Dermatologic Reactions to Immune Checkpoint Inhibitors : Skin Toxicities and Immunotherapy.

The development of immune checkpoint inhibitors [monoclonal antibodies targeting cytotoxic T lymphocyte-associated antigen-4 (CTLA-4), programmed cell death protein 1 (PD-1) or programmed death ligand 1 (PD-L1)] represents a major breakthrough in cancer therapy. Although they present a favorable risk/benefit ratio, immune checkpoint blockade therapies have a very specific safety profile. Due to their unique mechanism of action, they entail a new spectrum of adverse events that are mostly immune related [immune-related adverse events (irAEs)], notably mediated by the triggering of cytotoxic CD4

Capsaicin in the treatment of dermatologic disease.

Capsaicin cream is the first of a class of neuropeptide active agents to be introduced into dermatologic therapy. Capsaicin's effects appear primarily related to its ability to deplete the neuropeptide substance P from local sensory terminals in the skin. The use of capsaicin cream in the treatment of postherpetic neuralgia and psoriasis is discussed. I believe that capsaicin and other neuropeptide active agents may become important therapeutic modalities for the dermatologist in the near future.

Dermatologic Toxicity Occurring During Anti-EGFR Monoclonal Inhibitor Therapy in Patients With Metastatic Colorectal Cancer: A Systematic Review.

Monoclonal antibody inhibitors of the epidermal growth factor receptor (EGFR) have been shown to improve outcomes for patients with metastatic colorectal cancer (mCRC) without RAS gene mutations. However, treatment with anti-EGFR agents can be associated with toxicities of the skin, nails, hair, and eyes. Because these dermatologic toxicities can result in treatment discontinuation and affect patient quality of life, their management is an important focus when administering anti-EGFR monoclonal antibodies. The present systematic review describes the current data reporting the nature and incide

Pharmacovigilance analysis of cutaneous adverse drug reactions for cetuximab, panitumumab, and necitumumab based on EudraVigilance and WHO VigiAccess.

INTRODUCTION: Anti-EGFR monoclonal antibodies (mAbs)--cetuximab, panitumumab, and necitumumab--are cornerstone therapies for selected solid tumors but are frequently associated with cutaneous adverse drug reactions (ADRs). This study aimed to characterize and compare the dermatologic safety profiles of these agents using large-scale post-marketing pharmacovigilance data. METHODS: A comprehensive pharmacovigilance analysis was conducted using aggregated ADR reports from the WHO VigiAccess and European Medicines Agency (EMA) EudraVigilance databases. Disproportionality analyses were performed u

Dermatologic adverse events in pediatric patients receiving targeted anticancer therapies: a pooled analysis.

BACKGROUND: The dermatologic adverse events (AEs) of various molecularly targeted therapies are well-described in adult cancer patients. Little has been reported on the incidence and clinical presentation of such AEs in pediatric patients with cancer. To address this gap, we analyzed the dermatologic AEs reported across clinical trials of targeted anticancer therapies in pediatric patients. PROCEDURES: We conducted an electronic literature search (PubMed, American Society of Clinical Oncology annual meetings' abstracts, ClinicalTrials.gov, NCI's Pediatric Oncology Branch webpage) to identify

Dermatologic toxicities to inhibitors of cyclin-dependent kinases CDK 4 and 6: An updated review for clinical practice.

Cyclin-dependent kinase (CDK) 4/6 inhibitors (palbociclib, ribociclib and abemaciclib) have revolutionized the treatment of metastatic breast carcinoma. They currently form the first-line treatment, in combination with endocrine agents, for the management of locally advanced or metastatic hormone receptor-positive (HR + ), human epidermal growth factor receptor 2-negative (HER2-) breast cancer, the largest subtype of breast carcinoma. CDK 4/6 inhibitors have shown comparable efficacy outcomes with predictable and manageable adverse events. In this setting, dermatologic toxicity appears to be r

Taxane-induced scleroderma. Report of two cases.

Taxanes are a class of chemotherapeutic agents with common associated dermatologic adverse events, such as skin hyperpigmentation, hand-foot skin syndrome, paronychia and onycholysis. Taxane-induced scleroderma is rare. Few cases with skin findings resembling systemic sclerosis, have been reported after the administration of these agents. We report two cases with stage IV breast cancer, aged 66 and 71 years, who developed sclerodermic skin lesions in their extremities after starting treatment with placlitaxel and nabplaclitaxel respectively.

Widespread use of toxic skin lightening compounds: medical and psychosocial aspects.

Hyperpigmentation disorders and skin lightening treatments have a significant impact on the dermatologic, physiologic, psychologic, economic, social, and cultural aspects of life. Skin lightening compounds, such as hydroquinone and topical corticosteroids, are often used to treat hyperpigmentation disorders, such as melasma, or lighten skin for cosmetic purposes. Despite their established effectiveness, a multitude of dermatologic and systemic complications have been associated with these agents. Regulatory agencies have also recognized the adverse effects of skin lighteners and many countries

JAAD CME Part 1: Mechanism of Action of GLP-1 Receptor Agonists and Potential Pathways in Skin Health.

The prevalence of obesity and type 2 diabetes mellitus (T2DM) continues to rise in the United States (U.S.) and worldwide, contributing to substantial morbidity and mortality through metabolic dysfunction and systemic inflammation. Glucagon-like peptide-1(GLP-1)-based therapies, originally developed for glycemic control, have since demonstrated significant weight loss benefits and multiple effects across multiple organ systems. This review provides an overview of the historical development and mechanistic underpinnings of GLP-1 receptor agonists (GLP-1RAs) and dual GLP-1/glucose-dependent insu

A Closer Look at the Dermatological Profile of GLP-1 Agonists.

BACKGROUND/OBJECTIVES: Glucagon-like peptide-1 receptor agonists (GLP-1RAs) are widely used in treating type 2 diabetes and obesity, offering established metabolic and cardiovascular benefits. Emerging evidence suggests these agents also exert direct dermatologic effects. This systematic review categorizes these effects and explores their role in inflammatory skin diseases. METHODS: A comprehensive literature search was performed across EMBASE, PubMed, Web of Science, and Google Scholar for studies published from 2014 to 2025. Inclusion criteria were English-language, peer-reviewed original r

Disseminated Superficial Actinic Porokeratosis in a Woman With Metastatic Pancreatic Cancer During Chemotherapy With Capecitabine and Nab-Paclitaxel.

We report a case of disseminated superficial actinic porokeratosis (DSAP) in a 73-year-old woman with metastatic pancreatic cancer undergoing chemotherapy with capecitabine and nab-paclitaxel. This case suggests a possible association between DSAP and the use of these chemotherapeutic agents, emphasising the need for awareness of atypical dermatologic reactions in such patients.

Part 2: Management of intraoperative and perioperative bleeding.

With an increase in number of patients on antithrombotic therapies, management of bleeding during dermatologic surgery is increasingly important. As described in Part 1, perioperative discontinuation of antithrombotic therapies may increase the risk of embolic events thus the risks and benefits must be weighed carefully when deciding whether to continue or suspend therapy. However, continuing oral anticoagulants may result in increased intraoperative and postoperative bleeding. Here we describe various methods to effectively achieve hemostasis which include (1) mechanical methods to compress t

Cutaneous Adverse Effects of Hydroxychloroquine and Leflunomide in Connective Tissue Diseases: A Report of Three Cases and a Literature Review.

Connective tissue diseases (CTDs) are a group of systemic autoimmune disorders that require long-term treatment with immunomodulatory and immunosuppressive agents to control disease progression and inflammation. Among the most frequently used agents are hydroxychloroquine and leflunomide, which have proven efficacy in conditions such as systemic lupus erythematosus, rheumatoid arthritis, and Sjögren's syndrome. The use of these drugs has been correlated with dermatologic adverse reactions of varying severity, spanning from common and mild skin eruptions to rare but severe cutaneous syndromes.

Clinical Characteristics and Treatment of Immune-Related Adverse Events of Immune Checkpoint Inhibitors.

Immune checkpoint inhibitors (ICIs) have been changing the paradigm of cancer treatment. However, immune-related adverse effects (irAEs) have also increased with the exponential increase in the use of ICIs. ICIs can break up the immunologic homeostasis and reduce T-cell tolerance. Therefore, inhibition of immune checkpoint can lead to the activation of autoreactive T-cells, resulting in various irAEs similar to autoimmune diseases. Gastrointestinal toxicity, endocrine toxicity, and dermatologic toxicity are common side effects. Neurotoxicity, cardiotoxicity, and pulmonary toxicity are relative

Antithrombotic Drug Eruptions in Dermatology Practice: Selection Bias, Clinical Phenotypes, and Diagnostic Approaches.

Antithrombotic agents are among the most frequently prescribed medications in elderly patients and are essential for the prevention and treatment of cardiovascular and cerebrovascular diseases. Although cutaneous adverse reactions associated with anticoagulants and antiplatelet agents are generally considered uncommon, dermatologists frequently encounter these medications when evaluating patients presenting with pruritic eruptions. This apparent discrepancy may be explained by differences in patient populations. While cardiologists and neurosurgeons manage all patients receiving antithrombotic

Dermatologic Implications of Glycemic Control Medications for Patients with Type 2 Diabetes Mellitus.

Type 2 diabetes mellitus (T2DM) is a chronic disease with a rising prevalence worldwide that is characterized by hyperglycemia and insulin resistance. Metformin, which improves blood glucose levels and insulin sensitivity, is considered the first-line therapy for T2DM. In patients with T2DM who are overweight or obese, glucagonlike peptide 1 (GLP-1) and dual GLP-1/gastric inhibitory peptide (GIP) agonists are used as adjunct or alternative therapies given their dual benefits for insulin sensitivity and weight loss. Beyond the metabolic implications of T2DM medications, these agents also have h

Perioperative management of dermatologic surgery patients on systemic cancer therapy.

The rapid expansion of immunotherapy and targeted oncologic treatments leading to improved overall survival has led to a growing number of patients requiring dermatologic surgery for skin cancer while receiving systemic cancer therapy. However, perioperative management in this setting remains poorly defined. Dermatologic surgeons and oncologists must balance risks of impaired wound healing, bleeding, and infection against potential consequences of skin cancer progression versus treatment interruption, underscoring the need for multidisciplinary guidance. Here, we review perioperative managemen
