) is a service delivery model that embeds behavioral health consultants in primary care. PCBH has been implemented across several health care systems and has been shown to increase access to behavioral health services. Most of the patient access research in PCBH settings has focused on initial engagement with behavioral health services via the warm handoff, a process of referral where the primary care provider can connect the patient with an on-site behavioral health consultant the moment a need is identified. In contrast, fewer studies have ex

OBJECTIVE: To analyze the risk factors for unplanned discontinuation of continuous renal replacement therapy (CRRT) in intensive care unit (ICU) patients, and to provide a reference for clinical medical staff to identify high-risk patients early and formulate intervention measures. METHODS: A cross-sectional study design was adopted. Patients who received CRRT in the Central ICU of the First Affiliated Hospital of Dalian Medical University from March 17 to July 25, 2025 were enrolled. Is AI Patient Safety 4.0?: From Find-and-Fix to Continuous Intelligence: Toward Care That Needs No Advocate.

Three decades after To Err Is Human catalyzed the modern patient safety movement, roughly one in four hospitalized patients still experiences an adverse event, and nearly one in four patients who die or deteriorate in U.S. hospitals does so in the setting of a missed or delayed diagnosis. These figures have prompted an appropriately skeptical question: has patient safety made any real progress? This special article argues that the skepticism mistakes an unfinished agenda for a failed one, and that the safety field has evolved through a recognizable sequence of framings. It also argues that art

Investigating the Impact of Continuing Care Model on Quality of Life and Cardiac Function in Patients With Coronary Heart Disease.

AIMS/BACKGROUND: The transition from hospital to home is a critical period for patients with coronary heart disease (CHD), and inadequate post-discharge care can adversely affect recovery. Continuing care models have been proposed to address these challenges, but their relative effectiveness compared with standard care still requires further evaluation. Therefore, this study evaluated the impact of continuing care on quality of life and recovery of cardiac function in patients with CHD. METHODS: This single-center, retrospective cohort study included 135 discharged CHD patients. Based on nurs

How Should Surgeons Help Formerly Incarcerated Patients With Chronic Surgical Needs Maintain Care Continuity?

Stoma care is very challenging, and, with the added hardship of incarceration, patients find it very difficult to navigate living with a stoma and having it reversed in a timely fashion. Incarceration history adds to the clinical and ethical complexity of surgical care for patients who require an ostomy, especially when secondary to trauma. This commentary on a case canvasses strategies for responding to long-term needs of formerly incarcerated patients with an ostomy who need good follow-up care as much as they need support reintegrating into communities.

Continuity of care for TB patients at a South African hospital: A qualitative participatory study of the experiences of hospital staff.

BACKGROUND: Ensuring effective clinical management and continuity of TB care across hospital and primary health-care services remains challenging in South Africa. The high burden of TB, coupled with numerous health system problems, influence the TB care delivered by hospital staff. OBJECTIVE: To understand factors from the perspectives of hospital staff that influence the clinical management and discharge of TB patients, and to elicit recommendations to improve continuity of care for TB patients. DESIGN: Participatory action research was used to engage hospital staff working with TB patients

CE: continuing education article. Integration of palliative care for patients with stage 5 chronic kidney disease.

Patients with advanced chronic kidney disease (Stage 5 CKD) have palliative care needs similar to patients with cancer. The decision not to commence dialysis or to withdraw from active treatment can have a profound impact upon all those closely involved in the patient's care. It is essential that every effort is made to minimise the physical and psycho-social symptoms experienced by patients who require palliative care. Effective teamwork across professional boundaries and specialities will ensure that patients and their families are provided with maximum comfort during their final days. All m
