Pacing and clinical electrophysiology : PACE2025eng
Post-implantation infections of pacemakers or defibrillators represent a serious complication, impacting patient morbidity and mortality. We present a case involving a 75-year-old male with multiple comorbidities who developed pocket erosion of a previously implanted biventricular defibrillator, complicated by Staphylococcus haemolyticus infection. Following device extraction and targeted antibiotic therapy, an extravascular implantable cardioverter defibrillator (EV-ICD; Aurora EV-ICD) was implanted. The procedure was successfully performed, with a 2-month follow-up showing no signs of infect

BACKGROUND: Complete device and lead removal is recommended for management of infected implantable cardiac device. Management of large lead vegetation (2 cm) is still in debate.
METHODS: We report a series of patients involving percutaneous extraction of large vegetations (>2 cm) from ICD/pacing leads using the AngioVac Cannula in patients with infective endocarditis. This approach was used to debulk the ICD/pacing lead vegetations in order to minimize the risk of septic pulmonary embolism during lead explantation.
CONCLUSION: AngioVac Cannula can be used safely and effectively as an adjunct

BACKGROUND: The prevalence of cardiac device-related infections (CDIs) has mirrored the unprecedented increase in device usage. CDIs are currently one of the leading indications for extraction. Despite this, there is limited data regarding the clinical trends, management and outcomes associated with this complication.
METHODS: A review of a prospective registry of all patients undergoing device extraction between January 1, 2004, and June 15, 2009, at a single high-volume tertiary referral center was performed.
RESULTS: A total of 506 consecutive patients were identified. From these, 350 pat

Residual fibrous structures, referred as "ghosts", are occasionally encountered following lead extraction. Though typically detected in patients with cardiac device infections, their prognostic and therapeutic implications remain speculative. We describe a 62-year-old gentleman with history of sick sinus syndrome who presented atrial fibrillation with rapid ventricular response. The patient had history of transvenous lead extraction following pacemaker pocket infection 6 months earlier. In echocardiography, an echo-dense tubular lead-like structure was seen that followed the route of a pacer l

Cardiac implantable electronic device (CIED) lead erosion and device expulsion are rare complications that are associated with significant morbidity and mortality. We describe the case of a 74-year-old man who presented with fever, chills, and night sweats after spontaneous implantable cardiac defibrillator expulsion over a decade after its implantation. A transthoracic echocardiogram revealed a large intracardiac vegetation, and a combination of blood and hardware cultures identified Staphylococcus hominis, Corynebacterium striatum, and Citrobacter koseri. Management involved percutaneous veg

BACKGROUND: Implantation of cardiac implanted electronic device (CIED) has surged lately. This resulted in a rise in cardiac device-related infections (CDI) and inevitably, lead extractions. We examined the recent national trend in the incidence of CIED infections and lead extractions in hospitalized patients and associated mortality.
METHODS: Using the Nationwide Inpatient Sample for the years 2003-2011 we identified patients diagnosed with a CDI-associated infection as determined by discharge ICD-9 diagnostic codes. We examined the trend of device-related infections overall and in different

Cardiac management devices have become an integral part of our armament for treatment of heart diseases. However, complications may arise that mandate extraction of either the device or the lead. The noninterventional lead extraction has become a topic of avid debate as simple traction is associated with low success rates whereas laser-assisted extraction carries a high economic cost. Herein we present a case of 22-year-old pacemaker leads extracted with a novel "trouserslike technique" that could present an attractive alternative for leads implanted for more than 10 years when laser sheaths a

PURPOSE: Rapidly growing mycobacteria (RGM) are uncommon causes of cardiac implantable electronic device (CIED) infection and present unique diagnostic and therapeutic challenges. Data describing their clinical characteristics, management, and outcomes remain limited, and comparative information with tuberculous mycobacterial CIED infections is scarce. This study aimed to describe a single-center case series of RGM-associated CIED infections and to synthesize published cases to characterize clinical presentation, management, and outcomes, with exploratory comparison to Mycobacterium tuberculos

Cardiac electrophysiology procedures have evolved to provide improvement in morbidity and mortality for many patients. Cardiac resynchronization therapy (CRT), implantable cardioverter/defibrillator (ICD) placement and lead extraction procedures are proven procedures, associated with significant reductions in patient morbidity and mortality as well as improved quality of life. The applications and optimization of these therapies are an evolving field. The optimal use and outcomes of cardiac electrophysiology procedures require a multidisciplinary approach to patient selection, device selection

INTRODUCTION: Cardiac implantable electronic device (CIED) trans venous lead extraction (TLE) is technically challenging. Whether the use of a laser sheath reduces complications and improves outcomes is still in debate. We therefore aimed at comparing our experience with and without laser in a large referral center.
METHODS: Information of all patients undergoing TLE was collected prospectively. We retrospectively compared procedural outcomes prior to the introduction of the laser sheath lead extraction technique to use of laser sheath.
RESULTS: During the years 2007-2017, there were 850 att

INTRODUCTION: As the population ages, the number of elderly patients with implantable cardiac devices referred for transvenous lead extraction will dramatically increase in Western countries. The safety and effectiveness of lead extraction in elderly patients has not been well evaluated. We report the safety and effectiveness of transvenous lead extraction in octogenarians.
METHODS AND RESULTS: From January 2005 to January 2011, we reviewed data from consecutive patients ≥ 80 years referred to our institutions for transvenous lead extraction because of cardiac device infection or lead malfunc

Background: The lead breakage (LB) during transvenous lead extraction (TLE) increases procedural complexity, increases the risk of complications, and decreases procedural efficiency. This study aimed to identify protective and risk factors for the breakage of cardiac electronic device leads during extraction. Methods: Data were sourced from the EXTRACT prospective registry for TLE procedures conducted between January 2016 and June 2025. A total of 702 consecutive TLE procedures involving 1375 leads were enrolled. Multivariate logistic regression was used to identify independent protective and

Cardiac implantable electronic device leads can contribute to tricuspid regurgitation and also complicate surgical and transcatheter interventions to manage tricuspid regurgitation. Here we present a case of a patient with sinus node dysfunction and complete heart block who underwent extraction of a right ventricular pacing lead before tricuspid valve surgery. We review the data regarding the contribution of leads to tricuspid regurgitation and the benefits of lead extraction, risks of jailing leads during tricuspid interventions, and pacing considerations around tricuspid valve procedures.

A 68-year-old man, 54 months after having been implanted with a biventricular device, underwent successful extraction of the malfunctioning left ventricular (LV) lead using mechanical dilation. During LV lead reimplantation, venography documented stenosis of the coronary sinus (CS). To overcome the obstacle, balloon angioplasty was performed and a LV lead was then inserted into a lateral tributary of the CS. The procedure was complicated by local infection and, after 2 months, removal of the entire unit became necessary. During controlateral device implantation, a second angioplasty was carrie

PURPOSE: The leadless pacemaker (LP) is a promising new technology in pacing therapy that avoids a generator pocket and transvenous lead-related complications. However, extraction experience with simultaneous re-implant of a leadless device remains limited. We sought to study the outcomes of Nanostim LP retrieval and simultaneous reimplantation of Micra LP.
METHODS: Patients undergoing retrieval of the Nanostim LP and receiving a Micra LP implant between 2016 and 2018 at Mayo Clinic (Rochester, MN) were studied. The procedural and outcome data were collected from the electronic medical record

Superior vena cava (SVC) syndrome is a rare disease induced by thrombosis and consequent occlusion of SVC, negatively affecting morbidity and mortality. The incidence of SVC syndrome from central venous catheters and pacemaker or defibrillator leads is increasing. Optimal treatment of pacemaker or defibrillator-related SVC syndrome is not well defined. Lead extraction causes mechanical trauma to the vessel wall. In addition, subsequent device implantation on the contralateral side can be an added factor for venous occlusion. The use of leadless pacemakers could be an interesting option to redu

BACKGROUND: Superior vena cava (SVC) lacerations have been identified as the most lethal complication encountered during cardiac implantable electronic device lead extraction. The case fatality rate of these events approximates 50% due to rapid exsanguination. A novel, compliant balloon specifically designed for use in the SVC may provide hemostasis in the event of endovascular perforation. By temporarily occluding the compromised vessel, the endovascular balloon should delay hemodynamic collapse, provide a more controlled surgical field for repair, and thereby reduce the mortality of SVC tear

Cardiac device-related infective endocarditis complicated by end-stage heart failure poses a major therapeutic challenge because complete hardware removal is essential, whereas transvenous lead extraction may be poorly tolerated in patients with severe ventricular dysfunction and shock. We report a 53-year-old man with long-standing dilated cardiomyopathy and cardiac resynchronization therapy-defibrillator infection who presented with methicillin-sensitive Staphylococcus aureus bacteremia, pocket infection, respiratory failure, acute kidney injury, and a clinical presentation consistent with m
