Resistance to Antifungal Drugs.

Pathogenic fungi have several mechanisms of resistance to antifungal drugs, driven by the genetic plasticity and versatility of their homeostatic responses to stressful environmental cues. We critically review the molecular mechanisms of resistance and cellular adaptations of pathogenic fungi in response to antifungals and discuss the factors contributing to such resistance. We offer suggestions for the translational and clinical research agenda of this rapidly evolving and medically important field. A better understanding of antifungal resistance should assist in developing better detection t

[Drug resistant tuberculosis].

Drug-resistant tuberculosis, and particularly multidrug-resistant tuberculosis (MDR-TB) and extensive drug resistant TB (XDR-TB) as an increasing health problem and a serious challenge to TB control programs. MDR-Tb and XDR-TB are highly lethal in people living with HIV, with case of fatality rates of over 90%. At the same time, there are not many drugs effective in TB chemotherapy. Information about susceptibility patterns of Mycobacterium tuberculosis isolates against antituberculosis drugs is important aspect of tuberculosis control, and surveillance and analysis of local rates of TB drug r

[Resistance to antituberculous drugs].

Mycobacteria responsible for tuberculosis (M. tuberculosis, M. bovis, M. africanum) are susceptible to a very small number of antibiotics. As soon as these drugs were used in humans all gave rise to the selection of resistant mycobacteria. Study of the mechanisms of acquired resistance, with the help of the genetics of mycobacteria, led to a more accurate understanding of the mode of action of antituberculous drugs. The antibiotics isoniazid, pyrazinamide, ethionamide and ethambutol are mycobacteria-specific because they inhibit the synthesis of mycolic acids, which are specific constituants o

Resistance to the nitroheterocyclic drugs.

The nitroheterocyclic drugs have been available since the early 1960's for the treatment of anaerobic protozoa. The application of these drugs has widened since then and they are presently used to treat anaerobic pathogenic bacteria and protozoa. The activity of the nitroheterocyclic drugs depends on the all-important nitro group attached to the imidazole or furan ring. Although the nitro radicals, generated by reduction of the parent drugs, are similar for both families of nitroheterocyclics, the nitroimidazoles and the nitrofurans, the electron potential of each is different and thus the mec

Overcoming Drug Resistance by Paclitaxel Resistance in Triple-Negative Breast Cancer.

Drug resistance in solid tumors has emerged as a critical challenge, with limited effective therapeutic options available. In this study, we find that chemotherapy induces an increased tumor neoantigens burden (TNB) in breast cancer patients and both non-resistant and chemo-resistant triple-negative breast cancer 4T1 tumor-bearing mice. Consistently, proteomic analysis reveals that tumor antigens derived from in vitro chemotherapy-treated paclitaxel (PTX)-resistant 4T1 cells (chemo-rAg) are enriched in damage-associated molecular patterns and neoantigens, highlighting their potential to elicit

Drug-resistant salmonellosis.

After the development of plasmid-mediated resistance against first-line drugs, the emergence of chromosomally-mediated quinolone resistance in typhoid fever has become a major concern. Quinolone insensitivity and treatment failures with ciprofloxacin are being increasingly reported. However, third generation cephalosporins have come as a saviour. To overcome fluoroquinolone insensitivity, higher doses with longer duration of treatment is recommended. Moreover, return of chloramphenicol and furazolidone sensitivity shown by different studies is a welcome observation.

Why does drug resistance readily evolve but vaccine resistance does not?

Why is drug resistance common and vaccine resistance rare? Drugs and vaccines both impose substantial pressure on pathogen populations to evolve resistance and indeed, drug resistance typically emerges soon after the introduction of a drug. But vaccine resistance has only rarely emerged. Using well-established principles of population genetics and evolutionary ecology, we argue that two key differences between vaccines and drugs explain why vaccines have so far proved more robust against evolution than drugs. First, vaccines tend to work prophylactically while drugs tend to work therapeuticall

Drug-resistance patterns in women discovered. Resistance testing necessary, expert says.

If HIV-1 drug resistance is found in a woman's plasma, it's likely the same resistance is present in her genital tract, suggesting that drug resistance among pregnant, HIV-infected women, requires further investigation, a new study shows.

Management of Neisseria gonorrhoeae infection: from drug resistance to drug repurposing.

INTRODUCTION: Neisseria gonorrhoeae is a common sexually transmitted disease connected with extensive drug resistance to many antibiotics. Presently, only expanded spectrum cephalosporins (ceftriaxone and cefixime) and azithromycin remain useful for its management. AREAS COVERED: New chemotypes for the classical antibiotic drug target gyrase/topoisomerase IV afforded inhibitors with potent binding to these enzymes, with an inhibition mechanism distinct from that of fluoroquinolones, and thus less prone to mutations. The α-carbonic anhydrase from the genome of this bacterium (NgCAα) was also v

Drug repurposing: An approach to tackle drug resistance in S. typhimurium.

Drug resistant S. typhimurium pose important public health problem. The development of effective drugs with novel mechanism(s) of action is needed to overcome issues pertaining to drug resistance. Drug repurposing based on computational analyses is considered a viable alternative strategy to circumvent this issue. In this context, 1309 FDA-approved drugs molecules from Mantra 2.0 database were analyzed for this study, against S. typhimurium. Sixteen compounds having similar profiles of gene expression as quinolones were identified from the database, Mantra 2.0. Further, the pharmacophore chara

Drug induced superinfection in HIV and the evolution of drug resistance.

The rapid evolution of HIV drug resistance is a major cause of AIDS treatment failure. Superinfection, the infection of an already infected cell by additional virions, can be a major factor contributing to the evolution of drug resistance. However, the pattern and consequences of superinfection in HIV populations are far from fully understood. In this paper we study the implications of the fact that superinfection is regulated by HIV. We propose that superinfection is negatively associated with the success of the virus, so that more successful viruses are less likely to allow superinfection. W

Overcoming drug resistance in multi-drug resistant cancers and microorganisms: a conceptual framework.

Resistance development against multiple drugs is a common feature among many pathogens--including bacteria such as Pseudomonas aeruginosa, viruses, and parasites--and also among cancers. The reasons are two-fold. Most commonly-used rationally-designed small molecule drugs or monoclonal antibodies, as well as antibiotics, strongly inhibit a key single step in the growth and proliferation of the pathogen or cancer cells. The disease agents quickly change or switch off this single target, or activate the efflux mechanisms to pump out the drug, thereby becoming resistant to the drug. A second prob

Surveillance and analysis of drug resistance and drug resistance levels in multidrug resistant tuberculosis on the tropical islands of China.

OBJECTIVE: Multidrug-resistant tuberculosis (MDR-TB) remains a major public health challenge in China. Hainan, China's largest tropical island, possesses distinct socio-geographical features. However, the drug resistance patterns and molecular epidemiology of MDR-TB in this region have not been fully elucidated. This study aimed to assess the correlation between drug resistance genotypes and phenotypic resistance levels in multidrug-resistant Mycobacterium tuberculosis (MDR-MTB) strains collected from Hainan Island, using whole-genome sequencing (WGS) and phenotypic drug susceptibility testing

Modeling spatial evolution of multi-drug resistance under drug environmental gradients.

Multi-drug combinations to treat bacterial populations are at the forefront of approaches for infection control and prevention of antibiotic resistance. Although the evolution of antibiotic resistance has been theoretically studied with mathematical population dynamics models, extensions to spatial dynamics remain rare in the literature, including in particular spatial evolution of multi-drug resistance. In this study, we propose a reaction-diffusion system that describes the multi-drug evolution of bacteria based on a drug-concentration rescaling approach. We show how the resistance to drugs
