BACKGROUND: Decompressive craniectomy (DC) is a surgical intervention used following traumatic brain injury to prevent or alleviate raised intracranial pressure. However the clinical effectiveness of the intervention remains in doubt. The location of the craniectomy (unilateral or bifrontal) might be expected to change the brain deformation associated with the operation and hence the clinical outcome. As existing methods for assessing brain deformation have several limitations, we sought to develop and validate a new improved method.
METHODS: Computed tomography (CT) scans were taken from 27

INTRODUCTION: Cranial reconstruction after decompressive craniectomy (DC) has been shown to be associated with a relatively high complication rate (16.4%-34%) compared with standard neurosurgical procedures (2%-5%). Most studies that have previously attempted to formulate a multivariate model for identifying factors predictive of postoperative complications of cranioplasty either were unsuccessful or yielded conflicting results. Therefore, fuzzy logic-based fuzzy inference system (FIS), which has proven to be a useful tool for risk prediction in medical and surgical conditions, was used in thi

BACKGROUND AND PURPOSE: Suboccipital decompressive craniectomy (SDC) is a life-saving intervention for patients with malignant cerebellar infarction. However, long-term outcome has not been systematically analyzed.
METHODS: In this monocentric retrospective study we analyzed mortality, long-term functional outcome, and quality of life of all consecutive patients that were treated by SDC for malignant cerebellar infarction in our institution between 1995 and 2006.
RESULTS: A total of 57 patients were identified. All of them were treated by bilateral SDC. An external ventricular drainage was i

BACKGROUND AND OBJECTIVES: Cranioplasty (CP) is a critical neurosurgical intervention typically performed after decompressive craniectomy (DC). However, the optimal timing remains uncertain due to conflicting evidence and heterogeneous study designs. This pooled comparative meta-analysis aims to address this gap.
METHODS: We conducted a systematic search for studies reporting outcomes of patients who underwent primary DC followed by CP on September 2024. Ultra-early CP was defined as ≤35 days, early CP as ≤86 days, and late CP as >86 days after DC. Outcomes included functional recovery measur

BACKGROUND: Decompressive craniectomy (DC) is a last-tier treatment for managing refractory intracranial hypertension in patients with aneurysmal subarachnoid hemorrhage (aSAH), though concerns persist about whether it primarily prolongs survival in a state of severe disability. This study investigated patient characteristics, surgical indications, complications, and outcomes following DC in aSAH.
METHODS: In this Swedish, retrospective multi-center study, 123 aSAH patients treated with DC between 2008-2022 were included. Data collection included demographic details, aSAH characteristics, inj

Background: Photobiomodulation (PBM), a noninvasive therapy using red and near-infrared light, has shown promising benefits in neuropathology. However, the transmission of PBM to the central nervous system through the scalp's soft tissues and skull is unknown. We hypothesize that decompressive craniectomy (DC) for moderate-to-severe traumatic brain injury (TBI) improves PBM penetrance and enhances neurorehabilitation. Method: Adult male and female Long-Evans rats were utilized to evaluate PBM tissue penetrance and its efficacy on motor deficit recovery following TBI and DC. TBI was induced usi

There is little doubt that decompressive craniectomy can reduce mortality however, the results of a recent study has provided more evidence to inform the debate regarding clinical and ethical concerns that it merely converts death into survival with severe disability or in a vegetative state. The recently published RESCUEicp trial compared last-tier secondary decompressive craniectomy with continued medical management for refractory intracranial hypertension after severe traumatic brain injury. Patients were randomly assigned to decompressive craniectomy with medical therapy or to receive cont

OBJECT: Severe traumatic brain injury (TBI) is often accompanied by early death due to transtentorial herniation. Decompressive craniectomy, performed alone or in conjunction with evacuation of the mass lesion, can reduce the incidence of raised intracranial pressure (ICP). In this paper the authors evaluate mortality and morbidity and long-term outcomes in children who underwent decompressive craniectomy for severe TBI at a single institution.
METHODS: Children with severe TBI who underwent decompressive craniectomy at the Primary Children's Medical Center between 1996 and 2005 were identifi

Several case reports and small clinical series have reported benefits of decompressive hemicraniectomy in patients with intractable cerebral edema and early clinical herniation. Specific indications and timing for this intervention remain unclear. We present our experience with this procedure in a subset of 18 patients with massive cerebral edema refractory to medical management, treated with decompressive craniectomy over a 3-year period (1997 to 2000). Computerized tomography (CT) scans were independently analyzed by a neuroradiologist blinded to clinical outcome. Eleven male and seven femal

PURPOSE: Determine whether there is a difference in 5-year functional outcomes between pediatric head trauma cases who received DC vs. non-operative management (NOM).
METHODS: A seven-year (2017-2024) retrospective, case-control study of 195 pediatric trauma admissions (ages 2-18) at two Level-1 Trauma Centers. Subjects with primary head trauma and GCS ≤ 12 were included. Cases were divided into two matched cohorts based on age, sex, and injury severity. The DC group was characterized by surgical indication, and non-operative management (NOM) interventions were recorded by tier per Brain Trau

INTRODUCTION: Decompressive craniotomy or craniectomy (DC) is used to treat refractory intracranial hypertension in severe traumatic brain injury and malignant stroke. Heart rate variability (HRV) and blood pressure variability (BPV) may capture autonomic dysfunction and hemodynamic instability in this setting, but their role remains uncertain. This scoping review maps the evidence on HRV, BPV, and related hemodynamic variability in DC patients.
METHODS: We conducted a scoping review in accordance with PRISMA-ScR guidelines. MEDLINE, Embase, Cochrane CENTRAL, CINAHL, and Web of Science were s

OBJECTIVE: Cranioplasty (CP) is used to repair cranial defects after decompressive craniectomy. During this procedure, the temporal muscle can contract or retract toward the base and adhere to the scalp flaps above and/or below the dura. Several complications including functional and cosmetic problems can occur following CP. This study presents the technical notes and outcomes of CP.
METHODS: This retrospective observational study collect data of CP-procedures using unseparated muscle-dura technique performed at our hospital in 2019-2022. Technical note is presented regarding the lack of sepa

Traumatic brain injury (TBI) represents a severe pathology with important social and economic concerns, decompressive craniectomy (DC) represents a life-saving surgical option to treat elevated intracranial hypertension (ICP). The rationale underlying DC is to remove part of the cranial bones and open the dura mater to create space, avoiding secondary parenchymal damage and brain herniations. The scope of this narrative review is to summarize the most relevant literature and to discuss main issues about indication, timing, surgical procedure, outcome, and complications in adult patients involv

BACKGROUND: Prior studies have shown that decompressive craniectomy may be an independent risk factor for the development of post-traumatic hydrocephalus (PTH). It is upon this background that we chose to conduct our single-center retrospective study to establish the possibility of an association between decompressive craniectomy and PTH.
METHODS: A retrospective review involving a database of all patients with traumatic brain injury was undertaken. All referrals and admissions with traumatic brain injury, as defined by the Mayo Classification, from January 2012 to May 2022, were included in

Giant vertebral-basilar aneurysms are rare and represent 1% of intracranial aneurysms. Natural history and treatment are associated with severe clinical manifestations, such as ischemia, mass effect, hydrocephalus, and subarachnoid hemorrhage, leading to high mortality and morbidity. In this case, a 51-year-old male with no relevant medical history presented to the emergency department with severe pulsatile right temporo-occipital headache, radiating to the territory of the maxillary branch of the trigeminal nerve. Investigation revealed a giant unruptured vertebrobasilar aneurysm partially th

BACKGROUND: Primary decompressive craniectomy (DC) is considered for traumatic brain injury (TBI) patients with clinical deterioration, presenting large amounts of high-density lesions on computed tomography (CT). Postoperative CT findings may be suitable for prognostic evaluation. This study evaluated the radiographic predictors of clinical outcome and survival using pre- and postoperative CT scans of such patients.
METHODS: We enrolled 150 patients with moderate to severe TBI who underwent primary DC. They were divided into two groups based on the 6-month postoperative Glasgow Outcome Scale
