Endodontic effects of root planing in humans.

Although the deliberate removal of cementum by root planing remains part of periodontal therapy, its scientific basis is under challenge. A potentially harmful effect of root planing is the exposure of dentinal tubules to the oral environment; this study investigated the short-term endodontic effects of root planing in humans. Ten teeth with significant loss of periodontal attachment had one proximal surface root planed; the other proximal surface was not root planed. After 10-14 days, the teeth were extracted and processed for light microscopic and SEM examination. Hypersensitivity to thermal

Clinical evaluation of a one-bottle bonding system for desensitizing exposed roots.

PURPOSE: To evaluate the effectiveness of a one-bottle adhesive system as a desensitizing treatment, with or without a prior phosphoric acid-etch step. MATERIALS AND METHODS: Two different clinical procedures using a one-bottle adhesive bonding system were tested for its efficacy on desensitizing exposed roots. Thirty subjects with a history of dentin hypersensitivity were selected and 60 exposed sensitive teeth were divided into two groups (n = 30). In Group 1 samples, Scotchbond 1 bonding system (Single Bond in USA) was applied without a prior acid-etching, while in those of Group 2, the ex

Bioactive Glass Empowered Mineralizable Hydrogel for Effective Dentinal Tubule Occlusion.

Dentin hypersensitivity (DH) arises from exposure of dentinal tubules, and tubular occlusion is regarded as the most effective therapy. Bioactive glasses (BGs) that can produce hydroxyapatite (HAp) minerals through reacting with saliva provide a potential opportunity for sustainable tubular occlusion. Although effective, the promotion of BGs in DH treatment has been long suffering from their poor maneuverability in practice. Here we employ a pH neutral BGs composed of a ternary Ca-P-Si system, denoted as PSC, and integrate it into a polyvinyl alcohol hydrogel to construct a composite system th

Hypersensitive dentinal pain attenuation with potassium nitrate.

Dentinal hypersensitivity occurs when gingival recession exposes dentin at the cervical margins of teeth. Twenty-four periodontal patients, with postoperative hypersensitive dentin were treated by burnishing saturated potassium nitrate (KNO3) to relieve pain. Using a visual analogue scale with participants acting as their own control, a subjective assessment of pain was measured and compared before and after KNO3 application. Thirty-six regions involving 98 teeth were assessed. A significant reduction of sensitivity and pain was achieved by using a saturated KNO3 solution (p < .0001 Student-t)

Dentin bonding: overview of the substrate with respect to adhesive material.

The current challenge in adhesive dentistry is to develop dentin bonding systems that will reproducibly achieve high bond strengths similar to those obtained between resins and acid-etched enamel. Some of the limitations of dentin as a bonding substrate are that it changes its structure as it is prepared deeper, it is difficult to dry, and its smear layer is weak. Further, it is difficult to avoid contaminating proximal boxes with blood. Such contamination lowers dentin bond strengths to very low values. Decontamination of such dentin must be done prior to resin placement. The forces of polyme

Comparative analysis of hydroxyapatite and zinc oxide nanoparticles for effective dentinal tubule occlusion in dentin hypersensitivity management: a profilometric and scanning electron microscopic investigation.

BACKGROUND: The objective of this study was to evaluate the efficacy of hydroxyapatite nanoparticles (HANPs) in comparison to zinc oxide nanoparticles (ZnO NPs) in occluding dentinal tubules (DTs). Managing dentin hypersensitivity (DH) and assessing the resilience of anti-hypersensitivity treatments present significant clinical challenges. Hence, it is imperative to explore the impact of NPs on DH treatment. METHODS: In this study, twenty- seven orthodontically extracted teeth with no caries or restorations were employed to produce 27 dentin discs. These discs were created by slicing the teet

Dentine hypersensitivity: a review of its aetiology, pathogenesis and management.

Dentine hypersensitivity (DH) is defined as pain arising from exposed dentine and represents a distinct clinical entity. Reported cases are increasing particularly among the younger age groups and are thought to be due to acidogenic diets, destructive habits, poor tooth brushing techniques, and the increased use of tooth whitening products. Dentine exposure may be due to a number of processes, both physical and chemical, that lead to either loss of enamel/ cementum or loss of gingival tissue. These causative factors seldom act in isolation and include erosion, abrasion, attrition, abfraction,

The combined occluding effects of fluoride-containing dentin desensitizer and Nd-Yag laser irradiation on human dentinal tubules: an in vitro study.

The purpose of the present study was to evaluate the combined occluding effects of fluoride-containing dentin desensitizer and neodymium:yttrium aluminum garnet (Nd-YAG) laser irradiation on human dentinal tubules. All six of the groups of dentin samples (A-F) included in this study received applications of fluoride-containing dentin desensitizer. Groups B, D, and F also received Nd-YAG laser irradiation. Groups A and B served as controls, to allow observations of the occluding effects on the dentinal tubules before and after Nd-YAG laser irradiation. Groups C and D were treated with 0.5 M vit

Effect and Stability of Poly(Amido Amine)-Induced Biomineralization on Dentinal Tubule Occlusion.

In recent years, scientists have developed various biomaterials to remineralize human teeth to treat dentine hypersensitivity. Poly(amido amine) (PAMAM) dendrimers have become a research focus in this field. It has been demonstrated that PAMAM is able to create precipitates both on the surface of and within the dentinal tubules, however, there is little information about its effect on reducing dentine permeability in vitro. This study aimed to evaluate the in vitro effectiveness and stability of the fourth generation amine-terminated PAMAM on dentinal tubule occlusion, especially on dentine pe

Tooth brushing, tooth wear and dentine hypersensitivity--are they associated?

Evidence suggests that patients suffer the painful symptoms of dentine hypersensitivity when dentine is exposed and the dentinal tubule system is opened to the oral cavity to allow stimuli to trigger a neural response in the pulp via a hydrodynamic mechanism. The processes needed to localise lesions of dentine hypersensitivity include loss of enamel and/or gingival recession. Whilst tooth brushing with or without toothpaste appears to cause minimal wear to enamel (in the absence of acids), circumstantial evidence implicates tooth brushing with gingival recession and exposure of dentine. Other

The effects of desensitizing agents on dentin permeability using a hydraulic conductance device.

This in vitro study evaluated the effects of a sodium-fluoride varnish, two surface pre-reacted glass-ionomer (S-PRG)-containing agents, and an experimental glycerol-stabilized calcium phosphate cluster (GCPC) gel on dentin permeability using a custom hydraulic conductance apparatus. Horizontal 1-mm coronal dentin disks were prepared from extracted human molars, conditioned with 2% citric acid, and randomized to ClinPro Fluoride Treatment, PRG Barrier-Coat, PRG Pro-Care Gel, experimental GCPC gel, or distilled-water control (n=10/group). Hydraulic conductance was calculated at 3 psi from the t

Effect Of Calcium Silicate, Sodium Phosphate, and Fluoride on Dentinal Tubule Occlusion and Permeability in Comparison to Desensitizing Toothpaste: An In Vitro Study.

This study compared the ability of a calcium silicate-, sodium phosphate-, and fluoride-based (CSSPF) toothpaste (TP) in promoting dentinal tubule occlusion and reducing dentin permeability with that of other commercially available antisensitivity TPs. Seventy-eight dentin discs (1.0±0.1 mm thick) were prepared from the midcoronal area and were treated with 37% phosphoric acid for 2 minutes; then they were randomly divided into six groups according to treatments: No treatment [positive control (PC)], entirely covered with nail varnish [negative control (NC)], hydroxyapatite (HAP)-containing TP

Effects of a potassium nitrate mouthwash on dentinal tubules--a SEM analysis using the dentine disc model.

The concept of tubular occlusion as a method of dentine desensitisation is a logical conclusion from the hydrodynamic hypothesis put forth by Brannström. The aim of this study was therefore to investigate qualitatively by SEM whether a 3% potassium nitrate/0.2% sodium fluoride mouthwash occluded tubule orifices, and by x-ray microanalysis, to characterise the nature of the deposits if any, following application. Following the 'dentine disc model' methodology 1mm thick tooth sections from unerupted molars were obtained. These were treated with the test and control mouthwashes and subjected to s
