Twenty years ago, ketamine was introduced into clinical use in Germany. Since this time, advantages and disadvantages of "dissociative anesthesia" with ketamine have been reported in many studies. Improvements were seen in stabilization of blood pressure during induction of shock patients, induction of patients with asthma, and preservation of spontaneous breathing as well as protective reflexes. On the other hand, psychomimetic reactions and an increase in cardiac oxygen consumption restricted the clinical use of ketamine. The incidence of these side effects was reduced when ketamine was comb

Pain is widely studied and is considered a major clinical, social, and economic problem worldwide, although it remains poorly understood. For disaster victims, the complex picture, biologically, psychologically, and socially, only makes the situation even more complicated. This narrative review aims to describe specific aspects of pain and pain management in disaster victims. We reviewed relevant literature, both on pain management and selected specific data related to conflict victims. We discuss the complexity of the picture, its different aspects, and mechanisms. We discuss the limitations

Conducted in challenging environments such as disaster or conflict areas, operational medicine presents unique challenges for the delivery of efficient and quality healthcare. It exposes first responders and medical personnel to many unexpected health risks and dangerous situations. To tackle these issues, artificial intelligence (AI) has been progressively incorporated into operational medicine, both on the front lines and also more recently in support roles. The ability of AI to rapidly analyze high-dimensional data and make inferences has opened up a wide variety of opportunities and increa

BACKGROUND: Anthropogenic and natural hazards increase reliance on donated medicines. However, inappropriate donations (e.g., expired, unlabeled, or poorly matched to local needs) remain common and create negative impacts on disaster-affected communities. In Japan, the coordination mechanisms and roles among stakeholders remain insufficiently characterized. This study aimed to identify and characterize stakeholders involved in the donated medicine cycle in Japan and to examine their relationships. In addition, this study also obtained stakeholder opinions on our proposed collaborative initiati

UNLABELLED: Introduction How the burden of disease varies during different phases after floods and after storms is essential in order to guide a medical response, but it has not been well-described. The objective of this review was to elucidate the health problems following flood and storm disasters.

"Recommendations for the management of crush victims in mass disasters" aims to assist medics, paramedics and rescue team members who provide care during disasters. Development of the recommendations followed an explicit process of literature review and, also internet and face-to-face discussions. The chapters cover medical and logistic measures, to be taken both at the disaster field and in the hospitals, to cope with the problems created by a catastrophe. Recommendations were based on retrospective analyses and case reports on past disasters, and also expert judgment or opinion. Since there

This article presents an overview of the capabilities for clinical management of radiation injuries available at the Nikiforov Russian Center of Emergency and Radiation Medicine (NRCERM) of the Ministry of the Russian Federation for Civil Defense, Emergencies and Elimination of Consequences of Natural Disasters (EMERCOM). NRCERM is a federal state budgetary institution and the Russian Federation's head organization for providing medical assistance for persons overexposed to ionizing radiation, responders to radiation emergencies and people evacuated from radiation contaminated areas. The purpose was to assess disorders related to disaster victim identification (DVI) in a group of Danish forensic personnel that had performed disaster victim identification in Thailand after the 2004 tsunami. All individuals from the DVI team were screened using a questionnaire to identify disorders presenting in relation to DVI work in Thailand. All participants who had a positive screening result were examined clinically by an occupational physician and had a standard lung function test. Individuals with skin disorders were examined by a dermatologist and a skin patch was performed. Individ

From the experience of more than 130 emergency medical relief missions in over 50 countries/areas, the AMDA would like to propose a system of mobile clinical examinations to prepare for possible natural disasters in Japan. Such a system will require the development of vehicles equipped with a full range of laboratory equipment, which I believe will become a public property in the world, and contribute to the enhancement of medical services in disaster areas as well as in areas with less developed medical technologies. AMDA's recent medical relief activities include the support of the victims o

The means of designing an effective user interface software package varies from one application to another. Almost the entire ICT infrastructure was damaged following the impact of the tsunami tidal wave. Under such circumstances, transporting critically ill patients is a must and becomes the first priority. Many considerations are needed when designing a specific user interface for emergency situations in developing countries. In this study we proposed how to design the user interface in order to support emergency medical care in the early stages of disasters. The user interface was classifie

One of the greatest challenges in emergency medicine and in particular for emergency staff are disasters with huge amounts of victims. The development of a panic is a rare, but nonetheless an extreme escalation of an emergency situation. This paper describes the psychological and anthropological background of panic reactions and gives a summary of the current evidence in research. Accordingly, panic may be defined as the conclusive endpoint of an internal assessment that the probability to influence ones own survival in a life-threatening situation is close to zero. The possibilities and limit

Radiation oncology departments are uniquely vulnerable to disruptions such as cyberattacks, pandemics, and natural disasters, which can result in unacceptable treatment delays. Using the National Academy of Medicine Crisis Standards of Care framework and lessons learned from the University of Vermont Medical Center cyberattack, an ethical framework is proposed to guide decision making in radiation oncology during a crisis.

US has become an integral part of patient evaluation in emergency medicine. As discussed in this article, other potential uses for US are relatively early in their development. With the increased interest and study, however, there exists a real potential for US to be employed in an expanded clinical role in the evaluation of traumatically injured patients outside the standard FAST examination, for acute and chronic musculoskeletal injuries, and for triage of patients in disasters.

BACKGROUND: Social crises deeply affect mental health; however, the effects of different disaster types on emergency psychiatric admissions have not been adequately compared. The pandemic represents a chronic socially mediated stressor, whereas the earthquake constitutes an acute traumatic shock. This study compared emergency psychiatric consultations across three consecutive periods. METHODS: This was a single-center retrospective observational study. All eligible psychiatric consultations at Sütçü İmam University Faculty of Medicine Hospital during pre-pandemic (January 2019–January 2020), p

OBJECTIVE: In England, the Civil Contingencies Act (Contingency Planning) Regulations 2005 require National Health Service (NHS) Trusts which provide hospital accommodation and services in relation to accidents or emergencies to conduct Emergency Preparedness, Resilience and Response (EPRR) exercises. The NHS England EPRR Framework specifies the minimum frequencies of these exercises. This review assessed the number of Trusts conducting exercises in accordance with these frequencies one year after the national NHS COVID-19 response was stepped down.
METHODS: The outcome of interest was the nu

It is appreciated that there are national and jurisdictional variations in regard to post-mortem dissections following accidental death. The purpose of this paper is to plead that, since there will be nationals of many states involved in an aircraft disaster, there should be a uniform system of forensic investigation on a world wide basis. The justification for this recommendation is illustrated through the investigation of accident causes and through the solution of the many problems relating to the settlement of probate which arise in accidental death. The causes of accidents which may be di

This study aimed to assess the situations involving potential risks of over-exposure to radiation faced by workers during the decommissioning of the Fukushima Daiichi Nuclear Power Plant (FDNPP), and to prepare effective medical countermeasures. While deaths have occurred at regular intervals at the FDNPP site, they cannot be attributed to direct effects of radiation. Radiation exposure among decommissioning workers is strictly maintained within regulatory limits, and no deterministic or stochastic radiation effects have been observed. High-risk areas include the reactor buildings of Units 1-4
