Accommodation and visual discomfort: a laboratory, optometric and subjective analysis.

Near-vision activities can induce visual discomfort, accompanied by symptoms that suggest accommodative dysfunction. However, results reported in the current literature are inconsistent regarding the link between visual discomfort and accommodation. This study aimed to analyse the accommodative response before and after various visual tasks designed to induce visual discomfort. Twelve volunteers (26 ± 3years) participated. Five visual tasks, varying in duration, viewing distance, and the congruence between accommodation and vergence, were performed in random order. Objective static and dynamic

Accommodative esotropia associated with low-dose atropine for myopia control: a case report with longitudinal AC/A ratio.

Low-dose atropine (LDA) is effective for slowing myopia progression and preventing its onset in children. It is generally considered safe. The main adverse effects are mild photophobia and blurred vision at near secondary to accommodative dysfunction. Esotropia is a rare adverse event that can occur in temporal association with LDA. We report a pediatric case of near-esotropia that emerged after the initiation of 0.025% low-dose atropine for myopia control and onset prevention and resolved after discontinuation; the longitudinal accommodative convergence-to-accommodation (AC/A) ratio assessmen

The influence of interactions between accommodation and convergence on the lag of accommodation.

Several models of myopia predict that growth of axial length is stimulated by blur. Accommodative lag has been suggested as an important source of blur in the development of myopia and this study has modeled how cross-link interactions between accommodation and convergence might interact with uncorrected distance heterophoria and refractive error to influence accommodative lag. Accommodative lag was simulated with two models of interactions between accommodation and convergence (one with and one without adaptable tonic elements). Simulations of both models indicate that both uncorrected hypero

Suppression of vestibular nystagmus by forced convergence in normal human subjects.

BACKGROUND: It has been suggested that the principal mechanism of nystagmus suppression in the nystagmus blockage syndrome is either adduction of the eye or convergence. We examined this issue using the nystagmus of the vestibulo-ocular reflex (VOR) as a model. METHODS: A motorized, computer-controlled rotary chair was used to produce VOR in darkness, using either sinusoidal or velocity step stimulation. Left eye position was monitored and horizontal slow-phase eye velocity was calculated. Subjects were cued to converge or perform other gaze tasks. RESULTS: Convergence suppressed nystagmus.

The AC/A ratio, age and presbyopia.

Previous reports concerning the effect of age on the AC/A ratio have been equivocal. Therefore, the present study investigated both the stimulus (AC/As) and response (AC/Ar) ratios using a subjective haploscope-optometer in a relatively large sample of subjects (n = 42) over a wide range of ages (22-65 years). The AC/As showed a small but significant decrease with age (approximately 0.04 delta/D/year). When the older subjects (> 45 years of age) were excluded, however, there was no systematic age effect. The AC/Ar exhibited a small but significant increase with age (approximately 0.08 delta/D/

The fluctuations of accommodation and ageing.

OBJECTIVE: The aim of this study was to investigate the influence of age on the fluctuations of accommodation. METHOD: Fluctuations of accommodation were measured on six 'young' (16-25 years, mean age 22 years) observers and six 'older' (35-48 years, mean age 42 years) observers. Accommodation was continuously recorded using a SRI optometer whilst the observers steadily viewed a high contrast target at stimulus levels 0.25 D, 1.00 D and 2.00 D. Fluctuations were quantified using power spectrum analysis, and frequencies were grouped into bands: low (LFC 0.3-0.6 Hz and 0.3-0.9 Hz) and high (HFC

Vergence adaptation and the order of clinical vergence range testing.

When measuring horizontal relative vergence ranges, the conventional clinical procedure is to assess the base-in (or divergence) range before the base-out (or convergence) determination. The rationale for this order of testing is that the convergence responses stimulated during the base-out measurements may produce vergence adaptation (i.e., a fusional aftereffect), which may temporarily bias the subsequent base-in values in the base-out direction. However, the effect of reversing this order of testing (i.e, base-out before base-in) has not been directly evaluated. Accordingly, the present stu

Heterophoria.

With the large number of factors that contribute to the "position of rest" or fusion-free posture, it might be expected that heterophoria would be normally distributed. Data from a large sample of symptom-free subjects (N = 925) show that this is not so. Furthermore, the difference in the prism adaptation response in orthophoria, esophoria, and exophoria suggests that the presence of heterophoria in normal subjects is due to the partial saturation of prism adaptation.

Eye-lens accommodation load and static trapezius muscle activity.

The purpose of this experimental study was to investigate if sustained periods of oculomotor load impacts on neck/scapular area muscle activity. The static trapezius muscle activity was assessed from bipolar surface electromyography, normalized to a submaximal contraction. Twenty-eight subjects with a mean age of 29 (range 19-42, SD 8) viewed a high-contrast fixation target for two 5-min periods through: (1) -3.5 dioptre (D) lenses; and (2) 0 D lenses. The target was placed 5 D away from the individual's near point of accommodation. Each subject's ability to compensate for the added blur was e

Non-linearity of the response accommodative convergence to accommodation ratio.

PURPOSE: Previous studies have reported variation in stimulus accommodative convergence to accommodation (AC/A) ratio across differing accommodative stimuli. Response AC/A ratio was assessed across 4 accommodative demands to determine if these differences could be due to accommodative inaccuracies to stimuli. METHODS: Twenty-three student participants aged 18 to 26 years (mean age 20.3 ± 1.7 years) successfully completed all testing conditions. The modified Thorington technique was used at 4 m to measure heterophoria. The Shin Nippon SRW 5000 infrared autorefractor was used to determine accom

Adaptive disorders of accommodation and vergence in binocular dysfunction.

Disturbances of binocular vision are described clinically by the Duane-White classification in terms of the magnitude of the accommodative-convergence ratio (AC/A). Convergence excess and convergence insufficiency are assumed to result from high and low AC/A ratios respectively. It is assumed that the abnormal AC/A ratio is an independent variable that underlies abnormal phorias. However, recent studies have demonstrated that the AC/A ratio is inversely related to the adaptability of tonic accommodation (lens adaptation) and directly related to adaptability of tonic vergence (prism adaptation)

Virtual reality-based vision therapy versus OBVAT in the treatment of convergence insufficiency, accommodative dysfunction: a pilot randomized controlled trial.

BACKGROUND: Virtual reality is being increasingly applied in vision therapy. However, the differences in effectiveness, optimal treatment cycle, and prognosis between virtual reality-based vision therapy and traditional therapies remain unknown. The purpose of this study was to compare the effectiveness of virtual reality-based vision therapy and office-based vergence/accommodative therapy in young adults with convergence insufficiency or accommodative dysfunction. METHODS: The patients were randomly assigned to either the virtual reality-based vision therapy group or the office-based vergenc

Human eyes do not need monochromatic aberrations for dynamic accommodation.

PURPOSE: To determine if human accommodation uses the eye's own monochromatic aberrations to track dynamic accommodative stimuli. METHODS: Wavefront aberrations were measured while subjects monocularly viewed a monochromatic Maltese cross moving sinusoidally around 2D of accommodative demand with 1D amplitude at 0.2 Hz. The amplitude and phase (delay) of the accommodation response were compared to the actual vergence of the stimulus to obtain gain and temporal phase, calculated from wavefront aberrations recorded over time during experimental trials. The tested conditions were as follows: Cor

Effect of heterophoria type and myopia on accommodative and vergence responses during sustained near activity in children.

The influence of phoria-type and myopia on changes to vergence and accommodation during prolonged near-task was examined in 53 children. Participants were classified into phoria and refractive categories based on near phoria and cycloplegic refraction respectively. Measures of near phoria, binocular (BA) and monocular accommodation (MA) were obtained before and during a 20 min task when children binocularly fixated a high-contrast target at 33 cm through best corrective lenses. Vergence adaptation and accommodative adaptation were quantified using changes to near phoria and tonic accommodation

An investigation into the mechanisms causing antipodean strabismus.

BACKGROUND: Antipodean strabismus is a rare clinical entity in which a patient manifests an esotropia when fixating with one eye and an exotropia when fixating with the other eye. It has been described in the settings of marked uncorrected anisometropia, dissociated strabismus or combinations of paresis, and mechanical restriction of eye movement. METHODS: A retrospective review of four patients with antipodean strabismus. RESULTS: All patients demonstrated a unique and uncommon pattern in which there was esotropia with one eye fixating and exotropia with the other eye fixating. None of the

Roles of the lateral suprasylvian cortex in convergence eye movement in cats.

Ocular convergence and lens accomodation were evoked by microstimulation in the lateral suprasylvian area (LS cortex) in the parieto-occipital cortex in the cat. Electrolytic lesions in LS cortex reduced the amplitude and velocity of ocular convergence. Neurons in LS cortex discharged in relation to ocular convergence and/or lens accommodation. These results support the hypothesis that the LS cortex plays an important role in controlling ocular convergence The LS cortex receives visual inputs from cortical visual areas 17, 18 and 19, and in addition from the superior colliculus through the LP

Ophthalmologic features of Parkinson's disease.

Patients with Parkinson's disease (PD) commonly complain of impaired visual function and difficulty reading, despite normal visual acuity. Although previous studies have evaluated contrast sensitivity, color vision, visuospatial processing, visual hallucinations, and ocular movements, none has systematically evaluated the ocular complaints and ocular findings of PD patients. Thirty patients with early untreated PD and 31 control subjects without neurologic or known ocular diseases were ophthalmologically evaluated for the frequency of visual complaints, dry eyes, blepharitis, visual hallucinat

A new oculomotor model demystifies "Remarkable Saccades".

Hering's Law of binocular eye movement control guides most oculomotor research and supports diagnosis and treatment of clinical eye misalignment (strabismus). It states that all eye movements are controlled by a unitary conjugate signal and a unitary vergence signal that sum. Recent evidence of temporally asynchronous eye rotations during vergence (Chandna et al., 2021) challenges the viability of a unitary vergence signal. Helmholtz proposed an explanation of binocular control that does not require a vergence system, that the eyes are controlled independently. Yet independent control fails to
