Modern pharmacological therapies in nephrology: Progress, promises, and the forgotten role of diet.

The rapid expansion of pharmacological therapies for obesity, type 2 diabetes, and chronic kidney disease (CKD) has transformed clinical practice and broadened the therapeutic landscape for the management of metabolic diseases. These agents provide well-established cardiometabolic and renal benefits; however, their use should be understood within the context of an integrated approach, alongside interventions targeting lifestyle-related determinants of disease. Lifestyle-related factors, including dietary patterns and physical activity, remain central to the pathophysiology of metabolic disease

Management of ketogenic diet therapy in patients with ileostomy: A report of 2 cases.

This case series explores the feasibility and clinical considerations of implementing the ketogenic diet therapy (KDT) for epilepsy management in patients with ileostomies. Two pediatric patients with medication-resistant epilepsy and ileostomies continued KDT under clinical supervision. The first patient had an ileostomy placed while on KDT. Fluids and electrolytes were increased initially, although sodium (Na) levels remained within a normal range. The second patient had an ileostomy before starting KDT. Hyponatremia and acidosis occurred initially. With routine laboratory monitoring, and el

Inflammatory bowel diseases and the low-FODMAP diet: benefits and challenges in therapy.

Inflammatory bowel disease (IBD) is a chronic condition characterized by periods of exacerbation and remission, during which patients experience a range of gastrointestinal symptoms that negatively impact their quality of life. With the rising prevalence of the Crohn's disease (CD) and ulcerative colitis (UC), new therapeutic approaches, including nutritional strategies, are being sought to support therapeutic treatment. One of dietary strategy under investigation is a diet low in fermentable oligosaccharides, disaccharides, monosaccharides and polyols diet (LFD), which limits the intake of in

Diet quality and nutritional status in people living with HIV on dolutegravir therapy.

While antiretroviral therapy (ART) has improved life expectancy for people living with HIV (PLWHIV), it has also brought new health challenges, including poor diet quality and excess weight. This cross-sectional study evaluated the diet quality and nutritional status of PLWHIV on dolutegravir (DTG)-based therapy in a Brazilian specialized care service. Adults (≥18 years) who initiated DTG between February 2017 and March 2020 were included. Data on sociodemographic, clinical, anthropometric, and dietary aspects were collected through interviews and medical records. Among the 148 participants (9

Treatment of celiac disease: from gluten-free diet to novel therapies.

Gluten-free diet (GFD) is the cornerstone treatment for celiac disease (CD). This diet excludes the protein gluten a protein forum in in grains such as wheat, barley, rye and triticale. Gluten causes small intestines inflammation in patients with CD and eating a GFD helps these patients in controlling signs and symptoms and prevent complications. Following a GFD may be frustrating, however, it is important to know that plenty of foods are naturally gluten-free and nowadays is relatively easy to find substitutes for gluten-containing foods. Certain grains, such as oats, are generally safe but c

Metabolic Therapy in Glioblastoma-Mapping the Evidence for Ketogenic Diet as an Adjunctive Strategy.

Background/Objectives: Glioblastoma multiforme (GB) is an extremely aggressive tumor of the central nervous system, characterized by rapid growth, high invasiveness, and significant resistance to treatment. A growing body of data indicates that metabolic reprogramming of GB cells may be a therapeutic target, and that the ketogenic diet (KD)-by limiting glucose and inducing ketosis-may modulate tumor metabolism. The aim of this review was to provide a synthetic presentation of the current state of knowledge regarding the use of KD as an adjunctive therapy in the treatment of GB, with a particul

Development of a Practical Guide to Implement and Monitor Diet Therapy for Eosinophilic Esophagitis.

Dietary therapy for short- and long-term management of eosinophilic esophagitis is an effective yet poorly understood and underutilized treatment strategy. Despite several prospective trials demonstrating the efficacy of dietary therapies, successful clinical implementation is hampered by the need for a multidisciplinary approach including dietitian support and provider expertise. The availability of these resources is not readily available to most gastroenterologists. Without standardized guidance on starting or completing the diet for gastrointestinal providers and/or consulting dietitians,

A low-carbohydrate diet in place of SGLT2i therapy in a patient with diabetic cardiomyopathy.

SUMMARY: In patients with diabetes mellitus, the toxic milieu caused by abnormal glucose and free fatty acid handling can lead to heart failure (HF). Referred to as diabetic cardiomyopathy (DMCM), this syndrome often exists in the absence of conventional risk factors for HF such as history of myocardial infarction or hypertension. Low-carbohydrate diets (LCDs) have recently been endorsed as an efficacious therapeutic dietary approach to prevent and reverse cardiometabolic disease including type 2 diabetes mellitus (T2DM). LCDs improve systemic insulin resistance (IR), reverses cardiac remodell

Polyphenols as a Diet Therapy Concept for Endometriosis-Current Opinion and Future Perspectives.

Endometriosis represents an often painful, estrogen-dependent gynecological disorder, defined by the existence of endometrial glands and stroma exterior to the uterine cavity. The disease provides a wide range of symptoms and affects women's quality of life and reproductive functions. Despite research efforts and extensive investigations, this disease's pathogenesis and molecular basis remain unclear. Conventional endometriosis treatment implies surgical resection, hormonal therapies, and treatment with nonsteroidal anti-inflammatory drugs, but their efficacy is currently limited due to many s

[Evaluating the efficacy of diet therapy with protein component modification at Wilson disease].

Wilson disease (WD) is a rare hereditary disorder of copper metabolism, based on of the ATP7B gene mutation, resulting in defect of cooper excretion, which leads to accumulation of cooper in tissues and internal organs (especially in the liver and brain). The basic principle of diet therapy for patients with WD is a diet with reduced copper content, adherence to which is accompanied by significant dietary restrictions, so patients with WD, compared to other liver diseases, represent the most difficult contingent for adjustment of diet. The aim: to assess of the effect of diet therapy with modi

Orofacial granulomatosis and diet therapy: a review of the literature and two clinical cases.

Orofacial granulomatosis is a nonspecific term that contains a wide variety of granulomatous entities, which share a clinical and histopathological presentation. It manifests as persistent or recurrent orofacial swelling, amongst other findings. Idiopathic orofacial granulomatosis, characterized by an absence of systemic granulomatous disease, is a diagnosis of exclusion. The main differential diagnosis is Crohn's disease. Its pathogenesis is unknown, however, it seems to be immune-mediated. Patch-test sensitivity to multiple allergens is well documented. Currently, therapeutic options conside
