Hypoxic-ischemic encephalopathy remains a major cause of morbidity and mortality in preterm and full-term infants. Experimental data from animal studies suggest that interventions that improve survival of injured neurons and prevent delayed neuronal loss may decrease hypoxic ischemic brain injury. Considerable attention has focused on optimizing management of newborns in the period immediately after resuscitation from perinatal asphyxia to minimize delayed neuronal death. The evidence regarding the role of glucose in modifying post-asphyxia brain injury and resuscitation was reviewed to better

Introduction: Postpartum hemorrhage remains a leading cause of maternal morbidity and mortality worldwide. While antifibrinolytic agents such as tranexamic acid are effective in treating established postpartum hemorrhage, the benefit of prophylactic tranexamic acid remains debated. The presence and frequency of early postpartum hyperfibrinolysis during routine childbirth have not been thoroughly investigated. Material & Methods: This prospective observational study was registered on ClinicalTrials.gov (NCT05975112) and conducted at the Medical University Hospital Graz between June 2023 and Jun

UNLABELLED: The meconium aspiration syndrome is the first cause of respiratory distress in full term newborns. At birth, management includes systematical oropharyngeal suctioning, before and after shoulders delivery, followed by tracheal suction. During last 10 years, many trials were published which discuss again the value of this strategy.
AIM: To assess practices of the current management of births with a meconial amniotic liquid in the French maternities.
METHODS: Observational survey by written questionnaires sent to the 617 French maternities.
RESULTS: The rate of answers was of 54.3%

We developed a new scavenging mask for the administration of nitrous oxide to mothers-to-be during labor in order to minimize leakage of the patient's exhaled breath into the room and, therefore, decreasing staff exposure to nitrous oxide. The scavenging system was designed with an inner mask for gas delivery, an outer mask to capture exhaled breath, and a continuous exhaust airflow based on breathing flow rates. In this study we field tested the scavenging system by measuring personal exposures of nurses (N = 30) and room air concentrations (N = 33), while patients self-administered nitrous o

Ultrasound is a valuable tool commonly used in the delivery room. It has multiple applications. The objective of this study was to investigate whether systematic fetal weight estimation by ultrasound in the delivery room increases the risk of cesarean delivery. Monocentric cohort study. All parturients with singleton pregnancies who gave birth full-term at = 39 weeks were enrolled in the study. We excluded all patients with a contraindication to vaginal birth as well as those in whom fetal weight estimation (FWE) by ultrasound on day of delivery was deemed necessary in making obstetric decisio

OBJECTIVE: To evaluate less invasive surfactant administration (LISA) in the delivery room as a part of neonatal stabilization and reduce infants requiring intubation.
PROJECT DESIGN: A SMART aim was developed to implement LISA in >30% of infants (25.0-31.6 weeks' gestational age) with respiratory distress syndrome (RDS) in the NICU or delivery room (DR) and increase the number of infants never requiring intubation by 50% by 12/31/2023. Three sequential Plan-Do-Study-Act cycles were implemented. Statistical Process Control charts tracked time-ordered data.
RESULTS: Over 4 years, 184 patients

Advancements in prenatal detection have improved postnatal outcomes for patients with congenital heart disease (CHD). Detailed diagnosis during pregnancy allows for preparation for the delivery and immediate postnatal care for the newborns with CHD. Most CHDs do not result in hemodynamic instability at the time of birth and can be stabilized following the guidelines of the neonatal resuscitation program (NRP). Critical CHD that requires intervention immediately after birth is recommended to be delivered in facilities where immediate neonatal and cardiology care can be provided. Postnatal stabi

OBJECTIVE: To determine whether heated humidified high-flow nasal cannula (HHHFNC) is noninferior to NCPAP to provide DR respiratory support to preterm neonates of gestational age (GA) 28-36 wk.
METHODS: This randomized, controlled, noninferiority trial included 124 spontaneously breathing preterm neonates who developed respiratory distress soon after birth and/or had a FiO2 requirement > 0.3. Primary outcome measure was treatment failure within 24 h. The absolute risk difference with 95% confidence interval (CI) were calculated with a noninferiority margin of 10%. Secondary outcome variables

BACKGROUND: In case of suspicious CTG in labor a conservative attitude is recommended, when possible, through intrauterine resuscitation maneuvers.
AIMS: This study aims to evaluate the use of intrauterine resuscitation maneuvers (IRM) at two Italian hospitals; the secondary outcome is the assessment of the agreement in their application.
MATERIALS AND METHODS: Retrospective analysis was performed on the data of 80 deliveries (40 vs 40) from two different Italian hospitals, located in Novara (group 1) and in Borgomanero (group 2). In order to evaluate a varied series of CTG traces (normal an

Background Supraventricular tachycardia (SVT) is seldom considered a cause for fetal tachycardia; commoner etiologies including maternal fever and fetal distress are usually envisaged. Fetal arrhythmia can be missed as a diagnosis, potentially leading to suboptimal management. Cases Three cases are described where detection of fetal tachycardia >200 beats per minute (bpm) at 36, 40, and 38 weeks gestation resulted in emergency cesarean section for presumed fetal distress. Retrospective review of the cardiotocograph in two cases revealed baseline heart rates 120 to 160 bpm, with loss of trace

Germinal matrix hemorrhage-intraventricular hemorrhage (IVH) is the most common form of brain injury in preterm infants. Although severe IVH has declined over the years, it still affects approximately 6% of infants born before 32 weeks of gestation. Most IVH cases are detectable by the first 24 hours after birth; therefore interventions to prevent IVH should focus on antenatal management for pregnant women and delivery room management. Obstetrical interventions, including antenatal corticosteroids, maternal rather than infant transport, and possibly elective cesarean delivery have been associa

INTRODUCTION: Premature babies are at increased risk of hypothermia, core body temperature <97°F. Delivery room environment may contribute and lead to complications. The objective was to reduce hypothermia in babies <32 weeks of gestation in the delivery room to <40% using a checklist and sustain it for 6 months.
METHODS: We created a delivery room checklist in 2012. Chart review established a baseline rate of hypothermia (<97°F). The team analyzed the checklist's effect on hypothermia from 2012 to 2018 and utilized numerous interventions to maintain compliance. Chi-square test and Fisher's e

Bronchopulmonary dysplasia (BPD) is the most common chronic respiratory complication of preterm birth. Preterm infants are at risk for acute lung injury immediately after birth, which predisposes to BPD. In this article, we review the current evidence for interventions applied during neonatal transition (delivery room and first postnatal hours of life) to prevent BPD in extremely preterm infants: continuous positive airway pressure (CPAP), sustained lung inflation, supplemental oxygen use during neonatal resuscitation, and surfactant therapy including less-invasive surfactant administration.
