Altered passive eruption (APE)
and active secondary eruption (ASE): differential diagnosis and management.

The process of tooth eruption consists of two phases, active and passive. While the distinction is unequivocal, the mechanism is ambiguous. This article compares and contrasts altered passive eruption (APE) and active secondary eruption (ASE). Although these phases present with similar clinical manifestations, each has its own etiology, physiogenesis, and pathogenesis. Furthermore, the differential diagnosis between the two, and that between other mirroring conditions, is essential for arriving at a definite diagnosis and correct treatment strategies, which may be similar but have different co

Orthodontic Extrusion vs. Surgical Extrusion to Rehabilitate Severely Damaged Teeth: A Literature Review.

The need to rehabilitate severely compromised teeth is frequent in daily clinical practice. Tooth extraction and replacement with dental implant represents a common treatment choice. However, the survival rate for implants is inferior to teeth, even if severely damaged but properly treated. In order to reestablish a physiological supracrestal tissue attachment of damaged teeth and to arrange an efficient ferrule effect, three options can be considered: crown lengthening, orthodontic extrusion and surgical extrusion. Crown lengthening is considered an invasive technique that causes the removal …

Management of crown-root fracture tooth by intra-alveolar transplantation with 180-degree rotation and suture fixation.

Teeth with traumatic crown-root fractures extending subgingivally and involving the entire width of the tooth may have problems in endodontic isolation, periodontal maintenance, and restoration. A number of techniques, such as crown lengthening, and orthodontic or surgical extrusion, have been described for treatment of crown-root fractures. However, all of these techniques have limitations in terms of the extended time required for treatment, cost, and esthetic considerations. Intra-alveolar transplantation is an alternative treatment option in the management of complicated crown-root fractur…

[Treatment of subgingival crown root fracture: forced eruption--case report].

The conventional treatment methods for a subgingival crown root fracture are (1) extract the residual root and restore with a fixed bridge; (2) crown restoration with a deep subgingival margin; and (3) exposure of the fracture line by a crown lengthening operation and restoration with a crown. However, these methods require reduction of either the tooth structure or the periodontal support. Improvements in endodontic and orthodontic therapy followed Heithersay in 1973 to combine the use of endodontic and orthodontic therapy to manage subgingival crown root fractures--the so-called forced erupt…

Forced eruption of a solitary nonrestorable tooth using mini-implants as anchorage: rationale and technique.

PURPOSE: The need to place a restoration's margins on sound tooth material may require crown lengthening or forced eruption. The latter intervention may benefit from a severance of the tooth's circumferential fibers during the orthodontic process in an effort to stabilize the bone level. Orthodontic appliance design for the extrusive action, especially in the absence of neighboring teeth, may require the use of mini-implants, which are a useful adjunct in such situations. MATERIALS AND METHODS: This case history describes the management of a maxillary left canine that lacked sound tooth mater…

Orthodontic extrusion of an extensively broken down anterior tooth: a clinical report.

A predictable esthetic restoration is not limited to the restored teeth; it has to include the gingival unit and its interface with the teet … levating sound tooth material from within the alveolar socket. It has some advantages over surgical crown lengthening, which is less conservative considering the sacrifice of supporting bone and the negative change in the length of the clinical crowns of both the tooth and its neighbors. Prevalence and characteristics of periodontal surgery procedures in postgraduate periodontic clinics: an epidemiological analysis.

Periodontal diseases include gingivitis, periodontitis, and peri-implant disease. There are different ways to treat these diseases, such as surgical and non-surgical periodontal treatments like gingivectomy, open flap debridement, and crown lengthening.

Multidisciplinary treatment approach to restore deep horizontally fractured maxillary central incisor.

This case report demonstrates sequential periodontic, orthodontic and prosthodontic treatment modalities to save and restore deep horizontally fractured maxillary central incisor. The location of fracture was deep in the mucosa which reveals less than 2 mm of tooth structure to receive the crown. The procedures like surgical crown lengthening, endodontic post placement, orthodontic forced eruption, core build-up and metal-ceramic crown restoration were sequentially performed to conserve the fractured tooth. Forced eruption is preferred to surgical removal of supporting alveolar bone, since for…

Forced eruption for preservation of a deeply fractured molar.

BACKGROUND: The treatment of deeply fractured and irreparably damaged teeth often confronts the dentist with a complex problem: Besides extraction, re-implantation and surgical crown lengthening, forced eruption is a reasonable differential alternative. HISTORY AND CLINICAL FINDINGS: In the following case a second lower molar was deeply fractured as a result of an internal granuloma. The tooth had undergone endodontic treatment and had been shortened to alveolar bone level, as the patient had refused extraction. THERAPY: A forced eruption with fibrotomy was performed using an unphysiological…

Tooth preservation of deeply destroyed teeth by forced orthodontic extrusion: A case series.

Maintaining and restoring teeth exhibiting subgingival or even subcrestal defect extensions represent a common problem in daily practice. Such teeth are often deemed "unrestorable" due to a significant hard tissue loss and defect locations violating the biologic width. In order to achieve a sufficient 2-mm-ferrule design and reestablish the biologic width, both surgical crown lengthening and orthodontic extrusion have been suggested. However, surgical crown lengthening has a negative effect on the attachment level of adjacent teeth as well as esthetic disadvantages particularly in the esthetic…

Surgical extrusion of a partially erupted and crown dilacerated incisor.

A case report of treatment for an incompletely erupted maxillary central incisor with crown dilaceration in a 12-year-old boy is described. The tooth was repositioned with surgical extrusion and endodontically treated through use of calcium hydroxide paste. Clinical and radiographic examination 2 years after completion of combined surgical and endodontic treatment revealed periapical healing and no signs of root resorption.

A string of pearls.

Most complex interdisciplinary rehabilitative dental cases are associated with tooth eruption due to excessive wear or missing occlusal contacts. As the teeth erupt, the osseous tissues and dentogingival complex extrude with the tooth. Historically, prosthetic treatment demanded placing crowns on short teeth, functional crown lengthening onto root surfaces, or orthognathic surgery to impact teeth. This article will present a more current approach to the treatment of dentoalveolar extrusion.

Complicated Crown-root Fracture Management Using the 180-degree Rotation Method.

AIM: The aim of this article is to report a successful treatment of a complicated crown-root fracture. BACKGROUND: The management of complicated crown-root fracture in young patients is challenging as the fracture line is below the bone crest and the pulp is exposed. Treatment options include crown lengthening, orthodontic extrusion, and intentional replantation. CASE DESCRIPTION: Here, a case of complicated crown-root fracture of tooth number 11 in a ten-year-old boy is presented. Intentional replantation with 180° rotation and slight extrusion and fixation was performed. In addition, root …

Maxillary exostoses as contributing etiology to lip hypermobility and associated excessive gingival display: A clinical report.

Several etiologies may contribute to the development of excessive gingival display (EGD). However, little, if any, consideration has been given to the potentially significant role of prominent buccal maxillary exostoses (BMEs) in EGD etiology. Therefore, the aim of this report was to highlight the contribution of BMEs to EGD. Two patients complaining of EGD were evaluated, and BMEs were found associated with a hypermobile upper lip, vertical maxillary excess, and altered passive eruption. Both patients received esthetic crown lengthening (ECL) as the first surgical intervention. The BMEs were …
