Critical care challenges after vascular surgery.

PURPOSE OF REVIEW: Vascular surgical patients represent one of the most complex populations in the intensive care unit, due to a combination of extensive comorbidities and high perioperative risk. Postoperative complications such as major adverse cardiovascular events, acute kidney injury, neurological complications, thromboembolism and coagulopathy are common and often intersect. We present the latest evidence on complications most encountered in critically ill patients exposed to vascular surgery and best practices for their management. RECENT FINDINGS: Although short-term mortality rates f

What's new in pediatric critical care?

Pediatric intensive care medicine is a rapidly evolving field of medicine, with recent publication of landmark papers specific for the pediatric population. Progress has been made in modes of mechanical ventilation, including noninvasive ventilation in pediatric ARDS and after extubation failure, with updated guidelines on ventilator liberation. Improved technology and advancements in hemodynamic support allow for better care of our patients with heart disease. Sepsis burden in children remains high and continued efforts are made to improve survival. A nutritional plan with a tailored approach

Antibiotics in anesthesia and critical care.

Sepsis is life-threatening organ dysfunction due to a dysregulated host response to an underlying acute infection. Sepsis is a major worldwide healthcare problem. An annual estimated 48.9 million incident cases of sepsis is reported, with 11 million (20%) sepsis-related deaths. Administration of appropriate antimicrobials is one of the most effective therapeutic interventions to reduce mortality. The severity of illness informs the urgency of antimicrobial administration. Nevertheless, even used properly, they cause adverse effects and contribute to the development of antibiotic resistance. Bo

Obesity in the Critical Care Setting.

The prevalence of obesity continues to rise and is caused by many factors. Obesity places patients at risk for high blood pressure, diabetes, heart disease, and cancer. Although obesity in the normal population is associated with increased morbidity and mortality, obesity in critically ill patients has lower mortality. This is referred to as the obesity paradox, and although not fully understood, involves several mechanisms that demonstrate a protective factor in critically ill obese patients. However, despite the benefit, the management of critically ill obese patients faces many challenges.

Melioidosis in Critical Care: A Review.

KEY POINTS: (1) Diabetes, hazardous alcohol use, and/or significant heart disease are more likely to develop a critical illness with melioidosis. (2) Pneumonia is the most common presentation. Those with pneumonia or bacteremia are most likely to require intensive care unit admissions. (3) Culture is the mainstay for the diagnosis. However, it is noted that Burkholderia pseudomallei is often wrongly identified as Pseudomonas or other Burkholderia species by commonly available commercial techniques. (4) Therapy consists of an intensive phase with intravenous antibiotics to prevent mortality fol

Ultrasonography in the Critical Care Unit.

PURPOSE OF REVIEW: This article summarizes the utility and evidence supporting the use of ultrasound exams in the intensive care unit. RECENT FINDINGS: Point-of-care ultrasonography (POCUS) is widely used by intensivists managing critically ill patients whereby they can accurately and rapidly assess for many pathologies such as pneumothorax, pulmonary edema, hydronephrosis, hemoperitoneum, and deep vein thrombosis among others. Basic and advanced critical care echocardiography, including transesophageal echocardiography, are routinely performed to determine the etiology of hemodynamic instabi

Pressure Injuries Among Critical Care Patients.

Nutrition is an important component in the prevention and treatment of pressure injuries. Although the point at which insufficient nutrient consumption affects the body's capability to support skin integrity has not been demarcated, what is known is that reduced intake of food and fluids/water and weight loss can increase the risk of developing pressure injuries. Protein and its building blocks, amino acids, are essential for tissue growth and repair during all phases of wound healing. Sufficient macronutrients (carbohydrates, protein, fats, and water) and micronutrients (vitamins and minerals

Haemophagocytic lymphohistiocytosis in adult critical care.

Haemophagocytic lymphohistiocytosis (HLH) is a syndrome of severe immune dysregulation, characterised by extreme inflammation, fever, cytopaenias and organ dysfunction. HLH can be triggered by conditions such as infection, autoimmune disease and malignancy, among others. Both a familial and a secondary form have been described, the latter being increasingly recognised in adult patients with critical illness. HLH is difficult to diagnose, often under-recognised and carries a high mortality. Patients can present in a very similar fashion to sepsis and the two syndromes can co-exist and overlap,

Critical Care Aspects of Gallstone Disease.

Approximately twenty per cent of adults have gallstones making it one of the most prevalent gastrointestinal diseases in Western countries. About twenty per cent of gallstone patients requires medical, endoscopic, or surgical therapies such as cholecystectomy due to the onset of gallstone-related symptoms or gallstone-related complications. Thus, patients with symptomatic, uncomplicated or complicated gallstones, regardless of the type of stones, represent one of the largest patient categories admitted to European hospitals. This review deals with the important critical care aspects associated

Critical Care Management of Intracerebral Hemorrhage.

Spontaneous intracerebral hemorrhage (ICH), the most devastating and debilitating form of stroke, remains a major healthcare concern all over the world. Intracerebral hemorrhage is frequently managed in critical care settings where intensive monitoring and treatment are employed to prevent and address primary and secondary brain injury as well as other medical complications that may arise. Although there has been increasing data guiding the management of ICH in the past decade, prognosis remains dismal. In this article, the authors discuss the risk factors for ICH, the role of imaging, the maj

History of pediatric critical care medicine.

OBJECTIVES: to review and cite important individuals and events in the development of pediatric critical care medicine (PCCM). DATA SOURCES: A MEDLINE search was performed looking for citations of the history of PCCM. This yielded 85 citations of which 46 were obtained. Thirty nine of the 85 were rejected as inappropriate either by title (e.g., pediatric emergency medicine) or because they were in a foreign language only. After review of the 46, 21 were included in this review and the others rejected as inappropriate. Textbooks of PCCM were reviewed for chapters on the history of PCCM and four

Neurodiagnostic techniques in neonatal critical care.

This article reviews recent advances in the neurodiagnostic tools available to clinicians practicing in neonatal critical care. The advent of induced mild hypothermia for acute neonatal hypoxic-ischemic encephalopathy in 2005 has been responsible for renewed urgency in the development of precise and reliable neonatal neurodiagnostic techniques. Traditional evaluations of bedside head ultrasounds, head computed tomography scans, and routine electroencephalograms (EEGs) have been upgraded in most tertiary pediatric centers to incorporate protocols for MRI, continuous EEG monitoring with remote b

[Magnesium in critical care and anesthesiology].

Magnesium (Mg) is an essential ion for life and is involved in many important biological processes. Mg deficiency may have serious consequences and has been implicated in many diseases. Assessment of Mg status is difficult because Mg is essentially intracellular and there is no simple, rapid and accurate laboratory test to determine total body Mg. New, non-invasive techniques are being developed that could be used in the near future. Mg therapy is clearly indicated in "torsade de pointes" and preeclampsia. Mg therapy is not recommended in the treatment of acute myocardial infarction, stroke or

Critical care neurology: Five new things.

Critical care neurology has generated interest both because of the urgency to understand acute brain injury and because acute interventions could improve outcomes. Unifying themes include intervention without delay and early recognition of the potential for deterioration of the patient. While monitoring devices offer useful prognostic indicators, conducting a thorough clinical neurologic examination is paramount in determining the most effective course of patient management. Recent progress has been made in acute brain injury monitoring, more effective reversal of anticoagulation after cerebra

New anticoagulants in critical care settings.

Thromboembolic events commonly occur in critically ill patients, and although they do not consistently present with specific signs and symptoms, they are associated with high morbity and mortality. Antithrombotic agents are the mainstay of the prevention and treatment of venous thromboembolism, and they are also used for stroke prevention in atrial fibrillation, embolism prevention in heart failure, and anticoagulation of prosthetic valves. These drugs have been combined with antiplatelet therapy for the prevention of secondary acute coronary syndrome. Antithrombotic agents such as Aspirin, cl

[Therapeutic hypothermia in neurological critical care].

Therapeutic hypothermia (TH) is very effective for the treatment of acute experimental brain injury. In contrast to other neuroprotective methods or pharmacological agents, TH represents the only clinically proven neuroprotective therapy. TH led to a decreased mortality rate and improved functional outcome in patients with cardiac arrest. Investigations in severe head trauma are inconclusive up to now. Other important indications such as ischemic stroke remain to be investigated properly. Recent phase II studies are on the way to test safety and feasibility in the early time window of stroke i
