The impact of sugammadex dosing and administration practices on potential cost savings for pharmacy departments.

PURPOSE: Neuromuscular blocking agents (NMBAs) are commonly used during surgery, and restoring neuromuscular function at the end of surgery is vital in preventing complications of residual paralysis. Recent guidelines from the American Society of Anesthesiology recommend using sugammadex over neostigmine; however, sugammadex is significantly more expensive than neostigmine and may increase drug expenditure for pharmacy departments. This review summarizes evidence evaluating sugammadex dose adjustments and the potential of these dose adjustments to lead to cost savings for pharmacy departments.

Potential cost savings by prevention of adverse drug events with a novel medication review program.

OBJECTIVES: Preventable adverse drug events (ADEs) account for appreciable health care costs and patient morbidity and offer an attractive opportunity for health care providers to improve patient care and decrease costs. It has been suggested that pharmacist intervention can prevent admissions and readmissions due to ADEs. This study assessed the ADEs prevented through a novel medication review program, then estimated the potential cost savings of the prevented ADEs using the literature on cost and prevalence of ADEs that were treated. METHODS: An innovative pharmacist-run medication review w

Exploring Cost Savings with Specialty Biologic Drugs Administered to Adult Inpatients with Inflammatory Bowel Disease.

Background: Specialty infusion and self-injectable biologic drugs for the treatment of inflammatory bowel disease (IBD) are high-cost medications. When administered to hospital-admitted patients, these medications are not reimbursed on an individual basis but rolled into a per diem payment by most payers in the United States (US). Therefore, choosing to administer these medications in the inpatient setting may reveal negative financial implications for some health care institutions. Selecting an alternative site of care to administer these medications during the clinical management process may

Potential health gains in West and Central Africa through savings from lower cost HIV treatment.

OBJECTIVE: Prices of antiretroviral (ARV) drugs in lower income countries have decreased substantially over the past two decades, helping to facilitate greatly expanded access to antiretroviral therapy (ART). However, ART coverage in many parts of the world remains low. We investigate the extent of epidemiological benefits that might be expected if ARV drug prices decline further. DESIGN: A modeling study using data from seven countries in West and Central Africa (Cameroon, Democratic Republic of the Congo, Côte d'Ivoire, Niger, Nigeria, Senegal, and Togo). METHODS: We investigated how the t

Melanoma Extirpation with Immediate Reconstruction: The Oncologic Safety and Cost Savings of Single-Stage Treatment.

BACKGROUND: The timing of reconstruction following melanoma extirpation remains controversial, with some advocating definitive reconstruction only when the results of permanent pathologic evaluation are available. The authors evaluated oncologic safety and cost benefit of single-stage neoplasm extirpation with immediate reconstruction. METHODS: The authors reviewed all patients treated with biopsy-proven melanoma followed by immediate reconstruction during a 3-year period (January of 2011 to December of 2013). Patient demographic data, preoperative biopsies, operative details, and postoperati

Automated monitoring of hemodialysis adequacy by dialysis machines: potential benefits to patients and cost savings.

Hemodialysis adequacy can be quantified using ultraviolet absorbance of the spent dialysate, or by analysis of dialysate conductivity at the dialyzer inlet and outlet in response to changes in dialysate electrolyte concentration. These measurements can be made at every dialysis, including initial and acute treatments and can help detect access recirculation. No disposables or reagents are required. Cost may be reduced by reducing the need for blood sampling and laboratory analysis.

Cost savings from the provision of specific methods of contraception in a publicly funded program.

OBJECTIVES: We examined the cost-effectiveness of contraceptive methods dispensed in 2003 to 955,000 women in Family PACT (Planning, Access, Care and Treatment), California's publicly funded family planning program. METHODS: We estimated the number of pregnancies averted by each contraceptive method and compared the cost of providing each method with the savings from averted pregnancies. RESULTS: More than half of the 178,000 averted pregnancies were attributable to oral contraceptives, one fifth to injectable methods, and one tenth each to the patch and barrier methods. The implant and intr

An update on community water fluoridation, part 1: Mechanism of action, effectiveness, and cost savings.

BACKGROUND: Water fluoridation has been an effective means of caries prevention for more than 80 years. The authors reviewed evidence on the benefits of community water fluoridation (CWF) in preventing caries, its cost-effectiveness, and the impact of discontinuing CWF. TYPES OF STUDIES REVIEWED: The authors focused on observational studies identified in the dental literature from January 1, 2014, through December 31, 2025. The review was facilitated by means of an assessment of several national reports and independent panels' reports on the benefits of CWF in preventing caries. RESULTS: Res

Surgical tray leaning: carbon, efficiency and cost-savings in MAKO robotic-assisted total knee arthroplasty.

INTRODUCTION: Robotic-assisted total knee arthroplasty (RA-TKA) is associated with a higher carbon footprint compared with manual TKA. This review sought to reduce the carbon and financial costs associated with MAKO RA-TKA by 'leaning' surgical trays. METHODS: Surgeons routinely performing MAKO RA-TKA were consulted, and a consensus was reached on items from the standard knee instrument trays that were redundant and could be removed. Two new 'lean trays' were then introduced for MAKO RA-TKA. Carbon and financial savings were calculated based on the reduction in the number of trays requiring d

Impact of Positive Lifestyle Behaviors on Direct Health Care Cost Savings for Low Back Pain.

OBJECTIVE: This study aimed to investigate the relationship between a previously purpose-developed lifestyle behavior scale and health care cost savings related to low back pain (LBP). METHODS: This longitudinal study used data from the Australian Twin Back (AUTBACK) study. LBP and lifestyle behavior measures were collected at baseline. Physical activity (PA) was objectively quantified via an accelerometer. A lifestyle behavior scale was created using variables of body mass index, PA, smoking status, and sleep quality. Weekly health care use for LBP was collected over one year. Health care co

Cost and operating room time savings with single-position prone lateral lumbar interbody circumferential fusion.

OBJECTIVE: The objective of this study was to determine efficiencies associated with dual-position versus single-position lateral lumbar interbody fusion (LLIF). METHODS: Billing databases were queried for LLIF procedures performed by a single surgeon at a high-volume tertiary care center. Case-specific costs for each procedure were collected. Cases were matched by the number of levels treated. One-level and 2-level LLIFs with percutaneous pedicle screw (PPS) fixation (LLIF+PPS) using a single vendor system were included. Length of stay, operative time, and operative costs were compared for d

Real world evidence in effectiveness, safety, and cost savings of generic levothyroxine: a systematic review.

PURPOSE: Generic levothyroxine, approved through Abbreviated New Drug Application (ANDA), is available for generic substitution. But ANDA does not require nonclinical or clinical data to establish safety and efficacy. Post-marketing evidence in generic equivalence for marketed levothyroxine products is limited. We conducted a systematic review to synthesize evidence in effectiveness, safety, and cost savings between patients using generic and branded levothyroxine. METHODS: We systematically searched published literature from Medline, International Pharmaceutical Abstracts, APA PsycInfo, and

A managed alcohol program in Sydney, Australia: Acceptability, cost-savings and non-beverage alcohol use.

INTRODUCTION AND AIMS: Managed alcohol programs (MAPs) are a novel harm reduction intervention for people who experience long-term homelessness and severe long-term alcohol dependence. MAPs provide regulated amounts of alcohol onsite under supervision. Preliminary international evidence suggests that MAPs are associated with improvements such as reduced non-beverage alcohol consumption and decreases in some alcohol-related harms. There are currently no MAPs in Australia. We aimed to assess the feasibility of a MAP in inner-Sydney. DESIGN AND METHODS: A survey among eligible homeless alcohol-d

Suprapubic catheter insertion is an outpatient procedure: cost savings resultant on closing an audit loop.

OBJECTIVE: To explore, by an audit, the regional practice of inserting a suprapubic catheter (SPC), and to prospectively determine the proportion of patients that can be successfully managed on an outpatient basis in one department. METHODS: Both local and regional practice were determined by a retrospective analysis of the hospital database for all cases of SPC insertion between April 2005 and March 2006. In addition, a questionnaire was e-mailed to each of 11 urology departments. Locally, from August 2006 onwards, all patients scheduled for SPC insertion were referred to a new clinic, where

Saving money, costing women: the gendered price of hospital-at-home.

Hospital-at-Home (HaH) is increasingly promoted as a patient-centered and cost-effective alternative to conventional hospitalization. While its clinical and organizational benefits are well documented, less attention has been paid to the forms of work on which HaH relies and to their gendered implications. This article examines HaH as a situation where paid healthcare work is transformed into unpaid caregiving work, and analyzes how this process reinforces gender inequalities. The study draws on an ethnographic case study conducted over eighteen months in one of France's largest HaH institutio

Congenital toxoplasmosis in Austria: Prenatal screening for prevention is cost-saving.

BACKGROUND: Primary infection of Toxoplasma gondii during pregnancy can be transmitted to the unborn child and may have serious consequences, including retinochoroiditis, hydrocephaly, cerebral calcifications, encephalitis, splenomegaly, hearing loss, blindness, and death. Austria, a country with moderate seroprevalence, instituted mandatory prenatal screening for toxoplasma infection to minimize the effects of congenital transmission. This work compares the societal costs of congenital toxoplasmosis under the Austrian national prenatal screening program with the societal costs that would have
