Hyperbaric Oxygen Treatment for Arterial Gas Embolism With Transient Cortical Blindness Following Intravenous Ozone Therapy: A Case Report.

INTRODUCTION: Cerebral arterial gas embolism (CAGE) is a rare but potentially fatal condition that can occur when air enters the arterial circulation. CASE REPORT: This case report describes a 64-year-old female patient who experienced a sudden onset of left lower extremity weakness and bilateral vision loss following a session of intravenous ozone therapy. Initial diagnostic imaging did not reveal any acute abnormalities. However, her clinical presentation was consistent with CAGE. She was initially administered emergent hyperbaric oxygen (HBO₂) at 2.8 ATA, followed by additional sessions at

Psychophysical stress during a 24 h dive: A case study of an older male diver.

This case study investigated the psychophysiological stress responses of a diver during and after a 24-h hyperbaric exposure. Comprehensive biomarker analysis revealed relevant changes in oxidative stress parameters and cardiac muscle markers. Notably, a critical time point at 6 h (T2) emerged, characterized by peak oxidative stress and diminished antioxidant capacity, followed by progressive cardiac strain as evidenced by elevated creatine kinase isoenzime (CK-MBm) and N-terminal prohormone of brain natriuretic peptide (NT-proBNP) levels. Despite these physiological challenges, no pathologica

Exploring the use of full-face masks for ventilation in dive rescue.

INTRODUCTION: Early oxygenation is essential in a non-breathing scuba accident victim, but the need to exclude water has thus far prevented underwater ventilation, causing significant delay. The full-face mask (FFM) is a potential solution, but its safety and feasibility in this context has not been established. This is a preliminary study into the utility of FFMs for underwater ventilation. METHODS: The tidal volume and peak inspiratory pressure delivered by the OTS Guardian and Neptune III FFMs were measured using a RespiTrainer Advance Airway Management Trainer and an open circuit scuba sy

Decompression sickness or ciguatera intoxication: a case report.

This report presents the case of a diver who suffered from ciguatera intoxication (CTX). After three days of consecutive diving, he arrived at hospital with symptoms of chest pain, shortness of breath, nausea, and had sensory abnormalities across his body, arms and legs. Although his clinical signs and symptoms may have suggested cardiac involvement or decompression sickness (DCS), ultimately his condition was caused by CTX after eating fish. This case illustrates the similarities between DCS and food intoxication. The common pathology of CTX is described and its differential diagnosis with DC

Hyperbaric oxygen therapy attenuates urethral stricture formation after urethral injury: an experimental rabbit study.

PURPOSE: Urethral stricture (US) is a frequent and challenging urological entity characterized by spongiofibrosis and luminal narrowing, often leading to recurrent infections, urinary retention, and upper tract deterioration. Despite various endoscopic and open surgical options, high recurrence rates remain a major problem, and there is no established medical therapy to prevent stricture formation. To investigate the protective effect of hyperbaric oxygen (HBO) therapy on fibrosis and stricture formation in an experimental urethral injury model in rabbits using objective endoscopic, radiologic

Heliox versus trimix in closed-circuit rebreather diving: A comparative study of pulmonary and decompression stress in male divers.

Closed-circuit rebreather (CCR) and helium-based breathing mixtures are widely used in technical diving. This study compared the effects of Heliox and Trimix on pulmonary function and decompression stress during CCR dives. Nineteen divers performed a standardized dive (50 ± 3 msw, 147 ± 10 min) using Heliox (16% O2/84% He; n = 9) or Trimix (16% O2/32% He/52% N2; n = 10). Pre- and post-dive venous gas emboli (VGE), pulmonary function, lung ultrasound, heart rate variability, and inflammatory parameters were assessed. VGE were detected in all Heliox divers but in only 50% of Trimix divers. Helio

Identification of DCS risk factors in recreational diving: multifactorial model based on the DAN DSL Database 2024.

BACKGROUND: Recreational diving creates risk for decompression sickness (DCS), which can occur in SCUBA diving even if current decompression algorithms are respected. The aim of this study was to identify the primary risk factors for decompression sickness in real-world regular recreational diving. MATERIAL AND METHODS: This study analyzed 127,957 dives from 5,907 divers in the DAN DSL database (version 07/2024) to identify independent risk factors for DCS in recreational diving. RESULTS: Decompression sickness was reported in 628 dives, yielding an incidence rate of 0.49%. The most critical

Predicting nitrogen narcosis using the electroencephalogram recorded from experienced divers.

Nitrogen narcosis causes acute cognitive impairment in divers breathing compressed air at depth, increasing injury and fatality risk. Susceptibility varies widely between individuals, making real-time cortical monitoring a potential safety tool. We recorded 32-channel electroencephalography (EEG) from 45 divers (both sexes; aged 20-55 yr) at rest across four randomized crossover hyperbaric studies. We manipulated pressure (101-811 kPa) and breathing gas (air or heliox) to create two classes of recordings: narcotic (high-pressure air) or non-narcotic (low-pressure air; heliox at any pressure);

Middle ear barotrauma during hyperbaric oxygen therapy; a review of occurrences in 5,962 patients.

Hyperbaric-associated middle ear barotrauma (MEB) is one of the most common side effects of the exposure to hyperbaric oxygen (HBO2) therapy. This retrospective observational study of 5,962 patients undergoing longterm therapy for chronic conditions took place at the local Diving and Hyperbaric Medicine Unit (DHMU) in Villafranca-Verona (Italy), a DHMU that administers, in multiplace chambers, more than 20,000 HBO2 treatments per year. The study was designed to weight and analyze both the incidence and severity of MEBs at the facility. Thanks to a systematic recording method over eight years, …

Mixed Compartment Syndromes and Neuropathy Resolution With Hyperbaric Oxygen.

INTRODUCTION: Skeletal muscle compartment syndrome (SMCS) and exertional compartment syndrome (ECS) are two distinct conditions. This case report describes a situation in which both conditions resulted from the same triggering events. The importance of this case lies in the fact that the residual neuropathy associated with the SMCS resolved after a single hyperbaric oxygen (HBO₂) treatment. CASE REPORT: This report was generated from a retrospective analysis of the patient's combined compartment syndromes, SMCS and ECS, residual functional deficit, and resolution of the latter with HBO₂ treat

Evaluation of external ventricular drain performance under hyperbaric conditions: a bench study.

INTRODUCTION: External ventricular drains (EVDs) are widely used for intracranial pressure (ICP) monitoring and cerebrospinal fluid (CSF) diversion in neurocritical care. Hyperbaric oxygen treatment (HBOT) exposes medical devices to increased ambient pressure; however, limited data exists regarding EVD performance and reliability under hyperbaric conditions. METHODS: A bench study was conducted to evaluate EVD performance under hyperbaric conditions. A rigid, non-compressible saline-filled column was used to assess pressure measurement stability independent of compliance effects. Additional c

Lactate increases oxygen unloading of preconditioned blood from male elite breath-hold divers.

Physical performance can be improved in aerobic athletes and breath-hold divers (BHD) by limb exposure to repetitive ischemia: remote ischemic conditioning (RIC). RIC protects against cardiac ischemia, and its blood-borne transferable substrate could be lactate. Accordingly, lactate added to whale blood increases oxygen unloading and adult seals possess higher cardiac lactate dehydrogenase activity (LDHa) than terrestrial mammals. Because BHD and adult diving mammals share adaptations to hypoxia, including lactate metabolization during apnea, we hypothesized that BHD compared to BMI/VO2max-mat

Erythropoietin and Hematological Responses in Recreational Scuba Divers: Insight Into the Hyperoxic-Hypoxic Paradox.

PURPOSE: During scuba diving, breathing under elevated pressure leads to hyperoxia, which may induce oxidative stress and erythrocyte damage. Hyperoxia may also influence erythropoietin (EPO) production, a phenomenon referred to as the hyperoxic-hypoxic paradox. This study investigated changes in EPO levels and red blood cell (RBC) parameters after 1) a single dive, 2) three successive dives over 2 d, 3) the summer diving season. METHODS: 1) Divers ( N = 23) completed a single 30-min dive to 30 m. Blood samples were collected immediately pre- and post-dive, and 3 h post-dive. 2) Divers ( N =

Arterial blood gas changes in progressively deeper breath-hold dives.

Studies involving breath-hold dives as deep as 60 m have identified a spike in arterial Po2 ([Formula: see text]) and Pco2 ([Formula: see text]) apparently associated with lung compression by water pressure as depth increases. Competitive breath-hold dives may exceed 100 m, raising the possibility that a spike in [Formula: see text] might contribute to narcosis-like symptoms sometimes experienced. To predict the magnitude of this spike, three elite breath-hold divers were instrumented with radial artery catheters and had arterial blood specimens taken at the bottom on separate dives to 20, 40,

Hyperbaric oxygen therapy for fatigue recovery: Experimental evidence and optimal regimen from a mouse model established by a chronic multi-stressor paradigm.

AIMS: Fatigue in safety-critical occupations is multifactorial and often overlaps with chronic stress-related neurobehavioral impairment. Hyperbaric oxygen therapy (HBOT) may accelerate fatigue recovery, but its efficacy and optimal regimen are unclear. We evaluated HBOT and identified the optimal number of sessions in a mouse chronic multi-stressor-induced fatigue-like phenotype. MATERIALS AND METHODS: C57BL/6 J mice subjected to a multi-stressor paradigm (restraint, noise, forced running, and overcrowding for 15 days) underwent different post-fatigue interventions: natural recovery, normoba
