OBJECTIVES: The purpose of this study was to determine the prevalence of valvular aortic stenosis requiring surgery in patients with a pre-existing diagnosis of subaortic stenosis.
BACKGROUND: Classic teachings emphasize aortic regurgitation as the most common complication associated with discrete subaortic stenosis. We hypothesized that significant aortic stenosis may also be an important valve lesion associated with this condition.
METHODS: Clinical outcomes in patients with subaortic stenosis were examined. The primary outcome of interest was the prevalence of valvular aortic stenosis req

BACKGROUND: Certain conditions, including discrete subaortic stenosis and right ventricular outflow tract obstruction are well-known associations with ventricular septal defect. However, the association of discrete type subpulmonary and subaortic stenosis with ventricular septal defect has not been described to date.
PATIENTS: In this report we present our experience in nine patients with doubly committed subarterial ventricular septal defect in whom associated discrete subpulmonary and subaortic stenosis were diagnosed by two-dimensional echocardiography. The mean age of the patients was 5.7

This paper reports on two cases of more uncommon types of subaortic stenosis. A 2-year-old boy was found with accessory mitral valve leaflet (AMVL) attaching to the anterior leaflet, ballooning into the subaortic ventricular septum associated with a discrete subaortic membrane. The obstruction was successfully relieved by removal of the AMVL and resection of the membrane. A 19-day-old newborn with accessory tissue on the mitral valve (AMVT) causing subaortic stenosis, subaortic ventricular septal defect (VSD) and patent ductus arteriosus was operated on successfully. Accessory tissue excision

There is considerable heterogeneity among pathologic mechanisms responsible for subaortic obstruction in infancy. Although the relatively discrete forms of subaortic stenosis are uncommon in this age group, conoventricular malalignment, maladherent mitral valve, accessory endocardial cushion tissue, restrictive bulboventricular foramen, and a poorly expanded suboartic conus are responsible for most types of subaortic stenosis in this age group. The angiocardiographic recognition of these types of subaortic obstruction is aided by awareness of the anatomic framework in which they are found. For

BACKGROUND: Recurrence after surgical resection of discrete subvalvar aortic stenosis in children often requires repeat operation. Risk factors for recurrence are poorly understood. We sought to determine potential risk factors for recurrence and postoperative comorbidities in the long term.
METHODS: Retrospective chart review was performed on all pediatric patients who underwent surgical resection of discrete subaortic stenosis at our institution. Demographics, perioperative findings, and clinical data were analyzed for predisposing factors.
RESULTS: From 1991 to 2015, a total of 104 patien

Bicuspid aortic valve is the most common congenital cardiac anomaly and it may often coexist with other congenital cardiac anomalies. Its coexistence with discrete subaortic membrane, causing obstruction of the left ventricular outflow tract is very rare. A 21-year-old male patient presented with complaints of exertional dyspnea and dizziness. On transthoracic echocardiography, the parasternal short-axis view showed a bicuspid aortic valve, and parasternal long-axis color Doppler view showed a mosaic pattern in the subaortic region. A more careful examination of the parasternal long-axis views

Subaortic stenosis (SAS) is a congenital heart disease, and its association with hypertrophic cardiomyopathy is very rare and clinically underappreciated. We report here a case of a 45-year-old female who was admitted to our hospital with chest tightness and shortness of breath. Both transthoracic and transesophageal echocardiography revealed asymmetric left ventricular hypertrophy and a membrane-like echo below the level of the aortic valve. This patient was diagnosed with membranous SAS with hypertrophic cardiomyopathy. Screening of her immediate family members revealed that her son also had

BACKGROUND: A discrete subaortic membrane cannot only cause left ventricular outflow tract obstruction, but can grow onto the aortic valve leaflets. The late finding of this encroachment is aortic valve insufficiency or stenosis. Echocardiography is used to follow the progression of outflow tract obstruction, but its ability to show subaortic membrane encroachment onto the aortic valve is unclear. The purpose of this study is to determine the sensitivity and specificity of echocardiography for diagnosing whether a discrete subaortic membrane involves the aortic valve.
METHODS: A pre-operative

Subaortic stenosis caused by an accessory mitral valve is an exceedingly rare finding. We report the case of an asymptomatic 14-year-old patient, in whom transthoracic echocardiography revealed an accessory mitral valve in the left ventricular outflow tract, producing mild subaortic stenosis. Except for an aneurysm of the interventricular septum, with no shunt, there were no other anomalies. Transesophageal echocardiography provided details about the morphology and location of the accessory valve. Being asymptomatic and having only a mild gradient, antibiotic prophylaxis for infective endocard

Tetralogy of Fallot is often found in association with a wide variety of other cardiac lesions, but is rarely found in association with lesions causing obstruction to the left ventricular inflow or outflow. Subaortic stenosis has only rarely been reported in association with tetralogy of Fallot. We report a patient with Marfan syndrome who underwent repair of tetralogy of Fallot at five years of age. Discrete and progressive subaortic stenosis developed two years after the surgical correction, in a previously normal and unobstructed left ventricular outflow tract. Surgical removal of the acqui

Williams syndrome, initially described by Williams, Barratt-Boyes, and Lowe in 1961, consists of characteristic dysmorphic features, congenital heart disease, and distinctive behavioral and emotional traits. In addition to acquired and congenital heart disease, manifestations in the renal, endocrine, musculoskeletal, and central nervous system may have implications during the perioperative period. Congenital and acquired heart disease can be a significant issue as sudden death, related to abnormalities of the coronary arteries, has been reported perioperatively in these patients. The authors p

Between June 1972 and August 1989, we operated on 45 patients with fixed subaortic stenosis. Discrete membranous stenosis was present in 28 patients and tunnel stenosis, in 13. Four patients had subvalvar stenosis complicating double-outlet right ventricle. There were 33 male and 12 female patients. Mean age at operation was 7.1 +/- 4.3 years (range, 6 months to 21 years). Local resection of the fibrous membrane was performed in 26 patients. Local resection was combined with myectomy in 18 patients. Aortoventriculoplasty (modified Konno procedure) was required at operation in 3 patients. There

Patient is 1 month old, 3.72 kg boy, he was diagnosed as {S, L, L} double-inlet left ventricle (DILV), transposition of the great arteries, coarctation of aorta, rudimentary right ventricle. Patient was undergone Norwood procedure, with "swing-back technique"; end-to end anastomosis of ascending aorta with descending aorta, double-barreled style Damus-Kaye-Stansel (DKS), end-to side anastomosis of neoaorta to aortic arch. This technique has following advantages over other reported technique; discrepancy of great arteries were resolved without distortion ; reconstruction of the aorta without th

Left ventricular outflow tract obstruction may be dynamic, most commonly associated with hypertrophic cardiomyopathy, and, uncommonly, by congenital anomalies such as discrete subaortic stenosis. We describe a patient with Behçet's disease, presenting with a systolic murmur, fever, and syncope, in whom a diagnosis of subaortic obstruction caused by a pseudo-aneurysm dissecting the interventricular septum and associated with a complete atrioventricular block was made.

The clinical, echocardiographic, and catheterization findings in a patient with discrete subaortic stenosis, aneurysm of the membranous interventricular septum, and mitral valve prolapse are presented. Echocardiography showed a subaortic membrane, abnormal aortic valve motion, accentuated systolic anterior motion of the membranous interventricular septum, and prolapsing mitral leaflets. Cardiac catheterization confirmed the diagnoses. The possible functional interrelationship of these lesions is discussed.

This paper details a rare cause of subaortic obstruction, a discrete membrane combined with an anomalous muscle bundle and an accessory tissue on the mitral valve. A 13-year-old boy was referred to our hospital for surgical relief of subaortic obstruction and mitral stenosis. He had received two additional operations, the first was for coarctation of aorta when 5 years old and the second was for discrete subaortic stenosis when 6 years old. Recatheterization at this time revealed an peak systolic gradient of 44 mmHg across the left ventricular outflow tract, and an peak diastolic gradient of 7
