Contraception and the dermatologist.

Contraceptives are pertinent to dermatologists in 3 major instances: (1) prescribing combined oral contraceptives for the treatment of acne; (2) ensuring that women being treated with potential teratogens are on adequate contraception; and (3) counseling female patients regarding contraceptives that can worsen acne. Most modern combined oral contraceptives will benefit acne; however, there are some agents that may be more effective than others, primarily because of the progestin used in the agent. Long-acting reversible contraceptives should be first line for women on teratogenic medications,

Endometrial carcinoma in women under 40 years of age: comparison of cases in oral contraceptive users and non-users.

Women under 40 years of age rarely develop carcinoma of the endometrium, this age group comprising less than 3% of all cases in most large series. The Registry for Endometrial Carcinoma in Young Women Taking Oral Contraceptive Agents has recorded and studied 30 cases in women under 40 with a history of oral contraceptive administration, and these are compared with 25 cases in the same age group which were accessioned at the Armed Forces Institute of Pathology prior to the era during which these agents have been available, and with series reported in the literature totaling 137 cases with no co

[Internal medicine problems regarding contraception. Part I].

The increasing distaste for the "pill' is mainly due to uncritical comments disseminated by the mass media and to the rational and irrational anxieties of women. The doses of steroidal hormones in today's hormonal contraceptive agents are only a small fraction of the amounts used in the past. Consequently, the occurrence of adverse reactions is much smaller; thus, tendency to thromboembolism has decreased by more than 25% with ovulation inhibitors containing only 50 gamma of EE, and thrombotic incidents are very rare if the so-called "micropill" (30 gamma EE) is given. Hypertension is observed

Effect of oral contraceptives on rhythmic urinary metal excretion.

Thirteen women, six of whom were using oral contraceptive agents, between the ages of 23 and 34 years, free from illness, and without a previous clinical history of menstrual abnormality, were studied with chronobiological methods for circadian (about 24 hour) patterns of metal and creatinine excretinine excretion. Circadian rhythms were detected for the excretion of lithium, potassium, iron, sodium, copper, and creatinine. Statistically significant by lower 24 hour excretion (mesors) of lithium, potassium, and iron was detected for the subjects using oral contraceptive agents. Although all wo

Effect of long-term oral contraceptive therapy before pregnancy on maternal and fetal zinc and copper status.

Thirteen pregnant women who conceived within six months after terminating oral contraceptive agent therapy of at least three years' duration participated in this study. Nine pregnant women with no history of oral contraceptive agent use served as controls. At delivery, erythrocyte counts were reduced, and during the first trimester and at delivery mean corpuscular hemoglobin and volume values were increased and erythrocyte zinc values were reduced in the oral contraceptive agent group as compared with controls. These changes appeared to be subclinical in magnitude and oral contraceptive agents

Comorbid bipolar disorder and premenstrual dysphoric disorder: real patients, unanswered questions.

Large-scale clinical and epidemiological studies suggest a link between bipolar disorder (BD) and premenstrual dysphoric disorder (PMDD). However, smaller studies using prospective charting failed to find this association. Here, we report three cases of individuals with BD and comorbid PMDD who responded successfully to adjunctive contraceptive agents in the management of their severe premenstrual symptoms. While controlled trials investigating pharmacological and non-pharmacological treatments are awaited, adjunctive treatment of contraceptive agents and mood stabilizers may be an option in t

Emergency contraception in the emergency department.

BACKGROUND: On June 24, 2022, the Supreme Court overturned Roe v. Wade, which will limit legal abortion in many areas of the U.S., making the need for effective emergency contraception even more critical. METHODS: This narrative review focuses on the approach to providing safe and effective emergency contraception in the emergency department (ED) with a focus on agents that are used in the U. S.; however, many of the agents discussed are also available and utilized in other countries. RESULTS: Emergency contraception methods included in this review are, ulipristal, levonorgestrel, combined o

Fertility regulatory methods and development of complications after evacuation of complete hydatidiform mole.

In a prospective, simple randomised study, we evaluated the relative efficacy of hormonal (oral contraceptive pill) and non-hormonal (intrauterine contraceptive device) methods of contraception as fertility regulatory agents in patients with complete hydatidiform moles and assessed the development of complications and sequelae if any, following their use. Fifty-six patients each on oral contraceptive pills (OCP) and intrauterine contraceptive device (IUCD) were recruited into the study. Ten patients, six on OCP and four on IUCD discontinued for varying reasons. Five patients (two on OCP, and t

[Current issues in contraception].

INTRODUCTION: The authors analyze contemporary methods of contraception. Regarding oral contraception, they point to agents which decrease the efficacy of oral contraception. They also deal with agents which increase the level of estrogen, thus increasing side effects (paracetamol, vitamin C). ORAL CONTRACEPTIVES: Oral contraceptives may also have an impact on the efficacy of some medications (anticonvulsants, antidepresants). Health risks of oral contraceptives are also mentioned, as well as WHO's, guidelines for women using contraceptives based on risks and benefits. OTHER METHODS OF CONTRA

Atherosclerotic risk factors in cardiovascular disease.

Atherosclerotic risk factors in cardiovascular disease include several that may be modified by changes in life-style or with drugs. Oral contraceptive agents may affect lipoproteins and glucose tolerance in particular. This paper reviews the relationship between lipoproteins, abnormal glucose tolerance and atherosclerosis and the implications that these relationships have for all oral contraceptive users.

Evaluation of interaction potential of certain concurrently administered drugs with pharmacological and pharmacokinetic profile of centchroman in rats.

Centchroman (Ormeloxifene) is a nonsteroidal, selective estrogen receptor modulator, oral contraceptive and anticancer agent, and is intended for long-term use by women. In view of its vast clinical application and the interaction of steroidal oral contraceptives with certain commonly used therapeutic agents, evaluation of interaction of certain concomitantly administered therapeutic agents (ibuprofen, rifampicin, diazepam, salbutamol, nifedipine, paracetamol, haloperidol, and tetracycline), in terms of both the postcoital contraceptive efficacy and pharmacokinetic profile, with centchroman wa

First-time use of newer oral contraceptives and the risk of venous thromboembolism.

Recent epidemiologic studies reported that the risk of venous thromboembolism (VTE) was higher with the use of the newer third generation oral contraceptives than with second generation agents. Although the overall findings of these studies are similar, the results, as they relate to patterns and duration of oral contraceptive use particularly among first-time users, are inconsistent. We reanalyzed data from the Transnational case-control study to assess the risk of VTE associated with first-time use of oral contraceptives as a function of its duration of use. Over the period 1993 to 1995, 471

Contraception and Diabetes Mellitus.

The aim of this narrative review was to discuss data on contraception in diabetes mellitus (DM). Women with DM rarely discuss contraception with their physicians, and healthcare providers offer advice to a very limited number of them. Overall, 1 in 8 women with DM using contraception methods was found to use an ineffective one. A further issue relates to drug-drug interactions between anti-diabetic medications and oral contraceptives. Generally, anti-diabetic agents do not alter the pharmacologic profile of hormonal contraception. However, preliminary results indicate that some novel anti-diab

Reversible ischemic colitis in young women. Association with oral contraceptive use.

Ischemic colitis, a condition of middle-aged to elderly patients, occurs uncommonly in younger persons. In this study, we describe the clinical and pathological features of 18 young adults (mean age, 29 years; age range, 17-39 years) with spontaneous ischemic colitis, 17 of whom were women. All presented with abrupt onset of severe, cramping abdominal pain followed by hematochezia. Colonoscopic visualization of the mucosa showed segmental hyperemia, friability, and erosion affecting the distal transverse colon (three cases), splenic flexure (three cases), descending colon (five cases), and sig

Contraception in the puerperium.

Choice of contraception in the puerperium is complicated by the normal factors of lactation, involuting uterus and emotional lability. The individual's previous contraceptive experience is important, and new agents and techniques change current practices. In the puerperium, failure for IUDs is increased, and tubal ligation has more hazards. During lactation the standard combined oral contraceptives are not advised, but the microdose progestagen-only pill may be a most satisfactory agent-changing to more routine methods when menstruation starts. In a population of normal women, progestagen-only

[Antiretroviral HIV therapy and hormonal contraceptives].

HIV guidelines recommend assessment of conception issues for all people living with HIV. Studies have shown negligible risk of HIV transmission from well-treated patients with HIV, and therefore condoms are no longer recom-mended to reduce HIV transmission. Some antiretroviral agents are metabolised through the same enzyme systems in the liver as hormonal contraceptives, which can affect the plasma concentration of both drug classes and the effect of the drugs, including reduced contraceptive efficacy. This review discusses the interactions between antiretroviral agents and hormonal contracept

Male hormonal contraception: looking back and moving forward.

Despite numerous contraceptive options available to women, approximately half of all pregnancies in the United States and worldwide are unplanned. Women and men support the development of reversible male contraception strategies, but none have been brought to market. Herein we review the physiologic basis for male hormonal contraception, the history of male hormonal contraception development, currents agents in development as well as the potential risks and benefits of male hormonal contraception for men.

Emerging concepts in male contraception: a narrative review of novel, hormonal and non-hormonal options.

Access to reliable contraception is a pillar of modern society. The burden of unintended pregnancy has fallen disproportionately on the mother throughout human history; however, recent legal developments surrounding abortion have sparked a renewed interest in male factor contraceptives beyond surgical sterilization and condoms. Modern efforts to develop reversible male birth control date back nearly a century and initially focused on altering the hypothalamic-pituitary-testes axis. These hormonal contraceptives faced multiple barriers, including systemic side effects, challenging dosing regime
