[Continuous renal replacement therapy in children with acute renal failure--5 years of experience].

Continuous renal replacement therapy (CRRT) has became a modality of choice in chidren with acute renal failure (ARF), especially in cases of multiorgan failure (MOF) and in hemodynamically unstable patients in whom regular hemodialysis is difficult to reform. Newborns and infants with contraindications to peritoneal dialysis are another group of patients treated with CRRT. Retrospective analysis of CRRT therapy in 112 patients treated with (CVVHD, CVVH, CVVHDF, SCUF-continuous veno-venous hemodialysis/hemofiltration/ hemodiafiltration/ultrafiltration) between 2000-2005 is presented. Prognosis of Long-Term Continuous Renal Replacement Therapy and the Impact of Combined Continuous Intravenous Sodium Infusion Therapy.

INTRODUCTION: Patients requiring long-term continuous renal replacement therapy (CRRT) generally have poor prognoses. This study evaluated whether adding continuous intravenous sodium infusion (cIVNa) is associated with improved hemodynamics and outcomes in patients undergoing long-term CRRT for ≥ 7 days. METHODS: This secondary analysis included 46 patients with acute kidney injury who required ≥ 7 days of CRRT from a previously published cohort. Patients received either conventional CRRT (n = 24) or CRRT plus cIVNa (n = 22). Hemodynamic parameters, fluid balance, laboratory findings, and cl

Risk factors of continuous renal replacement therapy following total aortic arch replacement under moderate hypothermia.

BACKGROUND: Stanford type A aortic dissection (TAAD) has a sudden onset and high mortality, and emergency total aortic arch replacement (TAAR) is the main treatment option for TAAD. The mortality rate of patients with postoperative acute kidney injury (AKI) combined with continuous renal replacement therapy (CRRT) is remarkable higher than that of patients without AKI. However, incidence of AKI and risk factors for CRRT following TAAR isn't entirely understood. METHODS: From October 2018 to March 2021, all patients with Stanford type A dissection who underwent total arch replacement surgery u

Model-informed cefepime dosing in paediatric patients receiving continuous renal replacement therapy.

Critically ill children receiving β-lactam antibiotics while on continuous renal replacement therapy (CRRT) are at high risk of subtherapeutic or toxic antibiotic exposures due to patient-specific physiology and CRRT prescription parameters that influence drug pharmacokinetics. This case series describes the use of model-informed precision dosing (MIPD) to guide cefepime therapy in five critically ill paediatric patients receiving CRRT. Individual pharmacokinetic analyses were conducted using measured cefepime plasma concentrations and a previously published population PK model with a mechanis

Continuous renal replacement therapy for septic shock: practicalities for a pediatric intensivist.

UNLABELLED: High-volume hemofiltration (HVHF) has been proposed as an adjunctive extracorporeal strategy for children with sepsis or septic shock requiring continuous renal replacement therapy (CRRT), based on the biological premise that enhanced convective clearance may attenuate circulating inflammatory mediator peaks. The 2026 Surviving Sepsis Campaign pediatric guidelines issued a conditional recommendation favoring HVHF over standard-volume hemofiltration in this setting, despite low-certainty evidence. This narrative review critically examines the biological rationale, available pediatri

Continuous Renal Replacement Therapy for Cytokine Storm in Diabetic Kawasaki Shock Syndrome.

To report the successful use of continuous renal replacement therapy (CRRT) in a child with type 1 diabetes mellitus (T1DM) and intravenous immunoglobulin (IVIG)-refractory Kawasaki disease shock syndrome (KDSS). A 3-year, 10-month-old boy with poorly controlled T1DM and autoimmune thyroiditis presented with KDSS and diabetic ketoacidosis. Despite IVIG and high-dose aspirin, he developed refractory shock and metabolic acidosis. Continuous venovenous hemodiafiltration was initiated, leading to rapid hemodynamic stabilization, resolution of hyperinflammation, and metabolic normalization. CRRT ma

An economic model for different anticoagulation strategies in continuous renal replacement therapy.

PURPOSE: To evaluate the cost-effectiveness of regional citrate anticoagulation (RCA) or systemic heparin anticoagulation (SHA) during continuous renal replacement therapy (CRRT) in critically ill patients. METHODS: A decision-analytic model was developed to calculate an incremental cost-effectiveness ratio between RCA and SHA from a US healthcare payer perspective. Key differentiator was the risk of bleeding derived from the RCA and SHA groups in the RICH trial. Base case didn't consider the potential impact of bleeding on length of stay (LOS) or mortality. Three scenarios were considered: 1

Posterior ischemic optic neuropathy following continuous renal replacement therapy: a case report.

BACKGROUND: Posterior ischemic optic neuropathy (PION) is a rare cause of acute vision loss in intensive care unit patients. PION following continuous renal replacement therapy (CRRT) hemodialysis has not ever been reported. Here, we report a case of bilateral nonarteritic PION following the initiation of CRRT. CASE PRESENTATION: A 52-year-old man with hypertension and stage 4 chronic kidney disease was admitted due to metabolic acidosis, hyperkalemia, and acute exacerbation of CKD. CRRT caused transient hypotension upon initiation but corrected the metabolic acidosis and hyperkalemia six hou

Efficacy of Regional Citrate versus Heparin Anticoagulation in Continuous Renal Replacement Therapy.

BACKGROUND: Continuous renal replacement therapy (CRRT) is used in hemodynamically unstable patients with acute kidney injury (AKI). Heparin, the most commonly used anticoagulant, has a significant bleeding risk and is associated with heparin-induced thrombocytopenia. Regional citrate anticoagulation is an alternative anticoagulation strategy in CRRT. MATERIALS AND METHODS: A randomized controlled trial was conducted in the Intensive Care Unit over one year, from October 2020 to September 2021, in patients with AKI requiring CRRT. Fifty-two patients were randomized into two groups: group 1 re

Cost-effectiveness of the TherMax blood warmer during continuous renal replacement therapy.

Hypothermia is a common adverse event during continuous renal replacement therapy (CRRT), affecting multiple organ systems and increasing risk of poor health outcomes among patients with acute kidney injury (AKI) undergoing CRRT. TheraMax blood warmers are the next generation of extracorporeal blood warmers which reduce risk of hypothermia during CRRT. The purpose of this study is to elucidate the potential health economic impacts of avoiding CRRT-induced hypothermia by using the novel TherMax blood warming device. This study compares health care costs associated with use of the new TherMax bl

Severe Hypophosphatemia-Induced Acute Toxic-Metabolic Encephalopathy in Continuous Renal Replacement Therapy.

Acute toxic-metabolic encephalopathy (TME) is an acute condition of global cerebral dysfunction in the absence of primary structural brain disease. Severe hypophosphatemia leads to muscle weakness and involves the diaphragm but hypophosphatemia-induced TME is very rare. Herein, we report the case of a 43-year-old woman with encephalopathy with severe hypophosphatemia during continuous renal replacement therapy. She presented with features of oliguric acute kidney injury on diabetic kidney disease due to volume depletion. At admission, her mental status was alert but gradually changed to stupor

Continuous renal replacement therapy during extracorporeal membrane oxygenation: why, when and how?

PURPOSE OF REVIEW: The use of extracorporeal membrane oxygenation (ECMO) is increasing rapidly. Patients on ECMO have a high risk of developing acute kidney injury (AKI) and needing renal replacement therapy (RRT). The aim of this review is to describe different strategies of combining RRT and ECMO and to outline their advantages and drawbacks. RECENT FINDINGS: Fluid overload is the most common indication for RRT during ECMO, and continuous renal replacement therapy (CRRT) is the most commonly used modality. The optimal timing for initiation of CRRT should be individualized based on degree of

Therapeutic Drug Monitoring in Neonatal HSV Infection on Continuous Renal Replacement Therapy.

Optimal acyclovir dosing under continuous renal replacement therapy (CRRT) in neonates is unknown. We monitored serum acyclovir levels and herpes simplex virus 1 (HSV-1) DNA levels in a neonate with disseminated HSV-1 infection and renal failure undergoing CRRT. A full-term, 5-day-old female presented with a 2-day history of lethargy and fever. She developed fulminant hepatitis and was diagnosed with HSV-1 infection by real-time polymerase chain reaction. Acyclovir was initiated at 60 mg/kg/day, which was lowered to 20 mg/kg/day because of development of renal failure. She was placed on contin

Pediatric patients with multi-organ dysfunction syndrome receiving continuous renal replacement therapy.

BACKGROUND: Critical illness leading to multi-organ dysfunction syndrome (MODS) and associated acute renal failure (ARF) is less common in children compared to adult patients. As a result, many issues plague the pediatric ARF outcome literature, including a relative lack of prospective study, a lack of modality stratification in subject populations and inconsistent controls for patient illness severity in outcome analysis. METHODS: We now report data from the first multicenter study to assess the outcome of pediatric patients with MODS receiving continuous renal replacement therapy (CRRT). On
