[Approach to the patient in vegetative state. Part II: differential diagnosis].

A prerequisite to the diagnosis of vegetative state is the exclusion of apparently similar syndromes, in which the patient retains the consciousness partially or even completely. Some syndromes are not separate nosological entities and should be abandoned: the apallic state, the neocortical death, the decerebrate and decorticate state, the alpha-coma, the vigil or prolonged or irreversible coma are among them. Three conditions deserve special consideration. The term locked-in syndrome describes a patient completely paralysed and mute, but fully conscious, and is usually caused by ischemic lesi

Separable brainstem and forebrain contributions to ultrasonic vocalizations in infant rats.

Competing views persist concerning the functional significance of ultrasonic vocalizations (USVs) emitted by infant rats. One perspective holds that USVs result from an emotional state of fear and anxiety, the adult expression of which depends in part on forebrain mechanisms. Here the authors examine whether pups lacking forebrain input are capable of emitting USVs. Aspirations of neocortex and hippocampus or precollicular decerebrations were performed on 8-day-old rats. After the rats recovered, USV responses were recorded for 10 min at room temperature (Phase 1) followed by enhanced cooling

Complete recovery of consciousness in a patient with decorticate rigidity following cardiac arrest after thoracic epidural injection.

A 46-yr-old man with dysaesthesia (burning sensation) following herpes zoster in the left upper chest region was treated with a single thoracic (T2/T3) epidural injection (1.0% lidocaine 3 ml + 0.125% bupivacaine 3 ml) as an outpatient. Twenty minutes after the injection, a nurse noticed the patient to be unconscious with dilated pupils, apnoea and cardiac arrest. Following immediate cardiopulmonary resuscitation, the patient was treated with an i.v. infusion of thiamylal sodium 2-4 mg kg-1 h-1 and his lungs were mechanically ventilated. When the patient developed a characteristic decorticate

The C fibre reflex of the cat urinary bladder.

1. Reflexes evoked in bladder parasympathetic neurones by electrical stimulation of bladder C afferent fibres were studied in cats anaesthetized with alpha-chloralose. The responses were compared with the ordinary micturition reflex evoked by low-threshold Adelta afferents from bladder mechanoreceptors and mediated by a spino-ponto-spinal reflex pathway. 2. The bladder was catheterized for fluid instillations and pressure recordings. Efferent reflex discharges were recorded from the cut central end of a small distal bladder branch of the pelvic nerve. The remaining bladder pelvic nerve branche

Differing activities of medullary respiratory neurons in eupnea and gasping.

Our purpose was to compare further eupneic ventilatory activity with that of gasping. Decerebrate, paralyzed, and ventilated cats were used; the vagi were sectioned within the thorax caudal to the laryngeal branches. Activities of the phrenic nerve and medullary respiratory neurons were recorded. Antidromic invasion was used to define bulbospinal, laryngeal, or not antidromically activated units. The ventilatory pattern was reversibly altered to gasping by exposure to 1% carbon monoxide in air. In eupnea, activities of inspiratory neurons commenced at various times during inspiration, and for

Hypothalamic-midbrain dysregulation syndrome: hypertension, hyperthermia, hyperventilation, and decerebration.

Certain decerebrate lesions of brain stem or hypothalamus induce pharmacologically reversible hypertension and hyperthermia in animals. We observed three young patients with episodic decerebration, hyperthermia, hypertension, and hyperventilation during recovery from comas of different etiologies. The shared pathology on neurologic examinations and computed tomographic scans was hypothalamic-mesencephalic dysfunction, suggesting a diencephalic-brain-stem disconnection syndrome or brain-stem release mechanism. Propranolol was the most effective drug tested, but only two patients responded, one

Rigidity in rats due to radio frequency decerebration and effects of chlorpromazine and mephenesin.

Bilateral radio frequency (RF) lesions of the midbrain were produced in an attempt to establish a non-bleeding method of producing decerebrate rigidity in rats. Marked extensor rigidity occurred with a high reproducibility in hindlimbs without the appearance of voluntary movement. Stretch reflex tension induced by repetitive dorsiflexion of the hindfeet was employed as a measure of the intensity of rigidity. Chlorpromazine-HCl (0.1-1 mg/kg, i.v.) and mephenesin (10-50 mg/kg, i.v.) which depress intercollicular decerebrate rigidity, reduced the RF decerebrate rigidity. These results suggest tha

Prolonged time course for vibratory suppression of stretch reflex in the decerebrate cat.

We studied the effects of longitudinal tendon vibration on the stretch reflex of the soleus and gastrocnemius muscles in 11 decerebrate cats. Vibration was applied at amplitudes (40-80 micrometer) and frequencies (120-250 HZ) sufficient to provide a strong tonic vibration reflex. In keeping with previous reports, we found that during an established tonic vibration reflex, the force and emg response to superimposed ramp and hold stretch are largely suppressed. This suppression is most obvious during the dynamic phase of stretch where it gives rise to a complex force response resembling that of

Hyperbaric oxygen therapy for hypoxic-ischemic encephalopathy in non-fatal drowning.

Paroxysmal autonomic instability syndrome with dystonia (PAISD) is a possible complication that worsens the prognosis of hypoxic-ischemic encephalopathy related to non-fatal drowning. There are case reports of hyperbaric oxygen (HBO2) therapy enhancing recovery in such cases. We report a case of a 5-year-old boy admitted to the Pediatric Intensive Care Unit after a non-fatal drowning. He was transferred under mechanical ventilation and sedation, with hemodynamic instability and hypothermia. On admission he had a Glasgow Coma Score of 6. On the fifth day of admission he presented episodes of dy

Organisation of sensitisation of hind limb withdrawal reflexes from acute noxious stimuli in the rabbit.

Spatial aspects of central sensitisation were investigated by studying the effects on three hind limb withdrawal reflexes of an acute noxious stimulus (20 % mustard oil) applied to a number of locations around the body in decerebrate and in anaesthetised rabbits. Reflex responses to electrical stimulation of the toes were recorded from the ankle flexor tibialis anterior (TA) and the knee flexor semitendinosus (ST), whereas responses to stimulation of the heel were recorded from the ankle extensor medial gastrocnemius (MG). In non-spinalised, decerebrated, pentobarbitone-sedated preparations, f

A clinical conundrum: unilateral decerebrate rigidity.

A case is described in which, following a motorcycle accident resulting in a closed head injury, a young woman was rendered unconscious. On examination she was found to have unilateral decerebrate rigidity. Further clinical and radiological examination revealed that there was no damage to the limbs, vertebral column, or spinal cord. Readers are expected to be able to make an anatomical diagnosis on the basis of the facts presented.

Characteristic behavior of the respiratory muscles, esophagus, and external anal and urethral sphincters during straining, retching, and vomiting in the decerebrate dog.

To ascertain differences in the brainstem pattern generators for straining and retching, discharges of 230 respiratory single motor units from the intercostal nerves and discharges of the nerves to accessory respiratory muscles, the esophagus, and external anal and urethral sphincters were observed during straining, retching, and vomiting (fictive expulsion) in decerebrate and paralyzed dogs. Straining and retching were identified with coactivation of the phrenic nerve and the nerve to the rectus abdominis, which was elicited by distension of the rectum and stomach, respectively. Possible role of brain stem respiratory neurons in mediating vomiting during space motion sickness.

Vomiting is produced primarily by the coordinated action of the respiratory muscles. The object of this study was to determine if brain stem expiratory neurons control abdominal muscle activity during vomiting. We recorded the activity of 27 ventral respiratory group (VRG) expiratory neurons, which are known to be of primary importance for control of abdominal muscle activity during respiration. During fictive vomiting produced by emetic stimulants in decerebrate, paralyzed cats, only one-third of these neurons had the appropriate firing pattern to contribute to abdominal muscle activity durin

Effects of carbamazepine on muscle tone in the decerebrate cat.

Previous studies, which have demonstrated that carbamazepine (CBZ) possesses direct muscle spindle suppressant activity, suggest that CBZ may have therapeutic value in the treatment of hypertonic disorders characterized by high fusimotor drive. The effects of CBZ on motor tone in the midcollicular decerebrate cat were therefore examined. CBZ, in a concentration-dependent manner, reduced muscle tone, as measured by the force necessary to overcome hyperextension. Significant depression was first observed at a serum concentration of about 25 micrograms/ml, whereas 50% depression occurred at about

Halothane depresses the response of carotid body chemoreceptors to hypoxia and hypercapnia in the cat.

Halothane is known to inhibit the ventilatory responses to hypoxia and hypercapnia. In order to determine whether this inhibition was mediated by peripheral chemoreceptors, the authors measured the effect of halothane on the response of carotid body chemoreceptors to these stimuli. Cats were decerebrated under brief halothane anesthesia, paralyzed, and ventilated. Chemoreceptor activity was recorded from single- or few-fiber preparations of carotid sinus nerve, and the inspiratory drive was recorded from the whole phrenic nerve. Steady-state responses were measured at three levels of CO2 tensi

Inhibition of reflex vagal bradycardia by a central action of 5-hydroxytryptophan.

1 Vagally mediated reflex bradycardia was elicited in spinal cats with intravenous pressor doses of noradrenaline. Administration of 5-hydroxytryptophan (1.5 and 3 mg total dose) into the fourth cerebral ventricle reduced the reflex bradycardia. 2 Inhibition of central amino acid decarboxylase with R044602 prevented the effects of 5-hydroxytryptophan. After intravenous administration of 5-hydroxytryptophan, vagal reflex bradycardia was not affected. 3 Results suggest that 5-hydroxytryptophan acts in the CNS to inhibit baroreceptor-mediated vagal reflex bradycardia and that action is mediated v
