Ossification of laryngeal cartilages on lateral cephalometric radiographs.

This study was undertaken to identify the variations in the physiologic ossification of human laryngeal cartilages by evaluating the lateral cephalometric radiographs of healthy males and females referred to our facility. Lateral cephalometric radiographs of 359 patients (141 male and 218 female; ages 10 to 59 years) referred from various specialty clinics between April 1998 and March 2000 were used. Frequencies and confidence intervals were obtained and tabulated for both thyroid and cricoid ossifications as seen on these cephalometric radiographs. The interobserver agreement was strong for b

Surgical management for isolated cricoid fracture causing arytenoid immobility.

Cricoid cartilage fractures usually occur concurrently with disorders of laryngeal function. In, particular, displaced cricoid lamina fractures can affect arytenoid movement. However, functional, recovery may require proper repositioning of the cricoid lamina, which is associated with a high rate of, complications. Here we present a case in which an isolated cricoid cartilage fracture with arytenoid, immobility due to displacement of the fracture in the cricoarytenoid joint space was successfully, treated. Our findings suggest that a combination of external approaches with temporary, cricothyr

A case of tracheal injury with cricoid fracture due to blunt laryngeal trauma.

BACKGROUND: Laryngeal injuries, especially cricoid cartilage fractures, are rare but may present with airway emergencies, and appropriate airway management is vital. CASE PRESENTATION: A 24-year-old man admitted to the emergency room with isolated blunt cervical trauma after a motorcycle accident. The patient presented dysphonia, stridor, hemoptysis, cervical subcutaneous emphysema, and increasing respiratory distress. Oral intubation was initially successful, however, worsening subcutaneous emphysema and loss of effective ventilation suggested airway disruption. Emergency cricothyrotomy was

[Preliminary report: observation by video laryngoscope of glottal narrowing due to cricoid pressure].

BACKGROUND: When the applied cricoid pressure is too strong, or the place or direction of the pressure application is not appropriate, glottal closure may occur, but its details are unclear. METHODS: We evaluated possible changes in the size of the rima glottides due to backward pressure on the cricoid cartilage or backward pressure or backward, upward, and rightward pressure (BURP) on the thyroid cartilage using a video laryngoscope (Fine View' Laryngoscope, Tray Medical, Tokyo) in 6 adult males and 6 females with Cormack and Lehane grade 1. RESULTS: The right-to-left distance of the rima g

Injury- and age-linked differences in wound healing and stenosis formation in the subglottis.

We studied the effects of superficial and deep endolaryngeal trauma of the subglottic airway in young and adult rabbits. In both age groups a soft stenosis was formed as long as the cartilaginous cricoid ring is not involved. This stenosis comprised a thickened subepithelial zone of scar tissue, separated from the cricoid cartilage by a layer of fatty tissue. Injury of the internal side of the cricoid cartilage induced a compact mass of scar tissue with local differentiation into fibrocartilage. In young animals only, injury of the cartilage led to remodelling of the cricoid ring (indentation

The anatomy of the pediatric airway: Has our knowledge changed in 120 years? A review of historic and recent investigations of the anatomy of the pediatric larynx.

BACKGROUND: There is disagreement regarding the anatomy of the pediatric airway, particularly regarding the shape of the cricoid cartilage and the location of the narrowest portion of the larynx. AIMS: The aim of this review is to clarify the origin and the science behind these differing views. METHODS: We undertook a review of published literature, University Libraries, and authoritative textbooks with key search words and phrases. RESULTS: In vivo observations suggest that the narrowest portion of the airway is more proximal than the cricoid cartilage. However, in vitro studies of autopsy

Difficulty Inserting Cuffed Endotracheal Tubes in a Child: A Case Report.

We experienced difficulty inserting cuffed inner diameter (ID) 4.5- and 5.0-mm endotracheal tubes (ETTs) in a 5-year-old boy. Postoperative ultrasound investigations showed that the internal transverse width of the cricoid cartilage was 8.0 mm. The maximum outer diameter (OD) of the deflated cuff portion of the cuffed ID 4.5- and 5.0-mm ETTs was 8.5 and 9.6 mm, respectively. The OD of an uncuffed ID 5.5-mm ETT was 7.6 mm; this tube passed the cricoid cartilage. Hence, the transverse width of the cricoid cartilage and ETT diameter including cuff folds should be considered when selecting cuffed

Airway growth following cricotracheal resection in puppies.

A canine animal model was developed to study the effects of cricoid resection on cricoid cartilage growth and subglottic lumen size. Twenty-four healthy, postweanling mongrel puppies, aged 5 to 7 weeks, were randomly divided into two groups. The first group, consisting of 13 puppies, underwent cricotracheal resection with primary anastomosis. A second group, consisting of 11 puppies, served as controls and did not have surgery. Animals from both groups were killed at age 13 weeks. Most of the animals, however, were allowed to mature to adulthood and were killed between age 26 to 28 weeks. Comp

Biomechanical characterisation of equine laryngeal cartilage.

REASONS FOR PERFORMING THE STUDY: Upper airway obstruction is a common problem in the performance horse as the soft tissues of the larynx collapse into the airway, yet there is a paucity of information on biomechanical properties for the structural cartilage components. OBJECTIVE: To measure the geometry and compressive mechanical properties of the hyaline cartilage to improve understanding of laryngeal function and morphology. METHODS: A total of 11 larynges were harvested from Thoroughbred and Standardbred racehorses. During gross dissection, linear dimensions of the cricoid were obtained.

[The reconstruction of laryngeal function in subtotal laryngectomy by pedicled flaps].

OBJECTIVE: To explore the possibility of subtotal laryngectomy in the treatment of advanced laryngeal cancer and selection of reconstruction. METHODS: Forty patients were treated surgically by subtotal laryngectomy with preservation of arytenoid cartilage and perichondrium. The pedicled flaps between cricoid cartilage and arytenoid cartilage were sewed up. The new larynx was reconstructed by suturing the cricoid or trachea to the hyoid bone or the tongue base. RESULTS: The 3 and 5 year survival rates were 85.0% and 76.2% respectively. Decannulation rate was 92.5%. CONCLUSION: The reconstruc

A histologic study of the growth of the subglottis after endolaryngeal trauma.

In young rabbits the histologic features of the growth of the subglottis were studied after an endolaryngeal trauma. This lesion of the larynx leading to a subglottic stenosis, caused specific pathologic changes of the cricoid cartilage and the subepithelial tissues. Activation of chondrocytes resulted in proliferation and regeneration of destructed cartilage. The normal subepithelial structures were replaced by granulation tissue differentiating into a thick fibrous scar and into cartilage. This ectopic cartilage together with the repaired cricoid formed a composite thickened 'ring'-like cart

Value of laryngeal cartilage sclerosis as a predictor of outcome in patients with stage T3 glottic cancer treated with radiation therapy.

PURPOSE: To determine whether sclerosis of the laryngeal cartilages was a predictor of a poor outcome in patients with stage T3 glottic cancer treated with radiation therapy. MATERIALS AND METHODS: Thirty-three patients with stage T3 glottic cancer underwent computed tomography (CT) before radiation therapy. Twenty-two patients underwent posttreatment CT. The presence of cartilage sclerosis, cartilage erosion, marrow invasion, and cartilage necrosis was determined. RESULTS: Nineteen of the 33 patients had cartilage sclerosis at CT. Seventeen patients had sclerosis of a single laryngeal carti

Complete Bilateral Ossification of the Arytenoid Cartilages With Minimal Involvement of the Laryngeal Skeleton: A Unique Anatomical Variation.

The arytenoid cartilages are distinguished by their mobility and unique mixed composition of hyaline and elastic cartilage. Though laryngeal cartilage ossification is not rare, and its pattern is well-studied, sole complete and bilateral arytenoid cartilage ossification is an aberration. An 84-year-old woman, with no history of voice or breathing issues, was brought to the emergency department for a head-and-neck trauma following a fall. CT scans revealed a subdural hematoma and, incidentally, a bilateral ossification of the arytenoid cartilages, sparing the vocal processes but involving compl

The Cricothyroid Joint: A Practical Guide for Distinguishing Between Different Joint Types.

OBJECTIVE: Type A cricothyroid joint (CTJ) leads to a higher elongation of the vocal folds than Type B/C CTJ. Therefore, the determination for the CTJ type is important whether to perform a cricoid-thyroid approximation for a pitch elevation in transwomen with gender dysphoria. This study aimed to develop a tool for clinicians and radiologists for distinguishing between Type A (cricoid cartilage protuberance) and Type B/C (flat surface with/without cartilage of the cricoid) CTJs on high-resolution computed tomography (HRCT). STUDY DESIGN: This was a prospective study. METHODS: Analysis of 60

Strategy for surgical treatment of congenital subglottic stenosis in children.

BACKGROUND/PURPOSE: Congenital subglottic stenosis is a rare anomaly caused by thickened cricoid cartilage. We report our surgical techniques, comprising anterior cricoid split (ACS), laryngotracheoplasty (LTP), KTP laser ablation, and application of a tracheal opening retainer (TOR) into the tracheostomy site. METHODS: Nine patients have been treated since 1988. Four patients (median age 85 days; range 5 days to 6 months) underwent ACS. Another four patients (median age, 17 months; range, 5-57 months) underwent LTP using costal cartilage grafts, although two had undergone tracheostomy before

Simulation of Pediatric Endoscopic Cricoid Reduction and Expansion.

Endoscopic cricoid expansion and reduction are newer approaches to the management of pediatric bilateral vocal fold immobility and postlaryngotracheal reconstruction glottic insufficiency, respectively. These procedures offer a less invasive, endoscopic alternative to procedures that typically required open management with a more prolonged recovery. These technically demanding procedures are currently performed only in select centers, and there is no currently described training model for practicing them. We present a modification to a laryngeal dissection station that allows for simulation of

Pediatric laryngeal trauma: a case series at a tertiary children's hospital.

BACKGROUND: Pediatric blunt or sharp laryngotracheal injuries are infrequent because of the softer cartilages and the protection of the prominent mandible. These injuries usually occur secondary to striking furniture or via the "clothesline" injury. METHODS: We present five cases of pediatric laryngotracheal injury (thyroid cartilage, true vocal cords, cricoid cartilage, cricotracheal junction, and posterior tracheal wall). RESULTS: We examined the need for intubation, need for tracheostomy, length of intubation, length of hospital stay, interval until direct laryngoscopy, use of steroids, p
