Duodenal varices successfully treated with cyanoacrylate injection therapy.

Duodenal varices are a rare complication of portal hypertension secondary to liver cirrhosis. Compared to oesophageal varices, they bleed less often but are also more difficult to diagnose and treat. There is no established treatment for bleeding duodenal varices and different treatment strategies have been employed with variable results. The authors present a case of 52-year-old male who was admitted with melaena. Upper gastrointestinal endoscopy was performed which identified bleeding varices in the second part of duodenum. The varices were injected with cyanoacrylate and the outcome was fav

Endoscopic follow-up of cyanoacrylate obliteration of gastric varices.

Bleeding from gastric varices is a life-threatening condition. We report our experience with cyanoacrylate injection. Twenty three patients with portal hypertension and gastric varices underwent intra-variceal injection of a cyanoacrylate/lipiodol solution (1:1). Study endpoint was variceal obliteration. Mean follow-up was 25.3 months. Variceal obliteration was achieved in 87% of patients. Recurrence occurred in one patient (4.3%) and rebleeding in another case (4.3%). Mild abdominal pain was described in 13% of patients. Overall mortality was 21.7% and rebleeding related mortality rate was 4.

Intratumoral injection of cyanoacrylate glue in head and neck paragangliomas.

BACKGROUND AND PURPOSE: Substantial intraoperative bleeding during surgical removal of head and neck paragangliomas may be a major problem in the management of these highly vascularized tumors. Traditional preoperative embolization via a transarterial approach has proved beneficial but is often limited by complex vascular anatomy and unfavorable locations. We report our experience with the preoperative devascularization of head and neck paragangliomas by using direct puncture and an intralesional injection of cyanoacrylate. METHODS: We retrospectively analyzed nine consecutive patients with h

[Retrogastric abscess secondary to gastric varices obturation with cyanoacrylate].

T cell acute lymphoblastic leukemia (T-ALL) is an aggressive cancer that is frequently associated with activating mutations in NOTCH1 and dysregulation of MYC. Here, we performed 2 complementary screens to identify FDA-approved drugs and drug-like small molecules with activity against T-ALL. We developed a zebrafish system to screen small molecules for toxic activity toward MYC-overexpressing thymocytes and used a human T-ALL cell line to screen for small molecules that synergize with Notch inhibitors. We identified the antipsychotic drug perphenazine in both screens due to its ability to indu

Occupational allergic contact dermatitis from ethyl cyanoacrylate.

Glues based on cyanoacrylates are widely used as contact adhesives for metal, glass, rubber, plastics and textiles, as well for biological materials, including binding tissues and sealing wounds in surgery. In this paper, an apprentice cobbler with an occupational allergic contact dermatitis from an ethyl cyanoacrylate glue, in which the major monomer was shown to be the sensitizer, is reported. Initial patch testing with the cyanoacrylate glue dissolved in acetone with the Finn Chamber (aluminium) technique yielded false-negative reactions. Positive test reactions were obtained with the same

The prevention of carotid artery rupture with isobutyl-2-cyanoacrylate.

Carotid artery exposure and rupture is one of the most feared complications of head and neck surgery. The ideal method for preventing rupture of an exposed artery should be easy to perform, safe, effective and should spare local and regional flaps for later use in reconstruction. Isobutyl-2-cyanoacrylate (Bucrylate) is a commercially available compound that appears to meet these criteria. Test animals were divided into two groups. The carotid arteries of 12 dogs (group A) were exteriorized bilaterally and coated with Bucrylate unilaterally. Group B (four dogs) underwent the same procedure exce

Clip-assisted endoscopic cyanoacrylate injection vs endoscopic ultrasound-guided coil and cyanoacrylate injection for gastric varices: A propensity score-matched study.

BACKGROUND: The high rebleeding rate and severe adverse events have raised concerns regarding the safety of endoscopic cyanoacrylate as a conventional treatment for gastric variceal hemorrhage. Clip-assisted endoscopic cyanoacrylate injection (Clip-CYA) and endoscopic ultrasound-guided coil and cyanoacrylate injection (EUS-CG) are two currently used modalities. There are limited data comparing the two techniques. AIM: To compare the efficacy, safety, and procedural characteristics of Clip-CYA vs EUS-CG for treatment of gastric varices (GVs) with spontaneous portosystemic shunts. METHODS: Bet

Structural comparison of N-butyl-2-cyanoacrylate-Lipiodol (NL) and N-butyl-2-cyanoacrylate-Lipiodol-Ethanol (NLE) using scanning electron microscope.

PURPOSE: N-butyl-2-cyanoacrylate (NBCA) and Lipiodol mixture (NL) are widely used for emergency embolization due to their effective polymerization upon contact with blood. However, NBCA's strong adhesive properties can cause complications, leading to the development of an NBCA-Lipiodol-ethanol mixture (NLE), which has shown reduced catheter adhesion. This study aimed to observe the structural differences between NL and NLE polymers using scanning electron microscopy. MATERIALS AND METHODS: Four different ratios of NBCA, Lipiodol, and ethanol (NLE230, NLE221, NLE150, and NLE141) were examined.

Embolization with N-Butyl Cyanoacrylate: Properties, Techniques, Applications, and Pitfalls.

N-butyl cyanoacrylate (NBCA) glue embolization is a minimally invasive technique widely used in interventional radiology to achieve vascular occlusion. It is a liquid embolic agent that rapidly polymerizes on contact with ionic substances such as blood or saline, allowing immediate solidification and formation of a durable embolus. This property makes it especially effective in controlling arterial bleeding from small-caliber or tortuous vessels that cannot be accessed by using microcatheters or in cases requiring urgent hemostasis due to severe hemorrhage. The embolization process involves mi

Management of recanalization after cyanoacrylate closure: A single-center experience.

OBJECTIVE: Cyanoacrylate closure (CAC) is an established treatment for lower-extremity varicose veins. Recanalization after CAC is an uncommon but clinically relevant event for which optimal management strategies have not been well described. The objective of this study was to retrospectively review cases of CAC recanalization at a single center and to descriptively evaluate the secondary treatment approaches, including ultrasound-guided foam sclerotherapy (UGFS), endovenous laser ablation (EVLA), and radiofrequency ablation (RFA). METHODS: We retrospectively reviewed 335 patients who underwe

Clip-Assisted Cyanoacrylate Injection for the Management of a Proximal Colonic Varix.

Colonic varices are a rare manifestation of portal hypertension and an uncommon cause of lower gastrointestinal bleeding, particularly when involving the proximal colon. We report a case of bleeding hepatic flexure varices in a 23-year-old male with extrahepatic portal vein obstruction (EHPVO) and prior splenectomy with lienorenal shunt. The patient presented with recurrent hematochezia and severe anemia. Upper gastrointestinal endoscopy was negative for a bleeding source, while colonoscopy revealed large tortuous varices at the hepatic flexure with stigmata of recent hemorrhage. Contrast-enha

Endoscopic Injection Sclerotherapy Assisted by Cyanoacrylate and Clips for Gastroesophageal Varices.

Gastroesophageal variceal bleeding (GEVB) is the most common complication of liver cirrhosis and the leading cause of death in patients with liver cirrhosis. Traditional endoscopic sclerotherapy has been widely used for the treatment of variceal bleeding. However, it still has a high rate of rebleeding and may cause esophageal ulceration, stricture, or perforation. Here we present a sclerotherapy method, endoscopic injection sclerotherapy assisted by cyanoacrylate and clips (CISC), as a feasible and safe procedure for gastroesophageal varices (GEVs). The method uses clips to block the oral sid

Cyanoacrylate glue topical adhesive for bronchoscopic repair of tracheal laceration.

Iatrogenic tracheal laceration (ITL) is a rare and potentially life-threatening complication of endotracheal intubation. The lacerations most commonly occur at the posterior membrane and in emergency and difficult intubations. There is no consensus regarding the optimal management of ITL. Small lacerations can be managed conservatively; however, surgical repair might be indicated for large lacerations, patients with progressive symptoms or those with haemodynamic instability. In this report, we present a case of a 3cm iatrogenic posterior membrane laceration following emergency intubation, whi

[Recurrent Bacteremia after Cyanoacrylate Sclerotherapy of Ectopic Duodenal Varices].

Sclerotherapy with N-butyl-2-cyanoacrylate is the first-line endoscopic approach for gastric and ectopic variceal bleeding, but it can be associated with local or systemic complications. Episodes of transient bacteremia after the procedure are frequent, but documented cases of recurrent bacteremia are rare. The authors report a 47-year-old female patient with liver cirrhosis who underwent duodenal sclerotherapy with cyanoacrylate after upper gastrointestinal bleeding. Subsequently, she developed five episodes of bacteremia with unknown origin. A definitive diagnosis of recurrent bacteremia due

Cyanoacrylate glue reactions: A systematic review, cases, and proposed mechanisms.

OBJECTIVE: Cyanoacrylate glue closure was first used in humans 10 years ago to treat venous reflux of the axial veins. Studies have since shown its clinical efficacy in vein closure. However, great need exists to elucidate further the types of specific adverse reactions that cyanoacrylate glue can cause for better patient selection and to minimize these events. In the present study, we systematically reviewed the literature to identify the types of reported reactions. In addition, we explored the pathophysiology contributing to these reactions and proposed the mechanistic pathway with inclusio
