Progress in brain imaging, monitoring and physiopathology allows the identification of brain oedema from brain swelling, determination of its interstitial or intracellular nature, as well as blood-brain barrier permeability and the evaluation of the impact on cerebral haemodynamic. Common treatment of all types of cerebral oedema is based on prevention of self-sustained disorders due to increased intracranial pressure resulting in ischemic cerebral oedema. The specific treatment of each type of cerebral oedema is reviewed. Optimization of conventional anti-oedematous strategies is based on the

Increased intracranial pressure (ICP) is a pathological state common to a variety of neurological diseases, all of which are characterized by the addition of volume to the skull contents. Elevated ICP may lead to brain damage or death by two principle mechanisms: 1) global hypoxic-ischemic injury, as a consequence of reduced cerebral perfusion pressure (CPP) and cerebral blood flow; and 2) mechanical distortion and compression of brain tissue as a result of intracranial mass effect and ICP compartmentalization. All ICP therapies have as a goal, reduction of intracranial volume. In unmonitored

Available evidence suggests that both hydrostatic and osmotic forces are important in the development of acute respiratory distress syndrome (ARDS) or, more broadly, acute lung injury (ALI). More than 80% of ARDS patients in a large-scale randomized controlled trial (RCT) exhibited, at least intermittently, pulmonary artery wedge pressures (PAWP) above 18 mmHg. Retrospective analyses have shown that PAWP elevation is associated with increased mortality. Reduction in serum total protein (STP) has been shown, in a recent retrospective analysis of data from a sepsis patient population with a high

Head trauma causes approximately 75,000 deaths each year in the United States. Both hypoxia and hypovolemia are individual predictors of poor outcome in the patient with severe head trauma. Management begins in the field and is focused on ensuring oxygenation and maintaining a blood pressure that supports cerebral perfusion. In the trauma center, intracranial pressure monitoring may be helpful in guiding the management of increased intracranial pressure. Mannitol, given in intermittent bolus infusions, is the therapy of choice for increased intracranial pressure. Hyperventilation has been show

BACKGROUND: Laboratory measurements of osmolality and electrolyte concentrations are useful as a source of clinical and pathophysiologic information on kidney function. We obtained different conditions of baseline diuresis in 54 rats, which were then treated with furosemide 10 mg/kg. From measurements of plasma (P) and urine (U) osmolality and Na, we calculated the free water clearance (CH2O) and the contributions of Na+ and non-Na(+)-solutes to its changes during diuretic administration.
RESULTS: The results show that furosemide abolishes both urine diluting and concentrating ability, by red

BACKGROUND: Mannitol and hypertonic saline (HS) are used by clinicians to reduce brain water and intracranial pressure and have been evaluated in a variety of experimental and clinical protocols. Administering equivolume, equiosmolar solutions in healthy animals could help produce fundamental data on water translocation in uninjured tissue. Furthermore, the role of furosemide as an adjunct to osmotherapy remains unclear.
METHODS: Two hundred twenty isoflurane-anesthetized rats were assigned randomly to receive equivolume normal saline, 4.2% HS (1,368 mOsm/L 25% mannitol (1,375 mOsm/L), normal

Acute intracranial hypertension requires aggressive treatment with physiologic and pharmacologic measures guided by intracranial pressure monitoring devices. Therapy involves the use of diuretics, corticosteroids, and barbiturates in combination with hyperventilation, ventricular drainage, and general supportive measures. This review focuses on the pathophysiology of increased intracranial pressure and the pharmacologic agents used in its management.

In a 75-year-old man treated for cerebral oedema with mannitol, anuria developed after two days' treatment. Within a few hours after haemodialysis renal function returned. High doses of mannitol can cause acute renal failure, especially in patients with pre-existing renal impairment. Lowering of the concentration of mannitol in plasma by haemodialysis will rapidly restore renal function.

The cells of the renal medulla adapt osmotically to high extracellular tonicities by high concentrations of organic osmolytes. Intracellular accumulation of these substances is, however, relatively slow. The aim of the present study was to assess the effect of an abrupt rise in extracellular tonicity on intracellular osmotically active substances after prior reduction of medullary contents of organic osmolytes by chronic diuresis. Intra- and extracellular electrolyte concentrations at the papillary tip and the tissue contents of methylamines (glycerophosphorylcholine, betaine), polyols (myo-in

Changes in plasma antidiuretic hormone (p-ADH) and plasma osmolality (p-Osm) levels were studied in 63 patients with Ménière's disease before and after isosorbide administration and a glycerol test. Increments in both p-ADH and p-Osm levels were observed after isosorbide administration and the glycerol test. The p-ADH secretion appeared to be secondarily stimulated by an increase in the p-Osm level. This p-ADH level increase affects cochlear fluid homeostasis. Based on these results, we must consider both the p-Osm and p-ADH levels when treating patients with Ménière's disease by osmotic diure

On the basis of their experience in the intensive therapy unit, correlated with modern pharmacokinetic data, the authors discuss the adverse effects induced by polyvalent therapy in septic shock, and which may be listed in the group of iatrogenic pathointerference. To the biochemical and functional alterations of shock may be added the possible effects of antibioticotherapy, corticotherapy, plasma expanders, lipid emulsions, crystalloid solutions, alkaline correcting solutions, osmotic diuretics, oxygenotherapy, vasoactive drugs, heparin and epsilon-amino-caproic acid. BACKGROUND: Every day children and adults throughout the world die from acute community-acquired bacterial meningitis, particularly in low-income countries. Survivors are at risk of deafness, epilepsy and neurological disabilities. Osmotic therapies have been proposed as an adjunct to improve mortality and morbidity from bacterial meningitis. The theory is that they will attract extra-vascular fluid by osmosis and thus reduce cerebral oedema by moving excess water from the brain into the blood. The intention is to thus reduce death and improve neurological outcomes.
OBJECTIVES: To evaluate th

Oral urea has been used in the past to treat various diseases like gastric ulcers, liver metastases, sickle cell disease, heart failure, brain oedema, glaucoma, Meniere disease, etc. We have demonstrated for years, the efficacy of urea to treat euvolemic (SIADH) or hypervolemic hyponatremia. We briefly describe the indications of urea use in symptomatic and paucisymptomatic hyponatremic patients. Urea is a non-toxic, cheap product, and protects against osmotic demyelinating syndrome (ODS) in experimental studies. Prospective studies showing the benefit to treat mild chronic hyponatremia due to

BACKGROUND: Interstitial edema of rejecting organs can be correlated with the accumulation of hyaluronan in the transplant; since hyaluronan has strong water-binding capacity, treatment with the hyaluronan-degrading enzyme hyaluronidase reduces not only the hyaluronan content but also the water content of the graft. The aim of the present study was to investigate whether a further reduction of the water content would be the result if hyaluronidase was used in conjunction with classic diuretic substances.
METHODS: Five days after heterotopic heart transplantation (PVG to Wistar/Kyoto), recipie

Although there have been major advances in the treatment of heart failure, loop diuretics remain essential for the treatment of fluid retention. Patients with refractory heart failure become resistant to progressively larger doses of diuretics. Some of the strategies which may be helpful in restoring diuretic efficacy are reviewed.

SGLT2 inhibitors are antihyperglycemic drugs that protect kidneys and the heart of patients with or without type 2 diabetes and preserved or reduced kidney function from failing. The involved protective mechanisms include blood glucose-dependent and -independent mechanisms: SGLT2 inhibitors prevent both hyper- and hypoglycemia, with expectedly little net effect on HbA1C. Metabolic adaptations to induced urinary glucose loss include reduced fat mass and more ketone bodies as additional fuel. SGLT2 inhibitors lower glomerular capillary hypertension and hyperfiltration, thereby reducing the physi

Polyuria, or excessive fluid loss through the kidneys, is a common issue in neurocritical patients, often resulting from conditions such as fluid volume overload, osmotic diuretics, central diabetes insipidus (CDI), or cerebral salt wasting syndrome (CSWS). Notably, the specific cause of postoperative polyuria within 24 h in patients with tumors located in the supratentorial non-midline region remains poorly understood. To address this gap, we conducted a prospective observational study with 30 patients and found that eight (26.7%) experienced postoperative polyuria. Binary logistic regression
