Manual removal of the placenta after vaginal delivery: an unsolved problem in obstetrics.

The third stage of labor is associated with considerable maternal morbidity and mortality. The major complication is postpartum hemorrhage (PPH), which is the leading cause of maternal morbidity and mortality worldwide. Whereas in the event of PPH due to atony of the uterus there exist numerous treatment guidelines; for the management of retained placenta the general consensus is more difficult to establish. Active management of the third stage of labour is generally accepted as standard of care as already its duration is contributing to the risk of PPH. Despite scant evidence it is commonly a

Obstetric brachial plexus palsy following routine versus difficult deliveries.

Previous bio-engineering studies showed that intrapartum peak forces applied by the clinician were lower in routine deliveries than difficult deliveries. A total of 751 cases of obstetric brachial plexus palsy were included and divided into two groups: group I (248 patients) were born following routine deliveries and group II (503 patients) were born following difficult deliveries. Both groups were compared regarding the type of palsy and the rate of good/poor spontaneous motor recovery from the palsy. Group I subjects were more likely to have upper Erb palsy whereas those in group II were mor

Theory of obstetrics: an epidemiologic framework for justifying medically indicated early delivery.

BACKGROUND: Modern obstetrics is faced with a serious paradox. Obstetric practice is becoming increasingly interventionist based on empirical evidence but without a theoretical basis for such intervention. Whereas obstetric models of perinatal death show that mortality declines exponentially with increasing gestational duration, temporal increases in medically indicated labour induction and cesarean delivery have resulted in rising rates of preterm birth and declining rates of postterm birth. Other problems include a disconnection between patterns of gestational age-specific growth restriction

An enhanced method for identifying obstetric deliveries: implications for estimating maternal morbidity.

OBJECTIVES: The accuracy of maternal morbidity estimates from hospital discharge data may be influenced by incomplete identification of deliveries. In maternal/infant health studies, obstetric deliveries are often identified only by the maternal outcome of delivery code (International Classification of Diseases code = V27). We developed an enhanced delivery identification method based on additional delivery-related codes and compared the performance of the enhanced method with the V27 method in identifying estimates of deliveries as well as estimates of maternal morbidity. METHODS: The enhanc

[Surgical methods for delivery in modern obstetrics and their influence on maternal and infant health].

The article addresses issues of comparative characterization of deliveries involving surgery and impact thereof on the health of the mother and her child. Risk factors are identified that the mother and her child run in sectio cesarea, in application of obstetrical forceps, and in vacuum-extraction of the fetus. Cesarean section was found out to be the most acceptable mode of delivery in origination of organic and functional nervous system involvement in children but the most ill-chosen and unpropitious one in the mother, especially so in those groups at risk for bleeding, septic complications

Blood and amniotic fluid contact sustained by obstetric personnel during deliveries.

OBJECTIVE: The objective of this study was to characterize blood and amniotic fluid contact sustained by obstetric personnel during deliveries. STUDY DESIGN: Trained observers collected data on 1376 person procedures during 230 deliveries at Grady Memorial Hospital from May to October 1989. Rates of contact were compared by means of the chi 2 test. RESULTS: At least one blood or amniotic fluid contact occurred during 79 (39.1%) of 202 vaginal and 14 (50.0%) of 28 cesarean deliveries; a needle stick occurred in 4 (2.0%) of the vaginal deliveries. Obstetricians and midwives had the highest rat

Incidence of Obstetric and Foetal Complications during Labor and Delivery at a Community Health Centre, Midwives Obstetric Unit of Durban, South Africa.

The objectives of this retrospective cohort study were to estimate the incidence of obstetric complications during labor and delivery and their demographic predictors. A total of 2706 pregnant women were consecutively admitted to a midwife obstetric unit with labor pain between January and December 2007 constituted the sample. Among them 16% were diagnosed with obstetrical and foetal complications. The most frequently observed foetal and obstetric complications were foetal distress (35.5/1000) and poor progress of labor (28.3/1000), respectively. Primigravid and grandmultiparity women were 12

Obstetric anesthesia and postpartum mental health: implications from delivery to long-term recovery.

Obstetric anesthesiologists have traditionally focused on providing effective labor analgesia and ensuring safe cesarean delivery. However, growing evidence suggests that anesthetic choices and childbirth experiences influence maternal mental health beyond the immediate intrapartum period. Postpartum depression (PPD) and postpartum post-traumatic stress disorder (PP-PTSD) are linked to impaired mother-infant bonding, reduced quality of life, and adverse long-term outcomes in mothers and children. Therefore, this review aimed to synthesize the current evidence on antenatal risk factors, deliver

Factors associated with obstetric anal sphincter injuries during vacuum delivery among Chinese women.

OBJECTIVE: To investigate factors associated with obstetric anal sphincter injuries (OASIS) during vacuum delivery among Chinese women. METHODS: A retrospective cohort study of Chinese women who underwent vacuum-assisted vaginal delivery at a tertiary referral hospital in Taiwan between January 2010 and December 2016. Logistic regression analysis was used to compare various factors (maternal, delivery, and neonatal factors) between women with and those without OASIS. RESULTS: Among 18 744 Chinese women with singleton vaginal delivery, 3141 (16.76%) had vacuum assistance and 2634 (83.86%) wer

Timing of delivery in a high-risk obstetric population: a clinical prediction model.

BACKGROUND: The efficacy of antenatal corticosteroid treatment for women with threatened preterm birth depends on timely administration within 7 days before delivery. We modelled the probability of delivery within 7 days of admission to hospital among women presenting with threatened preterm birth, using routinely collected clinical characteristics. METHODS: Data from the Canadian Perinatal Network (CPN) were used, 2005-11, including women admitted to hospital with preterm labour, preterm pre-labour rupture of membranes, short cervix without contractions, or dilated cervix or prolapsed membra

Obstetric brachial plexus palsy associated with breech delivery. A different pattern of injury.

Most obstetric brachial plexus palsies are due to rupture of the upper roots in babies whose delivery was complicated by shoulder dystocia. If treated by early exploration and grafting, they have a favourable prognosis. We reviewed 36 babies who had had an obstetric brachial plexus palsy after a breech delivery and found that they had a different pattern of injury; 81% had avulsion of the upper roots. This injury cannot be treated satisfactorily by exploration and microsurgical grafting and carries a considerably worse prognosis for shoulder function.

Is the risk of obstetric anal sphincter injuries increased in vaginal twin deliveries?

OBJECTIVE: Vaginal twin deliveries have a higher rate of intrapartum interventions. We aimed to determine whether these characteristics are associated with an increased rate of obstetric anal sphincter injuries compared with singleton. STUDY DESIGN: Retrospective study of all twin pregnancies undergoing vaginal delivery trial was conducted from January 2000-September 2014. Sphincter injury rate compared with all concurrent singleton vaginal deliveries. Multivariable analysis was used to determine twin delivery association with sphincter injuries while adjusting for confounders. RESULTS: Abou

[Initial obstetrical management of post-partum hemorrhage following vaginal delivery].

OBJECTIVES: To define initial steps of obstetrical treatment of post-partum hemorrhage (PPH) after vaginal delivery. MATERIALS AND METHOD: We searched the Medline and the Cochrane Library and checked the international guidelines: HAS, RCOG, SOGC, ACOG and WHO. RESULTS: In case of PPH, the use of a collecting bag is recommended (professional consensus). All the concerned professional (midwife, obstetrician, anesthesiology team) must be warned immediately (professional consensus). If placenta is retained, manual removal needs to be performed and after placental delivery, manual uterine explora

Pilot study for a trial of ursodeoxycholic acid and/or early delivery for obstetric cholestasis.

BACKGROUND: Obstetric cholestasis (OC) is a serious problem in pregnancy. It affects about 4500 women per year in the UK. Affected women develop itching and occasionally jaundice. More importantly, the condition is associated with premature delivery, fetal distress and is believed to be an important cause of stillbirth. However, even now, there is no clear evidence as to whether the most popular treatment, a drug called ursodeoxycholic acid is beneficial to the baby, or even if it is safe in pregnancy. Nor do we know whether planned early delivery of the baby at 37-38 weeks, another popular tr

Mode of delivery and maternal outcome in subsequent delivery after an obstetric anal sphincter injury: a Finnish retrospective cohort study.

BACKGROUND: Obstetric anal sphincter injury (OASI) is a severe complication of vaginal delivery. In previous studies, parturients with a preceding OASI are at increased risk of a recurrent OASI (rOASI) in subsequent vaginal deliveries. In Finland, the rate of OASI is low compared to other countries, at 1.4% of deliveries, and the incidence of rOASI is not well-known. This study examined recurrence and mode of delivery after an OASI. METHODS: This historical cohort study includes 278 women who have experienced an OASI and have delivered again in Tampere University Hospital. Deliveries complica

Preterm Delivery and Postpartum Substance Use: Implications for Maternal Mental Health in Obstetric Care.

Introduction Preterm delivery is an obstetric complication linked to an increased risk of postpartum depression (PPD), which often goes undiagnosed. Individuals with PPD may self-medicate to manage depressive symptoms. While postpartum substance use is well-documented, few studies have assessed whether preterm delivery is associated with new-onset substance use. A greater understanding of birth trauma as a factor for new-onset substance use in postpartum women can broaden our understanding of postpartum mood disorders and propel greater inquiry into mental health care in peri- and postpartum w
