Arthroscopic triangular fibrocartilage complex debridement using radiofrequency probes.

The initial results of using radiofrequency probes for debridement of a torn triangular fibrocartilage complex were studied in 20 patients with a mean age of 44 (range 27-56) years presenting with ulnar-sided wrist pain. On arthroscopic examination, 18 central and two radial triangular fibrocartilage complex tears were identified and debrided to a stable rim using radiofrequency probes. The mean follow-up was 22 (range 9-35) months. Seventeen patients experienced substantial pain relief. In three, the pain was unchanged. The mean flexion extension arc was 132 degrees , pronosupination arc 155

Elephantiasis nostras verrucosa successfully treated by surgical debridement.

BACKGROUND: Elephantiasis nostras verrucosa represents a rare group of cutaneous changes comprising dermal fibrosis, hyperkeratotic, verrucous, and papillomatous lesions after chronic secondary, nonfilarial lymphedema. There is no standard treatment for this rare cutaneous manifestation. OBJECTIVE: This article describes debridement that is helpful when performed in addition to physiotherapy. METHODS: We present a patient who was treated by shaving the verrucous area with a blade of a freehand knife and by subsequent abrading of the mossy area using a motor-powered grinder. RESULTS: Reepith

Staphylococcal neonatal necrotising fasciitis: survival without radical debridement.

Necrotising fasciitis is a serious infection associated with high morbidity and mortality. The conventional treatment is radical resection of the involved area with antibiotic as well as intensive supportive therapy. We describe a case of extensive truncal necrotising fasciitis in a neonate, secondary to Staphylococcus aureus infection successfully treated with intensive antibiotic therapy and multi-organ support, followed by fasciotomies, drainage and betadine irrigation. The successful outcome without radical resection could be due to the viability of superficial skin, S aureus as the causat

Spontaneous streptococcal gangrenous myositis: survival with early debridement.

Spontaneous gangrenous myositis caused by Streptococcus pyogenes is usually fatal, but no longer uniformly so. There appears to be a spectrum of disease due to beta-hemolytic streptococci, from necrotizing fasciitis to pyomyositis to spontaneous gangrenous myositis. Survival is possible with early surgical debridement, reexploration at 24 to 36 hours, and intensive supportive care.

Retroperitoneal Laparoscopic Debridement and Drainage for Pancreatic Abscess.

In patients with severe necrotizing pancreatitis, pancreatic necrosis and secondary infection of surrounding tissues can quickly spread to the whole retroperitoneal space. Treatment of pancreatic abscess complicating necrotizing pancreatitis is difficult and has a high mortality rate. The well-accepted treatment strategy is early debridement of necrotic tissues, drainage, and postoperative continuous retroperitoneal lavage. However, traditional open surgery has several disadvantages, such as severe trauma, interference with abdominal organs, a high rate of postoperative infection and adhesion,

Diabetic Foot Ulcers: A Review of Debridement Techniques.

Diabetic foot ulcers (DFUs) are a prevalent complication of diabetes mellitus (DM) and lead to significant morbidity and mortality. Patients with DM have a lifetime risk of DFUs as high as 34%. The pathogenesis of DFUs is multifactorial, and the most common underlying causes are poor glycemic control, peripheral neuropathy, peripheral vascular disease, foot deformity, and poor foot care. Diabetic lower-extremity complications are also a significant burden in terms of healthcare costs. In the United States alone, the direct cost of diabetic foot care has been estimated to be $8,659 per patient,

[Arthroscopic debridement of the extensor carpi radialis brevis].

Surgical treatment of lateral epicondylitis is reserved for patients who, despite extensive conservative therapy, do not experience satisfactory relief of symptoms. As an alternative to the open procedure, arthroscopic debridement of the extensor carpi radialis brevis (ECRB) muscle is a simple and standardized procedure. The arthroscopic approach also enables the additional treatment of intra-articular pathologies such as loose bodies or osteochondral lesions. After diagnostic arthroscopy, the attachment of the ECRB is visualized via the anteromedial portal, so that under visual control the de

Arthroscopic Treatment of Lateral Epicondylitis: Tenotomy Versus Debridement.

PURPOSE: To compare the outcome of 2 arthroscopic techniques for treating recalcitrant lateral epicondylitis. METHODS: The study included patients undergoing arthroscopic treatment of lateral epicondylitis during 2 different time periods: April 2005 to October 2007 (tenotomy) and May 2009 to June 2010 (debridement). By using a patient-administered form, baseline information including QuickDASH (disabilities of the arm, shoulder and hand) score (primary outcome), visual analog scale (VAS) of pain, and VAS of function was recorded prospectively. To have the same follow-up period of minimum 4 ye

Successful management of cutaneous mucormycosis by delaying debridement.

Mucormycosis is a frightening medical condition which has baffled clinicians all over the world. Cutaneous mucormycosis is, in particular, extremely invasive, leading to high rates of morbidity and mortality. Timely intervention with antifungal drug Amphotericin B and early radical debridement are keys for favorable outcome.Three consecutive patients died of cutaneous mucormycosis despite being treated with Amphotericin B and an early extensive debridement. With disappointing results in these patients, the treatment protocol was changed. Instead of early aggressive surgical intervention, the d

Initial extremity war wound debridement: a multidisciplinary consensus.

A joint meeting of the Limb Trauma and Wounds Working Groups resulted in the establishment of 29 consensus recommendations for the conduct of initial extremity war wound debridement. Pre-operative, operative and post-operative phases of debridement were considered along with wound irrigation and dressings. Wounds where a different surgical approach is required, such as superficial soft tissue wounds at one end of the spectrum and complex wounds sustained in close proximity to explosions at the other, were also discussed. The recommendations represent the consensus opinion of orthopaedic, vascu

Exploring debridement options for chronic venous leg ulcers.

The presence of devitalized tissue in a wound is a common problem facing practitioners and is regarded by many as a major inhibitory factor in the wound-healing process and can act as a focus for microbial proliferation. Therefore, for wound healing to proceed in a logical and ordered fashion, it follows that any necrotic material should be cleared from the wound bed. Wound bed preparation is now recognized as crucial to facilitating ordered restoration and regeneration of damaged tissue. However, there is a clear lack of good clinical evidence to support available wound debridement options, p

Arthroscopic debridement after intraarticular low-velocity gunshot wounds.

An unexpected degree of joint contamination was detected during arthroscopic debridement of three intraarticular, low-velocity gunshot wounds to the knee. When bullet wounds traverse a joint, it is recommended that clothing be inspected for fabric defects and that arthroscopy be considered for the surgical debridement.

Arthroscopic Debridement: A Conservative but Effective Surgery in the Ankle.

Initially deemed unsuitable for arthroscopy due to complex anatomy, the ankle joint became accessible following Takagi's pioneering work in 1939. Since then, ankle arthroscopy has advanced significantly and can be employed for various procedures although it is most commonly used for procedures like arthroscopic debridement of the ankle joint. For the purpose of this review, arthroscopic debridement of the anterior ankle impingement syndrome will be discussed. This syndrome involves pain during dorsiflexion of the ankle due to soft tissue or bony impingement. The anterolateral variant, often fr

The wound debrider: a new monofilament fibre technology.

Debridement is a basic necessity to induce the functional process of tissue repair, especially in chronic wounds. In this pilot study the authors used a new debrider technology with specific monofilament fibres in a unique texture to evaluate its efficacy, safety and tolerability. In eleven patients, exhibiting all types of wound-associated debris (biofilms, slough, necrotic crusts and hyperkeratotic plaques), the debrider, wetted with physiological solution, was wiped without specific force over the wound for about 2-4 minutes. This led to removal of almost all debris leaving healthy granulat

[Maggots in the wound, debridement, disinfection and wound healing].

An 87-year-old man had a longstanding untreated large basosquamous carcinoma on his right ear. He was admitted to the emergency department at our hospital. A large portion of the auricle had perished, together with part of the tumour. Surgery was planned but two days before, the patient complained of an irritating loud noise in his ear. We discovered this was caused by maggots in his external acoustic meatus: myiasis. Dozens of maggots were removed. A striking finding was that the smell of the wound had disappeared and that the wound was much cleaner, with a reddish aspect and less necrosis. T
