BACKGROUND: Deep brain stimulation (DBS), an established treatment for some movement disorders, is now being used experimentally to treat psychiatric disorders as well. In a number of recently published case series, DBS yielded an impressive therapeutic benefit in patients with medically intractable psychiatric diseases.
METHODS: This review of the use of DBS to treat psychiatric disorders is based on literature retrieved from a selective Pubmed search for relevant keywords, reference works on the topic, and the authors' own research.
RESULTS: Studies have been performed on the use of DBS to

Research on DBS for psychiatric disorders is being carried out in several areas, including obsessive- compulsive disorder, major depression, addictions, Tourette's syndrome and eating disorders. The results in the literature are described. The need for rigorous screening by interdisciplinary and ethical teams is explained. The need for pooling patients across different centres, with common treatment protocols, in order to achieve necessary large-scale studies in psychiatric DBS with long-term follow-up is pointed out.

Surgery for psychiatric disorders first began in the early part of the last century when the therapeutic options for these patients were limited. The introduction of deep brain stimulation (DBS) has caused a new interest in the surgical treatment of these disorders. DBS may have some advantage over lesioning procedures used in the past. A critical review of the major DBS targets under investigation for Tourette's syndrome, obsessive-compulsive disorder, and major depression is presented. Current and future challenges for the use of DBS in psychiatric disorders are discussed, as well as a ratio

Deep brain stimulation (DBS) is used for advanced and medically intractable patients with Parkinson's disease (PD), essential tremor (ET), and dystonia who meet strict criteria after a detailed motor, cognitive, and psychiatric evaluation. The potential targets are the ventral intermediate nucleus (VIM) of the thalamus for tremor, the globus pallidus interna (GPI) and the subthalamic nucleus (STN) for PD, and GPI for dystonia. The optimal target for PD has not been determined yet, although STN DBS has been performed more frequently in recent years. The mechanism of DBS effect is believed to be

A 48-year-old man with severe, lifelong Tourette's syndrome (TS) characterized by forceful self-injurious motor tics and obsessive-compulsive disorder was treated with bilateral deep brain stimulation (DBS). The decision to treat was based on his progressive neurological impairment (left sided weakness secondary to spinal cord injury) because of his relentless, violent head jerks. Electrodes were implanted at the level of the medial part of the thalamus (centromedian nucleus, the substantia periventricularis, and the nucleus ventro-oralis internus). DBS resulted in a substantial reduction of t

Deep brain stimulation (DBS) is an effective surgical therapy for well-selected patients with medically intractable Parkinson's disease (PD) and essential tremor (ET). The purpose of this review is to describe the success of DBS in these two disorders and its promising application in dystonia, Tourette Syndrome (TS) and epilepsy. In the last 10 years, numerous short- and intermediate-term outcome studies have demonstrated significant relief to patients with PD and ET. A few long-term follow-up studies have also reported sustained benefits. When successful, DBS greatly reduces most of parkinson

Essential tremor (ET) is one of the most common movement disorders and can cause significant functional disability in some patients. Although medications can improve tremor in some patients, approximately 50% of patients have medication-resistant symptoms. In patients with ET who experience marked disability due to the tremor, surgical therapy, primarily deep brain stimulation (DBS) of the ventral intermediate (Vim) nucleus of the thalamus, is a viable treatment option. Multiple studies of DBS of the thalamus have shown that it is efficacious in the treatment of ET hand tremor, often with seco

Deep brain stimulation (DBS) of the ventral intermediate nucleus of the thalamus, subthalamic nucleus, and internal globus pallidus has been put forth as an alternative to surgical ablation for the treatment of movement disorders. In this paper, the authors discuss the history and putative physiologic mechanisms underlying DBS of these target regions. The authors then review empirical findings pertaining to the effects of DBS on neurological symptoms, cognitive functioning, and psychiatric symptoms in Parkinson's disease and essential tremor, the disorders for which the procedure has been most

Chronic deep brain stimulation (DBS) is a promising technique for the treatment of movement disorders. Thalamic stimulation is now an established surgical procedure for parkinsonian and essential tremor. Pallidal and subthalamic stimulation are under active investigation as treatments for Parkinson's disease. Although high-frequency DBS at these sites has similar behavioral effects as lesioning, the physiologic mechanisms underlying the beneficial effect of DBS is not well understood and may be extremely complex. DBS offers a potential advantage over ablative therapy because stimulation-induce

OBJECTIVE: To collect the information necessary to design the methods and outcome variables for a larger trial of scheduled deep brain stimulation (DBS) for Tourette syndrome.
DESIGN: We performed a small National Institutes of Health-sponsored clinical trials planning study of the safety and preliminary efficacy of implanted DBS in the bilateral centromedian thalamic region. The study used a cranially contained constant-current device and a scheduled, rather than the classic continuous, DBS paradigm. Baseline vs 6-month outcomes were collected and analyzed. In addition, we compared acute sch

BACKGROUND: Deep Brain Stimulation (DBS) is thought to improve the symptoms of selected neurological disorders by modulating activity within dysfunctional brain circuits. To date, there is no evidence that DBS counteracts progressive neurodegeneration in any particular disorder.
OBJECTIVE/HYPOTHESIS: We hypothesized that DBS applied to the fornix in patients with Alzheimer's Disease (AD) could have an effect on brain structure.
METHODS: In six AD patients receiving fornix DBS, we used structural MRI to assess one-year change in hippocampal, fornix, and mammillary body volume. We also used de

BACKGROUND: Dystonia is a group of movement disorders that can result in tremor, abnormal posturing, and difficulty with coordinated movements. Medically intractable dystonia can be treated with deep brain stimulation (DBS). Dystonia refractory to DBS can be rescued with DBS to another region. Here, the authors report one of the first cases of dystonia initially treated with subthalamic nucleus (STN) DBS rescued by globus pallidus (GPi) DBS.
OBSERVATIONS: A 42-year-old male presented with progressive and disabling left hemidystonia along with toe curling, foot inversion, and arm posturing. Pr

This article focuses on the evolution of deep brain stimulation (DBS) targets within the brain, beginning with the discovery of DBS's potential in non-Parkinson disease movement disorders. DBS has gained in popularity and applicability for a growing number of neuropathologic conditions with neural network disorders and dysfunction. Targets within the brain have been based frequently on historical sites used for ablative surgeries in years past, derived from experiment and experience but also have arisen via elucidation of neural networks, transmitter function and location, disease neuropatholo

Deep brain stimulation (DBS) is a clinically established procedure for treating severe motor symptoms in patients suffering from end-stage Parkinson's disease, dystonia and essential tremor. Currently, it is tested for further indications including psychiatric disorders like major depression and a variety of other diseases. However, ethical issues of DBS demand continuing discussion. Analysing neuroethical and clinical literature, five major topics concerning the ethics of DBS in clinical practice were identified: thorough examination and weighing of risks and benefits; selecting patients fair

BACKGROUND: The purpose of the present study was to evaluate deep learning-based image-guided surgical planning for deep brain stimulation (DBS). We developed deep learning semantic segmentation-based DBS targeting and prospectively applied the method clinically.
METHODS: T2∗ fast gradient-echo images from 102 patients were used for training and validation. Manually drawn ground truth information was prepared for the subthalamic and red nuclei with an axial cut ∼4 mm below the anterior-posterior commissure line. A fully convolutional neural network (FCN-VGG-16) was used to ensure margin ident

Transcranial electric stimulation (TES) is a non-invasive, economical, and well-tolerated neuromodulation technique. However, traditional TES is a whole-brain stimulation with a small current, which cannot satisfy the need for effectively focused stimulation of deep brain areas in clinical treatment. With the deepening of the clinical application of TES, researchers have constantly investigated new methods for deeper, more intense, and more focused stimulation, especially multi-electrode stimulation represented by high-precision TES and temporal interference stimulation. This paper reviews the

BACKGROUND: Deep brain stimulation (DBS) is a promising treatment option for treatment-refractory obsessive-compulsive disorder (OCD). Several stimulation targets have been used, mostly in and around the anterior limb of the internal capsule and ventral striatum. However, the precise target within this region remains a matter of debate.
METHODS: Here, we retrospectively studied a multicenter cohort of 82 patients with OCD who underwent DBS of the ventral capsule/ventral striatum and mapped optimal stimulation sites in this region.
RESULTS: DBS sweet-spot mapping performed on a discovery set

With the introduction of adaptive deep brain stimulation (aDBS) for Parkinson's disease, new questions emerge regarding who, why, and how to treat. This paper outlines the pathophysiological rationale for aDBS, which provides real-time modulation of the stimulation amplitude based on subthalamic beta (range 13-30 Hz) activity and related physiomarkers. We review clinical evidence comparing aDBS with conventional DBS in terms of motor improvement, side-effect reduction, energy efficiency, and technical developments, including sensing-enabled device characteristics, stimulation algorithms, and p
