[Principles of intensive care during the acute period of severe craniocerebral trauma in children].

Prospective analysis of the course of severe craniocerebral injury (CCI) in 37 children showed that the results of treatment depended on methods of intensive care. In children the outcome of CCI depends primarily on adequacy of maintaining effective cerebral bloodflow during the acute period of CCI irrespective of the terms and etiological factors of coma. Adequate cerebral perfusion pressure is attained with osmodiuretics, saluretics, and hemodynamic therapy. Hyperventilation is justified in cases with coma developing within 2 h after the injury; it should not lead to long (more than 1 h) dec

[A pharmacological analysis of the antiedematous action of GABA-ergic and opioidergic agents in craniocerebral trauma].

Experiments on rats with skull inquiry show that biculline (dose 2 mg/kg) removes antiedematous effect of fenibut (50 mg/kg), sodium hydroxybutirate (200 mg/kg), promedol (1 mg/kg), and synthetic analogs of encephalines DAGO and DSLET (100 mg/kg). Naloxon at a dose 1 mg/kg blocks antiedematous effect of ligands of opiate receptors, but does not change the effect of fenibut and sodium hydroxybutirate. The presence of close inter-regulatory interactions between GAMK-ergic and opioidergic systems in forming the traumatic correction is suggested.

[Cervical spinal injuries in craniocerebral trauma: their x-ray diagnosis on the day of the accident].

To investigate cervical spine injuries and to evaluate cervical radiography 100 patients were reviewed who were referred to computed tomography of the head because of blunt head trauma. 15 sustained cervical injury, 11 were located at the lower level of the cervical spine. Six fractures of the lower cervical spine were detected by additional radiographs. The diagnosis was established by computed tomography in three cases. To rule out cervical injuries in patients with incomplete visualisation of the lower cervical spine, cervical computed tomography should be performed in addition to lateral,

[Costs and proceeds from patients admitted via the emergency room with mild craniocerebral trauma].

The introduction of the diagnosis-related groups (DRG) in 2003 radically changed the billing of the treatment costs. From the very beginning, trauma surgeons questioned whether the introduction of the DRG could have a negative impact on the care of the severely injured. "Trauma centers in need" was the big catchword warning against shortfalls at trauma centers due to the billing via DRG. This situation was confirmed in the first publications after introduction of the DRG, showing a clearly deficient level of care of polytrauma cases. Over the years, adjustments have led to an improvement in th

[Noninvasive monitoring of cerebral oxygen saturation after severe craniocerebral trauma in a Jehovah's witness].

Patients with severe head injury run a high risk of developing secondary cerebral defects. Various methods have so far been described which vary in their continuity, invasiveness and technical aspects for the early detection and treatment of complications. Within this group of patients Jehovah's witnesses pose a particular problem due to their restriction concerning therapeutical possibilities. That may mean an additional danger to the cerebral O2-delivery. This case report highlights on the value of continuous, non-invasive cerebral O2-saturation measurement and its combination with the fiber

[Concomitant vertebral trauma in patients with craniocerebral injuries. 34 consecutive patients over 3 years].

Between February 1986 and January 1989, a total of 430 inpatients with the diagnosis of a head injury were prospectively registered and analysed for the association of a spinal trauma. Of these patients, 92% (n = 396) suffered a primary coma and 8% (n = 34) a secondary coma. On admission the ratio of coma, somnolence, and mild clouding was 6:1:3. In 8% (n = 34) the head injury was combined with a lesion of the spinal column. The cervical spine was affected in two-thirds of the cases (21/34). The clinical outcome was subdivided into three categories: dead (n = 10), paraplegic (n = 12), and 12 o

A study of magnetic resonance diffusion tensor imaging in the prognostic assessment of severe craniocerebral trauma.

BACKGROUND: Magnetic resonance diffusion tensor imaging (DTI) is recognized for its diagnostic capabilities in severe craniocerebral trauma but is less explored for its prognostic utility. This study assesses the prognostic value of DTI in predicting outcomes for patients with severe craniocerebral trauma. METHODS: We conducted a retrospective analysis of 125 patients who sustained severe craniocerebral injuries between March 2021 and September 2022. Patients were evaluated 90 days post-injury using the Glasgow Outcome Scale (GOS) and categorized into good (GOS 4-5, n = 74) and poor (GOS 1-3,

Correlation of preoperative inflammatory factors and emotional disorders with postoperative delirium in patients with craniocerebral trauma.

BACKGROUND: Traumatic brain injury (TBI) imposes a substantial societal and familial burden due to its high disability and fatality rates, rendering it a serious public health problem. Some patients with TBI have poor treatment outcomes and are prone to postoperative delirium (POD), which affects their quality of life. Anxiety has been linked to increased POD incidence in some studies, while others have found no correlation. AIM: To investigate the correlation of POD risk factors, preoperative inflammatory factors, and mood disorders in patients with TBI. METHODS: We retrospectively collecte

[Certain issues concerning medical certification and driving fitness assessment after craniocerebral traumas].

BACKGROUND: The incidence of craniocerebral traumas increases with a growing number of road accidents. It should be emphasized that it is rather difficult to determine precisely the craniocerebral traumas incidence rate, however, it is well known that road accidents are the major cause of these injuries. MATERIAL AND METHODS: The study embraced 43 men (mean age: 47.1; SD: 11.4) after craniocerebral traumas. Of the 43 men, 23 drivers had driving license of the following classes: A, Al, A2, AM, B, B1 and BE without professional driving permit, and 20 men worked as professional drivers licensed

Pediatric crushing head injury: biomechanics and clinical features of an uncommon type of craniocerebral trauma.

BACKGROUND: Head injuries constitute one of the leading causes of pediatric morbidity and mortality. Most injuries result from accidents involving an acceleration/deceleration mechanism. However, a special type of head injury occurs when the children sustain a traumatism whose main component is a static load in relation to a crushing mechanism with the head relatively immobile. PATIENTS AND METHODS: We report a series of children who sustained a craniocerebral injury of variable severity produced by head crushing. We also analyze epidemiological and clinical data, and biomechanics in these in

[Use of instenon in complex treatment of patients with optic nerve atrophy caused by craniocerebral trauma].

Instenon, a combined nootropic agent, was used on the step-type basis within a complex treatment of optic nerve atrophy caused by craniocerebral trauma. Our study revealed that instenon, when used within a complex treatment of a partial optic nerve atrophy caused by craniocerebral trauma, improved the visual acuity in 85.7% of patients and it expanded the total visual field in 76.2% of patients, whereas the routine therapy ensured the related improvements in 58.9% and 61.5% of patients, respectively. Besides, the drug provided for a better retinal electric sensitivity and for a better optic ne

[Trauma to the cranial nerve roots in the pathogenesis of brain stem lesions in severe craniocerebral injury].

A clinico-morphological analysis of results of 65 autopsies has shown that traumatization of cranial nerve roots at the moment of trauma is one of pathogenetical mechanisms of truncal lesions. A group of primary progressing truncal lesions was established which included 81.1% of patients who died at the hospital.

Unusual Projectile in Craniocerebral Trauma: A Medico-Legal Challenge in the Context of Riot-Control Interventions.

We report the case of a 15-year-old male who died instantly after being struck in the right occipitoparietal region by an unconventional projectile-a "crystal ball"-during a public protest in Medellín, Colombia. The projectile was fired from a 37 or 40 mm launcher modified with non-regulation ammunition. Autopsy findings, performed under the Minnesota Protocol, included cranial fracture, brain laceration, and subarachnoid hemorrhage. Although designed to be nonlethal, the improvised use of this kinetic ammunition converted it into a lethal weapon. This case highlights critical gaps in forensic

[Forensic medical assessment of acute blood loss in combination with craniocerebral trauma and alcohol intoxication].

Forensic medical findings have been analysed retrospectively for 242 deceased of acute blood loss: 186 (76.8%) corpses were male and 56 (23.2%) corpses were female. It was found that in acute blood loss (ABL) duration of the terminal period (TP) depended on its combination with craniocerebral trauma (CCT) or alcohol intoxication (AI). A short TP (several minutes) was observed in ABL patients with CCT (39.6%), a long-term TP--in ABL associated with AI (20-60 min, 40.4% cases; several hours, 56.3%; over 24 hours, 54.5%). Slow TP occurred primarily in mild AI (TP for several hours 55.6%, over 24
