Classification of Geriatric Low Back Pain Based on Hip Characteristics With a 12-Month Longitudinal Exploration of Clinical Outcomes: Findings From Delaware Spine Studies.

OBJECTIVE: The purpose of this study is to identify geriatric chronic low back pain (LBP) subgroups based on the presence of potentially modifiable hip impairments, using Latent Variable Mixture Modeling (LVMM), and to examine the prospective relationship between these subgroups and key outcomes over time. METHODS: Baseline, 3-month, 6-month, and 12-month data were collected from a prospective cohort of 250 community-dwelling older adults with chronic LBP. Comprehensive hip (symptoms, strength, range of motion, and flexibility), LBP (intensity and disability), and mobility function (gait spee

Examining Psychological Factors as Contributors to Pain, Disability, and Physical Function in Geriatric Chronic Low Back Pain: A Prospective Analysis of the Delaware Spine Studies Cohort.

In younger populations, risk factors from psychologically-focused theoretical models have become accepted as primary drivers behind the persistence of low back pain (LBP), but these risk factors have not been thoroughly assessed in older adult populations (60-85 years). To address this knowledge gap, we sought to examine longitudinal associations between both general and pain-related psychological risk factors and future pain intensity, LBP-related disability, and physical function (gait speed) outcomes in older adults with chronic LBP (n = 250). Questionnaires for general (ie, depressive symp

Metal and metalloid levels in blood of semipalmated sandpipers (Calidris pusilla) from Brazil, Suriname, and Delaware Bay: Sentinels of exposure to themselves, their prey, and predators that eat them.

Identifying animals as sentinels for humans and other animal species is an excellent method for understanding exposure to environmental contamination at different times and places. Shorebirds are useful sentinels because they have a world-wide distribution, eat a range of prey, and are eaten by a range of other species, including humans. We collected blood from semipalmated sandpipers (Calidris pusilla) wintering in northern (Suriname N = 71) and northeastern (Brazil N = 61) South America to examine levels of heavy metals and metalloids (arsenic, selenium), and compare them to blood levels in

Is use of Long-Acting Reversible Contraceptives (LARC) associated with reduced well-woman visits and STI testing? Evidence from female Medicaid clients aged 15-24 in Delaware.

OBJECTIVE: Contraceptive method type matters to sexually transmitted infection (STI) prevention, but contraceptive counseling often emphasizes method efficacy and the benefits of "forgettable" methods, including long-acting reversible contraceptives (LARCs). We aimed to explore associations between prescription method type and annual STI testing and investigated whether these associations relate to annual well-woman visits. METHODS: We constructed a panel of 20,949 young women (<25) enrolled in Delaware's Medicaid program from 2012 through 2019. Conditional logit regressions measured associat

Self-reported knee function can identify athletes who fail return-to-activity criteria up to 1 year after anterior cruciate ligament reconstruction: a delaware-oslo ACL cohort study.

STUDY DESIGN: Cohort study, cross-sectional. OBJECTIVES: To determine if self-reported knee function assessed with the International Knee Documentation Committee 2000 Subjective Knee Form (IKDC 2000) could discriminate between successful and nonsuccessful performance on return-to-activity criteria (RTAC) tests after anterior cruciate ligament (ACL) reconstruction. BACKGROUND: Selecting appropriate performance-based and patient-reported tests that can detect side-to-side asymmetries, assess global knee function, and determine a participant's readiness to return to activity after ACL reconstru

Patient Acceptable Symptom State Thresholds for IKDC-SKF and KOOS at the 10-Year Follow-up After Anterior Cruciate Ligament Injury: A Study From the Delaware-Oslo ACL Cohort.

BACKGROUND: Clinicians need thresholds for the Patient Acceptable Symptom State (PASS) and Treatment Failure to interpret group-based patient-reported outcome measures after anterior cruciate ligament (ACL) injury. Validated thresholds that are crucial for accurately discerning patient symptom state and facilitating effective interpretation have not been determined for long-term follow-up after ACL injury. PURPOSE: To calculate and validate thresholds for PASS and Treatment Failure for the International Knee Documentation Committee Subjective Knee Form (IKDC-SKF) and the Knee injury and Osteo

Evaluation of fentanyl test strip distribution in two Mid-Atlantic syringe services programs.

BACKGROUND: Fentanyl test strip (FTS) programs are designed to promote fentanyl awareness and reduce overdose risk by providing people who use drugs (PWUD) with a method of testing drugs for the presence of fentanyl prior to use. In 2018, two large syringe services programs (SSP) in the U.S. Mid-Atlantic-one in Baltimore City, Maryland and one in the state of Delaware-began distributing FTS. We evaluated the implementation of these programs. METHODS: A convenience sample of SSP clients was surveyed in Baltimore City (urban) and Delaware (urban, suburban and rural) to assess FTS utilization pa

Mercury in Red Knots (Calidris canutus rufa): Inter-Annual and Geographic Variation in Blood Mercury and Associated Toxicological Risk Along the Atlantic Flyway.

Shorebird populations are declining sharply, and trace metal contaminants may compound the many migration-related threats they face by affecting feeding, migration, and reproductive success. Despite this risk to the federally threatened Red Knot (Calidris canutus rufa), blood mercury in Red Knots has not been examined outside Delaware Bay, and the associated toxicological risk has not been assessed across the annual cycle. We report blood mercury from four cohorts: spring 2024 and spring 2025 at Delaware Bay, fall 2024 at Avalon, New Jersey, and spring 2025 at Kiawah Island, South Carolina. Bl

Ensuring Access to Opioid Treatment Program Services Among Delawareans Vulnerable to Flooding.

OBJECTIVE: To quantify potential flood-related access disruptions to medication-assisted treatment for opioid use disorder (OUD) among Delawareans. METHODS: Spatial flood risk maps and infrastructure, services, and hazard risk, transportation networks, opioid treatment programs (OTPs) for the State of Delaware were integrated to visually display the relationship between these layers. A complex network theory-based simulation model was used to assess both direct (e.g., inundation with flood water) and indirect (e.g., isolation) impacts of floods. RESULTS: Delaware is at increasing risk from f

Emerging babesiosis in the mid-Atlantic: autochthonous human babesiosis cases and Babesia microti (Piroplasmida: Babesiidae) in Ixodes scapularis (Acari: Ixodidae) and Ixodes keiransi (Acari: Ixodidae) ticks from Delaware, Maryland, Virginia, West Virginia, and the District of Columbia, 2009 to 2024.

The range of Babesia microti (Franca, 1910)-infected ticks is expanding, resulting in locally acquired human babesiosis cases occurring in new areas: Maryland (2009), the District of Columbia (2013), Virginia (2016), and West Virginia (2017). We collected host-seeking ticks from old fields, ecotones, forested habitats and animal hosts in Delaware, Maryland, and Virginia, 2010 to 2024. Ixodes scapularis Say, the tick vector of babesiosis, was captured in all 3 states. PCR revealed B. microti in 2.7% (36/1310) of I. scapularis, with site prevalence ranging from <1% to 12.5% infected. The first B

DementiaBank: Theoretical Rationale, Protocol, and Illustrative Analyses.

PURPOSE: Dementia from Alzheimer's disease (AD) is characterized primarily by a significant decline in memory abilities; however, language abilities are also commonly affected and may precede the decline of other cognitive abilities. To study the progression of language, there is a need for open-access databases that can be used to build algorithms to produce translational models sensitive enough to detect early declines in language abilities. DementiaBank is an open-access repository of transcribed video/audio data from communicative interactions from people with dementia, mild cognitive impa

Science Over Stigma: Saving Lives--Implementation of Naloxone Use in the School Setting.

Unintentional drug overdose is a leading cause of preventable death in the United States. Administration of naloxone hydrochloride ("naloxone") can reverse a potentially fatal opioid overdose and save lives. The school nurse is an essential part of the school team responsible for developing emergency response procedures and should facilitate access to naloxone for the management of opioid-related overdose in the school setting. Delaware has been leading efforts to provide education, increase awareness, and help erase the stigma of substance use disorder through school nurse collaboration with

Transgenerational health promotion.

The Adult Health and Development Program (AHDP) is an intergenerational, interdisciplinary health promotion and rehabilitation program in which nursing and other college students are paired with older adults to engage in activities to improve their health and well-being. The purpose of this article is to describe resources and strategies used to implement a new program. The University of Delaware's program uses the ACAEM Paradigm developed by the AHDP at the University of Maryland where the program has existed for 27 years. However, the University of Delaware emphasizes the concepts of transit

Avian haemosporidian parasite prevalence and diversity in two populations of the American kestrel (Falco sparverius).

Parasite communities vary among host species and across space. However, little is known about differences in parasite communities between geographically and genetically distinct populations of the same host species. American kestrels (Falco sparverius) are small falcons with regionally distinct genetic populations across North America. We sampled kestrels from Delaware and Utah for avian haemosporidian parasites (genera: Haemoproteus, Plasmodium, and Leucocytozoon) and used molecular barcoding of the parasite cytochrome b gene (cyt b) to quantify parasite genetic lineage diversity. We identifi
