The use of targeted muscle reinnervation for improved myoelectric prosthesis control in a bilateral shoulder disarticulation amputee.

A novel method for the control of a myoelectric upper limb prosthesis was achieved in a patient with bilateral amputations at the shoulder disarticulation level. Four independently controlled nerve-muscle units were created by surgically anastomosing residual brachial plexus nerves to dissected and divided aspects of the pectoralis major and minor muscles. The musculocutaneous nerve was anastomosed to the upper pectoralis major; the median nerve was transferred to the middle pectoralis major region; the radial nerve was anastomosed to the lower pectoralis major region; and the ulnar nerve was

Life-saving hip disarticulation following necrotizing myofasciitis from invasive mucormycosis after blast injury: A case report.

INTRODUCTION: Necrotizing myofasciitis is a rapidly progressing, life-threatening infection involving deep soft tissues, often caused by bacterial pathogens. However, fungal etiologies such as invasive mucormycosis, particularly in trauma patients, are rare and associated with high morbidity. Blast injuries create hypoxic environments conducive to fungal proliferation, complicating clinical management and often necessitating aggressive surgical interventions such as amputation. PRESENTATION OF CASE: We present the case of a 26-year-old male with no prior medical history, who sustained multipl

Percutaneous profunda femoris artery revascularization to prevent hip disarticulation: case series and review of the literature.

BACKGROUND: Little is known about the role of percutaneous revascularization of the profunda femoral artery (PFA) in patients with amputation stump ischemia who are at risk of hip disarticulation. METHODS: We identified 4 patients who were treated for persistent amputation stump ischemia by PFA percutaneous revascularization over a 3-year period. All 4 patients had significant cardiopulmonary comorbidities and 2 patients had at least 3 previous groin surgeries. The short- and long-term outcomes of the patients were evaluated with regard to stump salvage, wound healing rate, complications, and

Cortical brain activity in transfemoral or knee-disarticulation prosthesis users performing single- and dual-task walking activities.

INTRODUCTION: Walking with a prosthesis while performing secondary tasks increases demand on cognitive resources, compromising balance and gait. This study investigated effects of a secondary task on patterns of brain activity and temporospatial gait parameters in individuals using a prosthesis with or without a microprocessor-controlled prosthetic knee(MPK) and controls. METHODS: A cross-sectional study with repeated measures was performed. Twenty-nine individuals with amputations and 16 controls were recruited. Functional near-infrared spectroscopy was used to evaluate changes in oxygenated

Perioperative Management of a Patient With Decompensated Upper Limb Arteriovenous Malformation Undergoing Shoulder Disarticulation: A Case Report.

Surgical resection of arteriovenous malformations (AVMs) is indicated in the presence of life-threatening and severe morbidity, including symptomatic heart failure, ischemic pain, and recurrent bleeding, where other less invasive treatment strategies have been unable to halt the progression of disease. We present the challenges encountered in the perioperative care of a 23-year-old man with high output cardiac failure, gangrenous hand, and severe chronic pain undergoing shoulder disarticulation for a high-flow complex AVM of the upper limb.

Right Knee Ureaplasma Septic Arthritis in an Immunocompromised Host, Requiring Right Hip Disarticulation for Definitive Source Control.

INTRODUCTION: Ureaplasma urealyticum is a fastidious, urease-producing bacterium typically responsible for urogenital infections. In immunocompromised hosts, a full-thickness mucosal breach may be complicated by hematogenous spread, leading to septic arthritis in distant joints U. urealyticum is difficult to isolate on routine culture and requires special media or 16S rRNA polymerase chain reaction for detection. CASE REPORT: We report a 26-year-old immunocompromised female with neuromyelitis optica on long-term cyclosporine and Grave's disease, who developed chronic right knee U. urealyticum

Outcomes in Drainage Ankle Disarticulation vs Guillotine Transtibial Amputation in the Staged Approach to Below-Knee Amputation.

A transtibial amputation is the traditional primary staged amputation for source control in the setting of non-salvageable lower extremity infection, trauma, or avascularity prior to progression to proximal amputation. The primary aim of the study is to compare preoperative risk factors and postoperative outcomes between patients who underwent transtibial amputation versus ankle disarticulation in staged amputations. A retrospective review of 152 patients that underwent staged below the knee amputation were compared between those that primarily underwent transtibial amputation (N = 70) versus

Disarticulation-Associated Femoral Head Separation in Clinically Normal Broilers: Histologic Documentation of Underlying and Predisposing Cartilage Abnormalities.

Routine and quantitative histologic studies on femoral head separation (FHS) associated with coxofemoral joint disarticulation at necropsy were conducted on 125 femoral heads collected from 21- to 50-day-old clinically normal broilers. The study compared groups demonstrating grossly detached femoral heads (DFHs) with those having attached femoral heads (AFHs). Marked microscopic lesions compatible with osteochondrosis (OCD) consistently occurred along the separation surface in the DFH population. The histologic changes consisted of cartilage degeneration and necrosis sometimes forming small cl

Comparison of Quality of Life of Patients after Hip Disarticulation and Those after Stump Lengthening with Modular Prosthesis.

BACKGROUND: Disarticulation has been used less and less often in the treatment of musculoskeletal neoplasms; however, in some cases it allows the patient to achieve good oncological and functional outcomes. The aim of this paper is to present the possibilities, surgical technique and treatment outcomes of patients after hip disarticulation and stump lengthening with a modular prosthesis. MATERIAL AND METHODS: Three classic hip disarticulation surgeries and 2 hip disarticulation procedures with stump lengthening with a modular prosthesis were performed at the Department of Oncological Orthopae

Stifle Disarticulation as a Pelvic Limb Amputation Technique in a Cockatiel ( Nymphicus hollandicus ) and a Northern Cardinal ( Cardinalis cardinalis ).

This clinical report describes the use of stifle disarticulation in 2 avian patients as an alternative to transfemoral limb amputation. A northern cardinal ( Cardinalis cardinalis ) was presented for soft tissue swelling and a traumatic fracture of the third digit of the left limb, with secondary bacterial infection and necrosis. A 25-year-old cockatiel ( Nymphicus hollandicus ) was diagnosed with distal extremity necrosis after a nonunion of a closed tibiotarsal fracture. In both cases amputation was recommended due to the poor prognosis for healing and because both birds were poor surgical c

Coronary stenting with the half (disarticulated) Palmaz-Schatz stent: immediate results and six-month follow-up.

Coronary stenting with the half disarticulated Palmaz-Schatz stent is particularly suitable for ostial stenoses, diaphragm stenoses, stenoses distal to tortuous segments or coronary bends and localized dissections after balloon angioplasty. Nevertheless very few data regarding the half stent exist and follow-up data are nonexistent. From January of 1994 to December of 1995 a total of 207 half stents were implanted in 175 patients. Most patients had stable or unstable angina and in the majority of cases the stent was implanted due to localized dissection or to suboptimal result. The procedural

Targeted muscle reinnervation following external hemipelvectomy or hip disarticulation: An anatomic description of technique and clinical case correlates.

BACKGROUND: Targeted muscle reinnervation (TMR) has been shown to decrease or prevent neuropathic pain, including phantom and residual limb pain, after extremity amputation. Currently, a paucity of data and lack of anatomical description exists regarding TMR in the setting of hemipelvectomy and/or hip disarticulations. We elaborate on the technique of TMR, illustrated through cadaveric and clinical correlates. METHODS: Cadaveric dissections of multiple transpelvic exposures were performed. The major mixed motor and sensory nerve branches were identified, dissected, and tagged. Amputated perip

A Muscle Synergy Framework for Cross-Limb Reconstruction of Hand Muscle Activity Distal to a Virtual Wrist-Level Disarticulation.

Currently, myoelectric prostheses lack dexterity and ease of control, in part because of inadequate schemes to extract relevant muscle features that can approximate muscle activation patterns that enable individuated dexterous finger motion. This project seeks to apply a novel algorithm pipeline that extracts muscle activation patterns from one limb, as well as from forearm muscles of the opposite limb, to predict muscle activation data of opposite limb intrinsic hand muscles, with the long-range goal of informing dexterous prosthetic control.

A case of an elderly hip disarticulation amputee with rheumatoid arthritis who regained the ability to walk using a hip prosthesis.

[Purpose] We report a case of an elderly patient with rheumatoid arthritis who underwent hip disarticulation because of necrotizing fasciitis and regained the ability to walk independently with a prosthetic limb. [Participant and Methods] A 61-year-old female patient underwent right hip disarticulation due to severe necrotizing fasciitis of the right lower limb. Her chief complaint was that she was not able to walk inside her house or outdoors to perform instrumental activities of daily living. We applied a Canadian-type hip disarticulation prosthesis to the stump. The patient received in-hosp
