Clinical assessment of common patient complaints with complete dentures.

The purpose of this study was to investigate relationships between patient complaints with complete dentures and several factors such as age, gender, medical status and denture faults. Six hundred patients who received new dentures were assessed three months after insertion of the dentures. Patient complaints were scored according to their answers to specific questions. Denture quality was recorded by one prosthodontist using a validated classification of aspects of denture quality. No significant relationship was found when age, gender, and medical status were compared with patient complaints

Clinical morbidity and sequelae of treatment with complete dentures.

Wearing complete dentures may have adverse effects on the health of both the oral and the denture-supporting tissues. This article is a review of selected literature on the sequelae of treatment with complete dentures in the specific areas of residual ridge resorption, mucosal reactions, burning mouth syndrome, temporomandibular disorders, and patient satisfaction. Recent literature found with a Medline search from 1952 to 1996 is included in this review. Residual ridge resorption is an inevitable consequence of tooth loss and denture wearing, with no dominant causative factor having been foun

Overdentures for acquired and congenital anomalies: 1. Complete dentures.

Overdenture principles can be applied to dentitions that are relatively complete without preparation or alteration of the existing teeth. Such overdentures may cover the traditional denture-bearing tissues as well as the unaltered remaining teeth. The primary advantages derived are reversibility, simplicity, and cost effectiveness. The primary indications are restoration of congenital and acquired anomalies. A detailed clinical and laboratory technique for fabricating these overdentures is presented. Effective restorations require care and attention to detail using the principles of convention

Ultrastructural investigation of hard palate mucosa under complete dentures.

An ultrastructural investigation was made to study the tissue changes occurring under complete dentures. Biopsy from 45 patient specimens were obtained from lesions of hard palate mucosa. Narrow intercellular space with less distinct bridges, loosely knit arrangement of tonofilaments, prominent endoplasmic reticulum, and vacuolar conformation of mitochondria were observed in the cells of epithelium. The underlying connective tissue was characterized by a preponderance of newly formed collagen fibers and numerous fibroblasts. Macrophages were located in the lamina propria and perivascular regio

Impact of different chemical denture cleansers on the properties of digitally fabricated denture base resin materials.

PURPOSE: To compare the impact of three different chemical denture cleansers (CDCs) (Corega, chlorhexidine, and hydrogen peroxide) on the surface roughness, microhardness, and color stability of 3D-printed, computer-aided design and computer-aided manufacturing (CAD-CAM) milled, and heat-polymerized denture base material (DBM). MATERIALS AND METHODS: A total of 420 disc-shaped specimens (10 ± 0.1 × 2 ±0.1 mm) were fabricated using three different construction techniques: three-dimensional (3D) printing (n = 140), CAD-CAM milling (n = 140), and heat-polymerization (n = 140). The bond strength of a highly cross-linked denture tooth to denture base polymers: a comparative study.

PURPOSE: This study compared shear bond strength and type of bond failure between a highly cross-linked tooth and different denture base polymers. MATERIALS AND METHODS: Cross-linked denture teeth were bonded to either a heat-, an auto-, a microwave-polymerized denture base resin or a relatively new injection-molded, microwave-polymerizable polyurethane-based resin. Six experimental groups were established for each of the shear and peel tests. In four of the groups, teeth were used as received and bonded to each of the denture base resins; in the remaining two groups, they were treated with d

Bond strength of denture teeth to denture bases in CAD/CAM-milled, printed, and conventional fabrication techniques.

OBJECTIVE: This in-vitro study examined the bond between denture teeth (milled, printed, and prefabricated) and milled, printed, and autopolymerized denture bases. METHOD AND MATERIALS: In total, 88 specimens (11 groups, n = 8 per group) of denture teeth and denture base resins were fabricated using systems of three different manufacturers (Vita Zahnfabrik, Ivoclar Vivadent, and Formlabs). The groups consisted of combinations of six different teeth, five different denture base materials, and four different adhesive bonding options. Teeth were either printed, milled, or prefabricated, while de

Impact of Occlusal Overloads on Complications in Fixed Prosthetic Dentures.

Background/Objectives: Fixed prosthetic dentures (FPD) represent a reliable treatment option to rehabilitate oral function and aesthetics. The concentration of loads at a certain level of the bridge causes overloading of the bridge components. The objectives of this study were to identify the frequency and severity of complications in FPDs associated with occlusal overload for patients prosthetically treated with metal-ceramic and metal-acrylic FPDs. Methods: This retrospective cross-sectional clinical study included 306 patients of both sexes, aged 30+, with an FPD requiring rehabilitation. T

Do maxillary dentures protect the skull base from penetration injury?

KEY CLINICAL MESSAGE: Foreign bodies penetrating from the oral cavity can damage surrounding tissues. This case is considered an extremely rare and fortunate instance in which a maxillary denture appeared to weaken the external force and change the direction of the scissors, preventing damage to vital organs. ABSTRACT: The patient was a 73-year-old man. While on a ladder pruning a plant, he accidentally fell. The gardening scissors passed through the maxillary sinus from the maxillary alveolus and penetrated below the zygomatic arch. At the time of injury, the patient was wearing a metal-fram

Overeruption of teeth opposing removable partial dentures: a preliminary study.

One of the purposes of prosthodontic treatment is to prevent overeruption of opposing teeth, but there is currently minimal literature describing the efficacy of removable partial dentures (RPDs) in performing this function. This study investigated overeruption following RPD treatment. The study participants were 33 patients treated with RPDs, and overeruption was evaluated by comparing the surface computeraided design data of dental casts made at two different time points-before and after RPD treatment. Overeruption was observed in 38.1% of teeth opposed by the RPD, which was much less than t

Prosthodontic management of radiation induced xerostomic patient using flexible dentures.

Xerostomia causes discomfort for complete denture wearers as the tissues become dry and friable due to lack of lubricating properties of saliva. Common problems faced by such patients are glossitis, mucositis, angular chelitis, dysgeusia and difficulty in chewing and swallowing. This case report describes a new method in addressing such issues by using flexible complete denture construction in radiation induced xerostomic patient with minimal tissue damage during and after denture construction procedures.

Predictable reproduction of the buccal shelf area in mandibular dentures.

The objective of this study was to assess whether a periodontal probe measurement could be used to obtain a predictable reproduction of the buccal shelf areas in mandibular dentures. One hundred patients were measured for the anterior, middle, and posterior width of the buccal shelf with a periodontal probe. This measurement was then compared with that of the corresponding portion of the existing denture. At each portion, there were statistically significant differences in the width of the buccal shelf and that of the corresponding portion of the existing denture from dental clinics of general

Oral restoration with fixed partial dentures on transplanted abutment teeth.

Thirty-five fully developed teeth were autogenously transplanted for 24 partially edentulous adult patients to enable the placement of fixed partial dentures. Restoration was successfully carried out on 22 patients during the follow-up period up to 13 years after surgery. Two restorations failed, one because of root fracture and another as a result of ankylosis of the transplanted tooth. Six transplanted abutment teeth showed root resorption and/or loss of attachment without consequence for the dental restoration. The proportion of successful tooth transplantations was 97% after 1 year, 94% af

Gingival recession related to removable partial dentures in older patients.

One hundred forty-six patients, living in the community and attending a general dental practice, were examined on two occasions separated by 3 years to determine relationships among removable partial dentures, their design, and gingival recession. This study confirmed the relationship between the presence of a partial denture and an increase of gingival recession, but was unable to demonstrate any predictable relationship to the lack of tooth support, the material used for the dentures, or covering the gingival margins with lingual plates. These design factors may be less important than the ma

Osseoretention? Comparative assessment of particulate hydroxyapatite inserted beneath immediate dentures.

This paper presents the results of a study to observe the effect on bone resorption and residual alveolar ridge form, of placement of dense non-resorbable hydroxyapatite granules into fresh canine sockets, at the time of immediate denture provision. Thirty five subjects were randomly divided into two groups cross matched for age and sex. All subjects required bilateral extraction of at least a lower canine and an adjacent tooth. Results demonstrate that six months post-extraction the control group had lost four times more bone than the implant group. These results were highly significant (P=0.

Oral mucosal lesions associated with the wearing of removable dentures.

Lesions of the oral mucosa associated with wearing of removable dentures may represent acute or chronic reactions to microbial denture plaque, a reaction to constituents of the denture base material, or a mechanical denture injury. The lesions constitute a heterogeneous group with regard to pathogenesis. They include denture stomatitis, angular cheilitis, traumatic ulcers, denture irritation hyperplasia, flabby ridges, and oral carcinomas. Denture stomatitis is the most common condition which affects the palatal mucosa in about 50% of wearers of complete or partial removable dentures. Most of
