Clinical performance of a rule-based decision support system for mechanical ventilation of ARDS patients.

We developed a clinical decision support system--ventilation protocols--that managed tidal volume and ventilator rate settings during mechanical ventilation of patients with the Adult Respiratory Distress Syndrome (ARDS). We applied these protocols for a total of 10,903 hours in 40 ARDS patients. The clinical staff suspended the protocols for only 5% of the total application time due to medical procedures, surgeries, transient clinical problems not addressed by the protocols, or because of attending physician request. Of 3,148 instructions generated by the ventilation protocols, the clinical s

Health system implementation of clinical decision support to optimize respiratory pathogen profile utilization.

UNLABELLED: Overuse of respiratory pathogen profiles (RPPs) in immunocompetent patients offers limited clinical benefit and may contribute to unnecessary hospital costs. Evidence supports RPP use primarily in severely ill, immunosuppressed, or pediatric patients, while smaller, targeted tests are more appropriate for routine cases . The goal of this study was to evaluate the impact of a clinical decision support (CDS) intervention on RPP utilization and associated reagent costs. We reviewed RPP utilization across the health system and implemented a CDS tool with pre-approved clinical indicatio

LungGPT: A unified multimodal system for interpretable diagnosis and clinical decision support of respiratory diseases.

Respiratory diseases cause significant morbidity, yet diagnosis remains labor intensive and dependent on physician expertise. Here, we present LungGPT, a unified multimodal system trained on 147 million tokens of domain-specific electronic health records from 125,917 participants. LungGPT comprises two modules: LungGPT-Dx for respiratory disease diagnosis and early warning of critical illness, and LungGPT-Ex for interpretable diagnostic reasoning and treatment recommendations. In large-scale evaluations, LungGPT-Dx achieves a macro-average area under the curve (AUC) of 0.852 (95% confidence in

Reduction of Intravenous Antihypertensives through Clinical Decision Support in a Large Safety Net System.

BACKGROUND: Asymptomatic severe hypertension (also known as hypertensive urgency) is frequently encountered in the hospital. Previous evidence suggests that management with one-time doses of intravenous (IV) antihypertensives may increase adverse events. Despite this, single-dose treatment remains common in the emergency department and inpatient settings. METHODS: This quality initiative was launched at New York City Health + Hospitals, the largest safety net hospital system in the United States. The initiative involved two changes to electronic orders for IV hydralazine and IV labetalol: a n

Development and implementation of a clinical decision support system tool for the evaluation of suspected monkeypox infection.

Monkeypox virus was historically rare outside of West and Central Africa until the current 2022 global outbreak, which has required clinicians to be alert to identify individuals with possible monkeypox, institute isolation, and take appropriate next steps in evaluation and management. Clinical decision support systems (CDSS), which have been shown to improve adherence to clinical guidelines, can support frontline clinicians in applying the most current evaluation and management guidance in the setting of an emerging infectious disease outbreak when those guidelines are evolving over time. How Implementation Context Shapes Clinician Acceptance and Engagement with Guideline-Based Clinical Decision Support Systems in Primary Care: A Qualitative Study.

BACKGROUND: Guideline-based clinical decision support (CDS) tools have become central to promoting evidence-based, high-quality primary care. Despite widespread implementation, clinician adoption and sustained use remain inconsistent. Existing research has typically examined technological usability or organizational implementation factors separately, providing limited understanding of how these influences interact in real-world primary care settings. This study examined how organizational implementation context and clinician technology acceptance jointly shape adoption and sustained use of gui

Clinical decision support systems' effect in reducing medication errors and adverse drug events: an expanded review from Making Healthcare Safer IV.

Clinical decision support systems (CDSSs) are used to provide alerts and recommendations on prescribing and order verification with the intent of reducing the frequency and harm of medication errors (MEs) and adverse drug events (ADEs). This review assessed recent evidence on the effects of CDSSs on reducing MEs and ADEs. Related implementation outcomes such as alert overrides and unintended consequences of use were also assessed, and literature around the effective implementation of CDSSs was summarised. The review followed processes proposed by the Agency for Healthcare Research and Quality

Prototyping and evaluation of a clinical decision support system in long-term care in German nursing homes.

Demographic change in Germany is increasing the demand for professional long-term care. Given rising requirements and simultaneous staff shortages, the need for digital systems is growing to effectively support nursing staff and mitigate staffing bottlenecks. This article introduces a hybrid clinical decision support system (CDSS) designed to generate actionable intervention recommendations by combining structured data and narrative nursing documentation. The CDSS consists of three interacting components: (1) knowledge models that encode state-of-the-art nursing knowledge, (2) a rule-based exp

Clinical evaluation of a computer-assisted decision support and documentation system for the primary care of polytrauma patients.

BACKGROUND: Polytrauma management demands rapid, high-stakes decisions under severe time pressure, increasing the risk of error and deviation from evidence-based protocols. Computer-assisted decision support systems (CDSS) may counter these risks by improving adherence to guidelines and reducing cognitive strain. We developed TraumaFlow, a CDSS integrating the German S3 guideline and ATLS® principles, and evaluated its impact in real-world trauma care. METHODS: In a prospective study at a level 1 trauma center, 30 shock room cases were managed with TraumaFlow alongside the standard paper prot
