Vitamin-D levels and intensive care unit outcomes of a cohort of critically ill COVID-19 patients.

OBJECTIVES: The pattern of global COVID-19 has caused many to propose a possible link between susceptibility, severity and vitamin-D levels. Vitamin-D has known immune modulatory effects and deficiency has been linked to increased severity of viral infections. METHODS: We evaluated patients admitted with confirmed SARS-COV-2 to our hospital between March-June 2020. Demographics and outcomes were assessed for those admitted to the intensive care unit (ICU) with normal (>50 nmol/L) and low (<50 nmol/L) vitamin-D. RESULTS: There were 646 SARS-COV-2 PCR positive hospitalisations and 165 (25.5%)

Impact of regular collaboration between infectious diseases and critical care practitioners on antimicrobial utilization and patient outcome.

OBJECTIVE: Antimicrobial stewardship programs have been shown to help reduce the use of unnecessary antimicrobial agents in the hospital setting. To date, there has been very little data focusing on high-use areas, such as the medical ICU. A prospective intervention was done to assess guideline compliance, antimicrobial expenditure, and healthcare cost when an infectious disease fellow interacts regularly with the medical ICU team. DESIGN: A 3-month retrospective chart review was followed by a 3-month prospective intervention the following year. Two hundred forty-six total charts were reviewe

Predicting, describing, or measuring? How to align model purpose with validity and applicability in critical care outcome research.

PURPOSE OF REVIEW: Clinical models are commonly applied in critical care for both descriptive and predictive purposes. However, methodological rigour is often lacking in their development and validation. This review examines the principles underlying the multiple domains of model validity. We argue that a clear model purpose and theoretical framework are essential preconditions for validity. RECENT FINDINGS: Recently developed descriptive and predictive models illustrate different approaches to promoting validity. In developing SOFA-2, eCARTv5, Sepsis-3 and the PHOENIX paediatric sepsis crite

The frailty, outcomes, recovery and care steps of critically ill patients (FORECAST) study: pilot study results.

INTRODUCTION: Frailty is common in critically ill patients and is associated with increased morbidity and mortality. There remains uncertainty as to the optimal method/timing of frailty assessment and the impact of care processes and adverse events on outcomes is unknown. We conducted a pilot study to inform on the conduct, design and feasibility of a multicenter study measuring frailty longitudinally during critical illness, care processes, occurrence of adverse events, and resultant outcomes. METHODS: Single-center pilot study enrolling patients over the age of 55 admitted to an Intensive C

Association between critical care admission and 6-month functional outcome after spontaneous intracerebral haemorrhage.

BACKGROUND: There is uncertainty about the clinical benefit of admission to critical care after spontaneous intracerebral haemorrhage (ICH). PURPOSE: We investigated factors associated with critical care admission after spontaneous ICH and evaluated associations between critical care and 6-month functional outcome. METHODS: We included 825 patients with acute spontaneous non-traumatic ICH, recruited to a prospective multicenter observational study. We evaluated the characteristics associated with critical care admission and poor 6-month functional outcome (modified Rankin Scale, mRS > 3) usi

Hyponatremia in critical care patients: frequency, outcome, characteristics, and treatment with the vasopressin V2-receptor antagonist tolvaptan.

Hyponatremia is a common problem in critical care patients and is associated with increased duration of hospital stay and increased morbidity and mortality. The prevalence of hyponatremia in the intensive care unit (ICU) has been reported to be as high as 30% to 40%. Recent studies have found hyponatremia at ICU admission in up to 14% of patients in unselected groups; patients with hyponatremia were at elevated risk of mortality vs normonatremic patients. Most cases in the ICU are euvolemic or hypervolemic hyponatremia, with the syndrome of inappropriate secretion of antidiuretic hormone being

Impact of the Chelsea critical care physical assessment (CPAx) tool on clinical outcomes of surgical and trauma patients in an intensive care unit: An experimental study.

BACKGROUND: Critically ill patients following traumatic injury or major surgery are at risk of loss of skeletal muscle mass, which leads to decreased physical function. Early rehabilitation in an intensive care unit (ICU) is thought to preserve or restore physical functioning. The Chelsea critical care physical assessment (CPAx) is a measurement tool used to assess physical function in the ICU. OBJECTIVES: To determine whether the use of the CPAx tool as part of physiotherapy patient assessment, in two adult trauma and surgical ICU settings where early patient mobilisation forms part of stand

Incidence and outcome of critical illness amongst hospitalised patients with haematological malignancy: a prospective observational study of ward and intensive care unit based care.

To determine the incidence and outcome of critical illness amongst the total population of hospital patients with haematological malignancy (including patients treated on the ward as well as those admitted to the intensive care unit), consecutive patients with haematological malignancy were prospectively studied. One hundred and one of the 1437 haemato-oncology admissions (7%) in 2001 were complicated by critical illness (26% of all new referrals). Fifty-four (53%) of these critically ill patients survived to leave hospital and 33 (34%) were still alive after 6 months. The majority (77/101) we

Contribution of ethnicity and deprivation to paediatric critical care outcomes in the UK, 2008-21: a national retrospective cohort study.

BACKGROUND: Evidence from UK paediatric intensive care units (PICUs) demonstrates increased incidence of admission among children of Asian and Black ethnicity and children residing in more deprived areas. We aimed to investigate whether mortality in PICU is associated with ethnicity and child poverty. METHODS: This national cohort study included children aged 0-15 years who were admitted to PICUs in the UK between Jan 1, 2008, and Dec 31, 2021. Participating PICUs were affiliated with and reported outcome data to the Paediatric Intensive Care Audit Network (PICANet), from which we extracted d

Clinical and microbiological outcome in septic patients with extremely low 25-hydroxyvitamin D levels at initiation of critical care.

A relationship between vitamin D status and mortality in patients in intensive care units (ICU) has been documented. The present study aims to describe the clinical profile and sepsis-related outcome of critically ill septic patients with extremely low (<7 ng/mL) vitamin D levels at ICU admission. We conducted an observational study in the ICU of a teaching hospital including all patients admitted with severe sepsis/septic shock and undergoing 25-hydroxyvitamin D (25(OH)D) testing within the first 24 hours from admission. We studied 107 patients over 12 months. At ICU admission vitamin D defic

Sleep fragmentation in critically ill children: a review of contributing factors in the pediatric intensive care unit and neurodevelopmental outcomes.

Sleep is a critical neurobiological process essential for brain maturation, emotional regulation, cognitive development, and overall organ system homeostasis. In the pediatric intensive care unit (PICU), sleep architecture is frequently disrupted by environmental stimuli, sedation, and clinical interventions, resulting in sleep fragmentation. Unlike sleep deprivation, sleep fragmentation preserves sleep duration but impairs its continuity and depth, disproportionately affecting slow-wave sleep, that is essential for growth, healing, in addition to immune function and REM sleep, that is fundame
