Intravascular ultrasound and drug-eluting stents. Where are we now?

Drug-eluting stents (DES) have revolutionized interventional cardiology. The profound ability of DES to suppress neointimal hyperplasia results in a reduced need for revascularization and improved clinical outcomes. DES, however, are not immune to in-stent restenosis. This problem particularly affects DES implanted for "off-label" indications and in challenging clinical and anatomic scenarios. In addition, as compared to bare-metal stents, DES do not reduce the incidence of stent thrombosis. Therefore, optimization of DES deployment is becoming increasingly important and, in this regard, intra

Re-stenosis after drug-eluting stents in cardiac allograft vasculopathy.

BACKGROUND: Cardiac allograft vasculopathy (CAV) constitutes a primary cause of death after heart transplantation. Balloon angioplasty and bare metal stents have been used for revascularization but they are associated with a high risk of re-stenosis. Limited data have shown favorable results with drug-eluting stents (DES). This study examines the rate of re-stenosis for DES in CAV as well as predictors for its occurrence. METHODS: Cardiac transplant patients who received at least one DES for a previously untreated coronary lesion were included. These patients were retrospectively followed unt

Treatment of spontaneous coronary artery dissection with drug-eluting stents.

Spontaneous coronary artery dissection SCAD is a rare condition which may result in sudden coronary occlusion, acute myocardial infarction and sudden cardiac death. The prognosis of spontaneous coronary artery dissection is uncertain and optimal treatment is unknown. We describe a case of SCAD in a 29-year-old male with acute inferior wall myocardial infarction MI and post-MI angina that was treated with drug-eluting stents.

Drug-eluting stents for interventional revascularization of coronary multivessel disease.

The treatment of coronary artery disease has changed over the past 35 years since the introduction of coronary artery bypass surgery in 1968. Percutaneous coronary intervention, introduced in 1977 with balloon angioplasty, was accelerated by the establishment of elective stent placement in 1994, together with the development of suitable antiplatelet regimes. In 2002, DES were made commercially available, following the results of clinical trials in single lesions. A meta-analysis of four randomized clinical trials comparing bare stents to bypass surgery for multivessel disease conducted in the

Therapy for Atrial Fibrillation in Patients with Drug-Eluting Stents.

BACKGROUND: Despite guideline recommendations, evidence for the use of non-vitamin K antagonist oral anticoagulant (NOAC) monotherapy in patients with atrial fibrillation after implantation of a drug-eluting stent remains limited. METHODS: In this multicenter, randomized, open-label, noninferiority trial in South Korea, we assigned patients with atrial fibrillation who had undergone the implantation of a drug-eluting stent at least 1 year earlier in a 1:1 ratio to receive NOAC monotherapy or combination therapy (NOAC plus clopidogrel). The primary end point was net adverse clinical events, a

New Innovations in Drug-Eluting Stents for Peripheral Arterial Disease.

PURPOSE OF REVIEW: The purpose of this paper was to provide a review of the burden of peripheral arterial disease; to examine older therapies and their limitations; and especially to highlight new treatment innovations as well as the data supporting their use. RECENT FINDINGS: Building on the success of paclitaxel in the prevention of restenosis in the peripheral circulation, the newest generation drug-eluting stent is presented, which combines paclitaxel with a polymer-allowing the drug to be eluted slowly over 12 months. The positive results of the pilot MAJESTIC study led to the ongoing IM

Comparison of angioscopic findings among second-generation drug-eluting stents.

BACKGROUND: First-generation drug-eluting stents (DES) have reduced short-term stent failure as compared to bare-metal stents due to the inhibition of neointima hyperplasia, but instead increased the risk of very-late stent failure. Although better outcomes have been reported for second-generation DES than for first-generation DES, the difference in the angioscopic findings at 1-year follow-up has not been adequately elucidated among second-generation DES. METHODS: Consecutive 161 patients who received angioscopic examination at 1 year after implantation of second-generation DES, i.e. Nobori

Barriers to clopidogrel adherence following placement of drug-eluting stents.

BACKGROUND: Nonadherence to clopidogrel after drug-eluting stent (DES) placement is associated with in-stent thrombosis and adverse cardiac events. OBJECTIVE: To identify the incidence of and barriers associated with nonadherence to clopidogrel in patients receiving DES. METHODS: Patients who received a DES between March 1, 2004, and August 31, 2005, from a single academic medical center were eligible. Telephone interviews were conducted 6 or more months following discharge. Nonadherence was defined as premature discontinuation of or less than 80% adherence to clopidogrel. Patients were aske

Drug-eluting stents in the management of peripheral arterial disease.

Since major meta-analyses of randomized controlled trials in interventional cardiology showed the potential of drug-eluting stents in decreasing restenosis and reintervention rates after coronary artery stenting, one of the next steps in the treatment of arterial occlusive disease is the transfer of the active coating technology towards peripheral arterial interventions. In this manuscript, we aim to provide a literature overview on available peripheral (lower limb, renal, and supra-aortic) drug-eluting stent applications, debate the cost implications, and give recommendations for future treat

Effectiveness of everolimus-eluting stents in the treatment of drug-eluting stent versus bare-metal stent restenosis.

BACKGROUND: The efficacy of drug-eluting stents (DES) for the treatment of in-stent restenosis (ISR) after DES implantation is not well defined. This study compared the clinical outcome after the use of everolimus-eluting stents (EES) for the treatment of bare-metal stent (BMS) versus DES restenosis. METHOD: Ninety-four patients with 94 ISR were included in this study. Sixty-four patients had BMS-ISR and 30 patients had DES-ISR. Patients were treated by repeat PCI using an EES. The primary endpoint of the study was survival free of target lesion revascularization (TLR) at 12 months or DES-ISR

Relationship between intravascular ultrasound guidance and clinical outcomes after drug-eluting stents: the assessment of dual antiplatelet therapy with drug-eluting stents (ADAPT-DES) study.

BACKGROUND: Prior small to modest-sized studies suggest a benefit of intravascular ultrasound (IVUS) guidance in noncomplex lesions. Whether IVUS guidance is associated with improved clinical outcomes after drug-eluting stent (DES) implantation in an unrestricted patient population is unknown. METHODS AND RESULTS: Assessment of Dual Antiplatelet Therapy With Drug-Eluting Stents (ADAPT-DES) was a prospective, multicenter, nonrandomized "all-comers" study of 8583 consecutive patients at 11 international centers designed to determine the frequency, timing, and correlates of stent thrombosis and

Paclitaxel-eluting stents for the treatment of chronic total coronary occlusions: a strategy of extensive lesion coverage with drug-eluting stents.

The recanalization of a chronic total coronary occlusion (CTO) is hampered by a high rate of lesion recurrence. The goal of the present study is to assess the effect of paclitaxel-eluting stents in CTOs in a strategy of extensive stent coverage and the optional use of additional bare metal stents (BMSs). In 82 consecutive patients, a CTO (duration > 2 weeks) was successfully recanalized with implantation of one or more Taxus stents. These patients underwent a repeat angiography after 5.0 +/- 1.5 months and were assessed by quantitative angiography. The patients were compared with 82 clinically

Drug-Eluting Stent and Drug-Coated Balloon Compared to Full-Covered Drug-Eluting Stent for Femoropopliteal Lesions: CENTAUR Study.

BACKGROUND: Several studies have documented positive clinical outcomes associated with drug-containing devices, such as drug-eluting stents (DES) and drug-coated balloons (DCB), for femoropopliteal artery (FPA) lesions. Endovascular therapy (EVT) using drug-containing devices is the primary strategy, even for complex FP lesions. However, little is known about the clinical outcomes of the hybrid of DES and DCB therapies. AIMS: To compare the efficacy of hybrid therapy with that of a full-covered DES for FPA lesions in a real-world setting. METHODS: This retrospective multi-center study enroll

New-generation drug-eluting stents: focus on Xience V® everolimus-eluting stent and Resolute® zotarolimus-eluting stent.

Compared to bare metal stent angioplasty, first-generation drug-eluting stents (DES) have markedly reduced the incidence of in-stent restenosis. However, given the increased concerns over late and very late stent thrombosis, newer-generation DES were developed. To date, these DES have virtually replaced the use of first-generation DES worldwide. In this review article, we carefully consider the pre-clinical and clinical trials that have been performed with currently available, european conformity-marked and Food and Drug Administration-approved new-generation Resolute(®) and Xience V(®) DES.
