The role of the right hemisphere in the Capgras delusion.

Following a general review of the literature linking right hemisphere dysfunction to psychoses in general and Capgras delusion (CD) in particular, two studies are described that confirm the link. Study 1 revealed that, compared with matched psychiatric controls, 3 CD patients were particularly poor at matching pairs of faces shown briefly in the left visual field-which implies dysfunctioning right hemisphere performance. However there was no difference in performance when the faces appeared bilaterally, which fails to support the hypothesis that poor interhemispheric communication may give ris

Face-processing impairments and the Capgras delusion.

Investigations of two cases of the Capgras delusion found that both patients showed face-processing impairments encompassing identification of familiar faces, recognition of emotional facial expressions, and matching of unfamiliar faces. In neither case was there any impairment of recognition memory for words. These findings are consistent with the idea that the basis of the Capgras delusion lies in damage to neuro-anatomical pathways responsible for appropriate emotional reactions to familiar visual stimuli. The delusion would then represent the patient's attempt to make sense of the fact tha

[Psychiatric and parasitologic aspects of dermatozoon delusion].

Patients suffering from delusional parasitosis are not only seen by psychiatrists, but more often by dermatologists and parasitologists. Previous results published by researchers of differing specialities led to somewhat contradictory opinions concerning these patients. The main goals of the present study were, on the one hand, to examine the various and partly controversial hypotheses, which were mostly based on single case observations in a substantial sample of 107 patients with delusional parasitosis and, on the other hand, to investigate possible differences between patients who consulted

Visual hallucinations and sensory delusions in the elderly.

One hundred and fifty successive referrals to a psychogeriatrician were assessed for visual hallucinations. Forty-four (29.33 per cent) patients reported visual perceptual disturbances. No differences between hallucinators and non-hallucinators were found in terms of sex, age, length of illness, underlying psychiatric diagnosis or cognitive score. There was a significant correlation between presence of hallucinations and eye pathology (less than .001) and delusions (less than .001). The phenomenological characteristics of the visual hallucinations are analyzed. The "picture" sign is described

[A woman in her twenties with paranoid delusions].

BACKGROUND: A woman in her twenties was admitted to the psychiatric ward with suspected psychosis. Routine tests revealed a rare and potentially dangerous cause of her symptoms. CASE PRESENTATION: The patient had a history of bipolar II disorder, recurrent depressive disorder and hypothyroidism, and presented to the psychiatric emergency department with a one-week history of delusions of persecutory character and increasing insomnia. She had given birth four months prior to admission. Clinical examination was otherwise unremarkable. Bipolar psychosis was considered the most likely diagnosis,

Everything is connected: Inference and attractors in delusions.

Delusions are, by popular definition, false beliefs that are held with certainty and resistant to contradictory evidence. They seem at odds with the notion that the brain at least approximates Bayesian inference. This is especially the case in schizophrenia, a disorder thought to relate to decreased - rather than increased - certainty in the brain's model of the world. We use an active inference Markov decision process model (a Bayes-optimal decision-making agent) to perform a simple task involving social and non-social inferences. We show that even moderate changes in some model parameters -

A distinct inferential mechanism for delusions in schizophrenia.

Delusions, a core symptom of psychosis, are false beliefs that are rigidly held with strong conviction despite contradictory evidence. Alterations in inferential processes have long been proposed to underlie delusional pathology, but previous attempts to show this have failed to yield compelling evidence for a specific relationship between inferential abnormalities and delusional severity in schizophrenia. Using a novel, incentivized information-sampling task (a modified version of the beads task), alongside well-characterized decision-making tasks, we sought a mechanistic understanding of del

[Laboratory diagnosis of pseudoparasites, artifacts and parasitic delusions].

Common practice in a diagnostic parasitology laboratory involves distinguishing parasitic organisms from various artifacts. Artifacts mean pseudoparasites, supposed parasites and parasitic delusions. Pseudoparasites include undigested leftovers or coincidentally or purposely ingested nonparasitic organisms or their parts. Supposed parasites are wild organisms which were incorrectly identified as the components of faeces. In parasitic delusions, it is impossible to find any kind of parasites while patients describe imaginary parasites in detail. All the above categories of nonparasitic findings

Referential delusions of communication and interpretations of gestures.

Gestures are an important aspect of non-verbal communication, but people with schizophrenia have poor comprehension of them. However, the tests of gesture comprehension that have been used present only scenes in which interpersonal meaning is communicated, though there is evidence that people with psychotic disorders tend to perceive communications where none were intended. Such mistakes about non-verbal behaviour are the hallmark of a subtype of delusions of reference identified as delusions of communication. Thus we hypothesised that patients with delusions of communication would tend to mis

Violence in schizophrenia: role of hallucinations and delusions.

The study examines the relationship between hallucinations/delusions and violent behaviour in a sample of long-stay inpatients with chronic schizophrenia. Thirty-one subjects defined as violent and meeting DSM-111-R criteria for schizophrenia were compared with 31 matched non-violent schizophrenia patients with respect to detailed phenomenologies of auditory hallucinations using the Mental Health Research Institute Unusual Perceptions Schedule (Carter and Copolov, 1993; Carter et al., 1995) and delusions using the Maudsley Assessment of Delusions Schedule (Taylor et al., 1994). Delusions in Alzheimer's disease and multi-infarct dementia.

Neuropsychiatric symptoms such as delusions and misidentifications have been reported in dementia ranging from 10% to 73% in Alzheimer's disease (AD) patients and up to 40% in multi-infarct dementia (MID) patients. The aim of this study was to investigate in 61 AD and 31 MID patients both the frequency and the content of delusions during the course of illness and to evaluate the relationship between these and both functional and mental decline. The results indicated that delusion experiences had occurred in 45% of AD patients and in 38% of MID patients, occurring most frequently during the fir

Delusion of Pregnancy: An Updated Systematic Review.

Delusion of pregnancy (DoP) is a rare psychopathological symptom characterised by the fixed, delusional conviction of being pregnant in the absence of objective evidence. The systematic review by Bera and Sarkar remains, to this day, the principal quantitative reference on the phenomenon, based on 84 cases published up to August 2014. The aim of the present work is to update this case series by including literature published between September 2014 and August 2026, and to analyse any clinical and aetiological differences that emerge relative to the original sample. Thirty-two new cases were ide

A review and integration of models on delusion maintenance.

Contemporary definitions of delusions highlight their resistance to conflicting evidence as the core feature. However, most etiological models of delusions have focused on delusion formation rather than maintenance and we lack a coherent understanding of why delusions persist. We conducted a systematic literature search of models on delusion maintenance, extracted their core postulates, point to explanatory gaps, and derive an integrated framework. We identified 74 published accounts that include postulated mechanisms of delusion maintenance. We classified the models into six core perspectives

Capgras and Fregoli syndromes: delusion and misidentification.

Capgras and Fregoli syndromes are two psychotic and complex conditions also known as Delusional Misidentification Syndromes (DMSs). Their description dates back to the beginning of XX century, and many explanatory models have been formulated through myths, psychoanalytical and psychological hypotheses, as well as neurobiological proposals. Even if DMSs are not fully considered in the modern diagnostic manuals, they still remain intriguing phenomena to be clinically observed and explained. Also, the employment of psychotropics and physical techniques in the treatment of such conditions is not s

Kyrle's Disease in a Patient with Delusions of Parasitosis.

Acquired perforating disorders are a group of uncommon skin conditions characterized by transepidermal extrusion of altered dermal material, most often associated with diabetes mellitus and chronic kidney failure. Delusional parasitosis is a primary psychiatric disorder in which affected patients have fixed, false beliefs that their skin is infested by parasites, in the absence of any evidence supporting their statements. A 69 year old malepatient addressed the Dermatology Department for a skin eruption consisting of multiple umbilicated keratotic papules with a generalized distribution. The p

Neuroprotection in glaucoma--delusion, reality or hope?

The global definition of glaucoma considers it as an optic neuropathy with multifactor etiology, which affects the optic nerve head (ONH), provoking visual field loss and permanent impairment of visual function. Bearing in mind the fact that the exact pathogenic mechanism is still not completely established, glaucoma treatment strategies so far are based upon the identification of glaucoma risk factors. Among them, only elevated intraocular pressure (IOP) could undergo therapeutic treatment. However, in spite of the adequate therapy and IOP lowering, very often the disease is still progressing

The old book switcheroo: or anatomy of a delusion.

In this paper, an aging baby boomer "tells all" about his personal experience with some of the "pitfalls of organized religion," as observed from his somewhat unusual perspective as a Jew by birth and a Roman Catholic by choice, and a lifelong Asperger's sufferer, the butt of practical jokes in a variety of juvenile and adult settings. His conclusion, echoing H.L. Mencken's famous statement that "the cure for the evils of democracy is more democracy," is that the best antidote to these "pitfalls" may be found simply in the responsible practice of religion, including both Judaism and Christiani
