Transcatheter closure for decompression sickness with a patent foramen ovale: A case report.

A patent foramen ovale is one of the predisposing factors of neurotic decompression sickness. Transcatheter closure of a patent foramen ovale is effective in the secondary prevention of decompression sickness associated with intracardiac shunt. The size of the umbrella should not be limited to the diagnosis of a patent foramen ovale or an atrial septal defect but should be determined by the supporting force of the soft margin of the atrial septum. The surgical method of patent foramen ovale closure is the same as that of the closure of an atrial septal defect, but the closure umbrella of a pat

Relationship between clinical and radiologic findings of spinal cord injury in decompression sickness.

BACKGROUND: Decompression sickness may involve the central nervous system. The most common site is spinal cord. This study was conducted to determine the relationship between magnetic resonance(MR) imaging findings of spinal damage. METHODS: We conducted a retrospective review of 12 patients (male=10, female=2) who presented with spinal cord symptoms. We investigated their clinical features, neurological findings and radiologic findings. RESULTS: The depth and bottom time of the dive were 34.5 meters (range 22-56) and 22.7 minutes (range 10-55) respectively. Most divers ascended within appro

Pathophysiology of inner ear decompression sickness: potential role of the persistent foramen ovale.

Inner-ear decompression sickness (inner ear DCS) may occur in isolation ('pure' inner-ear DCS), or as part of a multisystem DCS presentation. Symptoms may develop during decompression from deep, mixed-gas dives or after surfacing from recreational air dives. Modelling of inner-ear inert gas kinetics suggests that onset during decompression results from supersaturation of the inner-ear tissue and in-situ bubble formation. This supersaturation may be augmented by inert gas counterdiffusion following helium to nitrogen gas switches, but such switches are unlikely, of themselves, to precipitate in

Case report on a diver with type II decompression sickness and viral meningitis.

A 33-yr-old man came to the emergency department with the chief complaint of a severe headache and decreased sensation in his right hand following a deep dive on scuba. Physical examination before recompression treatment was remarkable only for hypesthesia on the right hand. We diagnosed type II decompression sickness and the patient underwent standard recompression therapy. The patient experienced near-complete resolution of his symptoms, his only residual complaint being that of neck pain with head movement. To investigate other causes of headache, a computed tomography of the head was perfo

Gene expression of Decompression Sickness-resistant rats through a miRnome/transcriptome crossed approach.

Susceptibility to decompression sickness (DCS) is characterized by a wide inter-individual variability, the origins of which are still poorly understood. We selectively bred rats with at least a 3-fold greater resistance to DCS than standard rats after 6 generations. In order to better understand DCS mechanisms, we compared the static genome expression of these resistant rats from the 10th generation to their counterparts of the initial non-resistant Wistar strain, by a microarray transcriptomic approach coupled and crossed with a PCR plates miRnome study. Thus, we identified differentially ex

Decompression sickness followed by diabetic ketoacidosis and sepsis shock: an unusual case report.

Decompression sickness (DCS) is caused by abrupt changes in extracorporeal pressure with varying severity. Symptoms range from mild musculoskeletal pain to severe organ dysfunction and death, especially among patients with chronic underlying disease. Here, we report an unusual case of a 49-year-old man who experienced DCS after a dive to a depth of 38 meters. The patient's symptoms progressed, starting with mild physical discomfort that progressed to disturbance of consciousness on the second morning. During hospitalization, we identified that in addition to DCS, he had also developed diabetic

Bubbles in the skin microcirculation underlying cutis marmorata in decompression sickness: Preliminary observations.

INTRODUCTION: The cutaneous form of decompression sickness (DCS) known as cutis marmorata is a frequent clinical presentation. Beyond a general acceptance that bubbles formed from dissolved inert gas are the primary vector of injury, there has been debate about pathophysiology. Hypotheses include: 1) local formation of bubbles in the skin or its blood vessels; 2) arterialisation of venous bubbles across a right to left shunt (RLS) with local amplification in bubble size after reaching supersaturated skin via the arterial circulation; and 3) passage of arterialised venous bubbles to the cerebra

Delayed hepatobiliary injury in a decompression sickness patient after scuba diving: case report.

We report here the first case of liver injury in a 51-year-old man following a dive to a depth of 40 meters. He presented with typical neurological symptoms affecting the lower limbs. Five days later, he experienced delayed abdominal pain, followed by rapidly progressive liver and adjacent organ injury due to air emboli in the intrahepatic portal vein. He received supportive care and hyperbaric therapy with a U.S. Navy Treatment Table 6 and recovered. Decompression sickness is a disease of protean manifestations. More information about venous gas emboli may be useful for better assessing decom

MRI features of spinal cord decompression sickness presenting as a Brown-Sequard syndrome.

Decompression sickness often manifests as central nervous system impairment. We report a 49-year-old woman who developed an unusual case of spinal cord decompression sickness presenting as complete Brown-Sequard syndrome. Initial MRI revealed increased signal intensity in the left side of the cervical cord at the level of C2-C3. A second MRI at 10 days post-injury showed signal abnormalities corresponding to an infarction in the posterior spinal artery territory. After two weeks of intensive treatment with various HBOT regimens, the clinical outcome was still poor, but at six months after the

Somatosensory evoked potentials and neuraxial blood flow in central nervous system decompression sickness.

Seven adult, conditioned dogs were anesthetized with pentobarbital sodium and prepared for measurement of upper lumbar, mid-thoracic and lower cervical spinal evoked potentials (SEPs), cortical evoked potential (CEP), and aortic, right ventricular, and cerebrospinal fluid pressures. Following preparation, one animal was monitored by means of repeated evoked potentials for 2 h, at which time a [14C]iodoantipyrine autoradiographic blood flow study was performed. The 6 other animals were exposed to simulated dives in a compression chamber while anesthesia was maintained through a chamber penetrat

Preventive effect of rosiglitazone on liver injury in a mouse model of decompression sickness.

BACKGROUND AND AIMS: Severe decompression sickness (DCS) is a multi-organ injury. This study investigated the preventive effects of rosiglitazone on liver injury following rapid decompression in mice and examined the underlying mechanisms. METHODS: Mice were randomly divided into four groups: a control group, vehicle group, and rosiglitazone (5 and 10 mg·kg⁻¹) groups, the latter three being exposed to a pressure of 911 kPa. Haematoxylin and eosin staining, plasma levels of alanine transaminase (ALT), aspartate transaminase (AST) and lactate dehydrogenase and blood cell counts were used to eva

Decompression sickness in a vegetarian diver: are vegetarian divers at risk? A case report.

We present a case of a diver who suffered decompression sickness (DCS), but who also was a strict vegetarian for more than 10 years. He presented with symptoms of tingling of both feet and left hand, weakness in both legs and sensory deficits for vibration and propriocepsis after two deep dives with decompression. The initial clinical features of this case were most consistent with DCS, possibly because of a vulnerable spinal cord due to cobalamin deficiency neuropathy. This case illustrates the similarities between DCS and a clinically defined vitamin B12 deficiency. The pathophysiology of vi

Cutaneous decompression sickness after an air dive with oxygen breathed during decompression in a commercial diver with a persistent foramen ovale.

A 43-year-old commercial diver had cutaneous decompression sickness after a dive to 17 metres of seawater for 160 minutes breathing air with transfer under pressure and oxygen breathed during decompression in a dry chamber. He had worked as a commercial diver for 16 years without previous problems. A bubble contrast transthoracic echocardiogram showed a large atrial right-to-left shunt. His persistent foramen ovale (PFO) was closed using a transcatheter technique and he has returned to commercial diving. As far as we are aware, shunt-mediated decompression sickness has not been reported previo

Fit for diving after musculoskeletal decompression sickness: how to detect and manage bone lesions?

Musculoskeletal decompression sickness (MS DCS) is a clinical condition characterized by joint pain following scuba diving. Recent studies have shown a potential link between MS DCS and bone lesions, including dysbaric osteonecrosis. This article highlights the importance of early detection and management of bone damage in MS DCS patients. It is recommended that a specialist diving doctor be consulted for a comprehensive assessment to ensure an accurate diagnosis and treatment plan. Ordering a joint MRI two months after the accident is the best way to detect the presence of intraosseous edema,

Shunt-mediated decompression sickness in a compressed air worker with an atrial septal defect.

We report a compressed air worker who had diffuse cutaneous decompression sickness with pain in his left shoulder and visual disturbance characteristic of migraine aura after only his third hyperbaric exposure. The maximum pressure was 253 kPa gauge with oxygen decompression using the Swanscombe Oxygen Decompression Table. He was found to have a very large right-to-left shunt across a 9 mm atrial septal defect. He had transcatheter closure of the defect but had some residual shunting with release of a Valsalva manoeuvre. Thirty-two other tunnel workers undertook the same pressure profile and a
