Dexmedetomidine hemodynamics in children after cardiac surgery.

BACKGROUND: Dexmedetomidine has opposing effects on the cardiovascular system. Action in the central nervous system produces sympatholysis and a reduction in blood pressure, while peripherally it causes vasoconstriction leading to an increase in blood pressure. The purpose of our study is to define the concentration-response profile for these hemodynamic effects in children after cardiac surgery. METHODS: A simultaneous pharmacokinetic-pharmacodynamic analysis of data from 29 children given a single bolus of dexmedetomidine 1-4 mcg.kg(-1) following cardiac surgery was undertaken using mixed e

Sedation using dexmedetomidine in pediatric burn patients.

Maintaining appropriate sedation and analgesia in pediatric burn patients can be quite challenging and often requires high doses of analgesics and anxiolytics because tolerance quickly develops. Escalating doses of opioids and benzodiazepines provide little additional benefit while increasing the incidence of side effects. Dexmedetomidine (DEX) is a novel alpha2-adrenergic agonist that provides sedation, anxiolysis, and analgesia with much less respiratory depression than other sedatives. In addition, DEX stimulation of alpha2 receptors on pancreatic beta cells may inhibit insulin secretion. H

SAFETY AND EFFICACY OF DEXMEDETOMIDINE-KETAMINE COMBINATION VS DEXMEDETOMIDINE ALONE FOR PEDIATRIC DENTAL SEDATION: A SYSTEMATIC REVIEW AND META-ANALYSIS.

BACKGROUND: Dexmedetomidine (D) and ketamine are widely used for pediatric dental sedation, each presenting unique benefits and potential adverse effects. Combining these agents, referred to as DK, was suggested to provide a balanced approach to pediatric sedation. This is the first systematic review and meta-analysis to evaluate the efficacy and safety of DK versus dexmedetomidine alone in pediatric dental procedures. METHODS: A systematic search was conducted in PubMed, Scopus, Web of Science, Embase, and CENTRAL, identifying randomized controlled trials (RCTs) comparing DK to D alone. Elig

Comparison of intranasal dexmedetomidine-midazolam, dexmedetomidine-ketamine, and midazolam-ketamine for premedication in paediatric patients: a double-blinded randomized trial.

BACKGROUND: Paediatric patients are a population with a high level of anxiety. The prevention of perioperative stress in a frightened child is important to render the child calm and cooperative for smoother induction. Intranasal premedication is easy and safe, and the drug is rapidly absorbed into the systemic circulation, ensuring early onset of sedation in children and good effectiveness. METHODS: 150 patients in the age group 2-4 years, ASA class I, undergoing elective surgical procedures were enrolled. The patients were randomly divided into 3 groups: a DM group (receiving intranasal dexm

Comparison of dexmedetomidine alone with dexmedetomidine and fentanyl during awake fiberoptic intubation in patients with difficult airway: a randomized clinical trial.

BACKGROUND: Awake fiberoptic intubation (AFOI) is the procedure of choice for securing the airway in patients with a difficult airway when undergoing surgeries under general anesthesia. An ideal drug would not only provide conscious sedation but also maintain spontaneous ventilation, smooth intubation conditions, and stable hemodynamics. We compared the effects of dexmedetomidine alone and dexmedetomidine in combination with fentanyl at a dose lower than the standard dose for achieving conscious sedation during AFOI in difficult airway patients undergoing oral cancer and dental surgeries. Enhancement of allograft acceptance by combined dexmedetomidine and rapamycin.

BACKGROUND: Dexmedetomidine, an α2-adrenoceptor agonist, is known for its sedative effects and unique pharmacological mechanism. Research increasingly emphasizes its potential to modulate immune responses. Further investigation is needed to fully understand how dexmedetomidine influences T cell differentiation and its potential synergy with tolerance-promoting agents. This study aims to explore a new approach to enhance allograft acceptance by combining dexmedetomidine with the mTOR inhibitor rapamycin. METHODS: The effects of both drugs on T cell proliferation, regulatory T cell preservation

Combining melatonin with dexmedetomidine improves anesthesia in rats.

Melatonin (ML) and dexmedetomidine (DM) are used separately as anesthetic premedication or as an anesthetic in humans and laboratory animals. In this study, we aimed to investigate the anesthetic properties of both drugs combined. The anesthetic effects of several combinations of ML (50 and 100 mg/kg) and DM (50 and 100 μg/kg) were evaluated in rats by observing behavioral manifestations and recording the duration and depth of anesthesia. Five anesthetic intervals were established according to the loss and recovery of reflexes. While each individual drug did not induce an appropriate anestheti

Appraisal of the Neuroprotective Effect of Dexmedetomidine: A Meta-Analysis.

Dexmedetomidine is an adrenergic receptor agonist that has been regarded as neuroprotective in several studies without an objective measure to it. Thus, the aim of this meta-analysis was to analyze and quantify the current evidence for the neuroprotective effects of dexmedetomidine in animals. The search was performed by querying the National Library of Medicine. Studies were included based on their language, significancy of their results, and complete availability of data on animal characteristics and interventions. Risk of bias was assessed using SYRCLE's risk of bias tool and certainty was

Dexmedetomidine: A Sedation Alternative in the Intensive Care Setting.

In the last 20 years, the occurrences of drug shortages have increased in frequency as well as duration before returning to the mainstream market. This has prompted intensive care unit nurses and medical staff to seek alternate medication infusion options that provide safe yet effective sedation for patients admitted to intensive care units across the country. Dexmedetomidine (PRECEDEX) emerged in 1999 after the Federal Drug Administration approved it for intensive care use but was quickly embraced by anesthesia providers as it rendered patients undergoing procedures or surgery with adequate a
