A third of all Type 1 diabetic patients will develop proteinuria as a clinical sign of diabetic nephropathy. A 100-fold increase in relative mortality is observed in patients with persistent proteinuria. Proteinuria is preceded by a microproteinuric phase, which is reversible upon optimized metabolic control. Thereby, progression towards clinically manifest nephropathy can be delayed. Macroproteinuria and progressive renal insufficiency are not influenced by metabolic control, but by aggressive antihypertensive therapy.

In a comparative histopathologic autopsy study, 100 patients with diabetes mellitus were compared to 100 without this condition with respect to the presence of chronic pancreatopathy. No qualitative differences in specific alterations could be identified. In diabetic patients, however, intralobular and interacinar fibrosis, lipomatosis and atrophy of the parenchyma occurred more frequently and tended to be more severe. These alterations were also associated with the duration and severity of the diabetic condition and with the age of the patient and the degree of arteriosclerosis. On the other …

Hyperlipoproteinemia (HLP) incidence and its character were studied in 1343 patients with diabetes mellitus, depending on age, diabetes type and carbohydrate metabolism. It was found that HLP is more often observed in patients over 35 years as compared to the one in children and juveniles (in 51 and 30% of cases, respectively). The IV type of HLP is prevalent, regardless of age, diabetes type and carbohydrate metabolism. An increased HLP incidence is seen in patients of any age with the I diabetes type during carbohydrate metabolism decompensation. Such a dependence is not observed in patients…

Lipid metabolism indices were studied in 100 patients with diabetes mellitus of different severity and compensation by means of disk electrophoresis in polyacrylamide gel. Hyperlipoproteinemia, more often of the IV type, was revealed in 51% of patients. In decompensated diabetes, mainly types IV and V were found out, which may be regarded as secondary (depending on carbohydrate metabolism disorder) hyperlipoproteinemia. In compensated diabetes, type IV hyperlipoproteinemia was seen, which appears to be primary, not depending on carbohydrate metabolism damage.

The present status of knowledge about glucagon pathophysiology in diabetes is reviewed. 1) A-cells behave abnormally in all varieties of diabetes mellitus, spontaneous and experimental, except perhaps in case of pancreatectomized humans. These abnormalities are : hyperreactivity of A-cells to arginine, non suppressibility by glucose, and absence of stimulation following hypoglycemia. 2) These abnormalities appear as secondary in most instances : a) A-cells behave in a normal way in most studies with prediabetics ; b) plasma glucagon concentration is normalized by excellent control of diabetes …

It would appear from the literature that hypothyroidism is rare in male diabetics. Serologically, there is an established association between autoimmune thyroiditis and diabetes mellitus. However, very little clinical evidence has been produced to confirm these findings. A series of four male insulin-dependent diabetics with hypothyroidism, including three who had latent pernicious anaemia, were studied. Their case histories and a review of the subject are presented
