Atopic dermatitis and ichthyosis vulgaris.

Atopic dermatitis remains a common skin problem in the pediatric age group. General approaches to management focus on reducing inflammation and pruritus as well as preventing xerosis. Ichthyosis vulgaris is the most common form of the ichthyoses and often is associated with atopic dermatitis. Recognition of these conditions is necessary to institute therapy that will alleviate the discomfort experienced by affected individuals.

[Immune mechanisms of atopic dermatitis].

The syndrome of atopy consists of three major clinical entities, i.e., allergic rhinoconjunctivitis, allergic asthma and atopic dermatitis, and is usually associated with elevated serum IgE levels. Whereas the pathogenetic role of IgE is clearly established in the case of allergic rhinoconjunctivitis and allergic asthma, the occurrence of atopic dermatitis cannot be easily explained by hyperimmunoglobulinemia E. Atopic dermatitis clinically presents as eczematization of certain predisposed (mostly flexural) areas of the skin. The clinical and histopathological picture, as well as the emergence

Atopic dermatitis: new therapeutic considerations.

Atopic dermatitis is a genetically determined inflammatory condition in which the primary defect is expressed in one or more hematopoietic cells that infiltrate the skin. It is a multifactorial disease with inflammation triggered by a variety of factors. Among these, atopic dermatitis has been experimentally induced and reproduced by emotional-stress interviews and food challenges only. The inflammatory events of atopic dermatitis appear to initiated by mast cells, but eosinophils, monocytes, and T lymphocytes (predominantly CD4) also are present in lesions. The secondary effects of inflammati

Patch testing in atopic dermatitis.

Because patients with atopic dermatitis are less prone to type IV allergies, there has been controversy regarding the role of patch testing in these patients. However, recent observations indicate that patch testing with standard allergens often adds valuable information about contact sensitivity in these patients.

Atopic dermatitis and tuberculin reactivitiy.

Transient suppression of already established tuberculin reactivity by a wide-spread, allergic contact dermatitis as well as by primary irritant contact dermatitis has been recently reported. Similar transient suppression of tuberculin reactivity by the eczematous inflammation of atopic dermatitis is now shown to occur. Thus, when the dermatitis is active, atopic dermatitis patients show diminished tuberculin reactivity. While they are in remission, however, a significant increase of their tuberculin reactivity occurs. Before the onset of dermatitis, their tuberculin reactivity was normal. Heal

Atopic sensitization and atopic dermatitis in Estonian and Swedish infants.

BACKGROUND: Early life events seem to have a major impact on the development of tolerance or sensitization. OBJECTIVE: The aim of the study was to compare the prevalence of sensitization and atopic dermatitis (AD) during the first 2 years of life in Estonia and in Sweden. METHODS: Two groups comprising 110 Estonian and 123 Swedish infants were followed from birth up to 2 years of age. Data about symptoms of allergy, infections and use of antibiotics were obtained by questionnaires. Clinical examinations, skin prick tests (SPTs) with food and inhalant allergens, and blood sampling for IgE ana

Systemic therapies for atopic dermatitis.

Atopic dermatitis is a common, chronic, relapsing skin disorder characterized by eczematous lesions and intense pruritus, associated with reduced quality of life. Advanced systemic therapies (injectable biologics and oral Janus kinase inhibitors [JAKis]) for moderate-to-severe atopic dermatitis have transformed the options for patients. Dupilumab, a biologic targeting the interleukin-4 receptor, is now approved for patients as young as 6 months and improves comorbid allergic conditions, such as asthma and eosinophilic esophagitis. In the past 4 years, tralokinumab and lebrikizumab (both target

Sodium Intake and Atopic Dermatitis.

IMPORTANCE: The association of diet with atopic dermatitis (AD) remains poorly understood and could help explain heterogeneity in disease course. OBJECTIVE: To determine the extent to which a higher level of dietary sodium intake, estimated using urine sodium as a biomarker, is associated with AD in a large, population-based cohort. DESIGN, SETTING, AND PARTICIPANTS: This cross-sectional study of adult participants (aged 37-73 years) from the UK Biobank examined 24-hour urine sodium excretion, which was estimated using a single spot urine sample collected between March 31, 2006, and October

Genetic landscape of atopic dermatitis.

PURPOSE OF REVIEW: This review summarizes recent advances in identifying genetic risk factors for atopic dermatitis and how these genetic associations are being used to explore the causal relationships between atopic dermatitis and potential risk factors and downstream outcomes. RECENT FINDINGS: A recent large-scale GWAS meta-analysis has identified 91 genetic loci associated with atopic dermatitis. Rare variant studies have also identified new gain-of-function or loss-of-function variants implicated in atopic dermatitis, particularly for FLG and STAT6/JAK1 . Finally, there has been a surge i

Potential biomarkers of atopic dermatitis.

Atopic dermatitis (AD) is a chronic, recurrent inflammatory skin disease with a wide range of heterogeneity. Accurate biomarkers or predictors are the keys to instructing personalized tailored precise treatment. The development of technology such as transcriptomics, genomics, and proteomics provides novel insights into the possibility to find potential biomarkers. Meanwhile, emerging minimally invasive methods such as tape stripping were used to reveal different profiles of patients' skin without biopsy. Several potential biomarkers or predictors have been found. In this review, we summarized

Recent advances in atopic dermatitis.

The prevalence and disease burden of atopic dermatitis (AD) is substantial. AD causes significant impairment in quality of life. It is also associated with mental disorders as well as cardiovascular diseases. Many factors including race, environment, skin barrier dysfunction, immune regulatory abnormalities, and microbiome have been reported to affect the pathophysiology of AD. A variety of cell types including Th2, Th17, Th22, and type 2 innate lymphoid cells contribute to AD. Cytokines from these immune cells cause abnormal epidermal differentiation and skin barrier dysfunction. Moreover, mi

Food allergy and atopic dermatitis.

Atopic dermatitis (AD), characterized by intense pruritus, eczematous lesions, and a relapsing disease course, is a chronic inflammatory skin disease that affects both children and adults. AD often begins in infancy and is associated with atopic diseases in the personal or family history.1 Environmental factors may trigger AD by affecting the skin barrier and by triggering inflammation. The elicitation of T-helper type 2 cytokines further impairs the epidermal barrier and leads to the penetration of irritants and allergens into the epidermis and thereby perpetuating inflammation. The presence

Atopic dermatitis in elderly adults.

BACKGROUND: Atopic dermatitis (AD) in adults over 45 years of age (AD≥45) has been poorly studied. OBJECTIVES: To determine whether the AD phenotype varies according to the pattern of AD onset in AD≥45 patients and whether response to cyclosporine A (CsA) is influenced by age. MATERIALS AND METHODS: This monocentric retrospective study was performed in a French university department of dermatology. We included 409 AD<45 patients (111 treated with CsA) and 124 AD≥45 patients (26 treated with CsA). Among AD≥45 patients, 20% were categorised into Subgroup 1 (persistence of AD since childhood),

Management of Atopic Hand Dermatitis.

This article provides an overview of clinical aspects of hand eczema in patients with atopic dermatitis. Hand eczema can be a part of atopic dermatitis itself or a comorbidity, for example, as irritant or allergic contact dermatitis. When managing hand eczema, it is important to first categorize the subtype and identify potential culprit allergens or irritants. First-line therapy should be a combination of emollients and topical corticosteroids; possible alternatives include topical calcineurin inhibitors or coal tar. Second-line therapy includes UV therapy and systemic therapy, including azat
