Unexplained hypoglycemia during continuous nocturnal gastric drip-feeding in a patient with glycogen storage disease type ia: is it a dumping-like syndrome?

A 5 years old boy affected with Glycogen Storage Disease type Ia (GSD-Ia) with previous optimal metabolic control developed severe erratic hypoglycemic episodes during continuous nocturnal gastric drip-feeding (CNGDF) administered by nasogastric tube. The episodes of hypoglycemia were not related to pump failure or human errors or wrong position of the tube in the gastrointestinal tract. Hyperinsulinism was also considered in this patient but it was excluded mainly because hypoglycemia was only nocturnal. Moreover, hypoglycemic episodes disappeared when CNGDF was stopped and he was fed with no

Long-Term Outcomes of Pouch Resizing with Ring Augmentation for Dumping Syndrome and Weight Trajectory after Roux-en-Y Gastric Bypass: A Single-Center Experience.

INTRODUCTION: Dumping syndrome (DS) and recurrent weight gain are among the most relevant long-term challenges following Roux-en-Y gastric bypass (RYGB). Several treatment strategies have been proposed, including pouch resizing with ring augmentation (PRRA). This study presents long-term outcomes of PRRA, with a focus on weight trajectory and the management of DS symptoms. METHODS: A retrospective analysis of all PRRA procedures performed at a tertiary referral center between January 2008 and September 2023. Clinical data were obtained from electronic health records and patient questionnaires.

Long-term results after transoral outlet reduction (TORe) of the gastrojejunal anastomosis for secondary weight regain and dumping syndrome after Roux-en-Y gastric bypass.

BACKGROUND: Bariatric surgery has been proven to be the most effective therapy for obesity and Roux-en-Y gastric bypass (RYGB) is one of the most commonly performed procedure. However, weight regain and dumping syndrome occur over time. The transoral outlet reduction (TORe) procedure using an endoscopic suturing device may be an option to treat these conditions. We aimed to analyze outcome parameters and long-term results for this endoscopic technique. METHODS: A retrospective data analysis of patients who underwent TORe using an endoscopic suturing system at our institution from January 2015

Gastroenteric anastomosis in Billroth II gastrectomy with maintenance of the physiologic diameter of the normal pylorus to prevent the dumping syndrome. Part II. Results in 1,300 cases.

Clinical results of the use of a gastroenteric anastomosis with a diameter the size of the normal pylorus (1.9 cm) in 1,300 cases between 1954 and 1971 are presented mainly in terms of prevention of the dumping syndrome. We have had no patient with clinical manifestations of any degree of the dumping syndrome in the first two years after operation. These excellent results have led us to employ the method routinely. We recommend this method for its simplicity and, above all, its effectiveness in preventing the dumping syndrome.

[Copper deficiency anemia and neutropenia secondary to total gastrectomy].

We report a case of copper deficiency with anemia and neutropenia due to dumping syndrome after total gastrectomy. Parenteral nutrition containing cupric sulfate promptly improved the dumping syndrome, anemia and neutropenia. This case suggests that total gastrectomy with dumping syndrome can cause copper deficiency, and that copper deficiency should be considered when encountering similar patients with severe anemia and neutropenia.

Insulinoma-Induced Hypoglycemia in a Patient with Insulinoma after Gastrojejunostomy for Prepyloric Ulcer.

Hyperinsulinism due to dumping syndrome following gastric surgery is an uncommon condition. It is specified with hypoglycemic attacks. However, linking symptoms to dumping syndrome in each patient to whom gastric surgery was performed leads to inappropriate diagnosis and therapy. Insulinoma and other causes that give rise to hyperinsulinemia should not be ignored and these diagnoses should be excluded. In this paper, 71-year-old male patient who was followed up for 2 years with a false conclusion of dumping syndrome and operated on due to insulinoma diagnosed at endoscopic ultrasonography is p

Prediction of dumping after oesophageal cancer surgery.

INTRODUCTION: Dumping syndrome is a common consequence following oesophagectomy and may lead to reduced food intake, resulting in subsequent weight loss, and negatively impact health-related quality of life. This study aimed to develop a predictive model to facilitate preventive treatment planning by identifying individuals at an increased risk of developing dumping syndrome. MATERIALS AND METHODS: Data were obtained from a nationwide, population-based cohort of patients who underwent oesophagectomy for cancer between 2013 and 2020. Patient and clinical characteristics were retrieved from nat

Prevalence and pathophysiology of early dumping in patients after primary Roux-en-Y gastric bypass during a mixed-meal tolerance test.

BACKGROUND: Early dumping is a poorly defined and incompletely understood complication after Roux-en-Y gastric (RYGB). OBJECTIVE: We performed a mixed-meal tolerance test in patients after RYGB to address the prevalence of early dumping and to gain further insight into its pathophysiology. SETTING: The study was conducted in a regional hospital in the northern part of the Netherlands. METHODS: From a random sample of patients who underwent primary RYGB between 2008 and 2011, 46 patients completed the mixed-meal tolerance test. The dumping severity score for early dumping was assessed every

Unrecognized nocturnal hypoglycemia and glucose variability after gastrectomy: A prospective exploratory pilot study using continuous glucose monitoring with nutritional intervention.

INTRODUCTION: Dumping syndrome frequently complicates gastrectomy and is characterized by rapid postprandial glucose excursions and nocturnal hypoglycemia. Although asymptomatic hypoglycemia may persist after surgery, nocturnal hypoglycemia has been associated with cardiovascular risk and sudden death in patients with diabetes. Effective management strategies for post-gastrectomy patients remain limited. This study evaluated whether a diabetes-specific nutritional supplement designed to attenuate hyperglycemia could reduce glycemic variability and nocturnal hypoglycemia in patients with post-g

Symptoms, nutritional outcomes and quality of life after total gastrectomy with Roux-en-Y reconstruction: results of a cross-sectional study conducted on 80 long-term survivors.

Impaired functional outcomes are common issues after total gastrectomy, although seldom explored at a long distance from surgery. The primary aim of this study was to investigate the long-term postoperative functional and nutritional results after Roux-en-Y total gastrectomy (RYTG). Patients with gastric neoplasms or CDH1 mutation undergoing RYTG between 2000 and 2021, with a minimum follow-up of 12 months, were interviewed to assess their functional and nutritional status and to measure their quality of life (QoL) with the GIQLI, EORTC-QLQ20 and EORTC-STO22 questionnaires. Endoscopic Gastrojejunal Revisions Following Gastric Bypass: Lessons Learned in More Than 100 Consecutive Patients.

BACKGROUND: Weight regain and dumping after Roux-en-Y gastric bypass (RYGB) are long-term challenges thought to be due to dilation of the gastrojejunal anastomosis. The aim of this study was to analyze the feasibility, safety, and outcomes of endoscopic gastrojejunal revisions (EGRs) after its introduction in a tertiary bariatric surgery center. METHODS: From January 2016 to March 2018, we reviewed the electronic records of all patients undergoing EGR with the OverStitch suturing device. Demographics, procedure details, and outcomes were recorded. RESULTS: There were 107 patients (M:F = 29:7

Bilateral diffuse uveal melanocytic proliferation with multifocal diffuse integumentary melanocytic proliferation paraneoplastic syndrome: A case report.

Bilateral diffuse uveal melanocytic proliferation (B-DUMP) is a rare paraneoplastic syndrome typically presenting with bilateral visual loss. B-DUMP is associated with extraocular systemic malignancies with the most common being lung cancer in males and uro-gynaecological cancer in females (mainly ovarian cancer). Cutaneous and/or mucosal involvement in patients with B-DUMP has been reported but it is not well characterised. Herein, we present a female in her 70s with diagnosis of stage IV vaginal clear-cell carcinoma and metastatic melanoma of unknown primary that developed progressive bilate

Structural insight into ligand interactions of thymidylate synthase from white spot syndrome virus.

White spot syndrome virus (WSSV) is one of the deadliest crustacean pathogens, causing huge economic loss in global shrimp industry. WSSV encodes a thymidylate synthase (wTS) that is essential for DNA replication and viral proliferation, serving as a promising drug target against WSSV infections. To aid drug design, we solved wTS structures in complex with dUMP and dUMP/raltitrexed, at 2.28 Å and 1.43 Å resolutions, respectively. wTS forms a homodimer and each ligand-binding cavity is contributed by both monomers. In wTS-dUMP binary structure, the protein adopts an open conformation, with dUMP

Dumping in infancy diagnosed by radionuclide gastric emptying technique.

Two infants were diagnosed with dumping syndrome by a radionuclide gastric emptying method. Both patients presented with anorexia, weight loss, agitation and diaphoresis following bolus feeding by gastrostomy tube. One had documented hyperglycemia and glycosuria. Symptoms and signs of dumping in one patient were due to a gastrostomy placed in the antrum, whereby bolus tube feedings were inadvertently introduced directly into the duodenum. The second patient developed dumping symptoms after a Waterston colonic interposition was performed to correct a long gap esophageal atresia. Gastric emptyin

Metabolic problems after gastric surgery.

The findings on dumping syndrome (DS) are not consistent considering its relations with age, sex, weight/height, smoking habits, race, dose of oral glucose, the time elapsed since surgery, the function of exocrine pancreas nor the duration of ulcer symptoms. The patients after total gastrectomy (TG) may present relative postprandial lack of insulin. As a sign of long-term hyperglycemia elevated HbA1 has been measured in DS patients. Oral galactose test may reveal new features of DS. Abnormalities in splanchnic blood circulation as well as release of intestinal hormones are involved with DS. Di
