Sleep aid usage following benzodiazepine receptor agonist tapering and cognitive behavioral therapy for insomnia in middle-aged and older adults.

PURPOSE: To describe temporal patterns of use of sleep aids (benzodiazepine receptor agonists [BZRAs], non-BZRAs [e.g., melatonin]) from baseline to 6FU associated with cognitive behavioral therapy for insomnia (CBTI) combined with BZRA tapering and to assess the odds of BZRA use at 6FU associated with non-BZRA use at baseline. METHODS: Using diary data from a trial comparing CBTI combined with a masked BZRA taper method (MTcap) versus standard open BZRA taper (SGT), we determined the frequency of four temporal patterns of sleep aid use at 6FU relative to baseline: 1) "less BZRA-nonBZRA", 2)

Successful management of withdrawal symptoms from long-term intravenous dexmedetomidine using guanfacine in a pediatric patient: a case report.

INTRODUCTION: Dose reduction or discontinuation of dexmedetomidine, an alpha-2 adrenergic agonist sedative, after prolonged continuous administration may be associated with withdrawal syndrome. In adults, the adjunctive use of guanfacine, another alpha-2 adrenergic agonist, has been reported to facilitate the tapering and discontinuation of dexmedetomidine. Herein, we report the first pediatric patient in whom this approach was successfully used. CASE REPORT: A previously healthy 8-year-7-month-old girl developed febrile infection-related epilepsy syndrome and subsequently developed drug-resi

Steroid induced psychosis in a child with nephrotic syndrome: A case report.

BACKGROUND: and Importance: Steroids induced psychosis is one of major adverse effects of corticosteroids therapy especially in patients with disease causing low serum albumin level like nephrotic syndrome. The literature review about this adverse effects is lesser in children than adults population. The management is done by tapering dose of steroids in aid of immunosuppressive drug with an antipsychotics drug. CASE PRESENTATION: Here we present a case of 14 years old female child known case of nephrotic syndrome secondary to Focal segmental glomerulosclerosis with steroids induced psychosis

Glucocorticoid tapering strategies in polymyalgia rheumatica: a systematic literature review.

OBJECTIVES: Glucocorticoids (GCs) are the cornerstone of polymyalgia rheumatica (PMR) treatment, but the optimal tapering strategy remains unclear. This systematic review aimed to explore the impact of published GC tapering strategies on remission, flare rates, GC discontinuation, cumulative GC doses and patient-reported outcomes (PROs). METHODS: PubMed, EMBASE and Cochrane Library were systematically searched for prospective studies reporting GC tapering protocols and outcomes related to remission, flare, GC discontinuation, GC cumulative dosage and PROs. Two researchers independently extrac

Random Forest and Gradient Boosted Trees for Patient Individualized Contrast Agent Dose Reduction in CT Angiography.

This work aims to recognize the patient individual possibility of contrast dose reduction in CT angiography. This system should help to identify whether the dose of contrast agent in CT angiography can be reduced to avoid side effects. In a clinical study, 263 CT angiographies were performed and, in addition, 21 clinical parameters were recorded for each patient before contrast agent administration. The resulting images were labeled according to their contrast quality. It is assumed that the contrast dose could be reduced for CT angiography images with excessive contrast. These data was used t

Patient dose reduction for a localizer radiograph with an additional tin filter in chest-abdomen-pelvis, spine, and head computed tomography examinations.

This study aimed to evaluate the dose reduction potential of adding a tin filter to localizer radiographs (LR) on computed tomography (CT) examinations in both phantom and clinical studies. LRs were performed using combinations of 120 kVp and 20 mA (120/20), 100 kVp with a tin filter, and 50 mA or 20 mA (Sn100/50, Sn100/20). For the phantom experiment, entrance surface doses (ESD) of the LRs were evaluated for each protocol using an anthropomorphic phantom. This retrospective clinical study included 700 patients (300 for chest-pelvis, 200 for spine, and 200 for head CTs). The volume CT dose in

Dose reductions in patients with Waldenström macroglobulinaemia treated with ibrutinib.

Waldenström macroglobulinaemia (WM) is characterized by the presence of a MYD88L265P mutation. This mutation promotes growth and survival of malignant cells through Bruton tyrosine kinase (BTK) activation. Ibrutinib was the first BTK inhibitor approved for WM. Intolerance to ibrutinib frequently leads to dose reductions, though the impact of reducing ibrutinib dosing has not been systematically studied. We performed a retrospective study to determine the frequency and impact of reducing ibrutinib dosing in WM patients. With a median treatment time of 64 months, 96 (27%) of 353 WM patients requ

Opioid Management: Initiating, Monitoring, and Tapering.

Numerous guidelines targeting safe use of opioids for chronic pain have been published but substantial challenges persist in clinical application of best practice recommendations. This article describes a pragmatic approach to clinical care of adults with chronic pain receiving long-term opioid therapy. Three components of care are emphasized: (1) medical and mental health assessment before initiating opioid therapy, (2) clinical surveillance during the course of long-term opioid therapy, and (3) clinical considerations and strategies governing opioid tapering. A pressing need exists for ongoi

Long-term Risk of Overdose or Mental Health Crisis After Opioid Dose Tapering.

IMPORTANCE: Patients prescribed long-term opioid therapy are increasingly undergoing dose tapering. Recent studies suggest that tapering is associated with short-term risks of substance misuse, overdose, and mental health crisis, although lower opioid dose could reduce risks of adverse events over the longer term. OBJECTIVE: To assess the longer-term risks of overdose or mental health crisis associated with opioid dose tapering. DESIGN, SETTING, AND PARTICIPANTS: This is a cohort study using an exposure-crossover analysis. Data were obtained from the OptumLabs Data Warehouse, which includes

Eosinophilic Meningoencephalitis Occurred during Oral Corticosteroid Tapering and after Switching from Anti-IL-5 to Anti-IgE Treatment.

A 49-year-old man with severe eosinophilic asthma, sinusitis, and esophagitis was admitted with a sudden severe headache. The patient was diagnosed with eosinophilic meningoencephalitis based on frontotemporal abnormalities on brain magnetic resonance imaging and high eosinophil counts in the cerebrospinal fluid. His allergic-disease control levels were poor, requiring regular oral corticosteroid (OCS) use. He was switched from anti-interleukin (IL)-5 to anti-IgE therapy because of worsening urticaria and asthma symptoms during OCS tapering. We suspect this was a case of complex eosinophilic m

Evaluation of Clinical Outcomes Associated With Phenobarbital With Taper Compared to No Taper for the Management of Alcohol Withdrawal Syndrome.

BACKGROUND: Phenobarbital (PHB) has been shown to be an effective treatment of alcohol withdrawal syndrome (AWS), with multiple dosing strategies used (e.g., single-dose and symptom-triggered). Studies have often used tapered doses, typically following a front-loaded dose, despite PHB's long half-life which should lead to an ability to auto-taper. OBJECTIVE: The purpose of this study was to compare clinical outcomes associated with two PHB dosing strategies (taper [T], no taper [NT]) for AWS. METHODS: This retrospective cohort study compared adult patients admitted to the ICU from October 20

Metabolic parameters as a predictor of relapse in patients with schizophrenia undergoing tapering of antipsychotics.

Identifying patients at high risk of relapse during antipsychotic tapering remains a significant clinical challenge. This study aims to investigate whether a higher metabolic baseline is associated with increased risk of relapse in a cohort of 83 patients with schizophrenia undergoing tapering of antipsychotic medication. We used data from a tapering program that tapered antipsychotics in patients with schizophrenia during a period of six months, and patients were invited for a re-examination 12 months after baseline. We only collected blood samples at baseline. Mean age was 38.6 (SD 11.63) ye

Radiation Dose Reduction in Kidney Stone CT: A Randomized, Facility-Based Intervention.

OBJECTIVE: Kidney stones are common, tend to recur, and afflict a young population. Despite evidence and recommendations, adoption of reduced-radiation dose CT (RDCT) for kidney stone CT (KSCT) is slow. We sought to design and test an intervention to improve adoption of RDCT protocols for KSCT using a randomized facility-based intervention. METHODS: Facilities contributing at least 40 KSCTs to the American College of Radiology dose index registry (DIR) during calendar year 2015 were randomized to intervention or control groups. The Dose Optimization for Stone Evaluation intervention included
