Deceased donor liver transplantation (LT) is a crucial lifesaving option for patients with end-stage liver diseases. Although donation after brain death (DBD) remains the main source of donated organs, exploration of donation after circulatory death (DCD) addresses donor scarcity but introduces challenges due to warm ischemia. While technical advances have improved outcomes, challenges persist, with a 13% mortality rate within the first year. Delving into liver transplantation complexities reveals the profound impact of molecular signaling on organ fate. NOD-like receptor family pyrin domain c

BACKGROUND: The European Union Directives stipulate mandatory tests for the presence of any infections in donors and donations of substances of human origin (SoHO). In some circumstances, other pathogens, including fungi and parasites, may also pose a threat to the microbial safety of SoHO.
OBJECTIVE: The aim of the two systematic reviews is to identify, collect, and evaluate scientific evidence for the presence of fungal and parasitic infections in donors and donations of SoHO, and their transmission via transfusion and transplantation.
METHODS: An algorithmic search, one each for fungal an

The World Health Organization declares in its Guiding Principles on Human Cell, Tissue and Organ Transplantation that organizations must ensure that the personal anonymity and privacy of donors and recipients are always protected. Hence, most of the European transplantation programs forbid any form of direct contact between the family of the deceased donor and the transplant recipient. The anonymity is guarded to protect both parties against potential abuse, manipulation or financial pressure. However, legislation on anonymity of organ donation has come under discussion in the last few years,

CONTEXT: First proposed 2 decades ago, live kidney paired donation (KPD) was considered a promising new approach to addressing the shortage of organs for transplantation. Ethical, administrative, and logistical barriers initially proved formidable and prevented the implementation of KPD programs in the United States.
OBJECTIVE: To determine the feasibility and effectiveness of KPD for the management of patients with incompatible donors.
DESIGN, SETTING, AND PATIENTS: Prospective series of paired donations matched and transplanted from a pool of blood type or crossmatch incompatible donors an

Blood banking is undergoing a period of significant change as a result of several concurrent issues. Blood-transmitted diseases such as human immunodeficiency virus (HIV) and the alternatives to community-derived (homologous) blood such as autologous (patient's own) and designated (blood donor known to transfusion recipient) blood have had an impact on surgical transfusion practice. Many of these issues comprise the medicolegal elements of informed consent for elective blood transfusion, so that increasingly the need for a dialogue incorporating these issues between the transfusing physician a

The decision making process undertaken by a sibling who chooses not to donate a kidney to a brother or sister who needs a transplant is the subject of this article. Presented from the perspective of the potential donor client, the paper focuses on the moral and ethical concerns indicated when there are two equal but competing rights involved in the decision. It also addresses pertinent attitudinal dimensions and social and health issues that affect all parties--the client, the marital partner, the family of origin, and the patient's immediate family. The article concludes by addressing "what"

A persistent barrier to optimizing the utilization of deceased donor kidneys, nearly 30% of which are discarded, is the absence of objective, molecularly resolved metrics of organ quality. To address this gap, we integrated high-resolution proteoform imaging mass spectrometry (PiMS) of donor kidney biopsies with deep LC-MS-based intact proteoform profiling of recipient peripheral blood mononuclear cells (PBMCs) and standard clinical assays. This approach enables direct delineation of proteoform landscapes and reveals immunological and cell stress signatures. Across donor types, living donor (L

Liver transplantation : official publication of the American Association for the Study of Liver Diseases and the International Liver Transplantation Society2003eng
Infection occurs when microbial agents enter the host, either through airborne transmission or by direct contact of a substance carrying the infectious agent with the host. Human body fluids, solid organs, or other tissues often are ideal vectors to support microbial agents and can transmit infections efficiently from donor to recipient. In the case of blood transfusion and tissue transplantation, the main consequence of such a transmission is infection of the recipient. However, in the case of solid-organ transplantation, and particularly for liver transplantation, donor infections are not on

BACKGROUND: This paper concerns the requirements of the EU Tissue and Cells Directives with regard to the biological screening of donors of reproductive cells which are to be used for partner donation.
METHODS: We review the evidence regarding the risks of transmission of blood-borne viruses [hepatitis B (HBV), hepatitis C (HCV) and human immunodeficiency virus (HIV)] in the assisted reproductive technology (ART) setting. We document the experience in seven Irish ART clinics since the introduction of the legislation.
RESULTS: Even among those known to be HBV-, HCV- or HIV-positive, when curr

The Juvenile Diabetes Research Foundation (JDRF) Network for Pancreatic Organ Donors with Diabetes (JDRF nPOD) was established to obtain human pancreata and other tissues from organ donors with type 1 diabetes (T1D) in support of research focused on disease pathogenesis. Since 2007, nPOD has recovered tissues from over 100 T1D donors and distributed specimens to approximately 130 projects led by investigators worldwide. More recently, nPOD established a programmatic expansion that further links the transplantation world to nPOD, nPOD-Transplantation; this effort is pioneering novel approaches

INTRODUCTION: Past research has demonstrated that the urethral tonus is mainly under sympathetic control. Since 5 years, a beta 3-adrenoceptor (ADRB3) agonist is available in the treatment of overactive bladder syndrome. The presence of ADRB3 within the human urethra has not been demonstrated to date. Presence of ADRB3 in the urethra could influence urethral tonus. The aim of this study is to investigate the presence of ADRB3 in the human female urethra.
MATERIAL AND METHODS: We performed anatomical studies in five female specimens. Three specimens were obtained from the body donation program

Since 1968, when Leiden undertook the first successful European pediatric BM transplantation with a 7-year-old sibling donor, more than 300 young children have donated BM in our unit. We first retrospectively studied a cohort of 210 donors, younger than 13 years at donation, to survey procedures of donor eligibility and study immediate effects of BM donation. We then performed a long-term follow-up (FU) and health-related quality of life (HRQoL) study. Despite documentation of previous medical conditions, no child was declared unfit to donate. We found that iron deficiency anemia or low-iron s

In 1988, a moratorium on the use of federal funds for fetal tissue transplantation research (FTTR) halted the promise of a cure for millions of Americans suffering from Parkinson's disease, diabetes, and other debilitating conditions. Since the moratorium began, private and international experimentation continues with mixed success. In the foreground, however, the debate rages over federal funding for fetal tissue transplantation from induced abortions into humans. In the House of Representatives and the Senate, the debate culminated with the passage of the National Institutes of Health (NIH)

All cardiac allograft tissues are under potential contamination, requiring a validated terminal sterilization process or a minimal bioburden. The bioburden calculation is important to determine the bacterial burden and further decontamination and disinfection strategies for the valve processing. The aim of this study was to determine the bioburden from transport solution (TS) of heart valves obtained from non-heart-beating and heart-beating donors in different culture methods. The bioburden from TS was determined in 20 hearts donated for valve allograft tissue using membrane filter (MF) and di

AIM: This study analyzed determinants of advance-directive (AD) completion for end-of-life preparedness among older Korean adults.
METHODS: The study used data from 9,796 individuals in the 2023 National Survey of Older Koreans. Complex-sample weighted analyses and multivariate logistic regression were conducted to identify factors associated with AD completion among older Koreans. Sociodemographic, health-status, and death-preparation characteristics were analyzed.
RESULTS: Multivariate analysis revealed that the strongest predictors of AD completion were organ- or tissue-donation willingne

The assessment of dose and ultimately the health risk from intakes of radioactive materials begins with estimating the amount actually taken into the body. An accurate estimate provides the basis to best assess the distribution in the body, the resulting dose and ultimately the health risk. This study continues the time-honoured practice of evaluating the accuracy of results obtained using in vivo measurement methods and techniques. Results from the radiochemical analyses of the (241)Am activity content of tissues and organs from four donors to the United States Transuranium and Uranium Regist

Between June, 1984, and May, 1988, 130 patients from three renal units in the United Arab Emirates and Oman went of their own accord to Bombay, where they bought, through brokers, kidneys from living unrelated Indian donors for US$2600-3300. 131 transplants were done, and the 122 patients who survived the perioperative period returned to their original renal units for follow-up. Altogether there were 25 deaths (16 before the end of 3 months, 4 in the next 3 months, and 4 more by the end of the first year), which gave a patient survival rate of 81.5% at 1 year. The patients who died had multipl

It is clear that ethics committees face many dilemmas in developing sound public policy on organ transplantation through their roles in serving their institutions and their communities. With conscientiousnes and good faith, different committees may adopt different approaches to the same issue, and arrive at different solutions when subscribing to the same approach. It is valuable that committee members should be aware of other committees' approaches and disclose the reasons for their own in a frank, non-defensive way. It should also be remembered that a measure of legitimacy can attach to ethi
