Practice parameters for the treatment of narcolepsy and other hypersomnias of central origin.

These practice parameters pertain to the treatment of hypersomnias of central origin. They serve as both an update of previous practice parameters for the therapy of narcolepsy and as the first practice parameters to address treatment of other hypersomnias of central origin. They are based on evidence analyzed in the accompanying review paper. The specific disorders addressed by these parameters are narcolepsy (with cataplexy, without cataplexy, due to medical condition and unspecified), idiopathic hypersomnia (with long sleep time and without long sleep time), recurrent hypersomnia and hypers

The Therapeutic Potential of Amphetamine-like Psychostimulants.

This review delves into the therapeutic applications of amphetamine-type stimulants such as lisdexamphetamine dimesylate, mixed amphetamine salts, 3,4-methylenedioxymethamphetamine (MDMA), dextroamphetamine, and phentermine. These compounds have been investigated for their potential in treating a range of psychiatric disorders, including attention deficit hyperactivity disorder (ADHD), drug dependence, post-traumatic stress disorder (PTSD), and obesity. Lisdexamphetamine dimesylate has shown promise in effectively treating ADHD symptoms in both children and adults. Additionally, it has been ex

[New-onset trichotillomania during treatment with stimulant drugs. About two pediatric clinical cases].

Stimulant drugs are commonly used in pediatric population in the treatment of attention deficit hyperactivity disorder, and their side effects are well described, however trichotillomania does not appear as one of them. In the literature we found some published cases of trichotillomania in relation to methylphenidate and dextroamphetamine. We present two cases of new-onset trichotillomania in children followed up in our center by attention deficit hyperactivity disorder and treated with psychostimulant drugs (methylphenidate and lisdexamfetamine), as a probable adverse effect of this treatment

Sympathetic nervous system disorder of women that leads to pelvic pain and symptoms of interstitial cystitis may be the cause of severe backache and be very responsive to medical therapy rather than surgery despite the presence of herniated discs.

PURPOSE: To describe a unique pharmacologic treatment for backache that seemed to be related to herniated discs. METHODS: Dextroamphetamine sulfate was prescribed to a woman who developed acute lower backache which was attributed to herniated lumbar discs. RESULTS: Within hours the pain diminished and she was pain free within a week. The 100% relief has persisted for months. CONCLUSIONS: Acute backache can be added to the long list of pain syndromes related to impairment of the sympathetic nervous system that is effectively treated with sympathomimetic amines.

Modafinil as a potential motion sickness countermeasure.

INTRODUCTION: Motion sickness adversely affects military air and sea operations. Medications help prevent motion sickness but are frequently associated with side effects. Better medications or combinations of medications are needed. Dextroamphetamine has documented anti-motion sickness effects but also has a potential for abuse. Modafinil is a relatively new central nervous system stimulant that has none of the drawbacks of dextroamphetamine, but has not been evaluated for the treatment of motion sickness. METHODS: This double-blind, placebo-controlled study evaluated the anti-motion sickness

Sudden death of a child treated with imipramine. Case study.

Since 1990, there have been seven reported cases of sudden death in children treated with tricyclic antidepressants. This case study describes the sudden death of an additional child (8 years 9 months old, 29 kg in weight) treated with a tricyclic antidepressant, imipramine (100 mg orally, twice daily), and dextroamphetamine (10 mg orally every morning). Prior to death, there were signs of possible cardiovascular abnormalities after treatment was begun with imipramine. The report adds to the concern over the use of tricyclics in children.

Khat-induced hypnagogic hallucinations.

Khat is a plant whose leaves are chewed for their stimulating effect. This effect is attributed to cathinone, an alkaloid identical to dextroamphetamine. Khat chewing is widespread among eastern African and Yemenite populations and is believed to be innocuous. Our experience shows, however, that a substantial number of chronic khat chewers experience persistent hypnagogic hallucinations - a symptom that has not yet been described. Three vignettes illustrates this phenomena, which often interferes with psychiatric diagnosis. Different explanatory models are discussed, among them chronic suppres

Sympathomimetic amines in the treatment of chronic urticaria: two case reports.

Two patients are presented whose chronic urticaria had previously failed to improve with antihistamine therapy or glucocorticoids. Both demonstrated marked improvement with dextroamphetamine sulfate. Both patients also had idiopathic orthostatic edema for which the treatment of choice is amphetamines. When treatment was discontinued the urticarial lesions returned, only to decrease significantly again when therapy was resumed. Whether the response to amphetamines in chronic urticaria is unique to patients with idiopathic orthostatic edema or will also prove to be effective in treating typical

Response of HIV-related depression to psychostimulants: case reports.

Four depressed and cognitively impaired patients with HIV-related disease had a marked therapeutic response to treatment with psychostimulants. Use of dextroamphetamine and methylphenidate brought a prompt remission of depressive and cognitive dysfunctions without adverse side effects. The results suggest the need for further evaluation of psychostimulants in the treatment of HIV patients whose depression proceeds from an affective disturbance (either primary or secondary) or from a specific organic mental disorder. The importance of neuropsychiatric assessment of depressed HIV patients is str

Risk of Incident Psychosis and Mania With Prescription Amphetamines.

OBJECTIVE: Amphetamine prescribing has increased in the United States in recent years. Previous research identified an increased risk of incident psychosis with prescription amphetamines. The purpose of this study was to examine the impact of dose levels of prescription amphetamines on the risk of this rare but serious adverse outcome. METHODS: A case-control study using electronic health records was conducted to compare the odds of incident psychosis or mania with past-month exposure to prescription amphetamines. Case subjects were patients ages 16-35 hospitalized at McLean Hospital for inci

Amphetamine and Clonidine Toxicity Resulting in Posterior Reversible Encephalopathy Syndrome.

Amphetamine toxicity typically presents with hypertension and tachycardia. Conversely, clonidine acts as an agonist at central α2 and imidazoline receptors, which may cause brief initial hypertension followed by hypotension and bradycardia in overdose. We report a case of mixed ingestion resulting in posterior reversible encephalopathy syndrome (PRES) successfully treated with phentolamine.A 17-year-old male adolescent presented to the emergency department 2 hours after ingesting up to 25 each of clonidine 0.1-mg tablets and dextroamphetamine 10 mg extended-release capsules. He reported nausea

Treatment modalities for narcolepsy.

Narcolepsy, a lifelong disorder, requires long-term management of symptoms. Interventions may be nonpharmacologic, such as lifestyle changes, and pharmacologic for relief of daytime sleepiness. Pharmacologic treatment of narcolepsy has depended on the use of CNS stimulants to increase wakefulness, vigilance, and performance. The medications considered effective in the treatment of narcolepsy include dextroamphetamine, pemoline, methylphenidate, methamphetamine, and modafinil; only methylphenidate hydrochloride and dextroamphetamine are approved for use in the United States. The currently avail

The sympathetic neural hyperalgesia/edema syndrome, a common cause of female pelvic pain, manifesting as a pseudopheochromocytoma with marked clinical improvement with sympathomimetic amines.

PURPOSE: To show that a common but not well-known disorder of the sympathetic nervous system can present with symptoms suggesting a pheochromocytoma. MATERIALS AND METHODS: The standard treatment of this disorder (which is characterized by an abnormal water load test), i.e., sympathomimetic amine therapy, was given to a woman with paroxysmal tachycardia and hypertension. RESULTS: Over a period of six months, the treatment eradicated the paroxysmal symptoms to which all other therapies had failed. CONCLUSIONS: This condition recently named as sympathetic neural hyperalgesia edema syndrome ca

Gilles de la Tourette's syndrome. Familial occurrence and precipitation by methylphenidate therapy.

Gilles de la Tourette's syndrome was independently ascertained in two male cousins once removed. Previous studies have shown familial clustering of individuals with tics, but no consistent pattern of inheritance of Tourette's syndrome has been apparent. The onset and later exacerbation of symptoms in our younger patient were associated with the administration of CNS stimulants given for excessive motor activity. The adverse effects of methylphenidate and dextroamphetamine therapy on Tourette's syndrome supports the hypothesis that this condition results from a relative excess of CNS catecholam

Sonographic findings in methamphetamine-induced ischemic colitis.

Ischemic colitis is a clinicopathologic condition that commonly occurs in elderly patients with atherosclerotic disease or diabetes mellitus. Uncommon etiologies include vasculitis and use of drugs such as oral contraceptives, phenobarbital, nasal decongestants, dextroamphetamine, and cocaine. Recent studies have shown sonography and CT to be helpful in the evaluation of the colitides. We report the sonographic and CT findings in an unusual case of methamphetamine-induced ischemic colitis. Sonography and CT revealed diffuse thickening of the large bowel wall. Methamphetamine abuse should be co

The confounding effect of the development of idiopathic orthostatic edema and thyrotoxcosis on weight fluctuation related to effects on free water clearance in a woman with long-standing surgically induced panhypopituitarism and diabetes insipidus.

PURPOSE: To evaluate the effect of idiopathic orthostatic edema and the effect of thyrotoxicosis on weight fluctuation and fluid retention in the presence of surgically induced panhypopituitarism and diabetes insipidus controlled with hormone replacement. MATERIALS AND METHODS: Dextroamphetamine sulfate was used for weight gain when no other etiologic factor was found. Methimazole was used when weight loss occurred when serum T4 and free T4 indicated thyrotoxicosis. RESULTS: Sympathomimetic amine therapy very effectively controlled the weight gain and methimazole controlled the weight loss.

Safety and Efficacy of Monoamine Oxidase Inhibitors in Patients Who Use Psychoactive Substances: Potential Drug Interactions and Substance Use Disorder Treatment Data.

Monoamine oxidase inhibitors (MAOIs) remain an important option for patients with treatment-resistant depression (TRD) and other psychiatric conditions, despite potentially serious drug-drug interactions and associated dietary tyramine restrictions. However, they are rarely prescribed in patients with comorbid substance use disorders (SUDs) due to concerns about potential drug interactions and limited research in these populations. This narrative review investigates the use of MAOIs in patients who use psychoactive substances, exploring potential interactions while summarizing the relatively s

Sensitivity and specificity of QEEG in children with attention deficit or specific developmental learning disorders.

The sensitivity and specificity of QEEG-based discriminant functions were evaluated in populations of children diagnosed with specific developmental learning disorders and those with attention deficit disorders. Both populations of children could be distinguished from each other, and from the normal population, with high levels of accuracy. Pretreatment QEEG could be utilized to distinguish ADD/ADHD children who responded to dextroamphetamine from those who responded to methylphenidate, again with high levels of accuracy. This paper provides a replication of all presented discriminant function

Narcolepsy: treatment issues.

Pharmacologic management of patients with narcolepsy is usually based on treating the separate symptoms of cataplexy and excessive daytime sleepiness (EDS). For treating cataplexy, the most widely used medications include the antidepressants venlafaxine, imipramine, and protriptyline, usually at lower doses than prescribed with depression, and sodium oxybate. Monoamine oxidase inhibitors are also sometimes used, but much less frequently. The U.S. Food and Drug Administration has approved 4 medications for EDS: dextroamphetamine, methylphenidate, modafinil, and sodium oxybate. Sodium oxybate is

Tourette's syndrome: are stimulants safe?

Clinicians commonly encounter children with comorbid attention deficit hyperactivity disorder (ADHD) and the tic disorder Tourette's syndrome. The pharmacologic treatment of children with this particular combination of conditions has been problematic because of long-standing warnings against using psychostimulants due to concerns about worsening tics. The basis for these warnings was anecdotal clinical observations. A recent group of placebo-controlled, double-blinded clinical trials have consistently shown that certain stimulants, particularly methylphenidate and dextroamphetamine, are effect
