A survey of predilection sites and degree of disability associated with guineaworm (Dracunculus medinensis).

Of the 1,200 people surveyed for dracontiasis in Ribi and Kanje villages in Awe Local Government Area of Plateau State, Nigeria, 982 (82%) were infected. Most infections (98%) involved the lower limb. Worms emerging from the umbilicus, groin, palm, wrist, and upper arm were also encountered. Of the 982 infected persons 206 (21%) were totally incapacitated, 193 (20%) were seriously disabled, and 431 (44%) suffered mild incapacity, while 152 (16%) were unaffected. Of the 5 species of cyclopoid copepods found in their main water source, only Thermocyclops nigerianus and Mesocyclops aequatorialis

Surveillance for dracunculiasis, 1981-1991.

In 1986 the World Health Organization (WHO) designated dracunculiasis (guinea worm disease) as the next disease scheduled to be eradicated (by 1995) after smallpox. Dramatic improvement in national and international surveillance has played a key role in the global eradication campaign, which was initiated at CDC in 1980. About 3 million persons are still affected by the disease annually, with adverse effects on their health as well as on agricultural production and education. Over 100 million persons are at risk of having the disease in more than 20,000 villages in India, Pakistan, and 17 Afri

Distribution, behavior, and course of patency of Dracunculus insignis in experimentally infected ferrets.

From 106 ferrets experimentally infected with Dracunculus insignis, 273 female worms and 42 male worms were recovered. The percentage of female worms that mated, the location of all worms, and the rate of emergence of gravid females were recorded. Of 174 mature female worms recovered, only 11% had emerged before necropsy. Eighty-one percent of the male worms and 87% of all unmated (immature) females were found on the trunk of the animal or on the skin overlying it. Gravid female worms were found on the extremities (legs) 88% of the time. Almost one-third of gravid female worms were found on th

Dracunculiasis: an eradicable scourge.

As an incapacitating disease which has a direct negative effect on the self-sufficiency of rural populations in parts of Asia and Africa, dracunculiasis is a serious, but neglected, hindrance to economic development. It is the only communicable disease that is transmitted solely by drinking contaminated water. Several intervention measures have been shown to be effective in reducing or interrupting transmission, the most effective of which is provision of safe drinking water. Its vulnerability to well planned control measures has been demonstrated in India, the Ivory Coast, Nigeria, the Soviet

The existence of Dracunculus medinensis (Linnaeus, 1758) in Turkana, Kenya.

Dracontiasis has been previously reported in southern Sudan, northern Uganda and north-west Eritrea but this is the first report of autochthonous cases in Turkana, Kenya. Five Turkana tribesmen, three females from the same family, one young boy and an adult male were being treated for guinea-worm at the dispensary in Lokichogio, northern Turkana District. The three women had recently returned from southern Sudan, where the disease is endemic, but the two male tribesmen had only lived in and around the Lokichogio region. The methods used in obtaining water from water-holes dug in dry river beds

Concurrent therapeutic and behavioral interventions are associated with a reduced number of emerging Dracunculus medinensis worms in dogs in Chad.

Dracunculus medinensis (Guinea worm; GW) is a parasitic nematode that causes dracunculiasis (Guinea worm disease; GWD). The annual incidence of GWD in humans has been reduced by over 99.9% globally since the 1980s thanks to the implementation of complementary interventions. Dogs are now the primary hosts of GW and impede eradication efforts. The antihelmenthic drug, flubendazole (FLBZ), was suggested as a possible therapeutic intervention after it was found to be partially effective at reducing fertility of D. medinensis in experimentally infected ferrets. A 2019 clinical trial of FLBZ in Chad

Evaluating the Effectiveness of Potential Interventions for Guinea Worm Disease in Dogs in Chad Using Simulations.

Guinea worm (GW) disease (or dracunculiasis) is currently transmitted among dogs in Chad, which presents risks for the human population. We studied how interventions implemented at different levels might reduce the spread of GW disease (geographically and over time) and what levels of interventions might accelerate elimination. We built a multiple-water-source agent-based simulation model to analyze the disease transmission among dogs in Chad, as well as in geographic district clusters, and validated it using local infection data. We considered two interventions: 1) tethering, where infected d

The cost-effectiveness of an eradication programme in the end game: Evidence from guinea worm disease.

BACKGROUND: Of the three diseases targeted for eradication by WHO, two are so-called Neglected Tropical Diseases (NTDs)-guinea worm disease (GWD) and yaws. The Guinea Worm Eradication Programme (GWEP) is in its final stages, with only 25 reported in 2016. However, global eradication still requires certification by WHO of the absence of transmission in all countries. We analyze the cost-effectiveness of the GWEP in the end game, when the number of cases is lower and the cost per case is higher than at any other time. Ours is the first economic evaluation of the GWEP since a World Bank study in

Cutaneous emergence of Eustrongylides in two persons from South Sudan.

Two large, living worms were collected as they emerged from the lower limb of each of two persons in South Sudan. The worms were observed by staff of the South Sudan Guinea Worm Eradication Program during surveillance activities in communities at-risk for cases of Guinea worm disease (dracunculiasis). The worms measured 7 and 8 cm in length and were identified as fourth-stage larvae of Eustrongylides. This is the first report of such worms emerging from the skin; all five previous reports of human infection involved surgical removal of worms from the peritoneal cavity.

[Longitudinal study of Cyclopidae, intermediary hosts of the Guinea worm in the Sudano-Sahelian area (Burkina Faso)].

During 18 months covering two annual rainy seasons, 338 monthly water samples were taken in 29 ponds of two dracunculosis endemic villages of south-western Burkina Faso. The dynamics of ponds inhabited by Cyclopidae has been surveyed as far as their densities and their species are concerned. We identified a total number of 17 species from 6 genera of which the decreasing order in frequency is: Thermocyclops, Mesocyclops, Metacyclops, Cryptocyclops, Microcyclops and Allocyclops. The dominant species are: Thermocyclops crassus consimilis, Metacyclops margaretae, Thermocyclops oblongatus and Ther

Cooking copepods: The survival of cyclopoid copepods (Crustacea: Copepoda) in simulated provisioned water containers and implications for the Guinea Worm Eradication Program in Chad, Africa.

INTRODUCTION: The global Guinea Worm Eradication Program has reduced numbers of human infections of Guinea worm disease (dracunculiasis) to 49 cases in four countries. However, infections of domestic animals (dogs and cats) have recently been recognized and are increasing. Typically, Guinea worm (Dracunculus medinensis) transmission occurs via the ingestion of copepods from water. Despite several interventions, including tethering of dogs while worms emerge, the number of infected dogs continue to increase. One hypothesis is that dogs could be infected through the ingestion of copepods in prov

Challenges in the last mile of the global guinea worm eradication program.

OBJECTIVE: The objective of this study was to identify the existing challenges in the last mile of the global Guinea Worm Eradication Program. METHODS: Systematic Review of articles published from 1 January 2000 until 31 December 2019. Papers listed in Cochrane Library, Google Scholar, ProQuest PubMed and Web of Science databases were searched and reviewed. RESULTS: Twenty-five articles met inclusion criteria of the study and were selected for analysis. Hence, relevant data were extracted, grouped and descriptively analysed. Results revealed 10 main challenges complicating the last mile of g

Dracunculus insignis in ferrets: comparison of inoculation routes.

Three routes of inoculation were compared to determine the best method to infect ferrets with Dracunculus insignis. The traditional method of administering infected cyclops containing L3s through gavage was compared to intraperitoneal (i.p.) and subcutaneous (s.c.) inoculation of L3s. Ten of 18 (56%) gavaged ferrets became infected after receiving copepods containing approximately 100 L3s each; 44 adult worms were recovered from these 10 animals. Twenty-one of 28 (75%) animals inoculated with 50 L3s each became infected i.p.; 92 adult worms were recovered from the positive animals. Four of 5 (
