Clinical and imaging characterization of progressive spastic dysarthria.

BACKGROUND AND PURPOSE: To describe speech, neurological and imaging characteristics of a series of patients presenting with progressive spastic dysarthria as the first and predominant sign of a presumed neurodegenerative disease. METHODS: Participants were 25 patients with spastic dysarthria as the only or predominant speech disorder. Clinical features, pattern of MRI volume loss on voxel-based morphometry and pattern of hypometabolism on F18-fluorodeoxyglucose positron emission tomography (FDG-PET) scan are described. RESULTS: All patients demonstrated speech characteristics consistent wit

[Dysarthria-clumsy hand syndrome caused by cerebellar infarction].

A 80 year old woman suddenly complained of dysarthria and clumsiness of the right hand. CT scan showed an infarct in the territory of the lateral branch of the right superior cerebellar artery. This dysarthria-clumsy hand syndrome in a patient with paroxysmal atrial fibrillation suggests that the likely cause was an embolus from the heart.

Vowel Acoustics as Predictors of Speech Intelligibility in Dysarthria.

PURPOSE: This study sought to determine if alternative vowel space area (VSA) measures (i.e., novel trajectory-based measures: vowel space hull area and vowel space density) predicted speech intelligibility to the same extent as two traditional vowel measures (i.e., token-based measures: VSA and corner dispersion) in speakers with dysarthria. Additionally, this study examined if the strength of the relationship between acoustic vowel measures and intelligibility differed based on how intelligibility was measured (i.e., orthographic transcriptions [OTs] and visual analog scale [VAS] ratings).

[Blepharoptosis and dysarthria in a boy aged 2 years].

A boy, aged 2 years and 4 months, had a sudden onset of blepharoptosis of the right eyelid, accompanied by the mouth deviated to the right side, drinking cough, nystagmus, and developmental regression. Cranial MRI showed softening lesions formed after infarction of the right dorsolateral medulla oblongata, while head CT angiography showed no imaging of the proximal part of the V4 segment of the right vertebral artery. The child was diagnosed with dorsolateral medulla oblongata syndrome and was treated with gamma globulin to regulate immune function, with mannitol to reduce neuronal edema, with

Signs of dysarthria in adults with 22q11.2 deletion syndrome.

The aim of the study was to investigate how adults with 22q11.2 deletion syndrome (22q11DS) performed on dysarthria and intelligibility tests compared with a control group. Ten participants with confirmed 22q11.2 deletion, five males and five females with a mean age of 31 years (range: 19-49), were compared with a control group matched for gender and age (five males and five females, mean age: 32 years, range: 19-49). Assessment of non-verbal and verbal tasks reflecting respiration, phonation, oral motor function, velopharyngeal function, articulation, and prosody was performed as well as the

Speech rate treatments for individuals with dysarthria: a tutorial.

Research confirms the long-standing clinical observation that patients with dysarthria exhibit variability in speech rate. Thus, modifying speech rate has been documented as one of the best treatment options for these patients. In this tutorial, several published rate control interventions for dysarthric speakers are presented. Studies discussed utilized various interventions, including pacing boards, alphabet board supplementation, visual and auditory feedback, cueing and pacing strategies, and delayed auditory feedback. Interventions discussed represent a hierarchy from "rigid" strategies, w

Clinical study of the responsible lesion for dysarthria in the cerebellum.

There have been few reports describing the lesion for cerebellar dysarthria. We compared MRI findings of 4 reported patients (including our previously reported patient) to that of our patient who showed ataxic speech and ataxic gait. The lesions of 4 patients involved lobulus quadrangularis and lobulus simplex, and the lesion of the present patient involved lobulus semilunaris superior and lobulus simplex. Since lobulus simplex and lobulus quadrangularis were involved in many patients, we speculated that the cerebellar dysarthria of the present patient was due to the damage of these areas in t

Disorders of speech and language: aphasia, apraxia and dysarthria.

PURPOSE OF REVIEW: We review recent important papers pertaining to acquired aphasia, apraxia of speech and dysarthria with special attention to clinically significant work published in the last 12 months. RECENT FINDINGS: The role of the contralateral inferior frontal gyrus in language recovery after stroke is controversial, but is an area of active research, particularly in functional imaging studies. Recent treatment studies in poststroke aphasia have shown that intensity of language therapy may be more important than the method of therapy. Some studies have indicated that amphetamines, pir

Speech and language therapy for dysarthria in Parkinson's disease.

BACKGROUND: Dysarthria is a common manifestation of Parkinson's disease which increases in frequency and intensity with the progress of the disease (Streifler 1984). Up to 20% of Parkinsonian patients are referred for speech and language therapy (S & L T), its aim being to improve the intelligibility of the patient's speech. OBJECTIVES: To compare the efficacy of speech and language therapy versus placebo or no interventions in patients with Parkinson's disease. SEARCH STRATEGY: Relevant trials were identified by electronic searches of MEDLINE, EMBASE, CINAHL, ISI-SCI, AMED, MANTIS, REHABDAT

Paroxysmal dysarthria and ataxia in a patient with Behçet's disease.

The paroxysmal attacks which are frequently encountered in the course of multiple sclerosis (MS) are characterised by their sudden onset, short duration and frequent repetition. Such attacks have also been reported in some other diseases affecting the CNS, such as systemic lupus erythematosus. However, to our knowledge, they have not been reported in neuro-Behçet's disease (NBD). A patient with NBD who developed paroxysmal dysarthriaataxia attacks is presented, and the similarity of some clinical, laboratory, and neuroradiological aspects of NBD and MS are discussed with special emphasis on ma

Multidimensional Measurements of Dysarthria in Myotonic Dystrophy Type 1.

BACKGROUND: Myotonic dystrophy type 1 (DM1) is a heterogeneous neuromuscular disorder characterized by progressive muscle weakness and myotonia. Dysarthria is a known symptom of DM1, but literature is lacking about the patient's own perception in relationship to dysarthria characteristics and severity. AIMS: The aim of the study was to describe the acoustic speech characteristics of dysarthria in patients with DM1, examine the perceptually determined dysarthria severity through speech and language therapy assessment, gather subjective evaluations of speech and intelligibility from patients an

Does Native Language Matter in Perceptual Ratings of Dysarthria?

PURPOSE: Despite the general agreement that dysarthria characteristics are largely language-independent, few efforts have attempted a systematic comparison across languages. To examine the role of native languages in the perception of speech characteristics of dysarthria secondary to Parkinson's disease (PD), auditory-perceptual ratings of dysarthria, and confidence level of the judgments were compared between two listener groups: language-matched and language-crossed. METHOD: A total of 60 listeners (35 native speakers of Korean and 25 native speakers of American English) estimated speech ab

Vowel acoustics in dysarthria: speech disorder diagnosis and classification.

PURPOSE: The purpose of this study was to determine the extent to which vowel metrics are capable of distinguishing healthy from dysarthric speech and among different forms of dysarthria. METHOD: A variety of vowel metrics were derived from spectral and temporal measurements of vowel tokens embedded in phrases produced by 45 speakers with dysarthria and 12 speakers with no history of neurological disease. Via means testing and discriminant function analysis (DFA), the acoustic metrics were used to (a) detect the presence of dysarthria and (b) classify the dysarthria subtype. RESULTS: Signifi

[Dysphagia and dysarthria surgery for advanced-age myasthenia gravis].

Myasthenia gravis is often difficult to diagnose and treat in older subjects due to complications, previous history and reciprocal interaction with drugs used to treat complications. An 84-year-old woman with slowly progressive 2-year dysphagia and dysarthria had reached critical condition with aspiration pneumonia. She was diagnosed with thymoma-free myasthenia gravis and her respirator removed after being administrated an anticholinesterase drug. Her dysphagia and dysarthria did not improve. Because of severe osteoporosis with two previous lumbar compression fractures and excessive thinness,

Giant cell arteritis presenting as dysarthria and mediastinal mass.

Giant cell arteritis, Takayasu arteritis, and Horton disease are rare, idiopathic diseases that cause chronic inflammation and obliteration of large arteries, mainly the aorta and its major branches. Histological examination reveals multinucleated giants cells and clinical presentation is characterized by general symptoms and/or symptoms related to stenosis or occlusion of vessels. A case of a 50-year-old woman with neurological symptoms, cervicothoracic tumour with severe stenosis of the right subclavian artery and complete occlusion of common carotid artery is presented.

Patients' experiences of disruptions associated with post-stroke dysarthria.

BACKGROUND: Post-stroke dysarthria rehabilitation should consider social participation for people with dysarthria, but before this approach can be adopted, an understanding of the psychosocial impact of dysarthria is required. Despite the prevalence of dysarthria as a result of stroke, there is a paucity of research into this communication disorder, particularly studies that address the experiences of individuals. The available literature focuses mainly on the perceptions of others or includes groups of mixed aetiologies. AIMS: To investigate the beliefs and experiences of people with dysarth
