[Conventional and diffusion-weighted magnetic resonance imaging and proton spectroscopy in MELAS].

INTRODUCTION: MELAS (mitochondrial encephalomyopathy, lactic acidosis, and stroke-like episodes) is a mitochondrial hereditary dysfunction in which the physiopathological mechanism of cerebral lesions is not totally understood as yet. Typically, these lesions are described as having normal to increased apparent diffusion coefficient (ADC), and this has been used to distinguish stroke-like lesions from ischemic lesions. Notwithstanding this, within the last few years, there have been reports of diffusion restriction in stroke-like episodes. OBJECTIVES: Analysis of the diffusion characteristics

Diffusion-weighted magnetic resonance imaging of borderzone necrosis in paediatric tuberculous meningitis.

PURPOSE: Tuberculous meningitis (TBM) is associated with borderzone necrosis (BZN) of the brain parenchyma in areas adjacent to meningeal inflammation. Diffusion-weighted MRI (DWI) allows for accurate detection of cytotoxic oedema associated with necrosis. Detection and characterisation of BZN using DWI to explain its pathogenesis in TBM have not been performed previously in children. Our objective was to identify the prevalence and characteristics of BZN using DWI in children with TBM and to correlate it with the presence, degree and distribution of basal meningeal enhancement (BE) in the abs

Female urethral diverticular abscess clearly depicted by diffusion-weighted magnetic resonance imaging.

Diffusion-weighted (DW) magnetic resonance imaging (MRI) is a functional imaging to assess molecular diffusion. We report a first case of female urethral diverticular abscess examined by DW MRI. A 49-year-old woman presented with perineal pain, pyuria and inflammatory reaction. Conventional MRI revealed urethral diverticulum with the content of slightly lower-signal-intensity than urine on T2-weighted contrast. On DW MRI, the diverticulum was shown as a marked hyperintense lesion with restrained signal background of the surrounding tissue. The low apparent diffusion coefficient value of the di

"Prominent ear sign" on diffusion-weighted magnetic resonance imaging in relapsing polychondritis.

Relapsing polychondritis is a rare autoimmune disease characterized by recurrent inflammation of cartilage in multiple sites of the body, including the auricles. Central nervous system involvement appears rare. We encountered a case of relapsing polychondritis with encephalitis that could be diagnosed by the unique appearance of the auricle with signal hyperintensity on diffusion-weighted magnetic resonance imaging.

Propriospinal myoclonus: utility of magnetic resonance diffusion tensor imaging and fiber tracking.

Propriospinal myoclonus (PSM) is a rare movement disorder characterized by involuntary spinal-generated muscular jerks that spread rostrally and caudally to other spinally innervated muscles. Most patients have no clear etiology, and conventional MRI of the spinal cord is generally normal. Here we report the use of magnetic resonance diffusion tensor imaging (DTI) and fiber tracking to detect tract-specific abnormalities in a patient with propriospinal myoclonus. As the patient had the fragile-X premutation and antithyroid antibodies, spinal cord DTI abnormalities may be related to these condi

Serial changes on diffusion-weighted magnetic resonance imaging in encephalitis or encephalopathy.

It is recognized that diffusion-weighted magnetic resonance imaging is a sensitive method of detecting cerebral lesions in various neurologic disorders. This report presents two patients with acute encephalitis or encephalopathy who manifested similar serial changes on diffusion-weighted magnetic resonance imaging. Clinically, Patient 1, a 2-year-old male, was diagnosed as having hemiconvulsion-hemiplegia-epilepsy syndrome and Patient 2, a 9-month-old male, acute encephalitis associated with exanthema subitum. Despite the different etiology and the distribution of lesions, diffusion-weighted m

Diffusion-weighted magnetic resonance imaging in Marchiafava-Bignami disease: follow-up studies.

Marchiafava-Bignami disease (MBD), an acute toxic demyelination of the corpus callosum in alcoholics, is associated with poor evolution in the majority of patients. We report here the early and late diffusion magnetic resonance imaging (MRI) and apparent diffusion coefficient (ADC) studies of two patients suffering from MBD with favourable outcome. Diffusion and anatomical MRI changes were parallel to the clinical evolution, suggesting that MRI studies can be helpful for diagnosis and follow-up. Unlike in stroke, restricted diffusion on ADC maps does not seem to be a sign of irreversibility.

Neuro-cysticercosis with Japanese encephalitis: magnetic resonance imaging with diffusion and spectroscopy.

Neuro-cysticercosis (NCC) and Japanese encephalitis (JE) are common in tropical countries. Two cases of NCC with coexistent JE are presented, which share same socio-demographic and ecologic factors and have the same intermediate host (pig). Patients were on treatment for NCC and presented in comatose state. Sudden clinical deterioration of a patient with NCC should warrant a search for coexistent JE. We report findings of magnetic resonance spectroscopy and diffusion-weighted imaging of the JE.

Diffusion magnetic resonance imaging in intermediate form of maple syrup urine disease.

An 8-year-old boy with the intermediate variant of maple syrup urine disease is reported. On b = 1000 s/mm2 (heavily diffusion weighted) images of diffusion magnetic resonance imaging, there was symmetric high signal in the globus pallidus, mesencephalon, dorsal pons, and nucleus dentatus, consistent with restriction of the mobility of water molecules. Apparent diffusion coefficient (ADC) maps revealed low ADC values ranging from 0.42 to 0.56 x 10(-3) mm2/s in these regions, compared to those of apparently unaffected regions in the brain parenchyma ranging from 0.63 to 0.97 x 10(-3) mm2/s. Synthesizing vocal tract magnetic resonance imaging sequences with phoneme-aware diffusion models.

PURPOSE: Real-time speech MRI offers critical insights into articulatory dynamics for diagnostics, therapy, and speech science, but direct speech-to-MRI mapping remains highly challenging. We present a diffusion-based framework that transforms spoken language into dynamic vocal tract MRI sequences with phonemic-aware attention. This improves phoneme-level fidelity and introduces the speech-to-image phonemic fidelity score (SIPFS), a new metric to assess articulatory precision. APPROACH: We trained audio-conditioned diffusion models with phonemic-aware attention and contrastive learning on the

Diffusion-weighted magnetic resonance imaging for diagnosis of post-operative paediatric cholesteatoma.

OBJECTIVES: High rates of recidivism are reported after paediatric cholesteatoma surgery. Our practice has adapted to include non-echoplanar diffusion-weighted magnetic resonance imaging for the diagnosis of residual or recurrent cholesteatoma. This audit aimed to evaluate the performance of non-echoplanar diffusion-weighted magnetic resonance imaging in our paediatric population. METHODS: A retrospective review was conducted of non-echoplanar diffusion-weighted magnetic resonance imaging scans performed to detect residual disease or recurrence after surgery for cholesteatoma in children from

Diffusion tensor magnetic resonance imaging of the postoperative spine with metallic implants.

There has been a growing need to understand the mechanism of development of acute spinal cord injury (SCI) and to optimize treatment. The paramagnetic nature of metallic implants has hampered the application of diffusion tensor imaging (DTI) in postsurgical SCI monitoring. We describe here a successful implementation of spinal DTI in postsurgical SCI patients. Data were acquired using a single-shot turbo-spin-echo sequence, where an extra gradient is applied before the refocusing pulse train to eliminate contributions from the non-Carr-Purcell-Meiboom-Gill components following a diffusion prep

Characterizing cardiac involvement in amyloidosis using cardiovascular magnetic resonance diffusion tensor imaging.

BACKGROUND: In-vivo cardiovascular magnetic resonance (CMR) diffusion tensor imaging (DTI) allows imaging of alterations of cardiac fiber architecture in diseased hearts. Cardiac amyloidosis (CA) causes myocardial infiltration of misfolded proteins with unknown consequences for myocardial microstructure. This study applied CMR DTI in CA to assess microstructural alterations and their consequences for myocardial function compared to healthy controls. METHODS: Ten patients with CA (8 AL, 2 ATTR) and ten healthy controls were studied using a diffusion-weighed second-order motion-compensated spin

Diffusion-weighted magnetic resonance imaging to differentiate malignant from benign gallbladder disorders.

PURPOSE: To retrospectively evaluate the utility of apparent diffusion coefficient (ADC) and lesion to spinal cord ratio (LSR) in diffusion-weighted magnetic resonance (MR) imaging (DWI) as compared with morphological assessment alone, for differentiating malignant from benign gallbladder disorders. METHODS: This study was approved by the ethics committee, and written informed consent was waived. Ninety-one patients (13 malignancy and 78 benignancy) were reviewed. ADC was calculated using two DW images with different motion-probing gradient strengths (b=0, 1000s/mm(2)). LSR was measured by di

Diffusion tensor magnetic resonance imaging for single subject diagnosis in neurodegenerative diseases.

Although magnetic resonance imaging is a standard investigation in neurodegenerative disease, sensitive and specific markers for the underlying histopathological diagnosis are largely lacking. This report presents evidence to indicate that corticobasal degeneration and progressive supranuclear palsy, in particular, might be identifiable at a single subject level with diffusion tensor imaging. Patients with clinical diagnoses of Alzheimer's disease, semantic dementia and non-fluent primary progressive aphasia (n = 9 each) were contrasted with control subjects (n = 26) with the diffusion tensor
