Improving Equity of Care for Culturally and Linguistically Diverse Patients: Utilising Multilingual Software to Support Breath-Hold Breast Radiation Therapy.

Breath-hold techniques, including Deep Inspiration Breath Hold (DIBH) and Exhalation Breath Hold (EBH), are commonly used in radiation therapy to reduce radiation exposure to organs at risk while maintaining accurate tumour targeting. However, high failure rates during breath-hold simulations have been associated with patient anxiety and insufficient preparation-particularly when instruction is given only 20 min prior to the procedure. These challenges are amplified for patients from culturally and linguistically diverse (CALD) backgrounds. To address this, a multilingual mobile application (a

Drive for equity: the impact of youth-generated evidence on transportation policy.

Youth Participatory Action Research (YPAR) offers a powerful approach for young people to analyse, critique and mobilise for equity within adult-controlled transportation systems. This article, authored by an intergenerational team (students, teachers and a university researcher) utilising a collective autoethnographic approach, examines two school-based YPAR projects in Denver, Colorado, that sought to influence local transportation policy. One addresses a deadly street, and the other insufficient bus services. Findings suggest that youth policy demands achieved greater success when they were

Realist synthesis of co-production of services in a local authority context: what works, for whom, how and in what circumstances?

BACKGROUND: Co-production is increasingly promoted as a means of improving the design and delivery of local authority services, yet evidence on how and why it works remains limited. This realist synthesis explored how co-production operates within complex local authority systems, identifying the circumstances and mechanisms that enable or constrain meaningful involvement of stakeholders in co-production processes and sustained change to ensure that services are co-produced with professionals and those with lived experience. METHODS: Scoping searches identified key literature on co-production

Design of a card-based operational framework for the implementation of inclusive representation in clinical trials.

BACKGROUND: Despite growing recognition that trial populations should reflect the broad range of people who will receive therapies, implementation of representative enrollment remains inconsistent. Existing frameworks often lack role-specific guidance, phase-specific actions, and actionable tools to support sponsors-referring specifically to pharmaceutical companies-in embedding inclusive representation (IR) across the trial life cycle. To address these gaps, we developed a card-based operational framework (CBOF) that assigns clear responsibilities, aligns with regulatory expectations, and sup

Underreported Demographic Factors in Apraxia of Speech Treatment Studies: Recommendations for Inclusive Research.

PURPOSE: While acquired apraxia of speech (AOS) has been the focus of intervention research for decades, little information is reported about the demographic representation of study participants. This article examines the extent to which published AOS treatment studies report participants' age, sex/gender, race/ethnicity, language background, and socioeconomic status; identifies critical gaps in the literature; and considers implications for representativeness. This information supports the important call to capture these patient factors systematically and thereby advance equity in research an

The impact of organizational inclusion for a diverse workforce on organizational climate assessments and hospital recommendations among nurse practitioners.

BACKGROUND: While the nursing profession has advocated for workforce diversity, limited research has examined how institutional inclusion efforts are associated with the experiences of nurse practitioners (NPs), particularly in hospital settings. PURPOSE: To examine whether institutional inclusion efforts were associated with NPs' strong recommendation of their hospital and whether this relationship was explained by NPs' assessments of organizational climate. METHODS: This cross-sectional study analyzed 2023 data from 742 NPs across 412 U.S. hospitals, drawing on the Penn Nurses4All Survey,

Equity and Resilience in Higher Education: An Examination of Environmental Injustices Affecting College Students from Marginalized Populations Amid Climate Crises.

As climate crises become more severe, higher education institutions face unprecedented challenges in protecting and supporting their students. This conceptual paper explores the intersection of equity and resilience in higher education, emphasizing how structural inequities increase climate-related vulnerabilities among marginalized student groups, especially low-income, first-generation, and BIPOC students. Using the Social Ecological Model and the Whole Community Approach, this paper suggests that climate resilience in higher education must go beyond traditional emergency management to inclu

Structural Barriers and Admissions Practices in Physical Therapist Education: A Case Study of Ohio Programs.

Underrepresentation of marginalized groups persists in the physical therapy workforce, reflecting structural barriers within health professions educational programs' admissions processes. This qualitative study applied a structural competency (SC) framework to examine admissions practices across Doctor of Physical Therapy (DPT) programs in Ohio and identify systemic barriers to diversity. Semi-structured interviews were conducted with admissions representatives from 6 of Ohio's 12 DPT programs (50% response rate) and supplemented by analysis of publicly available admissions materials from all

Equitable Language Access in Neonatal Research.

Participation in neonatal randomized controlled trials (RCTs) is associated with improved or equivalent outcomes for infants and is widely viewed as a cornerstone of high-quality neonatal care. Equitable access to research participation is therefore essential to scientific validity, ethical integrity, and generalizability. Although substantial efforts have focused on improving the representation of racial and ethnic minority groups in neonatal research, the inclusion of families who speak languages other than English remains inadequately examined. In this perspective, we synthesize existing ev

Systems, Structures, and Support: Using a Socioecological Framework to Improve Health Equity for Children in Immigrant Families.

Children in immigrant families (CIF) represent one in four children in the United States and face disproportionate health risks shaped by immigration policy, structural racism, trauma, and barriers to care. Using a socioecological framework, this article reviews the current state of pediatric immigrant health and outlines opportunities for pediatricians to advance equity at the individual, organizational, and structural levels. Clinicians can follow expert clinical care guidelines, practice trauma-informed care and safe documentation practices, and partner with qualified medical interpreters.

Strategies to Reduce Gender Discrimination in Surgery: A Qualitative Study of Women Neurosurgeons and Cardiothoracic Surgeons.

OBJECTIVE: Despite growing numbers of women entering medicine, female neurosurgeons and cardiothoracic surgeons continue to report persistent gender discrimination in their workplaces. While previous efforts to address these disparities have focused on individual-level interventions, less attention has been directed toward the cultural and structural dynamics that sustain inequity within these high-intensity surgical environments. This study explores female surgeons' perspectives on how neurosurgery and cardiothoracic surgery might become more equitable and inclusive. DESIGN AND SETTING: This

Practical approaches to inclusive recruitment practices in Parkinson's disease research.

In Parkinson's disease (PD), inclusive research recruitment practices are essential to ensure that study findings are generalizable to diverse populations. The definition and implementation of inclusive recruitment practices are guided by the principles of equity, justice, engagement, and sustainability. However, practical implementation guidance is lacking. This paper shares insights from the Black and African American Connections to Parkinson's Disease (BLAAC PD) study, a PD genetics research study being conducted in the United States that enrolls individuals with and without PD. The inclusi

Gender-inclusive, culturally sensitive validation of the parental self-efficacy to respond to school refusal.

This letter responds to the recent article by Cardamone-Breen et al. (2025), which introduced the Parental Self-Efficacy to Respond to School Refusal Scale (PSES-SR). Their study provided the first rigorously validated task-related measure of parental self-efficacy in addressing adolescent school refusal and established an important psychometric benchmark through confirmatory factor analysis, concurrent and convergent validity, and strong internal consistency. While commendable, several limitations require attention to strengthen its global relevance. These include the predominance of highly e

Community-engaged participatory methods for embedding equity in trial design.

BACKGROUND: Certain populations are consistently under-represented in randomised controlled trials (RCTs), including people from minority ethnic backgrounds and those experiencing socioeconomic disadvantage, language barriers, or marginalisation within health systems. This under-representation contributes to widening health inequities. While UK guidance such as the National Institute for Health and Care Research INCLUDE resources supports more inclusive research, practical methodologies for embedding equity in trial design remain limited. This paper draws on two linked public involvement proje

Off the Books: A Scoping Review of the Minority Tax and Professional Advancement in Academic Surgery.

INTRODUCTION: The minority tax is an understudied phenomenon by which underrepresented in medicine (URiM) physicians are more often responsible for nonpromotable activities: mentorship, education, outreach, and committee service. This review aims to summarize definitions of minority tax, explore its possible impact in academic surgery, and present gaps in the literature. METHODS: A systematic search across SCOPUS, MEDLINE, EMBASE, ERIC, and PsycINFO was conducted. Original research articles studying URiM individuals in academic medicine/surgery were eligible. A priori outcomes of interest wer

Reimagining research equity and beneficial community impact: reflections from the GSK Africa Open Lab inaugural cohort.

Global health research continues to rely heavily on knowledge generated outside the regions that bear the greatest disease burden, reinforcing structural inequities in scientific leadership and innovation. The GlaxoSmithKline (GSK) Africa Open Lab (AOL) was established to address this imbalance by investing directly in African-led research capacity. This reflective analysis examined how participation in the inaugural GSL-AOL cohort influenced research capacity, scientific independence, and career development among early-career African researchers. The reflection was guided by the question: How

Strategies to improve recruitment to randomised trials.

BACKGROUND: Recruiting participants to randomised controlled trials (RCTs) is challenging. Identifying effective recruitment strategies would benefit health research: poor recruitment leads to underpowered trials, reducing the reliability of findings and increasing the risk of wasted resources, ethical concerns, and trial failure. Evidence to inform recruitment strategies is increasingly generated through Studies Within A Trial (SWATs), which are methodological studies embedded within host RCTs. This is an update of a review last published in 2018. OBJECTIVES: Primary: to quantify the effects
