OBJECTIVES: Management of carious teeth with signs and symptoms indicative of irreversible pulpitis is traditionally invasive, but emerging evidence suggests successful treatment outcomes with less invasive vital pulp treatment such as coronal pulpotomy. The objective of this systematic review is to determine whether coronal pulpotomy is clinically effective in treating carious teeth with signs and symptoms indicative of irreversible pulpitis.

The health of dental pulp tissue is critical to maintaining normal tooth function from the eruption of permanent teeth to the formation of the apex. The study evaluated the inflamed pulp response to the mineral trioxide aggregate (MTA) after direct pulp capping with the mechanical pulp exposure in rats' incisor. Forty-eight mandibular central incisors of twenty-four Sprague-Dawley rats which were prepared with the cavities of one mm diameter, and the pulp exposures were randomly assigned into two groups: MTA group and calcium hydroxide (Ca(OH)2) group. The direct pulp capping was performed aft

The dentine-pulp complex is a unique organ that provides nourishment to the dentine and supports the tooth's innervation and defence system. Preserving its health is a key aspect of minimally invasive dentistry. Vital pulp therapies (VPTs), as defined by the European Society of Endodontology, are 'treatment strategies aimed at maintaining the health of all or part of the pulp'. These therapies are particularly useful when pulp inflammation, caused by caries or trauma, threatens pulp vitality and may otherwise lead to necrosis and the need for invasive treatment. VPTs offer several advantages,

The goal of this narrative review was to summarize and compare treatment options for permanent teeth carious lesions that are radiographically close to the pulp chamber. Thanks to adhesive restorative materials, minimally invasive approaches are possible. In this context, the old question arises as to whether caries must be excavated completely, i.e., down to hard dentin, or whether it could be advantageous to leave some soft dentin, or even merely seal the whole lesion. To answer this question, the microbiological aspects of the lesion must be considered, along with the immunological response

Pulp capping is a necessary procedure for preserving the vitality and health of the dental pulp, playing a crucial role in preventing the need for root canal treatment or tooth extraction. Here, we developed an electrospun gelatin methacryloyl (GelMA) fibrous scaffold incorporating beta-tricalcium phosphate (TCP) particles for pulp capping. A comprehensive morphological, physical-chemical, and mechanical characterization of the engineered fibrous scaffolds was performed. In vitro bioactivity, cell compatibility, and odontogenic differentiation potential of the scaffolds in dental pulp stem cel

INTRODUCTION: Vital pulp treatments (VPT) aim to conservatively manage deep caries and/or damage to preserve the vitality and function of the pulp. This minimally invasive approach is still debated when performing direct pulp capping (DPC), a treatment in which a protective biocompatible material is directly placed over the dental pulp, due to microbial risks inherent with pulp exposure and difficulties in sealing the exposure to protect the pulp from subsequent bacterial ingress. Several limitations associated with Mineral Trioxide Aggregate (MTA) such as long setting time and discoloration h

Tooth decay, which leads to pulpal inflammation due to the pulp's response to bacterial components and byproducts is the most common infectious disease. The main goals of clinical management are to eliminate sources of infection, to facilitate healing by regulating inflammation indental tissue, and to replace lost tissues. A variety of novel approaches from tissue engineering based on stem cells, bioactive molecules, and extracellular matrix-like scaffold structures to therapeutic applications, or a combination of all these are present in the literature. Shortcomings of existing conventional m

Great caution is required when choosing materials coming into direct contact with pulpal tissue. The aim of this study was to assess the connective tissue tolerance to Cap-cap and Cinacal, clinically used for pulp capping, in experimental animals. On the basis of tissue reactions around implanted material specimens at intervals of 2, 14 and 28 days after application, the two materials were found to belong to a group of biocompatible materials. Marked infiltration with inflammatory cells around Cap-cap specimens during the first few days after injection pointed to irritative feature of the mate

Among the goals of pulp capping are to manage bacteria, arrest caries progression, stimulate pulp cells to form new dentin, and produce a durable seal that protects the pulp complex. This article will provide a general discussion of direct and indirect pulp capping procedures, offering practitioners a pragmatic and science-based clinical protocol for treatment of vital pulp exposures. A clinical case will be presented in which a novel light-cured resin-modified mineral trioxide aggregate hybrid material was used to manage a mechanical vital pulp exposure that occurred during deep caries excava

INTRODUCTION: The addition of some chemicals to mineral trioxide aggregate (MTA) may improve the physical properties of the biomaterial. The aim of this study was to compare a combination of MTA with/without CaCl(2) as pulp-capping agents in dogs' teeth.
METHODS: The dental pulp of 32 teeth in four dogs were exposed and capped with either MTA or MTA/CaCl(2). After 2 months, the animals were sacrificed, and the teeth were prepared for histological evaluation. Pulpal response to the two capping materials was evaluated in terms of necrosis, inflammation, and formation of a calcified bridge. The

Pulp-capping materials are commonly adopted in the clinic to form reparative dentin and thus protect dental pulp tissues from cases of deep caries, accidentally exposed pulps or partial pulpotomy. Some traditional pulp capping materials used in the clinic include calcium hydroxide and mineral trioxide aggregates. However, there are limitations to thin restorative dentin, and a long period of time is needed to cause degenerative changes in dental pulp. In this paper, injectable colloidal gels were developed to induce the formation of reparative dentin through a simple UV method from methacrylic

When pulp tissue is damaged by caries or trauma, vital pulp therapy (VPT) can help preserve the pulp tissue for long-term retention of teeth. However, the choice of pulp capping agent used in VPT is important for the successful preservation of the pulp tissue. Here we investigated the expression and biological function of human β-defensin 4 (HBD4) in dental pulp stem cells (DPSC) and explored its potential as a pulp capping agent. We examined the expression of HBD4 in DPSC in vitro using qPCR and immunofluorescence staining. We also looked at the effect of HBD4 on inflammatory factors in lipop

Vital pulp therapy (VPT) is devised to preserve and maintain vitality of pulpally involved teeth challenged by a variety of intraoral conditions. Notable progress has been made in this field due to a better understanding of pulp physiology, improved clinical protocols and advanced bioceramic materials paired with adhesive technology. With focused case selection, conservative VPT can provide reliable treatment options for permanent teeth diagnosed with normal pulps or reversible pulpitis.

The purpose of this study was to evaluate the water sorption, solubility, pH and ability to diffuse into dentin of a glycerol salicylate-based, pulp-capping cement in comparison to a conventional calcium hydroxide-based pulp capping material (Hydcal). An experimental cement was developed containing 60% glycerol salicylate resin, 10% methyl salicylate, 25% calcium hydroxide and 5% Portland cement. Water sorption and solubility were determined based on mass changes in the samples before and after the immersion in distilled water for 7 days. Material discs were stored in distilled water for 24 h,
