Milestones, barriers and beacons: Shared decision making in Canada inches ahead.

Canada's approach to shared decision making (SDM) remains as disparate as its healthcare system; a conglomerate of 14 public plans - ten provincial, three territorial and one federal. The healthcare research funding environment has been largely positive for SDM because there was funding for knowledge translation research which also encompassed SDM. The funding climate currently places new emphasis on patient involvement in research and on patient empowerment in healthcare. SDM fields have expanded from primary care to elder care, paediatrics, emergency and critical care medicine, cardiology, n

Shared decision-making in pediatric otolaryngology: Parent, physician and observational perspectives.

OBJECTIVE: To describe physician and parent behavior during pediatric otolaryngology surgical consultations, and to assess whether perceptions of shared decision-making and observed behavior are related. METHODS: Parents of 126 children less than 6-years of age who underwent consultation for adeontonsillectomy or tympanostomy tube insertion were prospectively enrolled. Parents completed the Shared Decision-Making Questionnaire-Patient version (SDM-Q-9), while surgeons completed the Shared Decision-Making Questionnaire-Physician version (SDM-Q-Doc) after the consultation. Visits were video-rec

A Parent-Facing Decision Aid About Childhood Vaccination: Co-development Study Adapting a Shared Decision-Making Approach to Support Evidence-Guided Decision-Making.

BACKGROUND: Childhood vaccination rates are declining, and more parents are becoming hesitant about vaccinating their children. Higher levels of vaccine hesitancy occur among parents with lower levels of educational attainment, who are also likely to have lower levels of health literacy. OBJECTIVE: We aimed to develop a web-based decision aid, considered a shared decision-making (SDM) approach, to inform and support Dutch parents in their decision-making about routine childhood vaccination, prioritizing the needs of hesitant parents and those with lower health literacy. We adapted the traditi

Shared Decision-Making in IPMN of the Pancreas: A Framework for Surgical Decisions Under Uncertainty.

BACKGROUND: Management of intraductal papillary mucinous neoplasms (IPMNs) requires choosing between surgical resection and active surveillance, yet current diagnostic tools cannot reliably distinguish which lesions harbor high-grade dysplasia or invasive cancer. As a result, many patients undergo major pancreatic surgery for low-grade disease, while others are observed until progression emerges. This uncertainty contributes to substantial variation in surgeon risk estimates, intervention thresholds, and treatment recommendations. Patients likewise differ in their tolerance for cancer risk, vi

Beyond Facilitation: Reconceptualising the Psychiatric Nurse's Role in Shared Decision-Making.

From the perspective of psychiatric nursing, this discussion paper reconceptualises the role of psychiatric nurses in shared decision-making (SDM) in mental health care. Although psychiatric nurses are commonly described as facilitators, educators, advocates, coordinators, or supporters of SDM, these representations may underrecognise how knowledge developed through psychiatric nursing practice contributes to decision-making. Drawing on nursing epistemology and contemporary understandings of SDM, this paper conceptualises psychiatric nursing knowing as relational, interpretive, contextual, and

Shared decision-making in ongoing psychotherapy: Negotiating frames, preferences and meaning.

Objective: Shared decision-making (SDM) is pivotal in healthcare to enhance patient involvement in treatment decision-making. In mental health, SDM is increasingly also recognized as a key approach for tailoring psychotherapy to patients' individual needs. Although research on SDM in mental health care has documented the impact of SDM on patient involvement and activation, little is known about the implications of SDM in psychotherapy. Method: To investigate how SDM is enacted in the therapeutic encounter, this study draws on audio-recordings of ten SDM dialogues in psychotherapy. Patients in

Calcific Constrictive Tuberculous Pericarditis in Pregnancy: A Shared Decision-Making Approach.

BACKGROUND: Calcific constrictive tuberculous pericarditis is rare in the United States and even more uncommon in pregnancy, but it carries significant morbidity and requires individualized, multidisciplinary counseling. CASE SUMMARY: A 40-year-old woman with a history of tuberculosis presented at 16 weeks' gestation with symptomatic calcified constrictive pericarditis. Despite medical management, she developed progressive hemodynamic compromise. Given her ongoing symptoms and preference to avoid pericardiectomy, she elected for second-trimester termination after shared decision-making. DISC

Divergence in Perceptions of Shared Decision Making in Pediatric Neuromuscular Scoliosis.

UNLABELLED: Background. Shared decision making (SDM) is a promising but poorly understood approach toward improving care for children with medical complexity. The objective of this study is to assess the quality of SDM using self-reported and observer-based measures in pediatric orthopedic clinic visits for neuromuscular scoliosis (NMS) surgical treatment decisions. Methods. We recruited 18 children with NMS at ≥8 y old, a Cobb angle of ≥50°, and no prior spine surgery at a quaternary children's hospital; their caregivers; and spine surgeons (n = 3). We video-recorded visits and administered p

Factors influencing shared decision-making in microtia reconstruction: A systematic review.

BACKGROUND: The decision to pursue microtia reconstruction is highly personalized, requiring shared decision-making (SDM). This systematic review analyzes patient and caregiver motivations and priorities guiding the SDM process to help inform clinician-led discussions. METHODS: A systematic review evaluated studies from 1/1/1990 to 4/25/2025. Included articles focused on SDM in pediatric microtia reconstruction. Descriptive analyses were performed. RESULTS: Five articles met criteria, including 235 study participants: 69 patients (ages 3 to 17 at time of surgery), 134 caregivers, and 32 heal

Shared decision making in Eosinophilic esophagitis: Integrating physician and patient perspectives.

BACKGROUND AND OBJECTIVES: Shared decision making (SDM) is a patient-centered approach for conditions where multiple, preference-sensitive treatment options exist and there is no single best choice. Eosinophilic esophagitis (EoE), an increasingly prevalent chronic immune-mediated disease, offers an example of how patients and physicians use SDM to navigate the challenging tradeoffs of weighing pharmacologic and dietary therapies. We aimed to identify communication challenges for SDM in EoE care from physician and patient perspectives and to explore how patient treatment preference archetypes i

Japanese Patients' Perceptions of Shared Decision-Making in Renal Replacement Therapy.

INTRODUCTION: Shared decision-making (SDM) is a key process in selecting renal replacement therapy (RRT). This study analyzed SDM perceptions, preferences, as well as patient- and facility-level factors influencing SDM perception among Japanese patients with chronic kidney disease (CKD) who selected RRT. METHODS: We conducted a cross-sectional survey of 475 adult patients with CKD from 49 medical facilities. SDM awareness and recognition, preferences for SDM timing and frequency, discussion content, and desired professional involvement were assessed. Patient- and facility-level factors associ

Shared Decision-Making in Chronic Stable Angina and Severe Aortic Stenosis.

Shared decision-making (SDM) is the process that promotes the agreement between treatment choices based on patient values and medical knowledge. For chronic stable angina, this involves facilitating decision-making between percutaneous coronary intervention and guideline-directed medical therapy according to personalized risks and benefits associated with these 2 treatment options. In severe aortic valve stenosis, SDM again comes to play, this time to select between transcatheter aortic valve implantation and surgical aortic valve replacement. The implementation of SDM in South Korea faces cha

Development of the World Federation of Hemophilia Shared Decision-Making Tool.

INTRODUCTION: The use of shared decision-making (SDM) in clinical settings is becoming more prevalent. The evolving and increasingly complex treatment landscape of haemophilia management has augmented the need and desire for SDM between patients and their healthcare team. SDM tools have been used in other chronic conditions and can be an effective form of education for patients and clinicians. AIM: The World Federation of Hemophilia (WFH) partnered with people with haemophilia (PWH), patient advocacy groups, and healthcare practitioners to form an expert working group to develop an educationa
