[Robot-assisted radical cystectomy with neobladder formation].

INTRODUCTION: Robot-assisted radical cystectomy (RARC) with intracorporeal neobladder formation is a complex surgical procedure. AIM: To describe the main stages of RARC and to analyze its short-term results. MATERIALS AND METHODS: RARC with ileocystoplasty was performed in 16 patients, most of whom were men (n=14). In 15 patients, the indication for surgery was bladder cancer (BCa), while one patient has radiation-induced sigmoid fistula with a formation of small, contracted bladder. During radical cystectomy (RC), the lower ureters were dissected, followed by posterior dissection of the bl

Radical Cystectomy in Female Patients - Improving Outcomes.

PURPOSE OF REVIEW: To review the methods of improving surgical, oncological, and functional outcomes in women with bladder cancer treated with radical cystectomy. RECENT FINDINGS: Οrthotopic urinary diversion (ONB) is a safe option for well-selected women as it combines high rates of daytime and nighttime continence with exceptional oncologic outcomes. It is considered safe even for patients with limited lymph node disease and trigone involvement, as long as a preoperative biopsy of the bladder neck or an intraoperative frozen section analysis of distal urethral margin rules out malignant dis

Total thoracoscopic combined lingulectomy and pericardial cystectomy.

Thoracoscopic anatomic segmentectomy is usually more complex than lobectomy. This video shows a 58-year-old female who suffered from limited bronchiectasis of the lingular segment of the left upper lung and a pericardial cyst on the same side. Both of these benign thoracic diseases can cause pleural adhesions. Repeated chronic inflammation contributes to hypervascularity and lymph node enlargement, making surgery more difficult. We used single-direction thoracoscopic segmentectomy via a three-port approach and successfully removed the lingula and pericardial cyst.

Prostate-sparing cystectomy: long-term oncological results.

OBJECTIVE: To analyse the oncological outcome of prostate-sparing cystectomy (PSC). PATIENTS AND METHODS: Between 1994 and 2006, 63 men were treated with PSC after meeting the inclusion criteria (no tumour at the bladder neck, no prostate cancer). The results were compared with patients who had a standard cystoprostatectomy (SC) during the same study period, after matching for clinical and pathological characteristics. RESULTS: The 3- and 5-year disease-specific survival rates were 77% and 66% in the PSC group, and 68% and 64% in the SC group (log-rank, P = 0.6). The local recurrence rate wa

Benefits of laparoscopic prostate-sparing radical cystectomy.

PURPOSE: Prostate-sparing radical cystectomy has been described in the literature and has proven to be a promising procedure owing to the continence and erectile function results without necessarily compromising the oncologic outcome in selected patients. TECHNIQUE POINTS: A transperitoneal approach is used. Lymph node dissection is performed with frozen section, and then the ureters are ligated and biopsied. The seminal vesicles are dissected, followed by complete mobilization of the bladder. Incision of the bladder neck is performed, followed by simple prostatectomy. Finally, bowel reconstr

[Cystectomy for indications other than bladder cancer].

A cystectomy for indications other than transitional cell cancer of the bladder or general bladder cancer is frequently performed in cooperation with other surgical specialties such as general surgery or gynecology. In these cases the urological procedure as well as the oncological and surgical concepts of other specialties have to be combined. We studied our cystectomy patients who had undergone such a combined procedure for a non-urothelial indication concerning perioperative and postoperative complications.A total of 204 cystectomies were performed by the Department of Urology at the Univer

Sigmoid neobladder in women after radical cystectomy.

PURPOSE: Neobladder reconstruction using an intestinal segment is the common procedure of choice in men after cystectomy. Recently this procedure has been extended to women. We present our surgical and functional experience with the sigmoid neobladder in women. MATERIALS AND METHODS: A total of 11 women 49 to 74 years old (mean age plus or minus standard deviation 60 +/- 8.5) with transitional cell carcinoma of the bladder underwent lower urinary tract reconstruction with a modified sigmoid neobladder after nerve and urethral support sparing cystectomy. After at least 3 months voiding pattern

[Spur valve jejunal interposition in choledochus cystectomy].

During 1983-1990, 113 patients with congenital choledochus cysts were operated upon with the procedure called the spur valve jejunal interposition following total cystectomy, and satisfactory results were recorded in two years follow-up. However, early complications happened in 26.5% patients e.g. postoperative bile leakage, blood vomiting, small intestinal intussusception, adhesive intestinal obstruction, chyliform ascites, and disruption of wound. Six months-2 years after operation, reflux into bile duct and rapid emptying were observed in some cases in barium examination, and abdominal pain

Vaginal recurrence after cystectomy for bladder cancer.

The records of 14 patients with vaginal recurrence following cystectomy were reviewed in an attempt to find factors responsible for the local failure. Comparison was made with female patients who underwent cystectomy for bladder cancer during the same period but who did not have vaginal recurrence. Patients with previous surgery and radiation to the pelvic organs appear to be at risk for local recurrence, probably because of the difficulties associated with surgical removal of the bladder. Patients with tumors other than transitional cell carcinoma, especially those located at the bladder neck

Post-irradiation total cystectomy for bladder cancer.

Treatment with radiotherapy (6000 rad) followed by total cystectomy was carried out in 125 patients with T1-T3 bladder cancer, the tumour being poorly differentiated (grade III-IV) in 115 cases. Survival was significantly better, if the irradiation had resulted in complete tumour regression before cystectomy, and if the tumour at the initial examination was of category T1. The relative cumulative survival after 5 years for patients with T1-tumours was 60%, as opposed to 40% for patients with T3-tumours. 4 patients (2.4%) died within 30 days of the cystectomy, but 13 patients (10%) died within

Does Time Matter in Early Radical Cystectomy? Comparing Outcome, Clinical, and Pathological Characteristics of Immediate versus Deferred Radical Cystectomy.

INTRODUCTION: Early radical cystectomy (eRC) can be performed for high or very high risk non-muscle-invasive bladder cancer (NMIBC). Whether immediate eRC is beneficial is still unclear. The objective of this study was to compare outcomes between immediate eRC, delayed eRC, and radical cystectomy (RC) at the diagnosis of muscle-invasive bladder cancer (MIBC). METHODS: The single-center cohort consisting of patients with RC between 2008 and 2020 was divided into 4 populations: patients who received immediate eRC within 3 months of diagnosis of NMIBC (IEC), patients who received eRC at recurren

Indications for lower urinary tract reconstruction in women after cystectomy for bladder cancer: a pathological review of female cystectomy specimens.

PURPOSE: In an attempt to identify women who may be appropriate candidates for orthotopic lower urinary tract reconstruction, archival cystectomy specimens from female patients undergoing cystectomy for primary bladder cancer were reviewed. These pathological findings should provide a better understanding of tumor involvement at the bladder neck (vesicourethral junction) and urethra in women with transitional cell carcinoma of the bladder. MATERIALS AND METHODS: Cystectomy specimens of 67 consecutive women undergoing surgery for biopsy proved transitional cell carcinoma of the bladder between

Development and validation of surgical training tool: cystectomy assessment and surgical evaluation (CASE) for robot-assisted radical cystectomy for men.

BACKGROUND: We aimed to develop a structured scoring tool: cystectomy assessment and surgical evaluation (CASE) that objectively measures and quantifies performance during robot-assisted radical cystectomy (RARC) for men. METHODS: A multinational 10-surgeon expert panel collaborated towards development and validation of CASE. The critical steps of RARC in men were deconstructed into nine key domains, each assessed by five anchors. Content validation was done utilizing the Delphi methodology. Each anchor was assessed in terms of context, score concordance, and clarity. The content validity ind

Automated algorithm for surgical outcomes abstraction for cystectomy.

BACKGROUND: Cystectomy is a potentially morbid procedure that presents a prime opportunity for quality improvement. We hypothesized that a novel electronic medical record (EMR)-based automated algorithm would demonstrate >90% sensitivity and specificity with significant agreement with institutional National Surgical Quality Improvement Program (NSQIP) data abstraction. STUDY DESIGN: We developed an EMR-based algorithm that automatically abstracts surgical outcomes and quality metrics for cystectomy. All cystectomies performed between January 2013 and March 2025 by urologists in our health sys

[Radical cystectomy for bladder cancer is a complex procedure].

Radical cystectomy (RC) for bladder cancer is a complex procedure with an inherent risk of complications and even postoperative mortality. Historically, RC was performed in 44 hospitals in Sweden, which gradually decreased over time, and since a formal regional centralisation of these operations in 2017, cystectomy care is currently provided by nine hospitals. In the Swedish national urinary bladder cancer register (SNRUBC), complications after RC has been registered with high coverage since 2012, with data presented in an interactive online data resource (RODRET). Over time, an increased medi

Simple cystectomy: outcomes of a new operative technique.

OBJECTIVE: To present an efficient technique for simple cystectomy. Urinary diversion for benign indications is a relatively rare procedure. However, diversion alone without accompanying cystectomy results in a significant risk of complications, such as pyocystis, hematuria, pain, and secondary carcinoma. METHODS: We retrospectively reviewed our institutional experience with this simple cystectomy technique, which included 23 patients from 2007-2010 performed by 3 surgeons. There were 14 females and 9 males. All patients had exhausted all other possible conservative therapies. Indication for

Laparoscopic partial cystectomy for treatment of adenocarcinoma of the urachus.

The treatment of choice for urachal carcinoma has traditionally been an open approach, either by radical cystectomy or the more recently adopted bladder-sparing approach of extended partial cystectomy and umbilectomy. We report for the first time a laparoscopic technique for an extended partial cystectomy with en bloc umbilectomy for the management of urachal carcinoma in a 41-year-old man.
