Cardiac resynchronization therapy (CRT) improves left ventricular function, especially in patients with left bundle branch block or those receiving chronic right ventricular pacing. CRT is typically accomplished by placing a right ventricular endocardial pacing lead and a left ventricular pacing lead via the coronary sinus to a coronary vein overlying the lateral or posterolateral left ventricle. CRT can be combined with an implantable defibrillator or with a pacemaker. Limited data are available to compare these two versions of CRT head to head. This review summarizes the relevant trials and

Advances in the field of defibrillation have brought to practice different types of devices that include the transvenous implantable cardioverter-defibrillator (ICD) with or without cardiac resynchronization therapy, the subcutaneous ICD (S-ICD), and the wearable cardioverter-defibrillator. To ensure optimal use of these devices and to achieve best patient outcomes, clinicians need to understand how these devices work, learn the characteristics of patients who qualify them for one type of device versus another, and recognize the remaining gaps in knowledge surrounding these devices. AIMS: The indications for implantable cardioverter-defibrillators (ICDs) have been expanding, especially for primary prevention of sudden cardiac death. Implantable cardioverter-defibrillator saves lives; however, in near end-of-life situations linked to incurable diseases, the question arises as to whether or not to turn off the ICD to avoid excessive numbers of shocks as the heart begins to fail. This study examined the wishes of a cohort of ICD recipients.
METHODS AND RESULTS: Consecutive recipients of ICDs for primary or secondary prevention of sudden cardiac death were examined during a

PURPOSE: The purpose of this study was to describe the experiences of adolescents with implantable cardioverter defibrillators (ICDs).
DESIGN AND METHODS: This qualitative study explored the lived experiences of 14 adolescents with ICDs using face-to-face, semistructured interviews.
RESULTS: Six themes were identified: life interrupted; still the same, except; sometimes it's tough; it cuts both ways; unique joule; and living life regular.
PRACTICE IMPLICATIONS: These findings suggest that nurses could make a significant impact on the lives of adolescents with ICDs by focusing their efforts

UNLABELLED: PROPOSED SERVICE: The service proposed is the use of implantable cardioverter defibrillators (ICDs) in the management of risk factors leading to sudden cardiac death (SCD). ICDs are similar in size to a pacemaker and are intended to prevent death due to life-threatening ventricular tachyarrhythmias.
EPIDEMIOLOGY AND BACKGROUND: SCD occurs in approximately 100,000 people annually in the UK and is usually due to ventricular tachyarrhythmia. Increasing numbers of people are surviving a first episode of ventricular tachyarrhythmia and are at high risk of further episodes. Standard tre

The implantable cardioverter defibrillator (ICD) is accepted as the therapy of choice in preventing sudden cardiac death. Multiple studies, such as Antiarrhythmics Versus Implantable Defibrillators (AVID), the Canadian Implantable Defibrillator Study (CIDS), the Cardiac Arrest Study Hamburg (CASH), and the Multicenter Automatic Defibrillator Implantation Trial (MADIT), have shown a substantial benefit in survival rates for patients treated with ICDs compared with antiarrhythmic drug treatment. The detection of spontaneous ventricular tachycardias (VT) is based primarily on the programmed heart

The placement of a transvenous implantable cardioverter defibrillator (ICD) system through a single infraclavicular skin incision has been a surgical goal for years. The development of a new investigational model of ICD with substantially reduced dimensions (volume, 83 cm3; mass, 132 g) has made the one-incision approach a clinical reality. Between March and September 1993, 4 female and 19 male patients (mean age, 60 +/- 9.6 years; range, 46 to 73 years) underwent implantation of this device for the treatment of ventricular fibrillation (n = 14) or ventricular tachycardia (n = 9). BACKGROUND: Defibrillators constitute a critical component of emergency care for ventricular fibrillation and pulseless ventricular tachycardia. Advances in defibrillation technology point to the potential need for reassessing current requirements for defibrillation protection and associated resuscitation guidelines.
OBJECTIVE: This study aimed to establish reference information for future directions in defibrillator requirements, defibrillation protection requirements, and resuscitation guidelines based on the parameters of commercially available defibrillators.
METHODS: Defibrillator brand

INTRODUCTION: Cardiac arrest (CA) during hyperbaric oxygen treatment (HBOT) is exceedingly rare with only a few cases reported. It is unknown if in-chamber defibrillation of a patient has been performed in Australasia. In-chamber defibrillation is potentially dangerous with the risk of fire in an oxygen-rich environment. Australasian Standards prohibit the use of currently available defibrillators licensed for in-chamber use, as they contain lithium batteries. This study aimed to investigate how CA is managed in Australasian hyperbaric medicine units (HMUs) and to establish if there is a need

INTRODUCTION: Wearable cardioverter-defibrillators (WCD) have emerged as a valuable tool in the management of patients at risk for life-threatening arrhythmias. These devices offer a non-invasive and temporary solution, providing continuous monitoring and the potential for prompt defibrillation when needed. In this study, we explore the use of WCD and evaluate arrhythmic events through comprehensive monitoring.
METHODS: From November 2022 to May 2024, we conducted an outpatient follow-up of 41 patients receiving WCD. Regular check-ups, remote monitoring and comprehensive echocardiography were

BACKGROUND: To date, only a few comparisons between subcutaneous implantable cardioverter-defibrillator (S-ICD) patients undergoing and those not undergoing defibrillation testing (DT) at implantation (DT+ vs DT-) have been reported.
OBJECTIVE: The purpose of this study was to compare long-term clinical outcomes of 2 propensity-matched cohorts of DT+ and DT- patients.
METHODS: Among consecutive S-ICD patients implanted across 17 centers from January 2015 to October 2020, DT- patients were 1:1 propensity-matched for baseline characteristics with DT+ patients. The primary outcome was a composi

The benefit of implantable cardioverter-defibrillator (ICD) therapy depends upon appropriate evaluation of a persisting risk of sudden death and estimation of the patient's overall survival. Assessment of a stable and unchangeable arrhythmogenic substrate is often difficult. Structural abnormality and ventricular dysfunction, the two major risk parameters, may recover, and heart failure symptoms can improve so that ICD therapy may not be indicated. Risk stratification can take time while the patient continues to be at high risk of arrhythmic death, and patients may need temporary bridging by a

Implantable cardioverter defibrillator implantation is performed under general anaesthesia. This report describes awareness or recall in two of 33 patients scheduled for implantation. After induction, anaesthesia was maintained using continuous infusions of propofol and atracurium and intermittent boluses of alfentanil. Propofol was given in the same arm as the implantation side. In these two patients propofol flowed out of the bleeding central venous access. To minimise the possibility of awareness, we advise that continuous infusions should be administered intravenously on the contralateral

BACKGROUND: Patients with heart failure having a low expected probability of arrhythmic death may not benefit from implantable cardioverter defibrillators (ICDs).
OBJECTIVE: The objective was to validate models to identify cardiac resynchronization therapy (CRT) candidates who may not require CRT devices with ICD functionality.
METHODS: Heart failure (HF) patients with CRT-Ds and non-CRT ICDs from the National Cardiovascular Data Registry and others with no device from 3 separate registries and 3 heart failure trials were analyzed using multivariable Cox proportional hazards regression for s

A 34-year-old male with dilated cardiomyopathy and advanced heart failure was referred for defibrillator implantation for primary prevention of sudden cardiac death. During implantation despite multiple right ventricular (RV) lead positions (outflow tract, septum, apex) and various defibrillation vector combinations (superior vena cava coil on/off, polarity reversal, tilt change), the maximum energy shock (40 J) from the implanted device was unsuccessful in restoring the sinus rhythm. In a straightforward fashion another regular single coil shocking lead was introduced to the azygos vein using

Implantable Cardioverter Defibrillators (ICDs) have undergone substantial evolution since the 1980s, with the aim of decreasing sudden cardiac death, whilst minimising device related complications. Traditional transvenous ICDs (TV-ICDs), though comprehensive and effective, are associated with risks with having leads inside the veins and heart. The emergence of the subcutaneous ICD (S-ICD) offered a solution for patients without a pacing indication, significantly reducing the risks of complications acutely and long term. The absence of pacing and need for specific ECG requirements limit its usa

BACKGROUND: Due to the poor long-term prognosis of patients with transthyretin cardiac amyloidosis (ATTR-CA), the role of primary prevention implantable cardioverter-defibrillators (ICDs) in this patient population remains controversial. We aimed to study the impact of primary prevention ICDs on survival in patients with ATTR-CA.
METHODS: Among 382 patients diagnosed with ATTR-CA at our institution between 2004 and 2018, 19 had primary prevention ICDs implanted. This cohort was matched in a 1:3 manner on the basis of age, gender, ejection fraction (EF) and ATTR-CA stage with 57 patients witho

BACKGROUND: Evidence for the benefit of implantable cardioverter defibrillators (ICD) in preventing sudden cardiac death (SCD) in older adults is mixed; age alone may not predict benefit. Frailty may help identify patients in whom an ICD does not improve overall mortality risk.
METHODS: Structured search of PubMed, Embase, Web of Science, and Cochrane Central Register of Controlled Trials on 1/31/2019, without language restriction, with terms for ICD, frailty, and mortality. Frailty was defined broadly using any validated single component (e.g., walking speed, weight loss) or multi-component

Sudden cardiac death (SCD) is an abrupt and unexpected death presumed to be due to cardiac causes. The primary etiologic mechanism is cardiac arrest due to an arrhythmia. Cardioverter-defibrillators detect abnormal arrhythmia and abort the rhythm with an electrical shock. Wearable cardioverter-defibrillators are devices in which the leads contact the chest on a vest worn by the patient. Wearable devices are for patients at temporary risk of SCD, when comorbidities prohibit placement of an implantable cardioverter-defibrillator (ICD), or as a bridge to ICD placement or heart transplantation.
