Visibility in Endourologic Workup (VIEW): Comparison of Gas and Saline Cystoscopy.

Introduction Macrohaematuria often compromises cystoscopic visibility. We therefore compared gas-based versus saline cystoscopy in a standardized ex vivo macrohematuria model to assess visibility, procedure completion, and observed procedural performance. Methods In a monocentric, two-arm, open-label, randomized crossover pilot, physicians performed cystoscopy on ex vivo porcine bladders modified to mimic human access. Macrohematuria was simulated using a blood-analogue solution. Each bladder was distended with ambient air (180 ml; manual syringes; no intravesical pressure control) or saline (

Blue light cystoscopy- as an improvised diagnostic modality for bladder tumours.

Bladder cancer remains a significant global health concern, being the 10th most common malignancy worldwide and the 6th most common neoplasia in males, with alarming annual incidence and mortality rates. The current narrative review was planned to delve into the multifaceted landscape of bladder cancer, exploring its epidemiology, risk factors and diagnostic modalities. While white light cystoscopy has long been considered the gold standard for bladder cancer diagnosis and surveillance, the emergence of blue light cystoscopy has ushered in a new era of early detection. Numerous studies have de

Efficacy and safety of intravesical alkalinized lignocaine for cystoscopy: a retrospective analysis.

PURPOSE: To study the effect of intravesical instillation of alkalinized lignocaine (AL) solution on pain scores and patient tolerance in patients undergoing cystoscopy. Bladder distension, trigonal irritation, and bladder neck irritation during cystoscopy under local anesthesia contribute significantly to patient discomfort. METHODS: This was a retrospective case-control study performed at a single center from January 2023 to July 2023 (Institutional Ethics Committee approval: T/IM-NF/Urol/23/194). Prior to cystoscopy, patients in control group (Group A) (LG group) received only 10 ml of 2%

A Case-Control Study on the Association between Salmonella Bacteriuria and Cystoscopy.

To date, there is only one published report of an outbreak of urinary tract infections by Salmonella species after cystoscopy. Disinfection procedures for cystoscope have come into question. The current study aimed to determine the odds of developing Salmonella bacteriuria after cystoscopy. A retrospective case-control study was conducted on all patients with Salmonella species in urine (case) and blood (control) from 2017 to 2019 in 16 hospitals in Eastern Ontario, Canada. Eight of the 11 patients had cystoscopy prior to Salmonella bacteriuria; three of the 74 patients had urological procedur

Cystoscopy-Guided Laparoscopic Excision of Prostatic Utricle: Report of a Case.

Prostatic utricle (PU) is incomplete regression of Müllerian duct and may cause recurrent urinary tract infections (UTIs), stone formation, postvoid dribbling, and recurrent epididymitis. Although surgical excision is recommended to avoid complications, surgical access to PU has been challenging. Cystoscopy-guided laparoscopic management of PU in a 3-year-old boy is reported to discuss use of other endoscopic aids in the surgical treatment of PU. He was admitted with disordered sexual development with karyotype of 47,XYY/46,XY and has been experiencing recurrent UTIs. Voiding cystourethrogram

Management of Patients with Normal Cystoscopy but Positive Cytology or Urine Markers.

This presentation considers follow-up after successful transurethral resection of a high-grade non-muscle-invasive tumour, with normal cystoscopy, followed by bacillus-Calmette-Guérin (BCG) therapy. Focusing on two possible outcomes, a positive cytology but a negative urinary biomarker result, versus positive biomarkers but a negative cytology, we discuss what the evidence and guidelines recommend and which test is more robust. PATIENT SUMMARY: Bladder cancer is usually assessed by examination of tissue taken from the bladder, either by surgery or by biopsy; however, trace elements in the urin

Can posthysterectomy cystoscopy be utilized as a screening test for bladder cancer?

It is currently undisputed that cystoscopy is required after each hysterectomy to confirm the integrity of the urinary tract before leaving the operating room. Sometimes, secondary findings are detected on cystoscopy like interstitial cystitis and exophytic tumors. We are reporting a case of asymptomatic bladder cancer discovered incidentally during a posthysterectomy cystoscopy. The chance of a woman having this cancer is about 1 in 85. This demonstrates the additional merit of performing cystoscopy after hysterectomy as a screening test for bladder cancer.

Use of laparoscopic-assisted cystoscopy for removal of urinary calculi in dogs.

Urinary calculi were removed by means of laparoscopic-assisted cystoscopy in 3 dogs. Two small abdominal incisions were made--1 for a laparoscope and 1 for placement of a Babcock forceps to aid in grasping and lifting the urinary bladder to the abdominal wall. A cystoscope and instruments for calculi removal were passed through a small cystotomy. Biopsy of the urinary bladder or other abdominal organs could also be performed by use of this technique. Laparoscopic-assisted cystoscopy was minimally invasive and provided clear images of the mucosal surface of the urinary bladder and proximal port

Epidural meperidine for control of autonomic hyperreflexia in a quadriplegic undergoing cystoscopy.

Epidural meperidine was used to control autonomic hyperreflexia (AH) during cystoscopy and transuretheral sphincterotomy, in a quadriplegic patient who had chronic spinal cord transection at C6 level. Meperidine 100 mg diluted in 10 ml saline was injected in the epidural space at L3-L4 level. Within 10 minutes and throughout the surgical procedure, the blood pressure stabilized at 125/70-140/80 mmHg. Epidural meperidine produces selective blockade of the spinal opiate receptors and hence may block the nociceptive reflexes below the level of cord transection and prevent AH.

Nalbuphine and Althesin anaesthesia. A controlled double-blind comparison for cystoscopy.

In a double-blind, randomized study of 52 unpremedicated outpatients undergoing cytoscopy, either nalbuphine 20 mg or saline 2 ml was given intravenously immediately before induction of anaesthesia with Althesin 0.05 ml/kg. Increments of Althesin 0.5 ml were used to maintain anaesthesia. The patients received nalbuphine required less Althesin than those who received saline (p less than 0.001) for a similar duration of surgery, and there was less movement during anaesthesia (p less than 0.001). During surgery hyperventilation was observed in the patients who received saline; those who received

Reducing Clinical Waste in Flexible Cystoscopy: A Prospective Quality Improvement Study.

AIMS: Flexible cystoscopy is a very common procedure that relies heavily on single-use consumables, resulting in significant procedural waste. This study aimed to evaluate whether a targeted waste-segregation intervention could reduce clinical waste generation during flexible cystoscopy without compromising clinical practice. METHODS: A prospective audit of 50 consecutive diagnostic flexible cystoscopies performed at a tertiary urology centre was undertaken. Twenty-five procedures were evaluated before intervention, followed by structured contamination-based waste segregation training for cli

Bacteriuria After Diagnostic Flexible Cystoscopy Without Antibiotic Prophylaxis: A Prospective Study.

INTRODUCTION AND HYPOTHESIS: Flexible cystoscopy is well tolerated by women. Reprocessing of cystoscopes requires sterilization or, at least, high-level disinfection (HLD). This study aims to assess the incidence of bacteriuria after flexible cystoscopy using ortho-phthalaldehyde HLD. METHODS: This is a prospective study. Seventy-eight women undergoing diagnostic outpatient flexible cystoscopy were included. Exclusion criteria were microbiologically confirmed or acute urinary tract infection, significant urethral stricture, and bleeding disorders. Cystoscopes were reprocessed using ortho-phth

Managing Bladder-Neck Stenosis With "Cystoscopy and Proceed" Method: Our Experience.

INTRODUCTION: Bladder-neck stenosis (BNS) is characterized by obstruction of urine flow due to improper relaxation of the bladder neck. Accurate diagnosis through cystoscopy and treatment via bladder-neck incision (BNI) can restore urinary flow and reduce the risk of related complications. Our experience suggests that cystoscopy for diagnosis is underused due to various patient and physician-related factors. OBJECTIVE: The primary objective of this study was to evaluate the effectiveness of the "cystoscopy and proceed" method in managing BNS, measured by improvement in uroflowmetry (Qmax). Se

Cystoscopy-assisted percutaneous removal of impacted urethral stone: a case report.

BACKGROUND AND PURPOSE: Although cystolitholapaxy is possible in children, the instruments are not available everywhere. For tiny impacted urethral stone, an alternate technique is described. CASE REPORT: A 3-year-old boy came with retention of urine and dysuria. On examination: bladder was full, suprapubic region was tender, and a stone could be felt at posterior urethra. We planned push back and suprapubic cystolithotomy. After general anesthesia, the stone was pushed back by instillation of lignocaine jelly into the urethra. Then, a cystoscope was introduced, and a stone was found inside t

Multiple intravesical linear staples identified during surveillance cystoscopy after laparoscopic nephroureterectomy.

Laparoscopic nephroureterectomy has faced technical criticism secondary to the unknown effects of urine-exposed intravesical or intraureteral staples. Although not reported, staples exposed to urine theoretically create a nidus for encrustation and stone formation. We present a case of a 66-year-old man after left laparoscopic ureterectomy for ureteral malignancy. Surveillance cystoscopy at 6 months revealed a nearly complete intravesical titanium staple line without encrustation. Successful transurethral removal of all staples with standard cystoscopic graspers revealed no immediate, or delay
